Subclinical Hypothyroidism: Prevalence, Health Impact, and Treatment Landscape

Subclinical hypothyroidism (sHypo) is defined as normal serum free thyroid hormone levels coexisting with elevated serum thyroidstimulating hormone (TSH) levels. sHypo is a common condition observed in clinical practice with several unique features. Its diagnosis should be based on an understanding of geographic and demographic differences in biochemical criteria versus a global reference range for TSH that is based on the 95% confidence interval of a healthy population. During the differential diagnosis, it is important to remember that a considerable proportion of sHypo cases are transient and reversible in nature; the focus is better placed on persistent or progressive forms, which mainly result from chronic autoimmune thyroiditis. Despite significant evidence documenting the health impacts of sHypo, the effects of levothyroxine treatment (LT4-Tx) in patients with sHypo remains controversial, especially in patients with grade 1 sHypo and older adults. Existing evidence suggests that it is reasonable to refrain from immediate LT4-Tx in most patients if they are closely monitored, except in women who are pregnant or in progressive cases. Future research is needed to further characterize the risks and benefits of LT4-Tx in different patient cohorts.

The novel viral disease COVID-19 is spreading globally, causing countless infected individuals and deaths. Thereare active discussions and debates on how to manage patients with chronic illnesses in the COVID-19 pandemicera. Since thyroid diseases are chronic and associated with autoimmune diseases as well as high tumors, thereis a need to assess the association between thyroid diseases and COVID-19. Recently, the American ThyroidAssociation and European Thyroid Association published a statement on thyroid disease management during theCOVID-19 pandemic. Further, clinical data from COVID-19 patients also indicate that COVID-19 may affect thyroidfunctions. Therefore, we reviewed published literature on COVID-19 and thyroid diseases and discussedapproaches to proper management of thyroid diseases during the COVID-19 pandemic.
급성심근경색증의 예후 지표로서 퇴원전 운동부하검사의 의의

24명의 급성 심근경색 남자 환자의 퇴원 전 운동부하 검사를 검토한 바 예후 판정의 지표로 유의한 변수는 5METS 미달하는 운동량, 분당 130미달하는 최고 심박수, 운동유발 협심증 및 심실성부정맥이었다. 이들 변수들의 심장 사고(급사, 재경색, 불안정협심증 등)의 적중율은 83∼100%, 특이도는 89∼100%, 예민도는 33∼53%이었다. Predischarge exercise test, obtained in 24 male patients with uncomplieated acute myocardial infarction, were analyzed to evaluate several variable of prognostic value to separate high risk group from low one and obtain guidelines for cardiac rehabilitation. The following results were obtained 1.Average age of 24 patients were 53 years, CPK 420U, duration of follow-up 22 months and test were done on 16th day of the disease. 2.Twelve cardiac events, occurred during the period of 22 months, were 2 reinfarction, 2 sudden death, 5unstable angina and 3 strokes. 3.Exercise test variables with high predictive value (83-100%) for subsequeut cardiac events were work load less than 5 METs, maximum heart rate less than 130/min, exercise induced angina and ventricular premature beats, in that order. Their specificity were very high (89-100%) in contrast to low sensitivity (33-53%). 4.No significant prognostic value were fecund in exercise time less than 6 minutes, double product, prior blood pressure response during exercise, ST depression less than 1 mm and ST elevation.

When and Why Does a Firm Choose a Certain Type of Internet Channel?YOO, Weon-Sang*This study explores the choice of Internet channels in the trucking industry. Two of the main Internet channels are considered: electronic transportation exchanges and company Websites. Six case studies are conducted in order to identify the key factors that influence a firm’s choice concerning the adoption of Internet channels. Based on these case studies, we generate a set of testable propositions that can be used as a foundation for future theoretical and empirical research. The case analyses highlight five main factors that influence the adoption of the Internet channels: size of the firm, percentage of business on the ongoing relationships, degree of fragmentation of the industry, transaction size, and size of the network.
