        수술실 간호사의 직무 만족과 조직몰입에 관한 연구

        Purpose: This study was done to examine the relationship of job satisfaction and organizational commitment of perioperative nurses. Method: The subjects of this study were 500 perioperative nurses from 11 hospitals. The data were collected by self-reporting questionnaires from Sep. 19 to Sep. 27, 2009. Results: There was statistically significant relationship among the five variables. The analyses of covariance of these five variables revealed overall significant (p<.05). Stepwise linear multiple regression analyses were used to examine the influence of these five variables. Results indicated that the variables for verbal abuse (p<.01), workplace climate (p<.01), internal marketing (p<.001), and job transfer (p<.001) contributed significantly to the job satisfaction (adjusted R square=.426), while the verbal abuse (p<.01), internal marketing (p<.01), leadership style (p<.001) and workplace climate (p<.001) did to the organizational commitment (adjusted R square=.351). Canonical correlation analyses revealed that internal marketing and workplace climate contributed most significantly both to job satisfaction and organizational commitment. Conclusion: This study found that all these five nursing managerial factors were important influential on both job satisfaction and organizational commitment of perioperative nurses. Addressing these factors with further research will surely improve the commitment of these nurses and ultimately lead to better perioperative nursing care.

        수술실 간호사의 직무 만족도 측정 도구 개발 연구

        Purpose: The purpose of this study was to develop a valid and reliable instrument to explore and measure job satisfaction as perceived by perioperative nurses, specifically from the perspective of contemporary perioperative nursing in Korea. Method: Items (69) were developed from the responses to two open-ended questions by 168 perioperative nurses in one teaching hospital; (a) reasons that make work enjoyable, (b) reasons that make work unpleasant. Finally 55 items were selected, excluding items with lower correlation with the total scale. The instrument was completed by 885 perioperative nurses from 89 different hospitals nationwide who were attending the annual conference of the Korean Association Operating Room Nurses (KAORN), yielding a response rate of 95.6%. Results: Ten factors were extracted by factor analysis, which explained 60.0% of the total variance; Reliability and internal consistency were established (Cronbach's alpha=.95). A smaller sample of 39 perioperative nurses completed the instrument on two occasions, with a 2-week interval between. Mean scores were computed and two sets of scores were analyzed by Weighted Kappa (W-Kappa=0.680, p=.029). Conclusion: This instrument is a concise, highly practical and flexible tool, which can be used for the perioperative nurse in a wide range of hospital settings.

        수술실 간호사의 배치전환, 언어폭력, 조직몰입간의 관련성에 대한 융복합 연구

        본 연구는 수술실 간호사의 배치전환과 언어 폭력 및 조직몰입 간의 관련성을 파악하고 조직몰입에 미치는 영향요인을 융복합적으로 분석하기 위해 시도되었다. 연구대상자는 수술간호사회 학술대회에 참석하고 연구참여에 동의한 312명의 수술실 간호사이다. 연구 자료는 SPSS 21.0 프로그램을 이용하여 분석하였다. 연구 결과, 간호사의 조직몰입은 배치전환과는 약한 정도의 양의 상관관계가 있었으며, 언어폭력과는 약한 정도의 음의 상관관계를 나타내었다 . 이들 변수가 조직몰입을 설명하는 설명력의 정도는 57.7%로 나타났으나 배치전환과 언어폭력은 유의하지 않았고 직무만족이 영향요인으로 밝혀졌다. 따라서 수술실 간호사의 조직몰입을 향상시키기 위한 차별화된 교육프로그램이 개발되어야 할 것이며, 수술실 간호사의 조직몰입에 유의미한 변수를 추가한 연구가 필요하다. This study was conducted to examine the relationship among job rotation, verbal abuse and organizational commitment and the incfluencing factors of organizational commitment of perioperative nurses. 312 perioperative nurses attended at the KAORN conference agreed to participate in this research were examined. To analyze the data, SPSS 21.0 program was used. Study findings revealed a statistically significant relationships with positive relationship between job rotation and organizational commitment, but negative relationship between verbal abuse and organizational commitment. The most influencing factor was job satisfaction explaining 57.7%, while job rotation and verbal abuse did not show influence significantly. Some differentiated education programs are needed to develop to enhance the organizational commitment as well as conducting further study including meaningful variables.

