        Feasibility of the Lilium α-200 portable ultrasound bladder scanner for accurate bladder volume measurement

        Purpose: The aim of this study was to investigate whether data obtained from the Lilium α-200 (Lilium Otsuka Co., Ltd., Japan) correlated with conventional frequency-volume chart (FVC) and post-void residual urine volume (PVR) obtained by urethral catheterization. Materials and Methods: This was a prospective multicentre study. Patients hospitalized for the treatment of lower urinary tract symptoms were included. Patients were evaluated with conventional FVC and Lilium α-200 for 2 days. PVR was measured by urethral catherization after urination at the end of the 2 day evaluation period. Results: A total of 42 patients were enrolled in this study. Voided volume and PVR measured by Lilium α-200 were significantly correlated with voided volume obtained from conventional FVC and PVR measured by urethral catheterization, respectively. There was considerable measurement error in voided volume measured by Lilium α-200 (−21.0±102.0 mL). In contrast, the error between PVR measured by the Lilium α-200 and PVR obtained by urethral catheterization was 2.4±52.0 mL. Additionally, high body mass index, but not sex, benign prostate hyperplasia, time zone of measurement (daytime vs. nighttime), and examiners (a urologist versus other healthcare providers) were significantly associated with inaccurate results in voided volume. Conclusions: Voided volume and PVR measured by the Lilium α-200 were correlated with voided volume obtained from conventional FVC and PVR measured by urethral catheterization, although accuracy of the measurements was not high. The Lilium α-200 is a useful device to easily measure approximate bladder volume.

      • Readability Comparison of Pro- and Anti-Cancer Screening Online Messages in Japan

        Background: Cancer screening rates are lower in Japan than those in western countries. Health professionals publish procancer screening messages on the internet to encourage audiences to undergo cancer screening. However, the information provided is often difficult to read for lay persons. Further, anti-cancer screening activists warn against cancer screening with messages on the Internet. We aimed to assess and compare the readability of pro- and anti-cancer screening online messages in Japan using a measure of readability. Methods: We conducted web searches at the beginning of September 2016 using two major Japanese search engines (Google.jp and Yahoo!.jp). The included websites were classified as "anti", "pro", or "neutral" depending on the claims, and "health professional" or "non-health professional" depending on the writers. Readability was determined using a validated measure of Japanese readability. Statistical analysis was conducted using two-way ANOVA. Results: In the total 159 websites analyzed, anti-cancer screening online messages were generally easier to read than pro-cancer screening online messages, Messages written by health professionals were more difficult to read than those written by non-health professionals. Claim ${\times}$ writer interaction was not significant. Conclusion: When health professionals prepare pro-cancer screening materials for publication online, we recommend they check for readability using readability assessment tools and improve text for easy comprehension when necessary.

        Coalescence behavior of dispersed domains in binary immiscible fluid mixtures having bimodal size distributions under steady shear flow

        Coalescence process of binary immiscible fluid mixtures having bimodal size distributions, prepared by mixing two pre-sheared samples at different shear rates, pre1 and pre2, under shear flow at a final shear rate, f, are examined by transient shear stress measurements and microscopic observations in comparison with the results for simply pre-sheared samples having narrow size distributions (unimodal distribution samples). Component fluids are a silicone oil (PDMS) and a hydrocarbon-formaldehyde resin (Genelite) and their viscosities are 14.1 and 21.0pasec at room temperature (ca. 20oC), respectively. The weight ratio of PDMS: Genelite was 7:3. Three cases, (pre1=7.2sec1, pre2=12.0sec1 and f=2.4sec1), (pre1=0.8sec1, pre2 =4.0sec1 and f=2.4sec1), and (pre1=7.2sec1, pre2=12.0sec1 and f=7.2sec1) are examined. In the first case, transient shear stress did not show any significant difference but domains larger than the initial state are observed at short times. In the latter cases, there exist undershoot of shear stress, reflecting existence of deformed large domains, which is confirmed by the direct observation. It is concluded that coalescence between large and small domains more frequently occur than coalescence between the domains with similar size in the bimodal distribution samples.

      • No Association between the CDX2 G543C Polymorphism and Risk of Gastric Atrophy and Cancer

        Ectopic expression of CDX2 in the stomach is closely associated with chronic Helicobacter pylori (H. pylori) infection and intestinal metaplasia. Whether CDX2 has tumor suppression or tumorigenesis potential remains to be elucidated. In this study, we investigated the association between the CDX2 G543C polymorphism (silent mutation) and the risk for H. pylori-induced gastric atrophy and cancer as well as H. pylori infection, using 454 Japanese subjects undergoing a health checkup and 202 gastric cancer patients. The frequency of the minor allele was the same as previously reported in China, but different from that reported in England. CDX2 G543C was not associated with risk of H. pylori infection, gastric atrophy, or gastric cancer, although the point estimate for non-cardiac differentiated gastric cancer as compared to controls with gastric atrophy was 2.22 (95%CI=0.17-29.4). In conclusion, our results indicate that the CDX2 G543C polymorphism is unlikely to affect the H. pylori infection-gastric atrophy-gastric cancer sequence.

