        Efficacy of Intracytoplasmic Sperm Injection for Leukocytospermia

        White blood cells (WBCs) are present in most human ejaculates, but abnormally high concentration of seminal leukocytes map reflect an underlying pathological condition. The World Health Organization (WHO) has defined leukocytospermia as status of more than 10^(6) WBC/mL of semen. The purpose of this study was firstly, to compare the outcomes between conventional IVF and ICSl in leukocytospermia, and secondly, to investigate whether ICSI may be an alternation treatment for patients with leukocytospermia. Total 346 cycles of conventional IVF and ICSI candidates underwent IVF cycles at Eulji Hospital Infertility Clinic. Semen Parameters including concentration, motility, morphology of spermatozoa and concentration of leukocytes were assessed from the raw ejaculates. There was no difference in sperm concentration, motility and morphology. The rates of fertilization and good embryo development from ICSI were significantly higher than those from conventional IVF in leukocytospermia (60.4% & 32.5% respectively for ICSI group and 44.4% & 28.5%, respectively for IVF group, P<0.001). The pregnancy rate after ICSI was also higher than that from conventional IVF (34.0% vs 29.1%. P<0.05). These results indicate that the presence of seminal leukocyes (>1×10^(6) WBC/mL of semen) is adversely related with fertilization, embryo development and pregnancy rate. Therfore the measurement of seminal leukocytes in routine semen analysis appears to be of prognostic value with regard to male fertilizing potential. In conclusion. it is suggested that ICSI is an alternative choice of treatment for patients with leukocytospermia.

        Component-based systems are generally known as useful for maintenance and extension. However, there are a number of difficult aspects in designing them, and skillful knowledge of component-based design is strongly requested. Against the problem, this paper presents a User Interface tool that helps to develop component-based healthcare systems more easily and effectively. This UI tool has been developed, aiming at enabling easier and faster development of such systems. Further it supports standardization and interoperability. We expect that use of the UI tool helps to add, delete, change medical contents, and maintain related systems with less effort and better performance.

        Colloidal stability and in vitro permeation study of poly(e-caprolactone) nanocapsules containing hinokitiol

        Nanocapsules containing hinokitiol (HKL) were prepared by an emulsion-diffusion method. Poly(e-caprolactone) (PCL), synthesized according to a ring opening method, was used as a wall material, and HKL dissolved in octyl salicylate (OS) was use as a core material. In an emulsification step in preparing nanocapsules, three kinds of emulsifiers were employed, namely cetyltrimethylammonium chloride (CTAC), sodium lauryl sulfate (SLS), gelatin. On a transmission electron microscope, the size was hundreds of nanometers. According to the results of pHdependent microelectrophoresis, the absolute value of the surface potential of the nanocapsules prepared using either CTAC or SLS was greater than 20 mV. In case the nanocapsules were prepared using the mixture of CTAC and gelatin, the value was between 0 mV and 20 mV. The nanocapsules prepared by using either CTAC or SLS were stable in terms of size over the pH range of 3–11. However, the nanocapsules prepared using the mixture of CTAC and gelatin was not stable in size over the same pH range. Following the results of in vitro permeation experiment, the nanocapsules enhanced the permeation of HKL. Especially, the nanocapsules prepared using SLS was prominent in promoting the permeation. This is probably because that SLS act as a permeation enhancer. However, the nanocapsules themselves hardly penetrated into hairless mouse skin, which was observed on a confocal laser scanning microscope.

        In Vitro Permeation Study of Hinokitiol: Effects of Vehicles and Enhancers

        <P> In vitro permeation of hinokitiol (HKL) through hairless mouse skin was investigated using a diffusion cell. Either propylene glycol (PG) or ethanol (EtOH) was used as a vehicle for HKL. After applying the HKL solutions of 0.5%. 1%, 2%, and 5% onto the skin, the amount of HKL transferred through the skin into the receptor solution, phosphate-buffered saline (PBS, pH7.4), was determined at a predetermined time intervals for 18 hr using a high performance chromatography (HPLC). EtOH was more effective than PG in terms of in vitro permeation of HKL. This is possibly because EtOH acts as a permeation enhancer. Another reason would be related to the higher thermodynamic activity of HKL in ethanol. To investigate the effect of an enhancer on the in vitro permeation, oleyl alcohol, 1-dodecyl-2-pyrrolidone (DP), and lauric acid were used as enhancers. Each was added to the HKL solution (1%) so that the concentration of the enhancer was 1%. Among the enhancers, DP was the most effective and it enhanced the permeation of HKL approximately 5-10 times.</P>

