선인장 양심실 보조장치의 설계 및 임상적용을 위한 평가

현재 개발되고 있는 맥동형 이식형 인공심장은 완전이식형 인공심장과 좌심실보조장치 뿐이다. 좌심실보조장치를 장착한 환자의 10~15%가 우심실의 보조를 필요로하고 완전이식형 인공심장의 장착을 위해서는 자연심장을 제거해야하는 상황에서 이식형 양심실보조장치에 대한 필요성이 대두되고 있다. 본 연구진은 완전이식형 인공심장으로 개발된 한국형 인공심장을 개선하여 양심 실 보조장치를 개발하였다. 양심실보조장치는 이동형 작동기식 미케니즘을 이용하고 있으며 에너지 변환장치와 감속기로 구성된 작동기와 혈액주머니, 그리고 내장형 제어기로 구성되어 있다. 선인장 펌프로 명명된 KAH350은 캐뉼라의 연결을 위한 커넥터를 포함하여 길이 177mm, 폭 164mm, 높이 67 이고 무게 780g이며 최대 심박출량은 5L/min 이다. 양심실보조장치로 개발된 선인장펌프는 좌심실보조장치로도 응용이 가능하다. 이식적합성과 생체적합성을 평가하기 위하여 좌심실보조장치로 5회, 양싱실보조장치로 6회의 동물실험을 수행하였다. 각각 최장 28일간 생존하였으며 11회의 동물실험중 장치의 결함에 의한 것이 3회 있었는데 이들은 모두 전자장치의 결함과 방수처리문제였다. 현재 이식적합성과 일박출량을 향상시킨 KAH400모델이 개발중이다. The types of pulsatile implantable artificial hearts that are on the way of development and have been developed are totally implantable artificial heart and implantable left ventricular assist device (LVAD). Approximately 10% to 15% of all patients Implanted with wearable VADs have required right heart support with another device. And it is reluctant to patient who should remove his or her own heart to be implanted with total artificial heart. These situations drive the development of implantable bi-ventricular assist device (BVAD). The Korean BVAD was develophed by modifying the moving actuator type Korean artificial heart. This electro-mechanical BVAD comprises actuator including energy converter and reduction gear train, blood sacs, and internal motor and energy controllers. The KAH350 which is named as 'Cactus Pump' is 177mm in length, 164mm in width, and 67mm in thickness including connectors and nuts. The weight and maximum cardiac output of Cactus Pump is 780g and 5L/min, respectively. The Cactus Pump that was developed as BVAD could be used as LVAD by attaching compliance caps on the 2 ports of one ventricle. The animal experiments were undergone 5 times for LVAD, and 6 times for BVAD. The best records were 28 days survival in both applications. There were 3 times of device failure and they were all associated with electrical connection and hermetic sealing. The KAH400 that has improved anatomical fitting characterisitcs and stroke volume is on development.

In this paper, the dynamic stability of a hydraulic excavator using ZMP concept is considered. When a load is moved in an excavator based on automation, an excavator often loses the stability and falls over. This is because a dynamic element is not included in the moment equilibrium equation that is used in order to judge a reversal. Consequently, reversal distinction algorithm including all a static and a dynamic element along a load movement in crane work is necessary. Zero Moment Point(ZMP) is a point on the floor where the resultant moment of the gravity, the inertial force of the manipulator and the external force is zero. This study is going to interpret the reversal stability of the excavator to which is applied ZMP concept through simulation.
Control Schemes of Quantum-Based Pico-Newton Force Measurement

<P>We propose control-measurement schemes for flux-quantum-based pico-newton force metrology. A micron-sized superconducting ring placed in magnetic field gradients can serve as a pico-newton force gauge. Constant-force steps can be realized by controlling the flux quanta in the superconductor ring. Two different schemes are proposed to control and measure the number of flux quanta in the ring. They are the fixed bias-flux mode and the fixed bias-current mode. In the fixed bias-flux (bias-current) mode, external bias-flux (transport bias-current) is fixed just below the resistive transition and entry of the flux quanta is monitored by counting the voltage bumps with increasing the bias current (the external flux). At the targeted number of flux quanta, bias current and flux are reset to zero. The number of flux quanta trapped in the ring can be confirmed by counting the voltage bumps with increasing the bias current up to the maximum critical current of the ring. Simultaneous flux control and force measurement is made by measuring force steps in field gradient with counting the number of flux quanta exciting the ring with increasing the bias current.</P>
Magnetocardiographic performance of single-layer second-order YBCO SQUID gradiometers

