Modified Fontan 씨 수술법을 이용한 삼첨판막 폐쇄증과 완전대혈관전위증

A 18-year-old female underwent surgical correction of tricuspid atresia and complete transposition of great arteries combined with atrial 8 ventricular septal defect and pulmonary stenosis. After the transection of main pulmonary artery just above the pulmonic valve, proximal portion of main pulmonary artery was closed with running suture and distal portion of main pulmonary artery anastomosed with right atrial appendage without valve insertion. Atrial septal defect was closed with running suture. Postoperative course was uneventful and she discharged on 18th postoperative day. Her condition is in very good until present. Modified Fontan`s operation without valve placement [in the condition of low pulmonary vascular resistance and good left ventricular function] may has a good result.

Tracheal stenosis is due to tracheostomy or prolonged intubation. Development of tracheal stenosis following tracheostomy is very serious complication. In recent practice, tracheostomy has became more popular because of increased occasions of major and minor traumas. At the Dept. of thoracic surgery, Chonbuk national university hospital, we have experienced one case of tracheal stenosis following tracheostomy for assisted ventilation. Chest X-ray revealed the narrowing of trachea at cervicothoracic junction due to previous tracheostomy. We resected the narrow segment & tracheal reconstruction was performed with an excellent result in postoperative periods. Postoperative air tracheogram did not reveal demonstrable narrowing of air filled trachea.

식도암의 외과적 치료

Between March 1978 and December 1985, 39 patients were admitted to our hospital & surgery was performed to 27 patients. Among 39 patients, male patients were 31 cases, female 8 cases and the age ranged from 42 years old to 69 yrs old with the average of 55 years old. Main symptoms of esophageal cancer were dysphagia [6%], weight loss [20%], retrosternal and epicardial discomfort [18%], hoarseness [13%], and hiccup [5%]. The anatomical locations of esophageal cancer were followed as; 51% in lower esophagus & cardia, 44% in middle, and 5% in upper esophagus. Among 27 cases, 5 cases were managed by feeding gastrostomy and jejunostomy due to inoperability, 19 cases by esophagogastrostomy, and 3 cases by colon bypass with the resection of esophageal cancer. Postoperative complications were noticed in 7 cases, such as anastomotic leakage in 2 cases [7%], respiratory insufficiency in 2 cases [7%], intussusception in 1 case [4%], wound dehiscence in 1 case [4%], and hepatitis in 1 case [4%]. Among 2 respiratory insufficiency, 2 patients die as a result of that complication and operative mortality was 7%.

중국고대 私屬層의 신분제적 속성

The problem that si-shu私屬(including yi邑 and the people) and si-shu 私屬’s status standing as servant in the feudal relation between master and servant how the situation lies and transition is investigated. The subjects of a feudal ruler in gong-yi公邑 and si-yi私邑 separately bear the expenses only to their lords, and as lords intention serve in the war. Si-yi私邑 have self-regulation and independence, acknowledged by the lords, but have subordination character too in some exceptional condition must be served in the war by the lords order. Each si-shu-tu私屬徒 obey and loyal to one’s directly belonging lords. From early chun-qiu春秋 periods power feudal princes nations combine nearby territory, but at the same time struggle for power between strong families get more intensified. The victor obtain hold the power of state and through parcelling the land of gong-shi公室 in lots and occupy, so gong-shi公室 become to si-shu私屬. The fu-shui-yi賦稅役 systems in each si-yi私邑 were operated by different form. However zhan-guo戰國 period draws near, the lords weaken power-familes through reform, constructed absolute monarchy. Hence the tide that rule all the people and state as si-shu私屬 of monarch unfold. This development are realized through the emergence and progress of xian縣 system. The wise and competent man were appointed as chief of xian縣 rather than from noble family do that. Be due to xian縣 system now si-yi私邑 become under the direct control of monarch and by that si-yi私邑 or si-shu私屬 become as it is an administrative district and region name. In addition the people become qi-min齊民 or gong-min公民, and fu-shui-yi賦稅役 systems become equalized and simplified. Thereupon a term ‘si-shu私屬’ now come into use legislative term that indicate released status from slaves under certain restrictions, ascended one grade status. Therefore it is can be said that in Chinese history new social status come into existence. They seem to be free oneself from slaves but not wholly. They are exempted from military service and labor duty, also in respect that the slaves owner pay suan-qien算錢 taxes they are equal to slaves. In place of the slaves owner are exempted from double pay suan-qien算錢 taxes, lose the right of by generation ownership and trade. In aspect of state the problem of growing slaves and other several problems derived from that can be cancelled. It could be said that the substance of status rise is become getting ownership and accumulate of individual property. So that also it could be said that si-shu私屬 getting shu-ren庶人 status become to have the obligation to pay suan-qien算錢 taxes. In addition, recently there are the tendency that ‘huan-huang-di宦皇帝’ is elucidated as si-shu私屬 or near attendant of huang-di皇帝, but in this thesis pointed out that such interpretation are insufficient, the greater part of that were ‘shumin-zai-guan-zhe庶民在官者’ and xu-tu胥徒 in zhou-li周禮 that are in public service by zhi-yi職役 service.

