        클로미펜에 부적절한 반응을 보이는 다낭성 난소 증후군 환자에서 Aromatase Inhibitor의 유용성

        Objective: To evaluate the effectiveness of aromatase inhibitor (AI) for ovulation induction in polycystic ovary syndrome (PCOS) patients with thin endometrium, hyper-responsiveness after clomiphene citrate (CC) treatment. Material and Methods: A prospective study was performed in 43 PCOS patients (50 cycles) with ovulatory dysfunction between March 2004 and September 2004. AI group (total 36 cycles) included the patients 1) with thin endometrium below 6 mm on hCG day after CC (n=17), 2) with more than 5 ovulatory follicles after 50mg of CC (n=4), 3) who do not want multiple pregnancy (n=14). Patients were treated with Letrozole 2.5mg for days 3 to 7 of the menstrual cycle. CC group (total 14 cycles) were treated with CC 50~100 mg. Results: In PCOS patients, ovulation was occurred 97.2% after AI use. Between AI group and CC group, there was no significant difference in the mean age, duration of infertility, interval of menstruation, basal FSH, prior treatment cycles, and the day of hCG administration. But, the number of mature follicles (${\geq}15mm$) was lower in the AI group ($1.08{\pm}0.45$ vs. $1.64{\pm}0.75$) (p=0.018), and the thickness of endometrium (mm) was significantly thicker in the AI group ($10.35{\pm}1.74$ vs. $9.23{\pm}1.61$) (p=0.044), and E2 (pg/ml) concentration on hCG day was lower in the AI group ($116.9{\pm}75.8$ vs. $479.5{\pm}300.8$) (p=0.001). Among the AI group, patients with prior thin endometrium (below 6 mm) during CC treatment showed $10.6{\pm}1.6mm$ in the endometrial thickness and $106.6{\pm}66.8pg/ml$ in $E_2$ concentration. Patients with more than 5 ovulatory follicles after CC showed decreased follicle number ($1.25{\pm}0.5$) compared to prior CC cycle. Conclusions: In PCOS patients, AI group showed significantly thicker endometrium, lesser number of mature follicles, and lower E2 concentration on hCG day than CC group. AI might be useful alternative treatment for ovulation induction in PCOS patients with thin endometrium and hyper-responsiveness after CC treatment.

        대학생의 지각된 스트레스, 스트레스 대처, 완벽주의, 자아존중감이 정신건강에 미치는 영향

        본 연구에서는 지각된 스트레스, 스트레스 대처, 완벽주의, 자아존중감이 대학생의 정신건강에 미치는 영향에 대해 살펴봄으로써 이들의 정신건강 증진을 위한 방법을 모색하고자 하였다. K도의 3개 대학에 재학 중인 209명의 남녀 대학생을 대상으로 설문조사를 실시한 후 SPSS 18.0을 이용하여 빈도, 피어슨 상관관계, 회귀분석을 실시하였다. 대학생의 정신건강은 지각된 스트레스, 과업중심 및 정서중심 스트레스 대처, 사회적으로 부과된 완벽주의, 자아존중감과 유의한 상관관계가 있었다. 본 연구결과를 토대로 한 대학생의 정신건강 증진을 위해 대학생의 사회성을 강화하고, 신체활동증진 및 자아존중감 증진을 위한 다학제적 교육프로그램 개발이 필요하다고 판단된다. This study was done to identify the new ways improving mental health of college students through search the influence on their mental health by their perfectionism, self esteem, perceived stress and its coping efforts. Participants consisted of 206 male and female college students who are attending in 3 universities in K province. Data was analyzed by frequencies, Pearson's correlation and multiple regression. Mental health were significant correlation with perceived stress, coping efforts on their tasks and emotional stress, perfectionism obtained from society, and self esteem. In order to improving mental health of college student, it is crucial to intensify sociality and develop multidisciplinary educational program on the basis of the results of this study.

        코로나19(COVID-19) 환경 속에서 K대학 신입생의 사회적지지, 회복탄력성 및 대학생활적응과의 관계

        본 연구의 목적은 K대학 신입생의 사회적지지, 회복탄력성 및 대학생활적응과의 관계를 확인하기 위한 실증적 조사연구이다. 연구대상은 K도 소재 K대학교 대학신입생 120명이고, 온라인 자가설문지를 이용하여 자료를 수집하였다. 자료수집기간은 2021년 4월 1일부터 2021년 5월 10일까지이다. 자료분석은 수집된 자료를 SPSS/WIN version 25.0 통계 프로그램을 이용하여 분석하였다. 대상자의 일반적 특성은 기술통계로 분석하였다. 그룹간의 차이는 독립 t-test, 일원배치 ANOVA로 분석하였다. 사회적지지는 7점 만점에 평균 5.35±0.99점, 회복탄력성은 5점 만점에 평균 3.35±0.34점, 대학생활적응은 9점 만점에 평균 4.93±0.63점 이었다. 대상자들의 대학생활적응과 회복탄력성(r=.380, p<.000), 사회적 지지와 회복탄력성(r=.372, p<.000)이 유의한 정적 상관관계로 나타났다. 결론적으로 이 연구결과를 통하여 코로나19(COVID-19)와 같은 위기 상황 속에서도 대학 신입생들의 대학생활적응과 관련된 사회적지지를 높일 수 있는 프로그램 개발을 제안하는 바이다. The purpose of this study was to a descriptive investigative study to identify the relationship between social support, resilience, and college life adaptation of freshmen at K University. The participants comprised 120 freshmen from K University in K-Do, and the data were collected using online self-questionnaires. Data collection was from April 1, 2021, to May 10, 2021. The general characteristics of the subject were analyzed by descriptive statistics. The data were analyzed by descriptive statistics, an independent t-test, a one-way ANOVA, and the Pearson's correlation coefficient through the SPSS 25.0 Program. The average social support was 5.35±0.99 out of a total of 7 points, the average resilience was 3.35±0.34 out of a total of 5 points, and the average college life adaptation was 4.93±0.63 out of a total of 7 points. College life adaption and resilience(r=.380, p<.000), social support and resilience(r=.372, p<.000) was shown to be a significant statistic correlation. In conclusion, through this research, we propose to develop programs that can increase social support related to college life adaptation of college freshmen even in crisis situations such as COVID-19.

