        해독금화산(解毒金花散)의 Methicillin-Resistant Staphylococcus aureus에 대한 항균효과

        Objectives Methicillin-Resistant Staphylococcus aureus (MRSA) is a human pathogen and a major cause of hospital-acquired infections. New antibacterial agents that have not been compromised by bacterial resistance are needed to treat MRSA-related infections. In this study, we investigated the antimicrobial activity ofethanol extract of Haedokgeumhwa-san (HGH) which prescription is composed of korean medicine against MRSA. Methods The antibacterial activity of HGH extract was evaluated against MRSA strains by using the Disc diffusion method, broth microdilution method (minimal inhibitory concentration; MIC), checkerboard dilution test, and time-kill test; its mechanism of action was investigated by bacteriolysis, detergent or ATPase inhibitors. The checkerboard dilution test was used to examined synergistic effect of ampicillin, oxacillin, ciprofloxacin, vancomycin, gentamicin and norfloxacin in combination with HGH ethanol extract. A time-kill assay was performed a survival curve which was obtained by plotting viable colony counts depending on time on bacterial growth. Results The minimum inhibitory concentration (MIC) of ethanol extract (HGH) ranged from 1,000 to 2,000 μg/mL against all the tested bacterial strains, respectively. We are able to confirm that HGH extract has potentially strong antibacterial activity. In the checkerboard dilution test, fractional inhibitory concentration index of HGH in combination with antibiotics indicated synergy or partial synergism against S. aureus. A time-kill study showed that the growth of the tested bacteria was considerably inhibited after 8 hr of treatment with the combination of HGH with selected antibiotics. For measurement of cell membrane permeability, HGH 250~1,000 μg/mL along with concentration of Triton X-100 (TX) and Tris-(hydroxymethyl) aminomethane (Tris) were used. In the other hand, N,N-dicyclohexylcarbodimide (DCCD) and Sodium azide (NaN3) was used as an inhibitor of ATPase. TX, Tris, DCCD and NaN3 cooperation against S. aureus showed synergistic action. Accordingly, antimicrobial activity of HGH was affected by cell membrane and inhibitor of ATPase. Conclusions These results suggest that Haedokgeumhwa-san extract has antibacterial activity, and that HGH extract offers a potential as a natural antibiotic against MRSA.         가미한다열소탕(加味寒多熱少湯)이 고지방식이 비만생쥐의 체지방감소에 미치는 영향

        Objectives In this study, it was investigated whether Gami-Handayeolso-Tang (HDYST) medication has anti-obesity effects in high fat diet (HFD)-fed obese mice. Methods The experimental animals were divided into five groups-normal diet-fed (ND), high fat diet-fed control (HFD), HFD+HDYST 150, HFD+HDYST 300, and HFD+orlistat as a positive drug. The obese markers such as body weight, diet efficiency ratio, serum levels of total cholesterol, triglyceride, lipid contents, leptin, adiponectin, and GOT/GPT were measured. Also, white adipose tissue, liver weight, abdominal fat mass, hepatic lipid contents, and mRNA expression of obese-associating genes were examined in obese mice. Results In high fat diet-fed mice, HDYST administration significantly decreased body weight, diet efficiency ratio, serum levels of total cholesterol, triglyceride, LDL-cholesterol, as well as leptin and GOT/GPT, compared to the HFD group in a dose-dependent manner. HDYST increased significantly the serum levels of HDL-cholesterol and adiponectin. It also reduced the accumulation of lipids, such as total lipid and triglycerides, in organs such as liver and abdominal adipose tissue. Moreover, HDYST administration significantly decreased the expression levels of fatty acid synthetic genes, such as sterol regulatory element- binding protein-1c (SREBP-1c), FAS and Stearoyl-Coenzyme A desaturase 1 (SCD-1), in the liver tissues, while it increased the messenger RAN (mRNA) levels of fatty acid catalytic genes, such as Peroxisome proliferator activated receptor alpha (PPAR-α), acyl-COA oxidase (ACO), and Carnitine palmitoyltransferase-1a (CPT-1a). Conclusions Based on the results above, HDYST reveals anti-obesity effects declining body fat accumulation through the regulation of fatty acid metabolism and leptin/ adiponectin serum levels. It therefore suggests that HDYST can be clinically useful for the treatment of obesity. (J Korean Med Rehab 2016;26(1):13-31)

