The objective of this article is to carry on the comparative study of the two books, Dongyisusebowon and Myungsunrok. Je-Ma Lee showed, in fact, the intellectual achievement in his book, Dongyisusebowon. And Lee showed more close relationships with Suk-Ji Han's idea. The common ideas from this comparative study of both scholars are as follows. 1. Lee understands the real character of human being according to Chu-Ztu(朱子)'s doctrine, with the supplement of Han's. 2. Lee explains the Sung-Jung(性情) in Cosmology and the theory of Human Nature, but he concentrates upon the Sung-Jung in the theory of Human Nature, especially in constitutional medicine. 3. Lee states the Tai-Geuk(太極) in the meaning of heart from the medical viewpoint, he agrees with the Han's idea. 4. I try to comprehend Myungsunrok in the Wang-Yang-Ming(王陽明)'s concept. 5. Han explains that Sung-Jung-I-Ki(性情理氣) is one concept with two aspects, Che-Yong(體用). In addition, Lee has formed his 'Chun-In-Sung-Myung(天人性命)' thought in harmonization with Han's idea and Chu-Tzu's idea.

목적: 가와사키병이 있는 소아에서 진단 및 결과 예측에 있어서 Bacille Calmette-Guerin (BCG) 접종 부위 반응의 임상적 중요성을 알아보고자 하였다. 방법: 2006년 1월부터 2017년 2월까지 가와사키병으로 서울아산병원에 입원한 1,058명의 환자를 대상으로 각 연령대에서 BCG 부위 반응의 발생 빈도를 조사하였다. 이 중 생후 18개월 미만인 416명의 환자를 BCG 부위 반응과 실험실 및 임상 소견들과의 연관성을 분석하기 위한 대상자들로 선정하였다. 분석은 완전형과 불완전형 발현 두 그룹으로 구분하여 수행되었다. 결과: 1,058명의 환자들을 관찰한 결과, BCG 부위 반응의 발생률은 생후 6-12개월(83%) 연령의 환자에게 가장 많았고, 12개월 후 연령이 증가함에 따라 감소했다(P<0.001). BCG 부위 반응은 생후 18개월 미만 환자의 70%이상에서 발생하였고, 경부 림프절 종대보다 빈발했다. 로지스틱 회귀분석결과, 결막염(P=0.781), 입술/구강의 변화 (P=0.963), 발진 (P=0.510), 경부 림프절 종대 (P=0.363), 사지 말단의 변화 (P=0.283) 및 관상 동맥류 (P=0.776)는 BCG 부위 반응과 관련이 없었다. 결론: BCG 부위 반응은 BCG 예방 접종을 받은 18개월 미만 소아에서 가와사키병의 주요 임상 소견과는 독립적으로 유용한 진단 도구가 될 수 있다는 것을 확인하였다. 관상동맥 합병증 결과는 완전 가와사키병 및 불완전 가와사키병 모두에서 BCG 부위 반응이 있거나 없는 그룹간에 차이가 없었다. Purpose: The purpose of this study was to investigate the clinical significance of Bacille Calmette-Guerin (BCG) site reaction in terms of diagnosis and outcome prediction in young children with Kawasaki disease (KD). Methods: The incidence of BCG site reaction in the respective age ranges was investigated in 1,058 patients who were admitted at Asan Medical Center between January 2006 and February 2017. The 416 patients under 18 months of age were enrolled as subjects for the analysis of the association between BCG site reaction and other laboratory and clinical findings. The analysis was performed separately in complete and incomplete KD groups. Results: The incidence rate of BCG site reaction was peaked at 6-12 months (83%) and decreased with increasing age after 12 months in 1,058 patients (P<0.001). The incidence rate was above 70% in KD aged less than 18 months and more frequent than those of cervical lymphadenopathy. The logistic regression analyses showed that the principal clinical findings including conjunctivitis (P=0.781), red lips/oral mucosa (P=0.963), rash (P=0.510), cervical lymphadenopathy (P=0.363), changes in extremities (P=0.283) and the coronary artery aneurysm (P=0.776) were not associated with the BCG site reaction. Conclusions: The BCG site reaction could be a useful diagnostic tool independent to principal clinical findings in KD developing in children aged <18 months, who underwent BCG vaccination. Outcome of KD patients was not different between groups with or without the BCG site reaction in both complete KD and incomplete KD.

