      • Algorithmic Approach for Safe Optimization and Surgical Planning in Hilar Blocks- Single Center Experience

        Aims: To study the outcomes of our algorithmic-approach for safe optimisation and surgical planning in patients with Hilar- Block. Methods: Retrospective-analysis of prospectively-maintained database of patients undergoing surgery for hilar-block from Jan2013-May2019 was done.Our approach includes Imaging, Biliary-decompression, Future-liver-remnant-Volume(FLR)augmentation based on CT-Volumetry and FLR-function assessment. Results: 45cases of hilar-blocks underwent resections.32were Hilar-cholangiocarcinoma,5-Intrahepatic-cholangiocarcinoma, 6-Ca-Gall-Bladder with hilar-block,2-IgG4-sclerosing- cholangitis-presenting as malignant-masquerade. The mean age was57±12years and 30(67%) were males. On MRCP, hilar-blocks types 2,3a,3b,4 were 3,15,17,10 respectively. Pre-operative biliary-decompression of FLR were done in 21cases[19 PTBD(Percutaneous-Transhepatic-biliary-drainage)/ 2 EBD(Endoscopic-biliary-drainage)]. Additional PTBD were done in 2 cases for inadequate fall in SB, and 3 for cholangitis. The mean SB(Serum-Total-Bilirubin) at presentation was 9.57±5.58mg/dl. The rate of fall of S.bilirubin was faster in patients < 50yrs of age and type-3 hilar-blocks than in type-4 hilar- blocks. PVE was performed in 14cases and FLR hypertrophy of 11.3± 3.03%was achieved.The quality of FLR was assessed with LAI(n=39),fibroscan(n=17), ICGR15(n=12), HVPG (n=35), and selective-remnant-biopsy(n=14,if HVPG >10 mm Hg,ICGR15> 15%, or in-suspected steatosis or fibrosis).After optimization, surgical procedures done were Right-Hepatectomy(7), Right-TriSectorectomy(7), Extended-Right-Hepatectomy(9), Left-Hepatectomy(6),Extended-Left-Hepatectomy(5), Left-Trisectorectomy( 8) and Bile-duct-excision-alone(3).Eleven patients required concomitant vascular-resections and reconstructions(- 8portal-vein-resections, 2 hepatic-arterial-resection,1both) to obtain R0. R0 and R1 resections were achieved in 42(93%) and 3 patients. Clavien-Dindo-Grade3/4 complications were 22.2%(n=10). 8(18%)patients had Post-Hepatectomy-Liver- Failure.Overall operative-mortality was 5/45(11.1%). Conclusions: Our algorithmic approach for safe optimization by preoperative-biliary-drainage, FLR-augmentation and FLR-functional- assessment have led to a high rate of R0 major liver resection and good outcomes in patients with hilar-blocks.Augmentation of FLR can also increase resectability in borderline resectable cases.

        Return Migrants and International Transfer of Technology : A Case Study of Azad, Jammu and Kashmir

        The study aims exploring contributions of return migrants, particularly, their role in international transfer of technology in Azad,Jammu and Kashmir(AJK). Besides, reasons for migration and impact on employment generation, as well as, characteristics of migrant(s) are identified who are most likely to promote the transfer of technology. For this purpose, a field survey was conduted in Azad, Jammu and Kashmir, an area with a high rate of international migration. Econometric models were estimated to draw empirical support for determining of the issues. The empirical evidences indicate that education level, demographic profile, career planning before migration, vocational training, duration of stay abroad and specific type of employer are some of the important determinants of migrants who are likely to transfer technology. The return migrants who spend on luxuries are less likely to contribute in this respect. Such migrants possess specific set of characteristics(s). As per our knowledge, this source of international transfer of technology has not been identified so far. Besides, different technologies like improvement in infoware, humanware, technoware and organisational skills etc, which contribute to improve human capital are identified. The study also provides information on re-employment pattern of return migrants. It further highlights impacts of return migrants on the promotion of local businesses and employment generation. Based upon the evidences, the study provides policy guideline to fully benefit from migrants. A labour policy to provide information about the types of feasible businesses, incentives to bring new technology and facilitation of setting up business could enhance local development. The study contributes by identifying an important new source of international transfer of technology(s) through migrants. The policy direction are provided to explored this new channel to transfer new technologies and, therefore, accelerate economic development in developing areas. The study would also be equally beneficial for other areas, which experience return migrants.

