      • 편도체중심핵내 요힘빈과 날록손의미세주입이 신기성으로 유발된 통각감소에 미치는 효과

        선행연구에서 동물이 새로운 환경에 노출되면 날록손에 역전되지 않는 통각감소 현상이 발생하며, 이러한 신기성으로 유발된 통각감소(novelty-induced hypoalgesia)는 세로토닌(serotonin: 5-HT)이나 노어피네프린(norepinephrine: NE)등의 모노아민계에 의해서 매개된다. 본연구에서는 정서/인지적 반응과 항유해적반응을 매개하는 하나의 신경구조물로 알려져 있는 편도체중심핵이 이러한 통각감소를 전달하는 데에 책임있는 영역인지, 그리고 NE의 작용에 아편계가 관여하는지를 알아보고자 하였다. 본실험에서는, 먼저 모든 쥐에게 양측 편도체중심핵을 표적으로 안내관을 심는 시술을 실시했다. 1주일 후 이들 쥐를 크게 노출집단과 비노출집단의 2 집단으로 나눈 후 노출집단만 23℃의 열판에서 매일 5분씩 7일간을 노출시켰다. 8일째에 각 집단을 다시 요힘빈집단과 생리식염수집단(요힘빈-노출집단, 요힘빈-빈노출집단, 생리식염수-노출집단 및 생리식염수-비노출집단)으로 분류한 다음 편도체중심핵의 양측에 해당의 각 약물을 미세주입하고 모든 동물을 49℃의 열판에 노출시켜서 뒷발바닥핥기 잠재기를 측정하였다. 각 동물을 열판장치에서 꺼내자마자 동일한 부위에 다시 날록손을 주입하고 앞에서와 동일하게 잠재기를 측정하였다. 실험결과, 열판기구라는 신기성환경에 의해서 통각감소가 유발되었음은 물론, 알파-2 NE 수용기 길항제인 요힘빈에 의해서는 비노출조건에서만 통각감소가 증가하였으며, 날록손의 경우는 비노출조건의 요힘빈집단에서만 통각감소가 감소하였다. 이들의 결과는 동물이 신기성환경에 접하면 공포와 함께 통각이 감소한다는 사실을 알려줄 뿐만 아니라, 편도체중심핵내의 NE가 이러한 효과를 매개 및 상승시킴을 의미한다. 그리고, 편도체중심핵에서 비노출조건의 요힘빈하에서만 아편계가 관여한다는 사실은 신기성환경으로 유발된 통각감소에 아편계가 단독으로 개입할 가능성은 미소하며 이런 종류의 통각감소에서 아편계보다는 NE계의 역할이 큼을 시사한다. Previous studies have reported that a novel environment renders animals hypoalgesic and this novelty-induced hypoalgesia(NIHA) is not reversed by naloxone-pretreatment; in fact, the NIHA seems to be mediated through the monoaminergic system such as serotoninergic- and noradrenergic substrate, according to the studies on the effects of i.p. injection of those receptor ligands on the NIHA. The present study is conducted to investigate not only whether the central amygdaloid nucleus(ACe), which is known as mediating anti-nociceptive response as well as emotional/cognitive ones including fear/anxiety response, is responsible for the NE-mediated NIHA, but also whether the opiate system modulates this kind of NE mediation. In the present experiment, all rats were first given bilateral cannulae-implantations into the ACe. 7 days later, after these rats were randomly assigned into two groups named 'exposure group(EXP)' and 'non-exposure group(NON)', only the EXP started to be exposed to the non-functional floor of the Hot-Plate Apparatus (at the temperature of 23℃) 5 minutes every day for 7 consecutive days. On the 8th day, after being divided into two subgroups such as yohimbine group and saline group and being supplied with bilaterally-microinjected yohimbine or saline equivalent to its condition into the ACe, each group received a hot-plate(49℃) test. The latency to lick a hind-paw was manually recorded. As soon as picked out of the Apparatus all the rats were re-provided with naloxone, one of opiate receptor antagonists, and were re-tested under the same conditions mentioned. The result is that the novelty rats produced more hypoalgesia than the adapted rats in the hindpaw-licking latency, and that the bilateral micro-injection of yohimbine, an α(2^-)NE receptor antagonist increaing NE release in the brain-neuron synaptic cleft by blocking the presynaptic autoreceptors, potentiated the hypoalgesia, only in the novelty rats; which indicates that the novelty-induced hypoalgesia appears to be carried through the NE substrate and that the ACe is the chief forebrain site conducting the NE-potentiated novety-induced hypoalgesia. As far as the effect of naloxone injection under the influence of the preinjected yohimbine and saline is concerned, the hypoalgesia was reduced only under the condition of novelty yohimbine, neither under that of novelty saline nor under that of non-novelty yohimbine and saline; which implicates the opiate substrast may participate in the hypoalgesia necessarily as the NE-substrate-dependant pattern rather than by itself. Thus, the NE system may play more important role than opiate system in mediating the novelty-induced hypoalgesia.

