        터널형 2급와동 충전재의 탄성계수와 전단결합강도 및 수복치의 변연융선 파절강도에 관한 연구

        An alternative design to conventional class Ⅱ cavity preparation for proximal carious lesions is the tunnel preparation. It preserves the marginal ridge intact, thus making it possible to maintain the natural contact relationship with the adjacent tooth and minimize tooth reduction. This in vitro study was purposed to evaluate the effect of the materials' elastic constants and shear-bond strength on the marginal ridge fracture resistance of teeth restored by the tunnel technique, and to find the materials of choice for tunnel restorations. ??, ??, ??, and Tytin were used as restorative material. The elastic constants of each restorative material were evaluated by ultrasonic pulse measurement. Young's modulus and bulk modulus of the restorative materials were evaluated in three speimens for each material type. The shear-bond strength of the restorative materials to the dentin surface was measured after thermocycling 400 times between 6 and 60℃, using ten specimens for each material type. For measuring marginal ridge strength, 60 sound extracted molar teeth were distributed into six groups by size. Sound molar teeth were used as a Control group and unfilled prepared teeth were grouped as Unrestored. Another four groups were named Resinomer group, Ketac-Silver group, Miracle Mix group, and Tytin group by type of restorative material. Tunnel cavity preparation was done with # ½, 2, and 4 round burs in sequence. Initial access to proximal surface was made through an occlusal access preparation started at least 2㎜ from the marginal ridge, and the proximal opening was formed about 2.5㎜ below the marginal ridge. After restoration and thermocycling, marginal ridge strength was measured using a universal testing machine. The results were as follows : 1. The Young's modulus of ?? was 63.95 GPa, followed by ?? 27.60 GPa, ?? 18.48 GPa, and ?? 10.74 GPa showing significant differences between the groups(P<0.05). The bulk modulus of the materials showed the same order as Young's modulus. The value of ?? showed 59.57 GPa indicating that it will deform less than other materials under the same stress. It was followed by ?? 23.57 GPa, Miracle ?? 12.50 GPa, and ?? 11.60 GPa. 2. The Resinomer group had a shear-bond strength of 7.41 MPa which was significantly higher than those of the Ketac-Silver group (1.80 MPa) and the Miracle Mix group (2.84 MPa)(P<0.01). All the specimens of Tytin group detatched from the dentin surface during thermocycling. 3. The mean marginal ridge strength of the Unrestored group(46.14 kgf) was significantly lower than that of the Control group (84.24 kgf)(P<0.01). The marginal ridge strength of teeth restored by the tunnel technique was, in order, Ketac-Silver group 74.06 kgf, Miracle Mix group 73.36 kgf, Resinomer group 63.47 kgf, and Tytin group 58.76 kgf. The Ketac-Silver, Miracle Mix, and Resinomer groups showed no significant difference with the Control group(P>0.05), but the Tytin group showed significantly lower strength compared to the Control group(P<0.05). The results showed that the marginal ridge strength of the teeth restored by the tunnel technique was not significantly lower than that of sound teeth. They also demonstrated that the bonding strength of the restorative material to the tooth surface should be high and the modulus of elasticity should not be lower than that of the tooth in order to restore the marginal ridge strength to its natural condition.

      • GO-31 : Simultaneous Borderline mucinous tumor, Mature cystic teratoma and Endometriosis in the Same Ovary

        The synchronous presentation of borderline mucinous tumor, mature cystic teratoma, and endometriosis in the same ovary has never been reported in the literature. Ovarian endometriosis coexisting with a dermoid cyst is extraordinarily rare, although these both conditions are common in reproductive women. A 42-year-old woman presented with progressive dysmenorrhea and abdominal fullness. Pelvic magnetic resonance imaging revealed a 21×9.0×19.0 cm sized multiseptated cystic mass with internal fat lobules and calcification in the whole pelvis. Laboratory examination showed an elevated serum CA-125 of 110.6 U/ml and CA19-9 of 186 U/ml. On section, the tumor showed multilocular cystic mass, filled with mucinous fluid, sebum and hairs. The authors present a case of the simultaneous borderline mucinous tumor associated with mature cystic teratoma and endometriosis in same ovary in a reproductive woman. Microscopically, three tumor types were revealed: borderline mucinous tumor, mature cystic teratoma, and endometriosis. All ovaries with mature cystic teratoma should be thoroughly sectioned because additional histogenetically distinct lesions occurring in a mature cystic teratoma could easily be missed through inadequate sectioning.

        Amniotic fluid embolism(AFE) is rare but fatal complication of pregnancy that can be characterized into immediate circulatory collapse, acute respiratory distress, disseminated intravascular coagulation defect. The diagnosis is made by identifying clinical symptoms. The mortality rates of AFE have been estimated at between 37 and 80%. We experienced a survival case of 32-years-old woman who presented with sudden onset of hypotension, massive uterine bleeding after vaginal birth. So we report it with a review of the literature.

