Single Use (Disposable) Duodenoscope: Recent Development and Future

Unlike simple forward-viewing endoscopes such as gastroscope or colonoscope, duodenoscope houses much more complex designto fulfil its function. This design differences leave duodenoscopes more prone to contamination from inadequate disinfection processand potential dissemination of pathogens. Recent reports on dissemination of infection through the duodenoscope mandated anoverhaul of duodenoscope utilization including development of a disposable duodenoscope. This article reviews the current state ofdisposable duodenoscope development, including reported early efficacy as well as its future direction and utilization.
도시 커먼즈(Urban Commons) 실현의 한계 및 가능성

이 연구는 최근 사회문제 대응의 정책적 사업들이 커먼즈(Commons)적 관점에서 진행되고 있는가를 분석하고, 그 한계를 통해 도시커먼즈((Urban Commons)로의 확대를 위한 시사점을 제시하고자 한다. 분석결과 공동자원 커먼즈화 방식에서는 공공이 주체가 되어 추진되었다는 특성과 커먼즈의 개념차원의 접근보다는 젠트리피케이션 대응차원의 정책적 접근에 따라 이루어졌으며, 이는 기존의 양분된 소유구조 인식이 강함이 원인으로 분석되었다. 또한 도시커먼즈 구성요소에서는 주로 공공소유 자원을 시작으로 이루어지고 있고, 제도나 공동체의 경우 시민출자가 매개되어 구성하는 것으로 분석되었다. 이에 따라 다음과 같은 시사점을 도츨하였다. 첫째, 향후 사회전반에 적용 및 확대 가능성의 확인이다. 둘째, 그럼에도 불구하고 여전히 도시커먼즈를 전면적으로 추진하기 위한 사회전반의 인식 부족과 공동자원 제공의 한계가 있다. 확대 단계까지 공공의 공동자원 제공이나 지원이 요구된다. 셋째, 도시커먼즈의 구성요소 중 제도와 공동체는 공동자원을 활용하는 지역주민에 의한 출자 등의 형태에서 보다 적극적으로 실현됨을 확인할 수 있다. This study analyzes whether policy projects for responding to urban problems are currently being conducted from a commons perspective, and attempts to present implications for expansion to urban commons through their limitations. As a result of the analysis, in the common resource commons method, the public was the subject and was promoted according to the policy approach in response to gentrification rather than the conceptual approach of the commons. This was analyzed as the cause of the strong recognition of the existing divided ownership structure. In addition, it was analyzed that the urban commons component mainly consists of publicly owned resources, and in the case of an institution or community, citizen investment is mediated to constitute an institution and a community. Accordingly, the following implications were derived. First, it is a possibility that it can be applied and expanded to the whole society in the future. Second, in order to directly attempt urban commons in the whole society, the provision or support of common resources in the public should be given priority until awareness is expanded across society due to the limitation of providing common resources. Third, among the constituent elements of the city commons, institutions and communities are more actively using forms such as local residents" investment, which directly utilizes common resources.

Acoustic thermography uses the physical property that changes the speed of sound by a medium temperature. The speed of sound is measured by geometric information of measurement space, a position of speakers & microphones and sound propagation delay. The important point is that the number of speakers and microphones are physically limited. Thus, every time delay in a path must be measured precisely. The well-known measurement method of time delay is General Cross Correlation(GCC) method that is completely dependent on the sampling frequency. In this study, we suggest Time Delay Spectrometry(TDS) for measuring time delay.

<P><B>Purpose:</B></P><P>In multiple myeloma, extramedullary progression is associated with treatment resistance and a high mortality rate. To understand the molecular mechanisms controlling the devastating progression of myeloma, we applied single-cell RNA-sequencing (RNA-seq) to myeloma in the bone marrow and myelomatous pleural effusions or ascites.</P><P><B>Experimental Design:</B></P><P>Bone marrow or extramedullary myeloma samples were collected from 15 patients and subjected to single-cell RNA-seq. The single-cell transcriptome data of malignant plasma cells and the surrounding immune microenvironment were analyzed.</P><P><B>Results:</B></P><P>Comparisons of single-cell transcriptomes revealed the systematic activation of proliferation, antigen presentation, proteasomes, glycolysis, and oxidative phosphorylation pathways in extramedullary myeloma cells. The myeloma cells expressed multiple combinations of growth factors and receptors, suggesting autonomous and pleiotropic growth potential at the single-cell level. Comparisons of the tumor microenvironment revealed the presence of cytotoxic T lymphocytes and natural killer (NK) cells in both the bone marrow and extramedullary ascites, demonstrating a gene-expression phenotype indicative of functional compromise. In parallel, isolated myeloma cells persistently expressed class I MHC molecules and upregulated inhibitory molecules for cytotoxic T and NK cells.</P><P><B>Conclusions:</B></P><P>These data suggest that myeloma cells are equipped with specialized immune evasion mechanisms in cytotoxic microenvironments. Taken together, single-cell transcriptome analysis revealed transcriptional programs associated with aggressive myeloma progression that support autonomous cell proliferation and immune evasion.</P>

육지부 지열의 지중열교환기 타입은 수직밀폐형, 지중수평형, 에너지파일형, 스탠딩컬럼웰형 네가지 방식으로 구분되어지며, 각 타입별 성능평가 방식은 한국에너지공단 신 · 재생에너지 설비의 지원 등에 관한 지침 중 별표1의 신 · 재생에너지 설비 원별 시공기준(제7조제1항 관련)을 적용하여 KOLAS 인증기관으로부터 별표3의 ‘1. 지중열전도도 측정기준’[1]에 따라 측정한 열전도도 값을 지열이용검토서 작성기준에 맞게 적용하여 설계한다. 그러나 제주도의 지열 지중열교환기 타입은 평가 방법이 없는 상황이다. 이에 본 연구에서는 제주형 지중열교환기 성능 평가 방법을 제안하고자 한다.
