Cariprazine — an Alternative Treatment for Clozapine-resistant Schizophrenia?

Treatment-resistant schizophrenia (TRS) poses a significant therapeutic challenge in psychiatric practice. Clozapine is recognized as a treatment of choice in TRS but is not always effective in alleviating patients’ symptoms. Additionally, clozapine therapy is associated with multiple side effects and monitoring requirements that often limit its use and negatively affect patients’ compliance with the treatment. Although clozapine augmentation options are available, there is currently no alternative monotherapy proven to be effective in TRS. We present a case of a young man with TRS who failed to respond to appropriate trials of risperidone, aripiprazole and also clozapine, and who experienced impairing adverse effects of clozapine that made further clozapine treatment not only futile but also detrimental to his health. He was successfully treated with cariprazine monotherapy, which culminated in the remission of his both positive and negative symptoms of psychosis as well as in the marked improvement in social functioning. Cariprazine, a newer atypical antipsychotic endowed with a D3-preferring mode of action, may offer a better tolerated and more acceptable treatment option for patients with difficult-to-treat psychotic symptoms.

Purpose: Optimal radiotherapy treatment volumes for patients with locally advanced hypopharynx squamous cell carcinoma should ensure maximal tumor coverage with minimal inclusion of normal surrounding structures. Here we evaluated the effectiveness of a direct 3-mm high-dose gross tumor volume to planning target volume expansion on clinical outcomes for hypopharynx cancers. Materials and Methods: We performed a retrospective analysis of patients with hypopharynx carcinoma treated between 2004 and 2018 with primary radiotherapy using a direct high-dose gross tumor volume to planning target volume expansion and with or without concurrent systemic therapy. Diagnostic imaging of recurrences was co-registered with the planning CT. Spatial and volumetric analyses of contoured recurrences were compared with planned isodose lines. Failures were initially defined as in field, marginal, elective nodal, and out of field. Each failure was further classified as central high-dose, peripheral high-dose, central intermediate/low-dose, peripheral intermediate/low-dose, and extraneous. Clinical outcomes were analyzed by Kaplan-Meier estimation. Results: Thirty-six patients were identified. At a median follow-up at 52.4 months, estimated 5-year overall survival was 59.3% (95% confidence interval [CI], 36.3%–74.1%), 5-year local and nodal control was 71.7% (95% CI, 47.1%–86.3%) and 69.9% (95% CI, 57.0%–82.6%), respectively. The most common failure was in the high-dose primary target volume. The gastrostomy tube retention rate at 1 year among patients without recurrence was 13.0% (95% CI, 3.2%–29.7%). Conclusion: Minimal high-dose target volume expansions for hypopharynx cancers were associated with favorable locoregional control. This approach may enable therapy intensification to improve clinical outcomes.

<P>The azimuthal correlations of D mesons with charged particles were measured with the ALICE apparatus in pp collisions at [FORMULA OMISSION] and p–Pb collisions at [FORMULA OMISSION] at the Large Hadron Collider. [FORMULA OMISSION], [FORMULA OMISSION], and [FORMULA OMISSION] mesons and their charge conjugates with transverse momentum [FORMULA OMISSION] and rapidity in the nucleon-nucleon centre-of-mass system [FORMULA OMISSION] (pp collisions) and [FORMULA OMISSION] (p–Pb collisions) were correlated to charged particles with [FORMULA OMISSION]. The yield of charged particles in the correlation peak induced by the jet containing the D meson and the peak width are compatible within uncertainties in the two collision systems. The data are described within uncertainties by Monte-Carlo simulations based on PYTHIA, POWHEG, and EPOS 3 event generators.</P>
Rings in which every ideal contained in the set of zero-divisors is a d-ideal

In this paper, we introduce and study the class of rings in which every ideal consisting entirely of zero divisors is a d-ideal, considered as a generalization of strongly duo rings. Some results including the characterization of AA-rings are given in the first section. Further, we examine the stability of these rings in localization and study the possible transfer to direct product and trivial ring extension. In addition, we define the class of $d_E$-ideals which allows us to characterize von Neumann regular rings.
Measuring displacements of a railroad bridge using DIC and accelerometers

Railroad bridges in North America are an integral but aging part of the railroad network and are typically only monitored using visual inspections. When quantitative information is required for assessment, railroads often monitor bridges using accelerometers. However without a sensor to directly measure displacements, it is difficult to interpret these results as they relate to bridge performance. Digital Image Correlation (DIC) is a non-contact sensor technology capable of directly measuring the displacement of any visible bridge component. In this research, a railroad bridge was monitored under load using DIC and accelerometers. DIC measurements are directly compared to serviceability limits and it is observed that the bridge is compliant. The accelerometer data is also used to calculate displacements which are compared to the DIC measurements to assess the accuracy of the accelerometer measurements. These measurements compared well for zero-mean lateral data, providing measurement redundancy and validation. The lateral displacements from both the accelerometers and DIC at the supports were then used to determine the source of lateral displacements within the support system.

The North Korean government of Kim Jong-un is experiencing multiple simultaneous challenges to its legitimacy, but few could be more serious than the inflow and circulation of information in society. This paper uses three case studies to specifically examine how the North Korean state is responding to this danger by actively projecting narratives of transformation: “re-defectors,” sports, and Kim Jong-un’s court orchestra, the Moranbong Band. In every case, it becomes clear that the state is employing an active strategy, not only responding to negative external portrayals, but also trying to shape its own image both within and without its borders. In order to understand how the state interacts with the North Korean public, this paper employs Thomas Callaghy’s trifurcated “domain consensus” as a framework by which to sub-categorize Pyongyang’s approach: normative, utilitarian, and coercive. It focuses on the first of these types of consensus formation, the normative, by exploring the information strategies used by the Kim Jong-un government as it seeks to promote a revised Weberian “reciprocity of expectations” with the population.
Confocal Microscopy in the Esophagus and Stomach

Probe-based confocal microscopy (pCLE) is actively being investigated for applications in the esophagus and stomach. The use of pCLE allows real-time in vivo microscopy to evaluate the microarchitecture of the mucosal epithelium. pCLE appears to be particularly useful in identifying mucosal dysplasia and early malignancies that cannot be clearly distinguished using high-definition white light endoscopy, chromoendoscopy, or magnification endoscopy. In addition, the ability to detect dysplastic tissue in real-time may shift the current screening practice from random biopsy to targeted biopsy of esophageal and gastric cancers and their precursor lesions. We will review the use of pCLE for detection and surveillance of upper gastrointestinal early luminal malignancy.