        개인채무자 구제제도의 이용현황과 개선방안에 관한 연구

        본 연구는 채무조정 또는 채무청산을 통해 과다채무자의 조속한 경제활동 복귀를 지원하여 국민경제의 지속가능한 성장을 도모하는 개인채무자 구제제도에 대한 관심이 크게 증가함에 따라 국내에서 이루어진 선행연구를 중심으로 국내 구제제도와 해외 구제제도에 대한 비교를 통하여 문제점을 검토한 후 다음과 같은 개선방안을 제시하였다. 첫째, 개인채무자의 경제적 회생을 지원하고 선의의 금융소비자와 채권자를 보호하기 위해 주택담보채권에 대한 별제권의 행사를 제한하기보다 면제재산의 범위를 탄력적으로 조정하여 성실한 채무자의 기본생활을 보장하는 방안을 강구할 필요가 있다. 예컨대 성실한 채무자에게는 압류금지재산에 임차보증금, 주거비, 생계비 등을 포함할 수 있다. 둘째, 채무조정은 연체가 발생한 이후부터 신용회복지원 약정 체결까지의 기간이 짧을수록 채무조정자의 생활에 도움이 될 수 있어 조기지원이 필요하다. 그리고 채무회수 측면에서도 가압류 이후에 지원이 이루어지면 회수율이 낮고 가압류 이전에 지원을 통한 정상화가 이루어지면 회수율이 높게 나타날 수 있어 조기지원이 보다 효과적일 것이다. 셋째, 공적 채무조정에 따른 채무변제기간은 대부분의 국가가 5년을 최장기간으로 운용하고 있으며 우리나라의 개인회생 변제기간도 최장 5년이다. 그러나 개인채무자가 5년안에 변제를 마무리하여 회생가능성을 높이고 선의의 금융소비자와 채권자를 보호하기 위해 채무변제기간을 채무자의 원리금 상환능력에 따라 탄력적으로 조정할 필요가 있다. 넷째, 해외 사적 구제제도는 대상채무와 신청자격의 제한이 없어 채무자의 접근성이 뛰어난 반면에 채권자의 동의를 얻지 못해 채무조정에 실패하는 비율이 높다. 우리나라는 채무자의 구제제도 이용에 따른 편의성 제고 및 법원의 심리적 부담경감 등을 기대할 수 있어 채무조정대상을 확대하여 사적 채무조정 활성화방안을 마련할 필요가 있다. Korea has continued to face economic depressions with global financial crisis, such as Asian economic crisis in 1997 and American sub-prime mortgages in 2008. The extreme depression appears increase in unemployment rate, reduction of individual income, and increase in bad credit grade. As a result, the number of individual bankruptcy has been increased as well. There suppose to be desirable relief system which helps rehabilitation of the individual debtors to revive by reducing varieties of social welfare costs and recycling available labour forces. However, an abusive execution of the relief system may genenrate moral hazard cases and lower the individual consciousness. Even though Korea’s relief system for the debtors has been improved over time, undiscerning introduction of an overseas relief system may appear as reckless implementation. Therefore, we should conduct in-depth research before trying to adopt the foreign relief systems. Furthermore, we should put efforts on readjusting and redeveloping the newly introduced system, thereby customizing to the pre-existed local domestic system environment.

