        캡사이신유도체를 함유하는 폴록사머 겔제제의 물리화학적 특성 : based Gel Containing Capsaicin Analog

        Physicochemical changes of poloxamer-based gel containing capsaicin analog (N-[3-(3,4-dimethylphenyl)propyl]-4-(2-aminoethoxy)-3-methoxypheny] acetamide) such as drug content, viscosity and surface tension were investigated during the storage conditions at three different temperatures (25, 40 and 60℃) over 90 days. No noticeable changes of color were observed when stored at 25 and 40℃. However, the color of white poloxamer gels turned yellow during storage at 60℃. The drug contents were unchanged during storage at 25℃ but had tendency to decrease at 40℃. The drug contents were highly decreased over 40-50% when stored at 60℃. The viscosity of a poloxamer-based gel was unchanged during storage at 25 and 40℃ but greatly increased at 60℃. The surface tension of a poloxamer-based gel was not changed at three different temperatures. The storage conditions of a poloxamer-based gel containing capsaicin analog can be considered for further clinical applications.

        Textual Configuration and Cultural Connotations of the Outlaws of the Marsh (Shuihu)

        The Outlaws of the Marsh (Shuihu) is by no means a story about folk rebelling alone. It is rich in cultural connotations of profundity hidden behind the legendary bushrangers. Its basic meaning-generating mode is consisted in the conflict between the "Dao" (Way) and "Shi" (Power): the former is exemplified by the moral ideal of the Shiren as intelligentsia while the latter by the power system of monarchy in ancient China. Hence the novel as such reveals mainly an expression of the value-orientation and spiritual confusion of the Confucian scholars in general.

        Structure-based lead discovery for protein kinase C zeta inhibitor design by exploiting kinase–inhibitor complex crystal structure data and potential therapeutics for preterm labour

        The protein kinase C (PKC) is a family of serine/threonine kinases with a broad range of cellular targets. Members of the PKC family participate at the diverse biologicalevents involved in cellular proliferation, differentiationand survival. The PKCisoformzeta (PKCf) is an atypicalmember that has recently been found to play an essential rolein promoting human uterine contractility and thus been raisedas a new target for treating preterm labour and other tocolyticdiseases. In this study, an integrative protocol was describedto graft hundreds of inhibitor ligands from their complexcrystal structures with cognate kinases into the active pocketof PKCf and, based on the modeled structures, to evaluate thebinding strength of these inhibitors to the non-cognate PKCfreceptor by using a consensus scoring strategy. A total of 32inhibitors with top score were compiled, and eight out of themwere tested for inhibitory potency against PKCf. Consequently,five compounds, i.e. CDK6 inhibitor fisetin, PIM1inhibitor myricetin, CDK9 inhibitor flavopiridol and PknBinhibitor mitoxantrone as well as the promiscuous kinaseinhibitor staurosporine showed high or moderate inhibitoryactivity on PKCf, with IC50 values of 58 ± 9, 1.7 ± 0.4,108 ± 17, 280 ± 47 and 0.019 ± 0.004 lM, respectively,while other three compounds, including two marketed drugsdasatinib and sunitinib as well as the Rho inhibitor fasudil,have not been detected to possess observable activity. Next,based on the modeled structure data we modified three flavonoidkinase inhibitors, i.e. fisetin, myricetin and flavopiridol,to generate a number of more potential molecularentities, two of which were found to have a moderatelyimproved activity as compared to their parent compounds.

        The Study of Variational Feed Rate in 4-Axis Machining of Blades

        As a core part of aerospace, space, and steam turbine plants, blades are generally machined via 5-axis linkage processing to satisfy the high precision requirements of the rigorous surface. To save costs in blade machining, many small- and medium-sized enterprises often combine standard 3-axis computer numeric control machines with the automatic indexing turntable. The traditional 4-axis machining method adopts a constant feed rate, which causes overcutting near the leading and trailing edges of the blade because of the rapid changes in tool orientation. To solve this problem, we propose a speed optimization method that utilizes variational speed to ensure that the decomposition velocity and acceleration of each axis do not exceed the allowable values. First, we guarantee the correct tool lead angle. Second, a corrected speed model is established to obtain the component speed of each axis and to determine the constraint conditions of maximum and accelerated speed. Third, a 4-axis post processor for blade processing is developed using Java advanced language combined with the optimization algorithm. The cutting experiment reveals that our proposed speed optimization method effectively controls the precision of the surface profile and overcomes the overcut phenomenon that often occurs in traditional 4-axis machining.

      • Combined Effects of Six Cytokine Gene Polymorphisms and SNP-SNP Interactions on Hepatocellular Carcinoma Risk in Southern Guangxi, China

        Cytokine gene single nucleotide polymorphisms (SNPs) are involved in the genesis and progression of hepatocellular carcinoma (HCC). We hypothesized that combined effects of cytokine gene SNPs and SNP-SNP interactions are associated with HCC risk. Six SNPs in cytokine genes (IL-2, IFN-${\gamma}$, IL-$1{\beta}$, IL-6, and IL-10) were genotyped in a study of 720 Chinese HCC cases and 784 cancer-free controls. Although none of these SNPs individually had a significant effect on the risk of HCC, we found that the combined effects of these six SNPs may contribute to HCC risk (OR=1.821, 95% CI=1.078-3.075). This risk was pronounced among smokers, drinkers, and hepatitis B virus carriers. A SNP-SNP interaction between IL-2-330 and IFN-${\gamma}$-1615 was associated with an increased HCC risk (OR=1.078, 95% CI=1.022-1.136). In conclusion, combined effects of SNPs and SNP-SNP interactions in cytokine genes may contribute to HCC risk.