        Experiences of Inpatients Living with Lung Cancer in South Korea

        Purpose: This study aimed to examine the processes of adaptation lung cancer patients undergo by examining their social interactions with medical staff, families, and other patients during hospitalization. Methods: Data were collected from February 2017 to January 2018 for a qualitative research study based on in-depth interviews with 10 participants who were inpatients at a cancer treatment hospital. Data were analyzed using the grounded theory of Corbin and Strauss. Results: Through comparative analysis, we identified the central phenomenon of the study was, ‘living in a body that is not related to one’s own will.’ The causal conditions were, ‘hit by a bolt out of the blue’ and ‘a disease that I do not want to inform others of.’ The contextual conditions were, ‘an encounter with reality’ and ‘breaking down the wall of fear.’ The intervention conditions included, ‘finding comfort in adversity,’ ‘a loaf of bread is better than the song of many birds,’ and ‘the communion of the same situation.’ Action/interaction strategies included ‘sharing information,’ ‘a word of encouragement,’ and ‘accepting the wave of change.’ Finally, the perceived consequences were ‘life redesign’ and ‘a life journey with limited time.’ Conclusion: This study describes the processes underlying how patients find meaning in their lives after a diagnosis of lung cancer. Based on their experiences, it is necessary to offer nursing interventions including admission rooms, equipment, and instruments to meet the unique needs of patients. The findings indicate that inpatient treatment for lung cancer needs more meaningful and effective management.

        중소병원 간호사의 전문직 정체성 갈등 경험 - Vol.7 No.2

        Purpose: This study was performed to explore the conflicts of job identity of nurses’ who were working at medium-sized hospitals. Method: The methodology of critical ethnography was used to identify the meanings and discourses of their experiences in the cultural context of the hospital organization. An field study was performed at a 200 bed private hospital from April 2002 to December 2005. The participants of the study were 13 nurses. The data was collected with the questionnaire, an in-depth interview, and participant-observation, and from documents of the hospital. For the analysis of the data, taxanomy developed by Spradley, text analysis of Werner and Schoepfle, and critical discourse analysis were applied. The reflective thinking, line by line and highlight analysis were repeated several times in the process of data analysis. Results: Participants had conflicts with their job identity from the automated work like a machine, that is, exclusion of self and humanity from their work. The second reason of job conflict was a non-systematized job description. The last one was unfairness in the reward system. There were two discourses under the job conflicts of nurses’; capitalism and hierarchical job structure. Conclusion: We should deserve more effort to get useful data for resolving the nurses’ job conflict. Studies on interpersonal conflicts are needed. In addition, studies of job conflicts in general hospitals which had a systematic and well-organized job description are needed.

        시뮬레이션 수업을 받은 간호대학생의 공감능력과 의사소통능력이 문제해결능력에 미치는 영향

        본 연구의 목적은 시뮬레이션 수업을 받은 간호대학생의 공감능력과 의사소통능력이 문제해결능력에 미치는 효과를 파악하기 위함이다. 대상자는 시뮬레이션 수업 경험자 3, 4학년으로 234명이다. 자료수집은 2021년 4월 26일부터 5월 10일까지이다. 자료 분석은 SPSS/WIN version 25.0 프로그램으로 기술통계, 차이 검정은 Independent t-test, One way ANOVA, Pearson’s correlation coefficient, Multiple regression으로 하였다. 연구결과 공감능력은 7점 만점에 5.29점, 의사소통능력은 5점 만점에 3.30점, 문제해결능력은 5점 만점에 3.51점이다. 공감능력과 의사소통능력은 문제해결능력에 양의 상관관계였다. 문제해결능력에 영향을 미치는 요인은 공감능력, 의사소통능력 및 교내실습기간 순이였다. 이에 대한 설명력은 64.6%였다. 결론적으로 시뮬레이션 수업을 받은 간호대학생의 문제해결능력을 향상시키기 위해서는 지속적인 시뮬레이션 수업을 통하여 공감능력 및 의사소통능력을 증진시키기 위한 전략이 필요하다. The purpose of this study was to investigate the effects of empathy and communication skills on nursing problem-solving skills of nursing students who received simulation classes. The subjects of this study were composed of 234 students in the 3rd and 4th grades who already took simulation classes. The data was collected from April 26th to May 10th, 2021. The data was analyzed with the SPSS WIN 25.0 program. Descriptive statistics and mean differences were analyzed using an independent t-test, one-way ANOVA, Pearson’s correlation coefficient, and multiple regression. As a result, the average points of empathy skills were 5.29 out of 7, communication skills were 3.30 out of 5, and problem-solving skills were 3.51 out of 5. Empathy skills and communication skills were positively correlated with problem-solving skills. The predicting factors on problem-solving skills were empathy and communication skills, on-campus practice period, and gender in order. The explanatory power of these factors was 64.6%. In conclusion, in order to improve the problem-solving ability of nursing students, a strategy is needed to enhance empathy and communication skills through continuous simulation classes.
