만성 통증 환자에서의 정신약물학적 치료

Chronic pain is a significant public health problem and is frustrating to those who suffer from it. Psychiatrists can provide effective treatments to manage chronic pain. In addition to diagnosing and treating comorbid psychiatric disorders, psychiatrists can use psychological treatments and develop multidisciplinary methods to provide comprehensive healthcare to patients who have chronic pain. Psychiatrists are especially skilled in pharmacological treatment, especially with psychotropic agents that have proven efficacy to treat chronic pain conditions. Based on this extensive knowledge and skills in the use of psychotropic agents and the basic understanding and interest in patient dynamics, it is important for psychiatrists to be involved in the treatment of patients with chronic pain. In Korea, psychiatrists are not usually involved in thetreatment of patients with chronic pain. I briefly discuss the role of psychiatrist in the treatment of patients with chronic pain. I also review psychotropic agents effective in treating chronic pain conditions and conclude that psychiatrists in Korea should be actively involved in the care of these patients. 만성 통증은 건강에 심각한 문제를 일으키며 증상을 가진 환자에게 좌절을 불러일으켜 삶의 질을 떨어뜨리게 된다. 정신과 의사는 이들 만성 통증 환자의 진료 및 치료에서 적극적인 역할을 해야 하며, 효과적이라고 알려진 최신 치료법과 관련된 기술을 제공할 수 있어야 한다. 정신과 의사는 정신과적 공존질환의 진단과 치료, 심리적 치료의 적용, 그리고 전반적인 건강증진을 제공할 수 있는 다분야적 접근뿐만이 아니라, 효과가 입증되어 있는 정신과 약물의 적절한 사용을 통해 만성 통증 환자들의 치료 성공률을 증가시키고 삶의 질 개선에도 도움이 되어야 할 것이다. 이에 저자는 만성 통증 환자의 치료에 있어 우리나라의 정신과 의사들이 많은 도움을 줄 수 있을 것으로 생각하며 앞으로 적극적인 참여가 필요함을 제안한다.

투석과 관련된 고관절과 경추의 아밀로이드증: 영상소견의 증례보고

Dialysis-related amyloidosis is a complication of long-term hemodialysis and it is characterized by the accumulation of β2-microglobulin in the osteoarticular structures. We describe here the imaging findings of a case of dialysis-related amyloidosis involving the hip and cervical spine in a 62-year-old woman who received long-term dialysis. We focus here on the CT and MR imaging findings of the cervical spine and we include a review of the relevant literatures. 투석과 관련된 아밀로이드증은 장기간 투석후에 발생하는 합병증으로 골관절 부위에 β2-microglobulin 침착된다. 장기간 투석후 고관절과 경추를 침범한 투석과 관련된 아밀로이드증의 62세 환자에서 경추부위를 중심으로 영상소견을 문헌 고찰과 함께 보고한다.

Legal Issues on Commercial Use of Location Information in Korea

The ability to obtain the location of an individual with some certainty and precision has opened the door to new uses of radio, telecommunication, and satellite technologies. At the same time, the use of these technologies raises numerous legal and other issues, from privacy to quality and accuracy of the information collected or provided, to liability for unwanted interception of the data that is transmitted wirelessly. In 2006, Korea enacted “Act for the Protection of Location Information and Regulation thereon(LIPA)”. Recently, LIPA was amended to additionally authorize the police to collect and use location information. Although LIPA has played leading role in the protection for the location information, there is still much room for exploring in the future with the advent of a new technology and the ever-lasting social changes.

The parotid gland is well affected by the inflammatory process. Acute suppurative parotitis may originate from a septic focus in the oral cavity, but acute suppurative parotitis in the pediatric population is uncommon. Salivary duct calculi such as Stenson’s duct calculi is the one of possible causes of suppurative parotitis. Because the presence of significant complicated conditions in children, prompt diagnosis and treatment such as surgical removal is needed. We have experienced a case of right supprative parotitis with Stensen’s duct sialolith in 2-year-old male. The right stenson’s duct calculi was diagnosed following axial computed tomography scan. The patient was treated with antibiotic therapy and sialolithotomy was done via intraoral approach. We report this case with a review of the literature.

미국법상 온라인 라이센스 계약의 유효성

Online vendors often seek to bind online consumers with contracts, frequently in the form of license agreements with standardized terms. Importantly, online vendors differ in the manner in which they present their license agreements and consequently, in the amount of notice they provide their consumers. Some automatically splash their license on the screen and require consumers to click an “I agree” icon; others merely create a link to their license and do not require a clicking acceptance. Courts have considered these various methods of online presentation, holding some licenses binding on consumers and others not. Courts have begun to utilize two broadly defined categories-clickwrap and browsewrap-in order to help characterize the various methods of presentation. A clickwrap license is an agreement in which the vendor requires the consumer to click an “I accept” icon or click-check an unchecked box for the agreement to take effect. The vendor may place the icon at the conclusion of an agreement, where the consumer must scroll downward through the entire list of terms to click on it. Alternatively, a court will likely characterize an online license as a browsewrap if the vendor utilizes a small link to the license, instead of automatically presenting it. Indeed, the vendor may even tuck the link in the corner of an interior webpage of the site so that consumers are unlikely to notice it. The characterization of an online license as either clickwrap or browsewrap is increasingly important because courts have found consumer assent to exist in the former, but not the latter. Beyond the recent cases dealing with online licenses, the case law surrounding shrinkwrap licenses is a useful guide to understanding what constitutes consumer assent in the online world. Accordingly, this article begins by analyzing the major cases surrounding shrinkwrap licenses and their relevance to licenses in cyberspace, and discusses various methods courts may use to strike or modify terms of an online license, even where the court finds consumer assent to the overall agreement. These methods include the unconscionability doctrine, consumer protection statutes, and federal preemption. U.S. courts will likely continue to find consumer assent in clickwrap licenses, but should rely on methods for striking or modifying terms within an online license.

레싱의 <현자 나탄>에 나타난 가족담론

본 논문은 오늘날 급격한 변모를 겪고 있는 가족문제를 생각하는데 레싱의 <현자 나탄>이 여러 가지 시사점을 제공할 수 있다는 전제에서 드라마의 가족상을 고찰한다. 드라마에 나오는 나탄의 가족은 나탄이 십자군 전쟁으로 아내와 자식들을 다 잃은 후에 양녀를 들이고 유모를 식구로 받아들여 새로 구성한 가족이다. 이것은 입양가족을 넘어 오늘날 대두되고 있는 ‘생활공동체 Lebensgemeinschaft’에 가깝다. 이 새로운 가족의 성공은 피는 물보다 진하다는 오래된 관념을 극복한 뒤에 가능하다. 레햐의 양아버지 나탄은 레햐를 친딸 이상으로 사랑하지만 동시에 탁월한 선생이 되어 강도 높은 교육을 시행한다. 집에서 나탄은 아버지와 교육자라는 이중 역할을 수행한다. 그의 교육은 책을 매개로 하는 것이 아니라 현실에서 발생하는 문제들을 소크라테스식 산파술로 해결하는 실천적 방식이다. 드라마는 가족의 범주 내에서 사랑, 교육, 종교, 우정의 문제를 천착해 나간다. 드라마의 마지막을 장식하는 감동적인 종교적 화해는 바로 가족적 가치와 보편적인 휴머니즘으로서의 우정이 하나가 되는 데서 성취된다. 요컨대, <현자나탄>은 핏줄과 무관한 삶의 공동체로서의 가족을 생각할 수밖에 없는 이 시대에 선구적인 모델을 제시한다는 점에서 다시 읽을 필요가 있는 고전이다.