        Patient self-reported symptoms using visual analog scales are useful to estimate endoscopic activity in ulcerative colitis

        Background/Aims: In clinical practice, colonoscopy has been regarded as the gold standard for the evaluation of disease severity as well as mucosal healing in ulcerative colitis (UC). Some activity indices incorporating patient symptoms as parameters have been shown to reflect the endoscopic activity of UC. The aim of this study was to examine whether self-reported symptoms with visual analog scales (VAS) can predict endoscopic activity. Methods: A cross-sectional study of 150 UC patients who underwent colonoscopy with submission of VAS scores of 4 symptoms: general condition, bloody stools, stool form, and abdominal pain (0: no symptoms, 10: the most severe symptoms). Each VAS score was compared with colonoscopic activity assessed with the Mayo endoscopic subscore (MES). Results: All VAS scores were significantly correlated with the endoscopic severity (Spearman correlation coefficients of general condition, bloody stools, stool form, and abdominal pain: 0.63, 0.64, 0.58, and 0.43, respectively). Mucosal healing defined as MES 0 alone was predicted by VAS score <1.5 on general condition or 0 on bloody stools with sensitivity of 0.84 and 0.76 and specificity of 0.66 and 0.76, respectively. Additionally, VAS score <2.5 on stool form predicted active lesions in distal colorectum alone with sensitivity of 0.67 and specificity of 0.66, suggesting that this item could predict the indication of topical therapy. Conclusions: Self-reported VAS scores on symptoms were correlated with endoscopic activity of UC. To clarify the relationship between VAS and mucosal healing, further validation studies are needed.         Preoperative Estimation of Future Remnant Liver Function Following Portal Vein Embolization Using Relative Enhancement on Gadoxetic Acid Disodium-Enhanced Magnetic Resonance Imaging

        To retrospectively evaluate relative enhancement (RE) in the hepatobiliary phase of gadoxetic acid disodium-enhanced magnetic resonance (MR) imaging as a preoperative estimation of future remnant liver (FRL) function in a patients who underwent portal vein embolization (PVE). In 53 patients, the correlation between the indocyanine green clearance (ICG-K) and RE imaging was analyzed before hepatectomy (first analysis). Twenty-three of the 53 patients underwent PVE followed by a repeat RE imaging and ICG test before an extended hepatectomy and their results were further analyzed (second analysis). Whole liver function and FRL function were calculated on the MR imaging as follows: RE x total liver volume (RE Index) and FRL-RE x FRL volume (Rem RE Index), respectively. Regarding clinical outcome, posthepatectomy liver failure (PHLF) was evaluated in patients undergoing PVE. Indocyanine green clearance correlated with the RE Index (r = 0.365, p = 0.007), and ICG-K of FRL (ICG-Krem) strongly correlated with the Rem RE Index (r = 0.738, p < 0.001) in the first analysis. Both the ICG-Krem and the Rem RE Index were significantly correlated after PVE (r = 0.508, p = 0.013) at the second analysis. The rate of improvement of the Rem RE Index from before PVE to after PVE was significantly higher than that of ICG-Krem (p = 0.014). Patients with PHLF had a significantly lower Rem RE Index than patients without PHLF (p = 0.023). Relative enhancement imaging can be used to estimate FRL function after PVE.

        Foxf2 represses bone formation via Wnt2b/β-catenin signaling

        Differentiation of mesenchymal stem cells (MSCs) into osteoblasts is a critical process for proper skeletal development and acquisition/maintenance of bone mass. However, since this regulatory mechanism has not yet been fully elucidated, the treatment of severe osteoporosis and fractures is a challenge. Here, through a comprehensive analysis of gene expression during the differentiation of MSCs into osteoblasts, we show that the forkhead transcription factor Foxf2 is a crucial regulator of this process. Foxf2 expression transiently increased during MSC osteoblastic differentiation. Overexpression of Foxf2 in MSCs inhibited osteoblastic differentiation, and conversely, knockdown of Foxf2 expression promoted this process. Osteoprogenitor-specific Foxf2 knockout mice developed a high bone mass phenotype due to increased bone formation. RNA-seq analysis and molecular experiments revealed that Foxf2 regulation of bone formation is mediated by Wnt2b. Knockdown of Foxf2 in mouse femurs enhanced bone regeneration in vivo. FOXF2 expression was correlated with hip bone mineral density in postmenopausal women with low bone mass. Finally, inhibition of FOXF2 promoted osteoblastic differentiation of human MSCs. This study uncovers a critical role of Foxf2 in the differentiation of MSCs into osteoblasts and provides insight into the pathogenesis associated with bone-related diseases such as osteoporosis and nonunion after fracture
