        2007년 대한감염학회 권장 성인 예방접종 권장안

        In 2007, the Korean Society of Infectious Diseases recommended an immunization schedule for adults residing in Korea. This recommendation includes four categories: standard immunization for healthy persons, supplementary immunization for persons with underlying illnesses, recommendations for health-care personnel and for international travelers. The Standard Immunization Schedule consists of universal vaccination (tetanus-diphtheria/tetanus-diphtheria-pertussis (Td/Tdap); hepatitis A vaccine for persons aged 20s; human papilloma virus vaccine for women under 26 years old; meningococcal vaccine for military recruits and college students in dormitories; influenza vaccination for persons aged 50 years or more and pregnant women; and pneumococcal vaccination for persons aged 65 years or more) and catch-up vaccination (hepatitis A vaccine for persons aged 30s; hepatitis B vaccine, varicella and MMR vaccines for persons who have uncertain specific immunities). The Supplementary Immunization Schedule recommends 4 vaccines, i.e., pneumococcal, influenza, meningococcal, and Haemophilus influenzae type b, appropriate to the underlying illnesses, and confirmation of the presence of antibodies to hepatitis A and hepatitis B. The Recommendation for Health-Care Personnel includes universal vaccination (influenza and pertussis) and catch-up vaccination (hepatitis B, measles-mumps-rubella and varicella for seronegative health-care personnel). The Recommended Immunization for International Travelers focuses reinforcement of vaccinations for healthy persons and includes certain immunizations necessary for international travel (yellow fever, meningococcal, plaque, tick-borne encephalitis, and cholera).         Pusan is the largest coastal city with a population of about four million in Korea. Because of increased and confused traffic, photochemical air pollution become a major urban environmental problem recently. The photo-chemical air pollution weather forecasting method preciser than existing air pollution forecast method has been developed to forecast ozone episode days with meteorological conditions using the data measured at 7 air quality continuous monitoring stations from June to September using 2 years (1994, 1995). The method developed in present study showed higher percentage correct and skill score than existing air pollution forecasting in KMA (Korea Meteorological Administration).

      • 거대 점오염원이 주변 대기질에 미치는 영향에 관한 연구

        In order to show the effect of a vast point pollutant source on air quality of Pusan Thermoeletric Power Plant and its surrounding area, air quality around Pusan Thermoeletric Power Plant was simulated by ISCLT-2 which was supplied by EPA. For this purpose the emission amount of SO_2, NO_2 and TSP was calculated and atmospheric stability was classified for a recent decade(1985~1994) in Pusan. A result of the emission amount showed that much amount of NO_2, NO_2 and TSP are emitted from industrial area. It was clear that NO_2 is much emitted from line source and industrial area. And as a result of classification of atmospheric stability, neutral, stable and unstable state were 58%, 24.1% and 17.9%, respectivly. The result of ai quality simulation by ISCLT-2 showed that Pusan Thermoeletric Power Plant is affecting on the increse of 2.0ppb, 3.0ppb and 5.0㎍/㎥, SO_2, NO_2, and TSP respectively at its surrounding area, site A-3 which was located westward 2.2㎞ distance from Plant

