        A Prospective Study on the Prevalence, Extent of Disease and Outcome of Eosinophilic Gastroenteritis in Patients Presenting with Lower Abdominal Symptoms

        Background/Aims: The epidemiology of eosinophilic gastroenteritis remains unclear. We aim to determine the prevalence of eosinophilic gastroenteritis in patients with lower abdominal symptoms. Methods: In a prospective study, colonoscopy was performed on 2,469 consecutive patients. Biopsies were taken from the terminal ileum and ascending, transverse, descending and sigmoid colon in all patients. Results: Sixty-four of the 2,469 patients (2.6%) had eosinophilic gastroenteritis. Only five of the 64 patients (7.8%) with eosinophilic gastroenteritis had endoscopic mucosal abnormalities during colonoscopy. Six of these 64 patients (9.4%) had severe disease at presentation, and seven of these 64 patients (10.9%) required systemic steroid treatment. An elevated absolute peripheral eosinophil count was independently associated with severe disease at presentation (4/6 [66.7%] vs 3/58 [5.2%], p=0.005; odds ratio [OR], 25.320; 95% confidence interval [CI], 2.628 to 243.910), and severe disease at the time of presentation was independently associated with the use of systemic steroid treatment (6/7 [85.7%] vs 0/57 [0%], p=0.008; OR, 18.021; 95% CI, 2.163 to 150.152). Conclusions: The prevalence of eosinophilic gastroenteritis is common, and patients usually present normal- appearing mucosa on colonoscopy. Those with severe disease at presentation usually have a raised absolute peripheral eosinophil count and should be commenced on systemic steroids as an initial therapy. (Gut Liver 2018;12:288- 296)

        Growth and Physiological Responses of a Tropical Toxic Marine Microgalga Heterosigma akashiwo (Heterokontophyta: Raphidophyceae) from Singapore Waters to Varying Nitrogen Sources and Light Conditions

        Strains of Heterosigma akashiwo have been identified in Singapore waters, but there have been few studies detailing the biology of such tropical strains. Management of such species is important for the integrity of aquaculture industries, as well as for food safety and public health. Coastal modifications may, however, be altering the productivity of such species. This study investigated the effects of two anthropogenic factors—nitrogen (N)-pollution and light-shading—upon a single strain of H. akashiwo which was isolated from Singapore waters. The study observed that H. akashiwo strains grew well under the pulsed supply of three N-sources— ammonium, nitrate, and urea. Growth rate values suggest its ability to out-grow diatom species, which typically dominate coastal environments. The light absorption capacity highlighted potential response dissimilarity under the different N-regime conditions. Light experiments also demonstrated the ability of H. akashiwo to tolerate decreased light conditions, which allow it to thrive at depth. The ability to accumulate intracellular stores of N-nutrients was also demonstrated and can be important for sustaining cell productivity. This study therefore indicated how poor management of coastal environments may enhance the bloom potential of such toxic bloomforming species.

        Effects of Eurycoma Longifolia Jack Supplementation on Recreational Athletes` Endurance Running Capacity and Physiological Responses in the Heat

        This study investigated the effects of supplementation of the nutraceutical product Eurycoma longifolia Jack on recreational athletes` endurance running capacity and physiological responses in the heat. Twelve Malaysian healthy male recreational athletes (Age: 23.3 (3.7) years old; VO2max: 45.1 (8.1) ml.kg-1.min-1) were recruited in this double blind, placebo-controlled, cross-over study. Subjects completed two endurance running trials in the heat (31°C, 70% relative humidity), performed on separate days, after consuming either 2 capsules of Eurycoma longifolia Jack (75 mg per capsule) or a placebo per day for 7 days before and one hour prior to the experimental trial. On trial day, after 5 minutes of warm-up at 50 % VO2max, the subjects were requested to run on the treadmill at 60 % VO2max for 60 minutes. This was immediately followed by a 20-minute time trial for determining endurance running capacity. Blood samples were taken before warm up, after warm-up, and every 20 minutes during the trial. Statistical analysis was performed using one-way ANOVA with repeated measures. Results showed that the endurance running capacity of E. longifolia was not significantly different from that of the placebo trial. Similarly, oxygen uptake, heart rate, skin temperature, tympanic temperature, ratings of perceived exertion, haemoglobin concentration, haematocrit level, plasma glucose concentration, and plasma free fatty acid concentration were not significantly different between the trials. These findings suggested that supplementation of the E. longifolia product at a dosage of 150 mg.day-1 for 7 days has not provided beneficial effects on endurance running capacity and physiological responses of recreational athletes in the heat. Higher dosage and longer duration of supplementation of the product may be warranted to evaluate further its endurance capacity during exercise.

        Principled practical knowledge in bridging practical and reflective experiential learning: case studies of teachers’ professional development

        “Heuristic approach” is a teachers’ professional development (TPD) method supported by principled practical knowledge (PPK) which focuses on principled practical guidance that develops teachers’ capacity in innovative learning designs. This study reports the key features of the design and implementation of a TPD programme aimed at facilitating teachers’ experiential learning in designing and implementing seamless inquiry science learning (SISL). The TPD is based on the notion of fast and frugal heuristics which are supported by PPK and lead to actual classroom action. Two teacher case studies were analysed to investigate the teachers’ learning trajectory underpinned by an experiential learning framework throughout the TPD programme. As learners of innovative pedagogy, the teachers explored the design and implementation of SISL with their students. The findings indicate that the TPD programme had a positive impact on three aspects of teachers’ experiential learning: (a) acquiring new pedagogy; (b) improving knowledge and skills in the seamless inquiry science lesson (SISL) design and implementation; (c) nurturing reflective practices in teachers concerning the designing and implementation of SISL lessons. The teachers’ professional reflection and growth through experiential learning processes demonstrate how heuristic-based TPD engaged them as creative and professional lesson designers, resulting in meaningful changes in classroom practices. Our data analysis concluded that the heuristic-based TPD should be designed in such a way that it is authentic, reflective, practice-based and focused on meaningful tasks for the teachers.