        관절염 모델에서 가미청열사습탕(加味淸熱瀉濕湯)의 진통 및 소염 효과에 관한 연구

        Objectives This study was carried out to find the effects of Gami-cheongyulsaseub-tang (hereinafter referred to GCST) on the inhibition of zymosan-induced pain in rats and collagen II-induced arthritis (CIA) in DBA/1J mouse. Methods As an acute inflammatory pain model, peripheral inflammation was induced by intraplantar injection of zymosan into the right hind paw in rats and then the hyperalgesia and pain regulating factors in spinal cord were analyzed. As a chronic inflammation model, the mixture of collagen II and complete Freund``s adjuvant was treated into mice to establish rheumatoid arthritis and then body weight, thickness of hind paw, pathological change of spleen, immunological rheumatoid factor (IgG1, IgG2a, IgG2b, IgM and anti-collagen II), pro-inflammatory cytokines, and bone injury were analyzed. Results In the acute inflammatory pain model, GCST significantly inhibited the thermal and mechanical hyperalgesia and the pain regulating factors, including Fos, CD11b, PKA and PKC, in the spinal cord with a dose-dependent manner. In the chronic rheumatoid arthritis model, GCST administration decreased arthritic index and paw edema as compared with CIA control group. In particular, GCST reduced significantly the serum levels of total IgG2a, IgG2b, IgM, and specific anti-collagen II, but not total IgG1. GCST also resulted in the attenuation of bone injury and spleen enlargement/adhesion in CIA mice. Moreover, the secretion of pro-inflammatory cytokines TNF-α and IL-1β in CIA mice was significantly reduced by GCST in a dose-dependent manner. Conclusions Comparison of the results in this study showed that GCST had anti-nociceptive and immunomodulatory effects. These data imply that GCST can be used as an effective drug for not only rheumatoid arthritic pain but also other auto-immune diseases. (J Korean Med Rehab 2015;25(1):27-44)

        삼황사심탕(三黃瀉心湯)이 난황 알부민으로 유도된 알레르기 Mouse모델에서 항알레르기 효과

        Objectives In this study, we investigated the inhibitory effects of Samhwangsasim-tang (S.H) on the allergic response caused by ovalbumin(OVA) sensitization and challenge in BALB/c mice. Methods The experimental animals were divided into five groups; 1) normal as negative control, 2) OVA-sensitized mice, 3) OVA-sensitized mice treated with 200 mg/kg of S.H 200, 4) OVA-sensitized mice treated with 400 mg/kg of S.H 400, and 5) OVA-sensitized mice treated with 5 mg/kg of Dexamethasone (Dex). Antigen sensitization for allergic mouse model was performed with twice injection of OVA for 2 weeks. After secondary injection, S.H was administrated orally into mice every day for 13 days and the inhibitory effect of S.H on allergic responses was evaluated. Results Treatment of S.H into allergic mice reduced significantly ear edema and infiltration of immune cells in ear tissues induced with OVA challenge in a dose-dependent manner. S.H reduced significantly the serum levels of Total Immunoglobulin(Ig)G and IgE, and particularly inhibited the production of OVA-specific IgE, but not OVA-specific IgG. The serum level of proinflammatory cytokine TNF-α and Th2-associated cytokine IL-4 also were significantly decreased by S.H adminstration in a dose denpendent manner. S.H attenuated OVA-induced secretion of IFN-γ, but not IL-12 which is a cytokine inducing the development of Th1 cells. It also reduced significantly the secretion of IL-4, which is a cytokine inducing the development of Th2 cells, after splenocytes were stimulated with OVA. However the secretion of IL-5 and IL-13 was influenced weakly or a little. Conclusions These results indicate that S.H could reduce the allergic response through inhibition of antigen-specific IgE and Th2-inducing cytokines. It suggest that S.H may be available clinically for the treatment of allergic patients. (J Korean Med Rehab 2014;24(3): 71-85)

        Rarely, an arteriovenous fistula (AVF) can follow a femoral arterial puncture for cardiovascular catheterization. Surgical repair has been suggested as the standard treatment. We report a 66-year-old woman with a pulmonary thromboembolism as a complication of surgical repair of a chronic femoral AVF. We believe that the repair of the AVF led to a sudden decrease in venous blood flow, leading to thrombus formation at the site of damaged endothelium. Therefore, doctors should consider all possible complications that can arise from a sudden change in blood flow as a result of repairing a chronic AVF.
