The Korea Cohort Consortium: The Future of Pooling Cohort Studies

Objectives: We introduced the cohort studies included in the Korean Cohort Consortium (KCC), focusing on large-scale cohort studies established in Korea with a prolonged follow-up period. Moreover, we also provided projections of the follow-up and estimates of the sample size that would be necessary for big-data analyses based on pooling established cohort studies, including population-based genomic studies. Methods: We mainly focused on the characteristics of individual cohort studies from the KCC. We developed “PROFAN”, a Shiny application for projecting the follow-up period to achieve a certain number of cases when pooling established cohort studies. As examples, we projected the follow-up periods for 5000 cases of gastric cancer, 2500 cases of prostate and breast cancer, and 500 cases of non-Hodgkin lymphoma. The sample sizes for sequencing-based analyses based on a 1:1 case-control study were also calculated. Results: The KCC consisted of 8 individual cohort studies, of which 3 were community-based and 5 were health screening-based cohorts. The population-based cohort studies were mainly organized by Korean government agencies and research institutes. The projected follow-up period was at least 10 years to achieve 5000 cases based on a cohort of 0.5 million participants. The mean of the minimum to maximum sample sizes for performing sequencing analyses was 5917-72 102. Conclusions: We propose an approach to establish a large-scale consortium based on the standardization and harmonization of existing cohort studies to obtain adequate statistical power with a sufficient sample size to analyze high-risk groups or rare cancer subtypes.

Ultra-precision positioning systems basically require high natural frequency and sufficient workspace. To cope with this requirement, flexure hinge mechanisms have been proposed. However, previous designs are hard to satisfy the functional requirements of the system due to difficulty in modeling and optimization process applying the independent axiomatic design. Therefore, this paper suggests a new design and design-order based on semi-coupled, axiomatic design. A spatial 3-DOF parallel type micro mechanism is chosen as an exemplary device. Based on preliminary kinematic analysis and dynamic modeling of the system, an optimal design is conducted. To check the effectiveness of the optimal parameters obtained by theoretical approach, simulation is performed by FEM.

Orbital Infiltration as the First Site of Relapse of Primary Testicular T-cell Lymphoma

A 43-year-old male presented with a painless left testicular mass. The pathologic diagnosis of the radical orchiectomy specimen was peripheral T-cell lymphoma, unspecified (PTCL-u). According to the Ann Arbor staging system, his initial stage was III because of the right nasopharyngeal involvement. After first-line chemotherapy with four courses of the CHOP regimen and this was followed by involved-field radiotherapy, he achieved complete remission. Two months later, disease recurred to the left ciliary body of the left eye without evidence of involvement at other sites. Although the patient received intensive chemotherapy with autologous hematopoietic stem cell transplantation, he ultimately died of leptomeningeal seeding. Because both the central nervous system (CNS) and the orbit are sanctuary sites for chemotherapy, orbital infiltration of lymphoma should prompt physicians to evaluate involvement of the CNS and to consider performing prophylactic intrathecal chemotherapy as a treatment option. 본 논문은 방향그래프에서 사이클을 찾는 알고리즘으로 Transitive Closure를 기본구조로 하고 있는 All Pairs Shortest Paths 알고리즘을 주로 사용하여 사이클을 찾을 수 있도록 변형하였다. 또한 이 알고리즘을 2차원 시스톨릭 배열에서 구현하면 O(N²)이지만, 본 논문에서는 매우 큰 크기의 방향그래프를 기존의 작은 용량(m)의 VLSI 구조에 맞추어 분할하여 3차원 Mesh of Tree에서 구현하여 속도를 향상시켰다. 이 분할 맵핑 방법을 사용하지 않고 3차원 Mesh of Tree에서 구현할 경우의 속도는 O(NlogN)이지만, 분할 맵핑 방법을 적응할 경우 그 속도는 O( (N/m)log(N/m)) 이 된다.

<P>As a treatment for solid tumors, dendritic cell (DC)-based immunotherapy has not been as effective as expected. Here, we review the reasons underlying the limitations of DC-based immunotherapy for solid tumors and ask what can be done to improve immune cell-based cancer therapies. Several reports show that, rather than a lack of immune induction, the limited efficacy of DC-based immunotherapy in cases of renal cell carcinoma (RCC) likely results from inhibition of immune responses by tumor-secreted TGF-β and an increase in the number of regulatory T (Treg) cells in and around the solid tumor. Indeed, unlike DC therapy for solid tumors, cytotoxic T lymphocyte (CTL) responses induced by DC therapy inhibit tumor recurrence after surgery; CTL responses also limit tumor metastasis induced by additional tumor-challenge in RCC tumor-bearing mice. Here, we discuss the mechanisms underlying the poor efficacy of DC-based therapy for solid tumors and stress the need for new and improved DC immunotherapies and/or combination therapies with killer cells to treat resistant solid tumors.</P>
우선순위화된 ATM망에서 이중 버퍼를 갖는 중첩 임계치 셀 폐기 기법의 성능 평가

ATM망은 다양한 서비스에 대한 성능 요구 사항을 만족시켜야 한다. 이러한 다양한 서비스 요구 사항을 만족하기 위하여 본 논문에서는 우선순위 스케줄링과 버퍼 할당 기법을 혼합한, 이중 버퍼를 갖는 중첩 임계치 셀 폐기 기법을 제안한다. 즉, 지연 요구 사항을 만족시키기 위하여 이중버퍼를 사용한 스케줄링 기법을 제안하고, 손실 요구 사항을 만족 시키기 위해 2개의 임계치를 가지는 버퍼 할당 기법을 제안한다. 또한 시스템 모델로서는 M/D/1/N 모델로 모델링하여 시스템 성능을 분석하고 이의 증명을 위해 본 논문에서는 마코프 체인을 이용해서 해석하는 방법을 제안한다. 시뮬레이션을 통하여 제안한 해석 방법의 타당성을 검증하고, 제안한 모델이 ATM망의 서비스 품질 요구사항을 충족시키는데 보다 나은 유연성을 제공함을 알 수 있었다.
