Actinomycosis is a chronic, progressive, suppurative disease which appears to be difficult to establish a correct preoperative diagnosis. Most of pelvic actinomycosis in women are related to a long time intrauterine device(IUD) inserted state. The causative agent is a branching, gram-positive anaerobic or microaerobic organism, not fungi. Ascending infection of the upper genital tract by actinomycosis may be clinically inapparent. We experienced a case of clinically inapparent tuboovarian actinomycosis in a 47-year-o]d women with IUD, discovered pathologically after laparotomy.

This thesis studed to evaluate the fatigue behavior and propagation of rail steel under welding line. Test of crack growth was performed by all member of rail under constant amplitude loading at the structures laboratory in Hongik University. The effect of the following parameters with initiated crack length on the bottom edge of rail were studied. Here, fracture mechanics mode is opening mode. and Testing Material is KS50N Rail. From analysis and experimented result on the three point bending in the lab, This paper presented a effect of crack growth, shape and remaining service life. Further more, according to the variable crack length, variable section and the ratio of section the fatigue behavior and propagation were studied.
자궁내막증이 있는 불임환자에서 복강액내의 Cytokine에 관한 연구

Objective: The presence of the various cytokines in human peritoneal fluid has been evaluated incompletely. Changes in cytokine levels may be related to activation of peritoneal macrophage and T-lymphocyte, development of endometriosis, and infertility. This study assesses peritoneal fluid levels of interleukin-6 (IL-6), interleukin-8 (IL-8), interleukin-10 (IL-10) and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) in infertile women with endometriosis and normal women without endometriosis. Design: Prospective and case-control study in university hospital. Materials and Methods: Cytokine levels in peritoneal fluid obtained during laparotomy or laparoscopy from 21 patients in infertile patients with endometriosis and 24 controls undergoing laparotomy or laparoscopy with no evidence of pelvic endometriosis were determined by enzyme-linked immunosorbent assay. Results: The mean levels of interleukin-6 in infertile patients with endometriosis and controls were $72.7{\pm}23.7$ pg/ml and $18.5{\pm}9.7$ pg/ml respectively (p=0.02). Similarly, the mean levels of interleukin-8 in infertile patients with endometriosis was significantly higher than that of controls ($445.0{\pm}89.6$, vs $45.1{\pm}48.4$, p=0.04). The mean concentration of interleukin-10 in infertile patients with endometriosis was significantly lower than that of controls ($1.09{\pm}0.04$ vs $2.19{\pm}0.03$, p=0.03). The level of tumor necrosis factor-${\alpha}$ was not significantly different between the two study groups. Conclusions: Increased IL-6 and IL-8 and decreased IL-10 levels in the peritoneal fluid may be related to pathogenesis in the endometriosis and infertility, suggesting that partially contribute to the disturbed immune regulation observed in infertili women with endometriosis.
자궁경부 상피내종양 및 자궁경부암에서 Cyclooxygenase - 2 의 발현

목적: 역학적 연구들에서 NSAID의 복용과 대장암의 발생 위험은 역비례 하는 것이 밝혀졌고, 위암, 대장암 등 여러 악성종양에서 cyclooxygenase-2(COX-2)의 과발현이 보고되어, NSAID의 주된 작용 부위인 cyclooxygenase-2가 종양발생에 중요한 역할을 할 것이라는 보고가 잇따르고 있다. 이에 저자들은 정상 자궁경부 상피에서 침윤성 자궁경부암까지의 각 단계에서 COX-2의 발현 빈도를 조사하고, COX-2가 자궁경부암의 암화과정에 어느 정도 관여하는지를 알아보고자 한다. 연구방법: 1999년 2월 1일부터 1999년 7월 31일 사이 한림대학교 성심병원 산부인과에서 자궁경부 생검, 원추절제 또는 외과적으로 절제된 자궁의 경부 조직 중에서 정상(n=6)으로부터 CIN I(n=4), CIN II(n=2), CIN III(n=15), 미세침윤암(n=5), 침윤암(n=7)까지 39예를 대상으로 면역조직화학적 염색을 시행하였다. 결과: 정상 자궁경부 조직의 16.7%, CIN I, II의 0%, CIN III의 80.0%, 미세침윤암의 60.0%, 그리고 침윤암의 71.4%에서 COX-2의 과발현을 관찰하였다. 정상 자궁경부, CIN I 등의 저등급 병변에 비하여 진행된 병변인 CIN III, 침윤성암 등에서 COX-2가 강하게 발현되는 것을 관찰하였다(p$lt;0.01). 결론: 이상의 결과로 COX-2가 자궁경부상피내종양의 암화과정에서 중요한 역할을 수행할 것으로 생각되며, 향후 자궁경부상피내암과 자궁경부암의 예방 및 치료에서 COX-2가 중요한 연구 대상이 될 것으로 사료된다. Objective: A large body of evidence suggests that cyclooxygenase-2(COX-2) is important in tumorigenesis of gastrointestinal cancer. The purpose of this study was to determine whether COX-2 was also expressed in cervical intraepithelial neoplasia(CIN) and invasive cervical carcinoma. Methods: We studied the expression of COX-2 in normal cervix(n=6), CIN I(n=4), CIN II(n=2), CIN III(n=15), microinvasive carcinoma(n=5) and invasive carcinoma(n=7) by immunohistochemistry. Results: We observed strong COX-2 expression in 16.7% of the normal cervix, 0% of the CIN I and II, 80.0% of CIN III, 60.0% of microinvasive carcinoma and 71.4% of invasive carcinoma. The intensity of immunostaining was stronger in the CIN III and invasive cancer than the normal cervix and CIN I lesions(p$lt;0.01). Conclusion: These results suggest that COX-2 may have a role in carcinogenesis of CIN and may be a target for the prevention or treatment of CIN and invasive cervical cancer.