        멸균품의 유효기간에 관한 연구 : 포장재와 보건환경 중심으로

        This study was conducted to justify the "prescribed shelf life" currently used in Samsung Medical Center and give recommendations based on scientific data to adopt for safe and cost-effective storage times for sterile goods. Since the early 1970's, predetermined expiration dating policy has been used in Korea on ail in-house sterilized products like any other countries; six months for the supplies wrapped in polyethylene peel pouches and two weeks for the rest. Recently, however, the studies suggest that current standards for preserving the sterility can be changing. It looks like we unnecessarily have too-short expiration date of sterilized supplies, that is far from being cost effective. Certainly, this research is an exciting project, especially in Korea, and will be leading and contributing to the paradigm shift of the conservative concept of shelf life for sterile supplies in Korean healthcare facilities. The major goal of this study was placed on the measurement of the recontamination time in packs wrapped in all kinds of wrapping materials used in S.M.C and stored in various conditions for periods ranging from three months to six months, and then develop a practical standard of "shelf life" based on the analyzed data. Objectives : To measure and compare the recontamination time of sterile packs among wrapppers and storage conditions, and develop a practical standard for the shelf life and storage conditions of sterile products. Samples and Method : The sterile integrity was investigated of the total 1.440 sterile sample packs containing gauze strip(1.5㎝×0.5㎝) we prepared and wrapped in nine wrapping materials (randomized reprocessed linens , linens reprocessed 99 times, linens reprocessed 120 times, new kraft paper, used kraft paper, new nonwoven fabrics, 6-time reprocessed nonwoven fabrics, new polyethylene peel pouches, used polyethylene peel pouches), and stored in four locations(closed cabinet of one of the typical wards, open shelf of one of ICUs, open shelf of separate storage room in ER, open shelf in OR), for three to six months, respectively in 1998. Each type of packs were opened weekly and the gauze strips inoculated into Brewer's thioglycollate broth at 35℃ for seven days. Resuslts : There was no growth found for any types of the pack and storage conditions studied. Discussion : This study-was conducted in an attempt to identify the possibility of extending the current shelf life for sterile goods we use in a sense of cost effectiveness. The findings suggest that we could extend our expiration dates, that means we can save a lot of materials and personnel time meeded for resterilizing, outdating, recleaning, rewrapping, and reshelving. Then it will initiate a great reform in the field- of healthcare in Korea, and now we are looking forward to this valuable revolution. We arc going to continue this study and take all steps need to apply this new.- concept to our practice, and then trying to extend to other hospitals.

        수술실 간호사가 경험하는 언어폭력과 배치전환이 환자 신체안전관리 활동에 미치는 영향

        본 연구는 수술실 간호사의 언어폭력과 배치전환에 대해 알아보고 이들 변수가 환자 신체안전관리 활동에 영향 을 미치는지 여부를 알아보기 위해 실시되었다. 연구 대상자는 수술간호사회 학술대회에 참석한 483명의 수술실 간호 사이다. 수집된 자료는 SPSS 21.0 프로그램을 이용하여 분석하였다. 연구결과, 수술실 간호사가 지각한 언어폭력의 정 도는 평균 2.92, 배치전환은 3.28, 환자신체안전관리 활동은 3,94였다. 언어폭력은 환자 신체안전관리 활동과 음의 상 관관계(r=-.11 p<.05)를 보여주었고 , 배치전환은 환자 신체안전관리 활동과 양의 상관관계(r=.12 p<.01)를 보여 주었다. 종속변수인 환자 신체안전관리 활동에 영향을 주는 요인은 배치전환(β=.172, p<.01)으로 환자 신체안전관리 활동의 7%를 설명하였다. 결론적으로 연구결과는 환자 신체안전관리활동을 개선시키기 위하여 언어폭력을 감소시키 고 배치전환을 전략적으로 활용할 필요성을 제시한다. Purpose :This study was conducted to examine verbal abuse and job rotation perceived by perioperative nurses and their influences on the patient physical safety management practice Methods :Self-reporting questionaires were distributed and collected from 483 perioperative nurses who attended annual conference of Korean Association of Operating Room Nurses. Data were analyzed using SPSS 21.0. program. Results :The mean score for verbal abuse was 2.92, 3.28 for job rotation, 3.94 for patient physical safety management practice. Verbal abuse had negatiive correlation with patient physical safety management practice(r=-.11 p<.05), while job rotation showed positive correlation with patient physical safety management practice (r=.12 p<.01). The influencing factor on the patient physical safety management practice was job rotation(β=.172, p<.01) explaining 7.1% of it. Conclusion: Study findings indicated that verbal abuse should be lowered ,while job rotation should be applied strategically to improve patient physical safety management practice.

        통원 수술환자의 안전한 귀가 결정을 위한 퇴실기준에 관한 연구

        Purpose: This study was conducted to compare three discharge criteria; 1) discharge criteria of S Hospital determined by nurses, 2) discharge readiness determined by patients, and 3) the Modified Post-Anesthetic Discharge Scoring System (MPADSS). The usefulness of MPADSS as a discharge criteria for the patients’safe return to home after ambulatory surgery was also evaluated. Methods: A total of 370 day surgery cases were investigated. The MPADSS was employed in every 30 min. in parallel with discharge readiness assessment by nurses and patients. The percentage of the patients who were categorized as being ready to discharge were compared according to three discharge criteria. Results: The percentage of patients scored to be as MPADSS > 9 in 30 min, 60 min, 90 min were 96.5%, 99.5%, 100% respectively. Whereas 11.1%, 44.3%, 71.1% of patients rated themselves as being ready to discharge and 2.7%, 23.5%, 54.3% of patients actually discharged by nurses according to discharge criteria of S Hospital. Conclusion: Nurses tend to keep patients longer in the hospital when compared to the patient's own assessment about their readiness to home and to that of MPADSS. Faster discharge in the evening than day time suggests patient discharge can be influenced by nursing factors. This brings out the importance of scoring system to determine the safe discharge. The MPADSS could be a useful tool in evaluating patients for safe discharge.