        뇌 동맥류성 지주막하출혈 환자간호의 질 평가 도구 개발

        Purpose: The purpose of this study was to develope a evaluation tool of the quality of nursing care for aneurysmal subarachnoid hemorrhage patients at Hunt & Hess Grade Ⅰ∼Ⅲ. Method: The data was gathered from April 15, 2002 to June 14, 2002. The tool proceeded in six steps according to Bloch's framework. Result: The results of this study are as follows: 1) The quality evaluation tool for neurosurgery ICU consisted of 25 standards and 138 criteria. The quality evaluation tool for ward consisted of 27 standards and 144 criteria.; 2) The content validities of the tools for neurosurgery ICU and ward were 0.96 and 0.98, respectively.; 3) The Pearson coefficients for evaluating the interrated reliability of the tools for neurosurgery ICU and ward were r=0.9106 (p< 0.1 ) and r=0.8807 (p<0.1), respectively. 4) The ranges of corrected item-total correlations for evaluating the construct validity of the tools for neurosurgery ICU and ward were 0.0279∼0.9354 and 0.017 4∼0.9682, respectively.: And 5) The ranges of Cronbach's a reliabilities for evaluating the internal consistency reliability of the tools for neurosurgery ICU and ward were 0.6040-0.8554 and 0.6212-0.9584, respectively. Conclusion: The tools developed in this study seem useful in evaluating the quality of nursing care for aneurismal subarachnoid hemorrhage patients at Hunt & Hess Grade Ⅰ∼Ⅲ.

        2.9% 과산화수소를 함유한 부착형 미백제의 임상적 효능과 안정성에 관한 연구

        2.9%의 과산화수소를 함유한 시험용 부착형 미백제(Medison dental whitening strip, Samsung medical Co.,Anyang,Korea)를 실험군 피험자 23명, 시험용 미백제에서 과산화수소를 제외한 동일한 제재를 대조약으로 대조군 피험자 24명에게 매일 1시간 30분씩, 2주간 적용하여 임상적 효능과 안전성을 평가하였다. 미백 효과는 Shade Vision, Chroma Mete 및 Vitapan classical shade guide를 이용하여 색변화를 측정하여 다음과 같은 결론을 얻었다. 1. 실험군에서의 전체 치아의 색변화량(ΔE^(*))은 세 가지 측정법에서 모두 대조군에 비해 통계적으로 유의한 차이를 보였고 (p < 0.05). 쉽게 인지할 수 있는 정도였다. 2. 색변화는 색좌표 상에서 명도의 증가와 녹색조 및 청색조로의 변화에 의해 얻어졌다. 3. 치아별로는 어두운 견치의 색변화량이 가장 컸고, 측절치, 중절치 순으로 감소하였다. 4. 47명의 피험자 중 5명이 경도의 과민증을 경험하였으나 곧 완화되었다, 치은염증지수와 전기치수검사 결과 시험 전후에 차이가 없어 안전성을 확인하였다. This study evaluated the effectiveness and safety of an experimental bleaching strip (Medison dental whitening strip, Samsung medical Co., Anyang, Korea) containing 2.9% hydrogen peroxide. Twenty-three volunteers used the bleaching strips for one and a half hour daily for 2 weeks. As control group, the same strips in which hydrogen peroxide was not included were used by 24 volunteers with the same protocol. The shade chang (ΔE^(*), color differnce) of twelve anterior teeth was measured using Shade Vision (X-Rite Inc., S. W. Grandville, MI, USA), Chroma Meter (Minota Co., Ltd. Osaka, Japan) and Vitapan classical shade guide (Vita Zahnfabrik, Germany). The shade change of overall teeth in the experimental group was significantly greater than that in the control group (p < 0.05) and was easily perceivable. The change resulted from the increase of lightness (CIE L^(*) value) and the decrease of redness (CIE a^(*) value) and yellowness (CIE b^(*) value). The shade change of individual tooth was greatest in canine, can smallest in central incisor. The safety of the bleaching strip was also confirmed. 〔J Kor Acad Cons Dent 31(4):269-281, 2006〕

      • 흡연에 의한 급성 호산구성 폐렴 2예

        최근 흡연을 시작한 젊은 성언에서 급성 호산구성 폐렴의 발생에 대한 증례가 보고되고 있다. 급성 호산구성 폐렴은 대개 발열을 동반하기 때문에 초기에는 감염성 폐렴으로 잘못 진단, 치료되는 경우가 많다. 아직 정확한 병태 생리 및 조직학적 소견은 밝혀져 있지 않지만 병력 청취 및 임상 양상, 방사선 소견의 관찰을 통해 급성 호산구성 폐렴을 의심하고, 진단을 위해 기관지 폐포 세척액 검사를 시행한다면 좀 더 쉽게 진단 내릴 수 있을 것이다. 본 저자들은 최근 처음 시작한 흡연에 의해 발생한 급성 호산구성 폐렴을 진단하고, 스테로이드를 투여하여 성공적으로 치료한 환자 2예를 경험하였기에 이를 문헌 고찰과 함께 보고하는 바이다. Acute eosinophilic pneumonia(AEP) is characterized by acute febrile respiratory illness associated with diffuse pulmonary infiltration and pulmonary eosinophilia. The specific etiology for acute eosinophilic pneumonia is elusive. By some investigators, cigarette smoking is suggested as a causative substance which can cause AEP. In recent, the authors experienced two cases of AEP following cigarette smoking. Both cases had characteristic features including age around 20 years, new onset smoking before occurance of AEP, diffuse infiltration on chest radiography, pulmonary eosinophilia based on bronchoalveolar lavage and acute improvement after steroid therapy. These clinical features are resemble with previous smoking induced AEP case reports. Base on these clinical features, cigarette smoking associated AEP could be diagnosed more easily.

