Background/Aims: Left-sided diverticulitis is increasing in Japan, and many studies report that left-sided diverticulitis is more likely to be severe. Therefore, it is important to identify the features and risk factors for left-sided diverticulitis. We hypothesized that left-sided diverticulitis in Japan is related to obesity and conducted a study of the features and risk factors for this disorder in Japan. Methods: Right-sided di-verticulitis and left-sided diverticulitis patients (total of 215) were compared with respect to background, particularly obesity-related factors to identify risk factors for diverticulitis. Results: There were 166 (77.2%) right-sided diverticulitis patients and 49 (22.8%) left-sided diverticulitis patients. The proportions of obese patients (body mass index ≥25 kg/m2, p=0.0349), viscerally obese patients (visceral fat area ≥100 cm2, p=0.0019), patients of mean age (p=0.0003), and el-derly patients (age ≥65 years, p=0.0177) were significantly higher in the left-sided-diverticulitis group than in the right-sided-diverticulitis group. The proportion of viscerally obese patients was significantly higher in the left-sided-diverticulitis group than in the left-sided-diverticulosis group (p=0.0390). Conclusions: This study showed that obesity, particularly visceral obesity, was a risk factor for left-sided diverticulitis in Japan. (Gut Liver 2013;7:532-538)

Flammability of non-flame-retardant and flame-retardant materials was studied by using cone calorimeter Also, relations between the results obtained by using cone calorimeter and those obtained by the flammability test of japanese fire Service Law were examined. The results are as follows: 1) The ignition time of the molten specimens is relatively long, whereas the ignition time of the non-molten specimens is short. None of remarkable difference of the ignition time has been found between non-flame-retardant and flame-retardant materials specimens. 2) The peak heat release rates of flame-retardant materials are smaller than those of non-flame- retardant materials. 3) The carbon monoxide and smoke evolved from flame-retardant materials generate much more than those evolved from non-flame-retardant materials. 4) Even if flame-retardant materials are passed by the flammability test of Japanese Fire Service Law, they burn easily under external radiative heating condition.

Study Design: Retrospective radiological comparative design.Purpose: To investigate whether conventional magnetic resonance imaging (MRI) could substitute three-dimensional (3D)-MRI for the calculation of the foraminal stenotic ratio (FSR) and clarification of which patients can be assessed more accurately using 3D-MRI.Overview of Literature: Previous studies have indicated that 3D-MRI is useful for diagnosing lumbar foraminal stenosis. The FSR obtained using 3D-MRI, described as the ratio of stenosis length, characterized by perineural fat obliteration, to the length of the entire foramen, could indicate the stenosis severity; however, this method is time-consuming and expensive. The FSR also can be calculated using conventional MRI.Methods: We investigated 154 foramina at L5–S1 in 77 patients. All the patients had degenerative lumbar disorders and had undergone both conventional MRI and 3D-MRI during the same visit. Differences between the FSRs calculated from conventional and 3D-MRI reconstructions and any correlations with the plain radiography findings were assessed.Results: In foramina that had a FSR of <50% on conventional MRI, the difference between the FSR obtained using conventional MRI and 3D-MRI was 5.1%, with a correlation coefficient of 0.777. For foramina with a FSR ≥50% on conventional MRI, the difference was 20.2%, with a correlation coefficient of 0.54. FSR obtained using 3D-MRI was significantly greater in patients who required surgery than in those who were successfully treated with conservative methods (88% and 42%, respectively). Segments with spondylolisthesis or lateral wedging showed higher FSRs than those without these conditions on both types of MRI.Conclusions: FSRs <50% obtained using conventional MRI were sufficiently reliable; however, the results were inaccurate for FSRs ≥50%. Patients with high FSRs on 3D-MRI were more likely to require surgical treatment. Therefore, 3D-MRI is recommended in patients with suspected stenosis detected using conventional MRI or plain radiographs.

Purpose Since oncological outcomes of transanal colorectal tube (TCT) placement, an endoscopic treatment for colorectal cancer (CRC) with acute colorectal obstruction (ACO), remain unknown, this study analyzed long-term outcomes of TCT placement for stage II/III CRC with ACO. Materials and Methods Data were retrospectively reviewed from consecutive patients with distal stage II/III CRC who underwent surgery between January 2007 and December 2011 at two Japanese hospitals. One hospital conducted emergency surgery and the other performed TCT placement as the standard treatment for all CRCs with ACO. Propensity score (PS) matching was used to adjust baseline characteristics between two groups. Results Among 754 patients with distal stage II/III CRC, 680 did not have ACO (non-ACO group) and 74 had ACO (ACO group). The PS matching between both hospitals identified 234 pairs in the non-ACO group and 23 pairs in the ACO group. In the non-ACO group, the surgical quality was equivalent between the two institutions, with no significant differences in overall survival (OS) and disease-free survival (DFS). In the ACO group, the rate of primary resection/anastomosis was higher in the TCT group than in the surgery group (87.0% vs. 26.1%, p < 0.001). No significant differences were noted between the surgery and the TCT groups in OS (5-year OS, 61.9% vs. 51.5%; p=0.490) and DFS (5-year DFS, 45.9% vs. 38.3%; p=0.658). Conclusion TCT placement can achieve similar long-term outcomes to emergency surgery, with a high rate of primary resection/anastomosis for distal stage II/III colon cancer with ACO.