근래 道信禪師 禪法 연구에 대한 반론(2)

  In understanding the history of early Zen-sect, the issue of Zen Dharma of Dao-xin道信 and his genealogy are very important. Recently, there often has been a tendency to conclude the character of Zen Dharma of Dao-xin and deny historical evidence easily, by reasoning from a mere assumption and distortion. This tendency is due to not having the profound understanding on the Zen Dharma in early Zen-sect, and it must be considered with caution. The present writer pointed it out in the part I and II section of this thesis. The Zen Dharma of Dao-xin succeeded to Lanka-Zen of early Zen-sect as the way it is, since the great monk Dharma. I made it clear that the Lanka-Zen should not be distinguished from the Prajna-Zen, and such classified terms of Zen Dharma are not suitable for actual Zen Dharma.<BR>  The practice of vipa?yan? to mind(看心) and the practice of transcendence of vipa?yan?(絶觀) ? the practice of non-watch(不觀) ? the practice of non-thinking(不思) ? the practice of non-practice(不行) are not heterogeneity in Zen Dharma. This Zen Dharma can not be divided into gradually realizing Dharma and directly realizing Dharma, but directly realizing Dharma has this Zen Dharma as a chain of practice system.<BR>  The profound meaning of Zen Dharma can be understood better through directly realizing Dharma advocated by Zen-master Mahayana摩訶衍禪師 in "Disputation in Tibet (792-794)". The issue of the relations between teacher and pupil of Dao-xin and Niu-dou Fa-long牛頭法隆 also can not be determined by simple view point from documents criticism. And it is necessary to give consideration to the peculiar way of Zen-Sect.<BR>  I"m sure that the result of this thesis will correct the former fixed ideas on the Zen Dharma of Southern and Northern Zen-Sect.

『능가경』의 禪法과 초기 禪宗(3) - 如來禪ㆍ祖師禪의 문제

  "Tath?gata zen" which is stated in Lanka s?tra is not conditioned things in the casual state because that is Tath?gata"s protection-pra ctice for Bodhisattva. "Patriarch zen" which is called in later ages is not something particular with "Tath?gata (purity) zen" that is said by Shen-hui神會 and Ma-zu馬祖ㆍZong-mi宗密. By the way the special designation of "Patriarch zen" have some natural reason and historical background. That is, the first : they presented the type of awakening by conversation and to ask of matters directly, the second : the practice of transcendence of vipa?yan?, the third : the Dharma succession through direct transmission from mind to mind, the four : the zen of sudden enlightenment and sudden practice, non-forced practice, the five : misunderstanding on the [progressing into Tath?gata zen] for [Tath?gata zen]. Generally speaking the "Patriarch zen" is fundamental to Lanka zen, and include sanction of accomplishment on the assumption of embodiment in Dharma, and that is Patriarch"s personal transmission.