        Auranofin Enhances Sulforaphane-Mediated Apoptosis in Hepatocellular Carcinoma Hep3B Cells through Inactivation of the PI3K/Akt Signaling Pathway

        The thioredoxin (Trx) system plays critical roles in regulating intracellular redox levels and defending organisms against oxidative stress. Recent studies indicated that Trx reductase (TrxR) was overexpressed in various types of human cancer cells indicating that the Trx-TrxR system may be a potential target for anti-cancer drug development. This study investigated the synergistic effect of auranofin, a TrxR-specific inhibitor, on sulforaphane-mediated apoptotic cell death using Hep3B cells. The results showed that sulforaphane significantly enhanced auranofin-induced apoptosis by inhibiting TrxR activity and cell proliferation compared to either single treatment. The synergistic effect of sulforaphane and auranofin on apoptosis was evidenced by an increased annexin-V-positive cells and Sub-G1 cells. The induction of apoptosis by the combined treatment caused the loss of mitochondrial membrane potential (ΔΨm) and upregulation of Bax. In addition, the proteolytic activities of caspases (-3, -8, and -9) and the degradation of poly (ADP-ribose) polymerase, a substrate protein of activated caspase-3, were also higher in the combined treatment. Moreover, combined treatment induced excessive generation of reactive oxygen species (ROS). However, treatment with N-acetyl-L-cysteine, a ROS scavenger, reduced combined treatment-induced ROS production and apoptosis. Thereby, these results deduce that ROS played a pivotal role in apoptosis induced by auranofin and sulforaphane. Furthermore, apoptosis induced by auranofin and sulforaphane was significantly increased through inhibition of the phosphoinositide 3-kinase (PI3K)/Akt pathway. Taken together, the present study demonstrated that down-regulation of TrxR activity contributed to the synergistic effect of auranofin and sulforaphane on apoptosis through ROS production and inhibition of PI3K/Akt signaling pathway.

        Partial nucleotide sequencing and phylogenetic analyses of Newcastle disease virus and infectious bursal disease virus isolated in South Korea

        The present study was conducted to investigate the genetic profile of two prevalent avian pathogens in Korea namely, Newcastle disease virus(NDV) and infectious bursal disease virus(IBDV). Two farms located in Yeongi-gun, Chungnam were selected for this study. The two viruses were isolated from various organs(spleen, trachea, bursa of Fabricius) of deceased chickens that showed clinical symptoms of Newcastle Disease or Infectious bursal disease like swelling and congestion of the F bursa, facial edema, lacrimation, greenish yellow diarrhea as well as pathological signs like airsacculitis, haemorrhages in the intestines and so on. For analysis of NDV and IBDV, a 466 and 435 base pair fragments corresponding to the HN and VP2 regions which are highly conserved among related strains of NDV and IBDV, respectively, were amplified by RT-PCR and analyzed by sequencing. Comparison of the VP2 region showed a 99.3% homology between the Korean IBDV isolate and the BJ836-attenuated vaccine strain. In contrast, the HN region of the Korean NDV isolate only has an 83 to 84% homology with the vaccine strains LaSota, B1 and VGGA. Our findings reveal that the prevalent NDV strain in Korea is genetically different from the vaccine strains and may explain the recent outbreaks of Newcastle disease in the region.

        Identification of Novel Methylation Markers in Hepatocellular Carcinoma using a Methylation Array

        <P>Promoter CpG island hypermethylation has become recognized as an important mechanism for inactivating tumor suppressor genes or tumor-related genes in human cancers of various tissues. Gene inactivation in association with promoter CpG island hypermethylation has been reported to be four times more frequent than genetic changes in human colorectal cancers. Hepatocellular carcinoma is also one of the human cancer types in which aberrant promoter CpG island hypermethylation is frequently found. However, the number of genes identified to date as hypermethylated for hepatocellular carcinoma (HCC) is fewer than that for colorectal cancer or gastric cancer, which can be attributed to fewer attempts to perform genome-wide methylation profiling for HCC. In the present study, we used bead-array technology and coupled methylation-specific PCR to identify new genes showing cancer-specific methylation in HCC. Twenty-four new genes have been identified as hypermethylated at their promoter CpG island loci in a cancer-specific manner. Of these, <I>TNFRSF10C</I>, <I>HOXA9</I>, <I>NPY</I>, and <I>IRF5</I> were frequently hypermethylated in hepatocellular carcinoma tissue samples and their methylation was found to be closely associated with inactivation of gene expression. Further study will be required to elucidate the clinicopathological implications of these newly found DNA methylation markers in hepatocellular carcinoma.</P>