The objective of this study is to estimate the distribution of a zone disturbed by excavation (EDZ) around tunnels that have been excavated at about 500 m depth in pre-Tertiary hard sedimentary rock. One of the most important tasks is to evaluate changes in the dynamic stability and permeability of the rock around the tunnels, by investigating the properties of the rock after the excavation. We performed resistivity and acoustic tomography using two boreholes, 5 m in length, drilled horizontally from the wall of a tunnel in pre-Tertiary hard conglomerate. By these methods, we detected a low-resistivity and low-velocity zone 1 m in thickness around the wall of the tunnel. The resulting profiles were verified by permeability and evaporation tests performed at the same boreholes. This anomalous zone matched a high-permeability zone caused by open fractures. Next, we performed resistivity monitoring along annular survey lines in a tunnel excavated in pre-Tertiary hard shale by a tunnel-boring machine (TBM). We detected anomalous zones in 2D resistivity profiles surrounding the tunnel. A low-resistivity zone 1 m in thickness was detected around the tunnel when one year had passed after the excavation. However, two years later, the resistivity around the tunnel had increased in a portion, about 30 cm in thickness, of this zone. To investigate this change, we studied the relationship between groundwater flow from the surroundings and evaporation from the wall around the tunnel. These features were verified by the relationship between the resistivity and porosity of rocks obtained by laboratory tests on core samples. Furthermore, the profiles matched well with highly permeable zones detected by permeability and evaporation tests at a horizontal borehole drilled near the survey line. We conclude that the anomalous zones in these profiles indicate the EDZ around the tunnel.

Background/Aims The administration of liquid nutrients to patients is often accompanied by complications such as gastroesophageal reflux. To prevent gastroesophageal reflux, high-viscosity liquid meals are used widely, however, it still remains controversial whether high-viscosity liquid meals have any effect on the rate of gastric emptying. The present study was conducted with the aim of determining whether high-viscosity liquid meals had any effect on the rate of gastric emptying and mosapride might accelerate the rate of gastric emptying of high-viscosity liquid meals. Methods Six healthy male volunteers underwent 3 tests at intervals of > 1 week. After fasting for > 8 hours, each subject received one of three test meals (liquid meal only, high-viscosity liquid meal [liquid meal plus pectin] only, or high-viscosity liquid meal 30 minutes after intake of mosapride). A 13C-acetic acid breath test was performed, which monitored the rate of gastric emptying for 4 hours. Using the Oridion Research Software (β version), breath test parameters were calculated. The study parameters were examined for all the 3 test conditions and compared using the Freidman test. Results Gastric emptying was significantly delayed following intake of a high-viscosity liquid meal alone as compared with a liquid meal alone; however, intake of mosapride prior to a high-viscosity liquid meal was associated with a significantly accelerated rate of gastric emptying as compared with a high-viscosity liquid meal alone. Conclusions This study showed that high-viscosity liquid meals delayed gastric emptying: however, mosapride recovered the delayed rate of gastric emptying by high-viscosity liquid meals. The anxiety of tiny metal contamination into resin materials in the manufacturing process has become one of the serious issues. Because it may often cause of overheat in lithium-ion batteries, flat panel displays, and so on. Therefore, a study on detecting method of the tiny metals is a significant issue. The authors have been investigating the high-frequency induction heating (IH) based approach for tiny metals detection. In this paper, effective magnetic circuit design and IH conditions are discussed and demonstrated from theoretical and experimental point of view.

The gastrointestinal motility effects of endogenous incretin hormones enhanced by dipeptidyl peptidase-IV (DPP-IV) inhibitors have not yet been sufficiently investigated. The aim of this study was to determine whether single pre-prandial sitagliptin, the DPP-IV inhibitor, administration might have an effect on the rate of liquid gastric emptying using the 13C-acetic acid breath test. Methods Ten healthy male volunteers participated in this randomized, two-way crossover study. The subjects fasted for overnight and were randomly assigned to receive 50 mg sitagliptin 2 hours before ingestion of the liquid test meal (200 kcal per 200 mL, containing 100 mg 13C-acetate) or the test meal alone. Under both conditions, breath samples were collected for 150 minutes following the meal. Liquid gastric emptying was estimated by the values of the following parameters: the time required for 50% emptying of the labeled meal (T1/2), the analog to the scintigraphy lag time for 10% emptying of the labeled meal (Tlag), the gastric emptying coefficient and the regression-estimated constants (β and k), calculated by using the 13CO2 breath excretion curve using the conventional formulae. The parameters between the 2 test conditions were compared statistically. Results No significant differences in the calculated parameters, including T1/2, Tlag, gastric emptying coefficient or β and k, were observed between the 2 test conditions. Conclusions The present study revealed that single-dose sitagliptin intake had no significant influence on the rate of liquid gastric emptying in asymptomatic volunteers.
The Effect of Asynchronous Carrier on Matrix Converter Characteristics

In a matrix converter, input side and output side are coupled with each other through switching elements. Since disturbances on either side affect directly on the other side, it requires a high-speed on-line control system to compensate them.<br/> We proposed in previous papers a new control strategy and an on-line control circuit for a matrix converter. The control circuit could keep the output voltage at commanded value against fluctuation in the supply voltage. Furthermore wave forms of the output voltage and the input current were always kept sinusoidal. The switching pattern was generated by comparing modified voltage references with a carrier. The carrier was synchronized with the supply voltage using a PLL system, which made the control circuitcomplicatedand costly.<br/> This paper discusses on the possibility of an asynchronized carrier. Experiment results show the input current distortion in case of asynchronous carrier is bigger than that of synchronous earner However, the detenoration can be minimized by increased carrier frequency.