        Elevated On-Treatment Diastolic Blood Pressure and Cardiovascular Outcomes in the Presence of Achieved Systolic Blood Pressure Targets

        Background and Objectives: This study aimed to investigate the association between cardiovascular events and 2 different levels of elevated on-treatment diastolic blood pressures (DBP) in the presence of achieved systolic blood pressure targets (SBP). Methods: A nation-wide population-based cohort study comprised 237,592 patients with hypertension treated. The primary endpoint was a composite of cardiovascular death, myocardial infarction, and stroke. Elevated DBP was defined according to the Seventh Report of Joint National Committee (JNC7; SBP <140 mmHg, DBP ≥90 mmHg) or to the 2017 American College of Cardiology/American Heart Association (ACC/AHA) definitions (SBP <130 mmHg, DBP ≥80 mmHg). Results: During a median follow-up of 9 years, elevated on-treatment DBP by the JNC7 definition was associated with an increased risk of the occurrence of primary endpoint compared with achieved both SBP and DBP (adjusted hazard ratio [aHR], 1.14; 95% confidence interval [CI], 1.05–1.24) but not in those by the 2017 ACC/AHA definition. Elevated on-treatment DBP by the JNC7 definition was associated with a higher risk of cardiovascular mortality (aHR, 1.42; 95% CI, 1.18–1.70) and stroke (aHR, 1.19; 95% CI, 1.08–1.30). Elevated on-treatment DBP by the 2017 ACC/AHA definition was only associated with stroke (aHR, 1.10; 95% CI, 1.04–1.16). Similar results were seen in the propensity-score-matched cohort. Conclusion: Elevated on-treatment DBP by the JNC7 definition was associated a high risk of major cardiovascular events, while elevated DBP by the 2017 ACC/AHA definition was only associated with a higher risk of stroke. The result of study can provide evidence of DBP targets in subjects who achieved SBP targets.

        <P>Accurate diagnosis of ovarian clear cell carcinoma (CCC) is important because of its poor prognosis with chemoresistance and a high recurrent rate. The clinicopathologic characteristics and prognostic significance of the cell cycle regulator [early mitotic inhibitor-1 (Emi1)] and galactoside-binding protein (Galectin-3) were evaluated. Among 155 CCCs from 18 hospitals in Korea between 1995 and 2006, 129 pure CCCs were selected with consensus using immunohistochemical stains for hepatocyte nuclear factor-1β, Wilms' tumor protein, and estrogen receptor. The expressions of Emi1, Galectin-3, p53, and Ki-67 labeling index were analyzed with clinicopathologic parameters and the patient's survival. The mean age of the patients was 49.6 yr; the tumors were bilateral in 10.9%, and the average size was 12 cm. Adenofibromatous component was found in 7%, and endometriosis in 48.1% of the cases. Psammoma body was seen in 16.3%. Disease-free survival and overall survival rates were 78.3% and 79.1%, respectively. The International Federation of Obstetrics and Gynecology (FIGO) stage was the most important prognostic indicator. Emi1 expression (>5%) was seen in 23.3% of CCCs, and associated with high FIGO grades and poor overall survival (P<0.05). High Galectin-3 (80%) expression was seen in 59.7% of CCCs, and associated with FIGO stages III and IV, and high Ki-67 labeling index. High Ki-67 labeling index (50%) and p53 expression (50%) were seen in 27.1% and 18.6% of CCCs, respectively, but there was no clinicopathologic and prognostic significance. On the basis of the fact that the expression of Emi1 in CCC was correlated with a high histologic grade and worse overall survival, target therapy using inhibitors of Emi1 may be tried in the management of CCC patients with Emi1 expression.</P>