        Automatic Detection and Classification of Rib Fractures on Thoracic CT Using Convolutional Neural Network: Accuracy and Feasibility

        Objective: To evaluate the performance of a convolutional neural network (CNN) model that can automatically detect and classify rib fractures, and output structured reports from computed tomography (CT) images. Materials and Methods: This study included 1079 patients (median age, 55 years; men, 718) from three hospitals, between January 2011 and January 2019, who were divided into a monocentric training set (n = 876; median age, 55 years; men, 582), five multicenter/multiparameter validation sets (n = 173; median age, 59 years; men, 118) with different slice thicknesses and image pixels, and a normal control set (n = 30; median age, 53 years; men, 18). Three classifications (fresh, healing, and old fracture) combined with fracture location (corresponding CT layers) were detected automatically and delivered in a structured report. Precision, recall, and F1-score were selected as metrics to measure the optimum CNN model. Detection/diagnosis time, precision, and sensitivity were employed to compare the diagnostic efficiency of the structured report and that of experienced radiologists. Results: A total of 25054 annotations (fresh fracture, 10089; healing fracture, 10922; old fracture, 4043) were labelled for training (18584) and validation (6470). The detection efficiency was higher for fresh fractures and healing fractures than for old fractures (F1-scores, 0.849, 0.856, 0.770, respectively, p = 0.023 for each), and the robustness of the model was good in the five multicenter/multiparameter validation sets (all mean F1-scores > 0.8 except validation set 5 [512 x 512 pixels; F1-score = 0.757]). The precision of the five radiologists improved from 80.3% to 91.1%, and the sensitivity increased from 62.4% to 86.3% with artificial intelligence-assisted diagnosis. On average, the diagnosis time of the radiologists was reduced by 73.9 seconds. Conclusion: Our CNN model for automatic rib fracture detection could assist radiologists in improving diagnostic efficiency, reducing diagnosis time and radiologists’ workload.

        Ram semen preserved at 0℃ with soybean lecithin Tris-based extender substituted for egg yolk

        Objective: The present study evaluated the preservation of ram semen at 0℃ using soybean lecithin with a Tris-fructose extender. Methods: Semen was collected by artificial vagina ejaculation from six rams with proven fertility. High quality ejaculates were diluted by soybean lecithin (0.25%, 0.5%, 0.75%, 1.0%, 1.25%) using Tris-fructose extender and control (Tris-fructose egg yolk extender), respectively. The ejaculates were diluted to a concentration of 5×108 sperm/mL, followed by cooling to 0℃ in 90 min and maintaining the temperature for 12 days. The diluted semen samples were examined and recorded for sperm progressive motility, acrosome integrity at 0, 24, 72, 144, 216, 288 h, respectively. Two hundred and twenty-three ewes were inseminated for 216 h with optimal soybean lecithin concentrated semen or control via trans-cervical insemination. Results: The results showed that there were no differences in sperm progressive motility at 0, 24, 72, and 144 h (p>0.05). After 216 h, the sperm progressive motility in the control group and 0.5% concentration groups was significantly higher when compared to 0.25% concentration (p<0.05). The 0.5% concentration group demonstrated the highest survival rate and had no difference with the control group (p>0.05). At 216 h, the sperm progressive motility of all groups was still above 50%. The acrosome integrity of all groups was decreased with prolongation of storage time, but there was no difference at each time point (p>0.05). There was no significant difference in the lambing rate and pregnancy rate between the 0.5% concentration group and the control group (p>0.05). Conclusion: These results suggest that ram sperm is capable of fertilization after preservation at 0℃ with 0.5% of soybean lecithin in Tris-based extender substituted for egg yolk and produce normal offspring after insemination.

        Percutaneous Sacroplasty for Painful Sacral Metastases Involving Multiple Sacral Vertebral Bodies: Initial Experience with an Interpedicular Approach

        Objective: To report our initial experience of percutaneous sacroplasty (PSP) with an interpedicular approach for treating painful sacral metastases involving multiple sacral vertebral bodies. Materials and Methods: This study prospectively enrolled 10 consecutive patients (six men and four women; mean age, 56.3 ± 13.8 years) who underwent PSP for painful sacral metastases involving multiple sacral vertebral bodies from March 2017 to September 2018. Visual analogue scale (VAS) scores, Oswestry disability index (ODI) values, and the number of opioids prescribed to the patients were assessed before and after PSP. The procedure duration, length of hospitalization, and complications were also recorded. Results: Mean VAS and ODI declined significantly from 6.90 ± 1.20 and 74.40 ± 5.48 before the procedure to 2.70 ± 1.34 and 29.60 ± 14.57 after the procedure, respectively (p < 0.01). The median number of opioids prescribed per patient decreased from 2 (interquartile range [IQR] 1–3) pre-procedure to 1 (IQR 0–3) post-procedure (p < 0.01). Nine of the 10 patients showed no or decreased opioid usage, and only 1 patient showed unchanged usage. The mean procedure duration was 48.5 ± 3.0 minutes. The average length of hospitalization was 4.7 ± 1.7 days. Extraosseous cement leakage occurred in three cases without causing any clinical complications. Conclusion: PSP with an interpedicular approach is a safe and effective treatment in patients with painful sacral metastases involving multiple sacral vertebral bodies and can relieve pain and improve mobility.