We have studied noise properties and magnetocardiographic performance of single-layer second-order YBCO SQUID gradiometers made on STO substrates. A three-loop pickup coil is directly coupled to a step-edge junction SQUID with the coupling polarity of the center loop opposite to that of two side loops. Optimized device parameters for balancing were obtained from 8.8 mm × 3.0 mm devices on 10 mm × 10 mm substrates and scaled up to the larger devices for use in cardiomagnetic measurements. A 17.6 mm × 6 mm gradiometer with a 5.8 mm baseline showed an unshielded gradient noise of 0.84 pT/cm<SUP>2</SUP>/Hz<SUP>1</SUP>2/ at 1 Hz, which corresponds to an equivalent field noise of 280 fT/Hz<SUP>1</SUP>2/. A large balancing factor of 10<SUP>3</SUP> allowed us to record magnetocardiogram of a human subject.
Critical currents of $YBa_2Cu_3O_7$ step-edge Josephson junctions on $SrTiO_3$ (100) substrates

We have studied critical currents of $YBa_2Cu_3O_7$ step-edge junctions with different step orientations with respect to the major axes of $SrTiO_3$ (100) substrates. The junctions were prepared by pulsed laser deposition and argon ion milling with photoresist mask. We investigated current-voltage characteristics and critical current of the junctions as a function of the angle. The junction critical current showed an angle dependent modulation with maxima near 0 or 90 degree and minima near 45 and 135 degrees. The experimental results were analyzed based on the microstructure of the junction along the step and the d-wave symmetry of $YBa_2Cu_3O_7$ superconductor.

Effect of cobalt and rhodium promoter on NOx storage and reduction (NSR) kinetics was investigated over Pt/BaO/Al2O3. Kinetics of 2% cobalt loading over Pt/BaO/Al2O3 demonstrated highest NOx uptake during lean cycle, while reduction efficiency during rich cycle appeared most poor. In contrast to this, rhodium showed suppressing effect of NOx uptake during lean cycle and demonstrated an enhanced effect for the higher efficiency of NOx reduction during rich cycle. Thoracoscopy in Management of Chest Trauma: Our Three-year Jeju Experience

Purpose: The role for minimally invasive surgery in chest trauma is vague, one that recently is more frequently performed, and one attractive option to be considered. Thoracoscopic surgery may improve morbidity, mortality, hasten recovery and shorten hospital stay. Methods: A total of 31 patients underwent video assisted thoracoscopic surgery for the treatment of blunt and penetrating chest trauma from June 9th, 2013 to March 21st, 2016 in Jeju, South Korea. Results: Twenty-three patients were males and eight patients were females. Their ages ranged from 23 to 81 years. The cause of injury was due to traffic accident in 17 patients, fall down in 5 patients, bicycle accident in 2 patients, battery in 2 patients, crushing injury in 2 patients, and slip down, kicked by horse, and stab wound in one patient each. Video assisted thoracoscopic exploration was performed in the 18 patients with flail chest or greater than 3 displaced ribs. The thoracoscopic procedures done were hematoma evacuation in 13 patients, partial rib fragment excision in 9 patients, lung suture in 5 patients, bleeding control (ligation or electrocautery) in 3 patients with massive hemothorax, diaphragmatic repair in two patients, wedge resection in two patients and decortication in 1 patient. There was only one patient with conversion to open thoracotomy. Conclusion: There is a broad range of procedures that can be done by thoracoscopic surgery and a painful thoracotomy incision can be avoided. Thoracoscopic surgery can be done safely and swiftly in the trauma patient. [ J Trauma Inj 2017; 30: 33-40 ]

<P>We have studied electrical transport characteristics of individual double-wall carbon nanotubes (DWNT) under high bias voltages. As the bias voltage applied to the carbon nanotubes increases, the outermost shell of the DWNTs broke down sequentially, which enabled us to determine the current-carrying capacity of each shell. The maximum current-carrying capacity per shell was about 150 µA, which is well above that of any other previous reports. </P>