      • Pose Estimation of 5-DOF Manipulator using On-Body Markers

        Manipulators are helpful in performing various hazardous tasks like sanitization with chemicals in germs infected areas, spraying pesticides in fields, pick and place of heavy and hazardous materials where direct human intervention is difficult. For manipulators to perform its assigned task accurately, prior estimation of its pose needs to be pinpointed. End-effector grasping and arm manipulation require estimation of 3D object poses. Recently, a number of procedures and databases for vision-based estimation of object pose have been advised. However, it is not clear about the performance of the developed algorithms for visual pose estimation of robot manipulation. In this paper we present the pose estimation of a 5-dof PhantomX Reactor Arm using On-Body/Aruco Markers. Forward and inverse kinematics were used to estimate the pose from the position coordinates calculated using computer vision techniques. This paper implements an approach aimed at estimating the pose of a camera, affixed to a robotic manipulator, against a target object. We adopt a single camera single-shot technique that minimizes the reprojection error over all the rigid poses. The simulation and experimental results using industrial monocular camera for different sizes of On-body markers were presented.

        Genetic Studies on Production Efficiency Traits in Hariana Cattle

        The data on 512 Hariana cows, progeny of 20 sires calved during period from 1974 to 1993 maintained at Government Livestock Farm, Hisar were considered for the estimation of genetic parameters. The means for first lactation milk yield (FLY), wet average (WA), first lactation peak yield (FPY), first lactation milk yield per day of first calving interval (MCI) and first lactation milk yield per day of age at second calving (MSC) were 1,141.58 kg, 4.19 kg/day, 6.24 kg/day, 2.38 kg/day and 0.601 kg/day, respectively. The effect of period of calving was significant (p<0.05) on WA, FPY and MCI while the effect of season of calving was significant only on WA. Monsoon calvers excelled in performance for all the production efficiency traits. The effect of age at first calving (linear) was significant on all the traits except on MCI. Estimates of heritabilty for all the traits were moderate and ranged from 0.255 to 0.333 except for WA (0.161). All the genetic and phenotypic correlations among different production efficiency traits were high and positive. It may be inferred that selection on the basis of peak yield will be more effective as the trait is expressed early in life and had reasonably moderate estimate of heritability.

        Impact of Nonlordotic Sagittal Alignment on Short-term Outcomes of Cervical Disc Replacement

        Objective: To evaluate outcomes of cervical disc replacement (CDR) in patients with nonlordotic alignment. Methods: Patients who underwent CDR were retrospectively reviewed and divided into 3 cohorts: (1) neutral/lordotic segmental and C2–7 Cobb angle (L), (2) nonlordotic segmental Cobb angle, lordotic C2–7 Cobb angle (NL-S), and (3) nonlordotic segmental and C2–7 Cobb angle (NL-SC). Radiographic and patient-reported outcomes (PROMs) were compared. Results: One-hundred five patients were included (L: 37, NL-S: 30, NL-SC: 38). A significant gain in segmental lordosis was seen in all cohorts at < 6 months (L: -1.90° [p = 0.007]; NL-S: -5.16° [p < 0.0001]; NL-SC: -6.00° [p < 0.0001]) and ≥ 6 months (L: -2.07° [p = 0.031; NL-S: -6.04° [p < 0.0001]; NL-SC: -6.74° [p < 0.0001]), with greater lordosis generated in preoperatively nonlordotic cohorts (p < 0.0001). C2–7 lordosis improved in the preoperatively nonlordotic cohort (NL-SC: 8.04°) at follow-up of < 6 months (-4.15°, p = 0.003) and ≥ 6 months (-6.40°, p = 0.003), but not enough to create lordotic alignment (< 6 months: 3.89°; ≥ 6 months: 4.06°). All cohorts showed improvement in Neck Disability Index, visual analogue scale (VAS) neck, and VAS arm, without significant difference among groups in the amount of improvement ( ≥ 6-month PROMs follow-up = 69%). Conclusion: In patients without major kyphotic deformity, CDR has the potential to generate and maintain lordosis and improve PROMs in the short-term, and can be an effective treatment option for patients with nonlordotic alignment.

        Microwave Dielectric Characterization of Binary Mixtures of 3-Nitrotoluene with Dimethylacetamide, Dimethylformamide and Dimethylsulphoxide

        Dielectric relaxation measurements on 3-nitrotoluene (3-NT) mixture of dimethylacetamide (DMA), dimethylformamide (DMF) and dimethysulphoxide (DMSO) have been carried out across the entire concentration range using Time domain reflectometry technique at 15, 25, 35 and $45^{\circ}C$ over the frequency range from 10 MHz to 20 GHz. For all the mixtures, only one dielectric loss peak was observed in this frequency range and the relaxation in these mixtures can be well described by a single relaxation time using Debye model. Bilinear calibration method is used to obtain complex permittivity ${\varepsilon}^{*}({\omega})$ from complex reflection coefficient ${\rho}^{*}({\omega})$ over frequency range 10 MHz to 20 GHz. The excess permittivity, excess inverse relaxation time, Kirkwood correlation factor, molar energy of activation are also calculated for these mixtures to study the solute-solvent interaction.