      • Autotaxin-lysophosphatidic acid-LPA receptor 1 signaling can be a prominent target for development of therapy of epithelial ovarian cancer

        목적: Tumor microenvironment reportedly plays critical roles in drug resistance and proliferation of CSCs; however, the functional role of CSC-secreted factors has not been clarified. 방법: We isolated cancer stem cells (CSCs) through sphere culture of A2780 epithelial ovarian cancer (EOC) cells and primary human EOC cells, and identified lysophosphatidic acid (LPA) as a CSC-derived autocrine factor using mass spectrometry. 결과: LPA treatment stimulated CSC-like properties, including drug resistance and high tumorigenic potential. CSCs were enriched in aldehyde dehydrogenase- high subpopulation, in which autotaxin, an LPA-producing enzyme, was highly expressed. LPA signaling was mediated through LPA receptor 1-dependent activation of the PI3K-AKT pathway. Abrogation of autotaxin-LPA-LPA receptor 1-AKT signaling axis significantly suppressed CSC characteristics, which provides a potential target for CSC treatment. EOC shows high mortality due to development of resistance to chemotherapy and relapse. 결론: CSCs have been implicated in drug resistance and relapse of ovarian cancer. Our findings suggested that ATX-LPA-LPAR1-AKT1 signaling axis is critical for maintenance of cancer stem-like characteristics in an ovarian CSC subpopulation through an autocrine loop. Inhibiting ATX-LPALPAR1- AKT1 signaling axis by chemical inhibitor or knockdown of gene expression increased the sensitivity of CSCs to chemotherapeutic reagents. Our findings provide therapeutic opportunities for development of relapse-free treatment of EOC.

        Perception of Child Abuse and Child Disciplinary Practice among Adults Abused as Children

        Objectives: The aim of this study was to compare differences in perception and knowledge of child abuse and child disciplinary practices according to the history of child abuse victimization. Methods: A questionnaire survey on child abuse was conducted with 491 adults raising children. We compared the perception and knowledge of child abuse and child disciplinary practices between two groups of adults with and without a history of childhood abuse victimization. Results: The group with a history of childhood abuse had lower levels of knowledge of child abuse (F=6.990, p<0.01) and engaged in more negative disciplinary practices (F=5.974, p<0.05) than those without. However, no differences in the perception of child abuse were observed between the two groups. Conclusion: The results suggest that adults with a history of childhood abuse have lower levels of knowledge of child abuse and use more negative disciplinary practices in raising their children. This highlights the need to administer not only educational but also more direct hands-on interventions to vulnerable parents in order to foster healthy parenting and disciplinary practices.

        Pregnancy-related osteoporosis and spinal fractures

        Pregnancy-related osteoporosis is a very rare condition characterized by the occurrence of fracture during pregnancy or the puerperium. Despite its relative rarity, it can be a dangerous condition that causes severe back pain, height loss and disability. Normal physiologic changes during pregnancy, genetic or racial difference, obstetrical history and obstetrical disease, such as preterm labor or pregnancy-induced hypertension, are presumed risk factors of pregnancy-related osteooporosis. However, exact etiology and pathogenesis are uncertain. The management and natural history are still poorly defined. Traditional medications for osteoporosis are calcium/vitamin D and bisphosphonate. Concerns with bisphosphonate include accumulation in bone and fetal exposure in subsequent pregnancies. The newly developed medication, teriparatide, has shown good results. We report six cases of pregnancy-related osteoporosis and spinal fracture with literature review.

      • The role of Oxytocin and Oxytocin Receptor in Human Placenta according to Gestational age

        목적: Oxytocin (OXT) is a peptide hormone that has a central role in the regulation of parturition and lactation in uterus. OXT signals is mediated by the OXT receptor (OXTR) that are demonstrated by a species-specific expression. In the present study, we examined the synthesis of OXT and OXTR in human placenta tissue according to gestational age. 방법: Depending on gestational age, 48 placentas were divided into early preterm, late preterm, and term groups, and the expression of OXT and OXTR was evaluated. First of all, we revealed that OXT and OXTR mRNA and protein were present in normal placenta tissue via Q-PCR, Dot-blot assay, and Western blot assay. In order to explore the regulatory mechanism, the placental BeWo cells were administrated with E2, or P4 and the expression of OXT and OXTR was tested. In addition, in order to confirm estrogen receptor (ER) signaling, we administrated ER antagonists on BeWo cells. 결과: Both OXT and OXTR levels in normal placenta were increased gradually at the late stage of pregnancy, suggesting that local OXT may have a critical role in the function of placenta. The mRNA and protein levels of OXT and OXTR were increased by E2, while P4 did not. As a results, both OXT and OXTR were significantly altered by ERα antagonist (MPP), while ERβ antagonist (PHTPP) was not widely changed. 결론: These results suggested that OXT and OXTR were controlled by mainly E2 in placenta, thereby, may play physiological functions in human placenta at late stage of pregnancy.