      • 노인의 기능저하예방을 위한 표준운동프로그램 개발 및 효과검증 연구

        본 연구의 목적은 일상생활 속에서도 누구나 쉽고 편하게 따라 할 수 있는 노인의 신체기능 저하예방 및 개선에 적합한 표준운동프로그램을 개발한 후, 그 효과를 검증하는 것이다. 연구대상은 서울시 마포구 화신경로당에 등록되어 있고, 본 프로그램에 참여한 총 36명(남성노인 10명, 여성노인 26명)의 노인으로 선정하였으며, 평균연령은 77세이다. 프로그램은 2011년 8월 19일부터 10월 18일까지 총 3개월 동안 주 2회씩 12주간, 총 18회를 실시하였으며, 안무가 1명과 보조연구원 1명이 프로그램을 진행하였다. 조사도구는 설문지로 SFT(Senior Fitness Test)를 사용하였으며, 노인 체력 테스트로 본 프로그램의 효과분석을 실시하였고 세부적으로는 상·하체 유연성, 상·하체 근력, 평형성, 민첩성 에 관한 테스트를 실시하였다. 사후검사는 프로그램 18회를 모두 진행하고 10월 18일 프로그램이 종료된 후, 사전검사와 동일한 방법으로 실시하였다. 자료처리는 SPSS Windows 15.0 version 을 이용하여 빈도분석, 대응표본 t-test를 실시하였으며, 그 결과는 다음과 같다. 첫째, 참여노인의 체력 요인에 대한 차이분석 결과, 평형성을 제외한 하체유연성, 상체유연성, 하체근력, 상체근력, 평형성, 민첩성 모든 요인에서 통계적으로 유의하게 증가하였다. 둘째, 참여노인의 신체구성비에 대한 차이 분석 결과, 체지방율과 BMI는 통계적으로 유의하게 감소하였고, 기초대사량과 근육량은 유의하게 증가하였다. 또한 체지방량은 감소하여 프로그램에 따른 긍정적인 효과가 검증되었다. The purpose of this study was to verify the effect of poor physical function of the elderly in everyday life that anyone can easily and comfortably suitable for the prevention and improvement to develop a standard exercise program after. Distribution of Hwa-Sin-dong, Mapo-gu, Seoul incarnation is registered in the senior center, was selected and a total of 36 patients (10 patients, elderly men and elderly women, n = 26) participated in this program for the elderly, the age range from 61 years to 91-year-old and the average age 77-year-old. Program for three months from August 19, 2011 10 18 in 12 weeks, a total of 18 times, twice a week was conducted by research assistant choreographer with 1 program. Research tool, a detailed questionnaire to the elderly as a fitness test this program was carried out to analyze the effects of SFT (Senior Fitness Test), the upper and lower body flexibility (right, left), upper and lower body strength, balance (right, left), agility test was conducted. Post-inspection program 18 times in the same way as the pre-test and after the program ends Oct. 18, and all proceeds were conducted. Data processing were analyzed using SPSS Windows 15.0 version to the corresponding sample t-test was conducted, and the results are as follows. First, the analysis for differences in participation in physical factors of the elderly, lower body flexibility, balance, except for lower body strength, upper body flexibility, upper body strength, balance, and agility in all factors statistically significantly increased. Second, the difference analysis results for the participation of the elderly in the Composition of the body, was a statistically significant reduction in the percentage of body fat and BMI, basal metabolic rate, muscle mass increased significantly increased. Also reduced body fat, according to the program has a positive effect was verified.

        Lidocaine, Thrombin, Epinephrine 의 항균효과

        목적 : 기관지내시경 검사에서 흔히 사용되는 국소마취제인 lidocaine과 내시경시 지혈목적으로 사용되는 thrombin과 epinephrine이 각종 균주에 미치는 항균효과에 대해 알아보고자 하였다. 재료 및 방법 : 균주는 가톨릭대학교 성모병원에서 2004년 3월부터 2004년 9월까지 임상검체 에서 동정된 S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa 각각 42, 42, 42, 43주를 대상으로 하였다. Lidocaine, thrombin, epinephrine 감수성 검사는 National Committee for Clinical Laboratory Standards (NCCLS, 2002)의 기준에 따랐다. 결과 : Lidocaine은 S. aureus, S. pneumoniae, P. aeruginosa에서 MIC_(50), MIC_(90) 모두 20,000 ㎍/mL 이었다. K. pneumoniae는 각각 10,000 ㎍/mL이었다. Thrombin은 S. aureus와 P. aeruginosa에서 MIC50 500 lU/mL 과 MIC_(90) 500 IU/mL 이상이었고, K. pneumoniae에서는 MIC_(50)과 MIC_(90)이 모두 500 lU/mL이상이었으나 S. pneumoniae에서는 MIC_(50)과 MIC_(90)은 125 IU/mL이었다. Epinephrine은 K. pneumoniae, S. pneumoniae에 대한 MIC_(50), MIC_(90)가 모두 >500 ㎍/mL이었고, S. aureus와 P. aeruginosa에 대한 MIC_(50), MIC_(90)가 모두 500 ㎍/mL이었다. 결론 : 기관지 내시경 검사에서 흔히 쓰이는 lidocaine, thrombin, epinephrine 등의 약제들이 호흡기 질환의 흔한 병원균인 S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa 균주들에 대해 항균 효과가 있을 수 있어 세균배양검사에 영향을 미칠 수 있겠다. Background : We performed this stody to find out about antimicrobial effect of lidocaine which is commonly used local anesthetic, and thrombin and epinephrine used for hemostasis during bronchoscopic procedures. Materials and Methods : The microorganisms that were cultured from specimens obtained during bronchoscopy were Staphylococcus aureus (n=42), Streptococcus pneumoniae (n=42), Klebsiella pneumoniae (n=42), and Pseudomonas aeruginosa (n=43) collected from St. Mary's Hospital, from March to Sep 2004 were used for susceptibity testing. Susceptibility to lidocaine, thrombin, and epinephrine were tested according to the National Committee for Clinical Laboratory Standards. Result : MIC_(50) and MIC_(90) of lidocaine for S. aureus, S. pneumoniae, P. aeruginosa were all 20,000 ㎍/mL and that for K. pneumoniae were 10,000 ㎍/mL. MIC_(50) and MIC_(90) of thrombin for both S. aureus and P. aeruginosa was 500 IU/mL and above 500 IU/mL, respectively; that for K. pneumoniae were all above 500 IU/mL and for S. pneumoniae they were 125 IU/mL, MIC_(50) and MIC_(90) of epinephrine for K. pneumoniae and S. pneumoniae were above 500 ㎍/mL; that for S. aureus and P. aeruginosa were 500 ㎍/mL. Conclusion : We observed possible antimicrobial effect of lidocaine, thrombin, and epinephrine in vitro against pathogens such as S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa, which are common respiratory microorganisms. The use of these agants could affect the result of bacterial culture.