The acid leachate derived from the sulfide mineral such as pyrite can cause problems such as aging of infrastructure and environment contamination around the civil construction site. The purpose of this study is to assess the environmental effect of acid leachate derived from pyrite in the Miryang area under road construction. In this study, 13 samples of situ core were used for the net acid generation (NAG) experiment. The chemical composition including pH, oxidation and reduction potential (ORP) and electrical conductance of water samples produced from the NAG test was analyzed. In addition, five polished thin sections of rock cores were made for electro-microprobe analyses. In the results of the NAG tests, 7 samples showed lower values than pH 3.5. It strongly indicated that these areas are under the environmental and infrastructure damaged by the acid leachate. The chemical type of the 7 samples were classified as the Fe(Ca)-SO4 type, which is totally a different type compared to general groundwater. The concentration of total sulfur ranges from 0.004% to 12.5%. 6 rock samples are plotted on a potentially acid forming zone in the relationship diagram between the total sulfide and NAG-pH. In conclusion, it is suggested that a protection method against environmental damage and infrastructure corrosion by the acid leachate should be prepared in all of the areas under road construction.
TRPM7 Is Essential for RANKL-Induced Osteoclastogenesis

The transient receptor potential melastatin type 7 (TRPM7) channel is a widely expressed non- selective cation channel with fusion to the C-terminal alpha kinase domain and regarded as a key regulator of whole body Mg<sup>2+</sup> homeostasis in mammals. However, the roles of TRPM7 during osteoclastogenesis in RAW264.7 cells and bone marrow-derived monocyte/macrophage precursor cells (BMMs) are not clear. In the present study, we investigate the roles of TRPM7 in osteoclastogenesis using methods of small interfering RNA (siRNA), RT-PCR, patch-clamp, and calcium imaging. RANKL (receptor activator of NF-ՊB ligand) stimulation did not affect the TRPM7 expression and TRPM7-mediated current was activated in HEK293, RAW264.7, and BMM cells by the regulation of Mg<sup>2+</sup>. Knock-down of TRPM7 by siTRPM7 reduced intracellular Ca<sup>2+ </sup>concentration ([Ca<sup>2+</sup>]<sub>i</sub>) increases by 0 mM [Mg<sup>2+</sup>]<sub>e </sub>in HEK293 cells and inhibited the generation of RANKL-induced Ca<sup>2+ </sup>oscillations in RAW264.7 cells. Finally, knock-down of TRPM7 suppressed RANKL-mediated osteoclastogenesis such as activation and translocation of NFATc1, formation of multinucleated cells, and the bone resorptive activity, sequentially. These results suggest that TRPM7 plays an essential role in the RANKL-induced [Ca<sup>2+</sup>]<sub>i</sub> oscillations that triggers the late stages of osteoclastogenesis.
Intracoronary tenecteplase: a treatment option for failed mechanical thrombectomy

A 57-year-old female presented with chest pain of 4 hours duration associated with posterior ST segment elevation. She had no past medical history of the hypertension, diabetes, or dyslipidemia. she never smoked. Troponin T was elevated. Echocardiography showed a decreased left ventricular ejection fraction of 42% with regional wall motion abnormalities of the lateral, posterior and anterior wall. She was treated with aspirin, clopidogrel and atorvastatin before intervention. Coronary angiography revealed atotal occlusion of the ectatic proximal left circumplex artery (LCX) with TIMI 0 flow as infarction- related artery with massive thrombus burden. An unfractionated heparin was administered and a 0.014-inch guide wire passed across the occlusion. First, we injected a abciximab of 8.25 mg at the site of the thrombus through an aspiration catheter (Thrombuster Catheter, Atrium, Japan). We tried thrombus removal several times with the aspiration catheter. However, ectatic change remained occluded. Second, we performed ballooning several times using a balloon with 2.5 mm diameter However, the huge thrombus was not broken. Third, we tried thrombus removal with another catheter (Export Catheter, Medtronic, USA) with lager luminal tip. Very small amounts of thrombus wereaspirated. Fourth, ballooning was performed a couple of times with lager balloon, but this did not improve the angiographic thrombus burden. Finally, a 35 mg bolus of intracoronary tenecteplase was administered at the site of the thrombus through the aspiration catheter. Ten minutes later coronary angiography revealed nearly complete removal of the thrombus and TIMI 3 flow in a large ectatic artery. And we could identified the true culprit lesion which was located in the just proximal site of the bifurcation (between distal LCX and obtus marginal branch). IVUS (iLab, Boston Scientific, USA) showed large plaque burden, plaque rupture, and residual thrombus at the culprit lesion. IVUS-guided drug eluting stent implantation(Promus PREMIRE stent 4.0 mm × 12 mm, Boston Scientific, USA) was performed. IVUS did not show mal-apposition or under-expansion of the stent. Her inpatient stay was uncomplicated and she was discharged 4 days after her initial presentation.
Prevalence of antibodies against hepatitis E virus in wild boars (Sus scrofa) in Korea

We determined the nationwide seroprevalence of hepatitis E virus (HEV) infection in the wild boar population in Korea. Enzyme-linked immunosorbent assay (ELISA) results showed that 42% of the 528 wild boars that were hunted between 2013 and 2014 were anti-HEV antibody positive. Furthermore, all Korean provinces showed an HEV seroprevalence between 9.8% and 51.1%, suggesting that wild boar HEV infection occurs throughout the country. Importantly, infected wild boar could act as a potential reservoir for HEV and could aid transmission to other animals and humans.