      • IL-32γ attenuates airway fibrosis by modulating the integrin-FAK signaling pathway in fibroblasts

        <P><B>Background</B></P><P>Fibrosis in severe asthma often leads to irreversible organ dysfunction. However, the mechanism that regulates fibrosis remains poorly understood. Interleukin (IL)-32 plays a role in several chronic inflammatory diseases, including severe asthma. In this study, we investigated whether IL-32 is involved in fibrosis progression in the lungs.</P><P><B>Methods</B></P><P>Murine models of chronic airway inflammation induced by ovalbumin and <I>Aspergillus melleus</I> protease and bleomycin-induced pulmonary fibrosis were employed. We evaluated the degree of tissue fibrosis after treatment with recombinant IL-32γ (rIL-32γ). Expression of fibronectin and α-smooth muscle actin (α-SMA) was examined and the transforming growth factor (TGF)-β-related signaling pathways was evaluated in activated human lung fibroblasts (MRC-5 cells) treated with rIL-32γ.</P><P><B>Results</B></P><P>rIL-32γ significantly attenuated collagen deposition and α-SMA production in both mouse models. rIL-32γ inhibited the production of fibronectin and α-SMA in MRC-5 cells stimulated with TGF-β. Additionally, rIL-32γ suppressed activation of the integrin-FAK-paxillin signaling axis but had no effect on the Smad and non-Smad signaling pathways. rIL-32γ localized outside of MRC-5 cells and inhibited the interaction between integrins and the extracellular matrix without directly binding to intracellular FAK and paxillin.</P><P><B>Conclusions</B></P><P>These results demonstrate that IL-32γ has anti-fibrotic effects and is a novel target for preventing fibrosis.</P><P><B>Electronic supplementary material</B></P><P>The online version of this article (10.1186/s12931-018-0863-3) contains supplementary material, which is available to authorized users.</P>

        Clusterin Modulates Allergic Airway Inflammation by Attenuating CCL20-Mediated Dendritic Cell Recruitment

        <P>Recruitment and activation of dendritic cells (DCs) in the lungs are critical for Th2 responses in asthma, and CCL20 secreted from bronchial epithelial cells (BECs) is known to influence the recruitment of DCs. Because asthma is a disease that is closely associated with oxidative stress, we hypothesized that clusterin, an oxidative stress regulatory molecule, may have a role in the development of allergic airway inflammation. The aim of this study was to examine whether clusterin regulates CCL20 production from the BECs and the subsequent DC recruitment in the lungs. To verify the idea, clusterin knockout (Clu(-/-)), clusterin heterogeneous (Clu(+/-)), and wild-type mice were exposed intranasally to house dust mite (HDM) extract to induce allergic airway inflammation. We found that the total number of immune cells in bronchoalveolar lavage fluid and the lung was increased in Clu(-/-) and Clu(+/-) mice. Of these immune cells, inflammatory DCs (CD11b(+)CD11c(+)) and Ly6C(high) monocyte populations in the lung were significantly increased, which was accompanied by increased levels of various chemokines, including CCL20 in bronchoalveolar lavage fluid, and increased oxidative stress markers in the lung. Moreover, HDM-stimulated human BECs with either up- or downregulated clusterin expression showed that CCL20 secretion was negatively associated with clusterin expression. Interestingly, clusterin also reduced the level of intracellular reactive oxygen species, which is related to induction of CCL20 expression after HDM stimulation. Thus, the antioxidant property of clusterin is suggested to regulate the expression of CCL20 in BECs and the subsequent recruitment of inflammatory DCs in the airway.</P>
