역설적 반응에 의하여 동시에 결핵성 뇌농양 및 결핵성 복막염이 발생한 증례 1예

역설적 반응은 감수성 있는 항결핵제를 복용중인 결핵환자에서 치료시작 당시의 병변이 악화되거나 새로운 병변이 생기는 현상이고 이는 치료실패와 감별을 요한다. 일반적으로 역설적 반응은 기존 항결핵 치료로 좋은 결과를 보이므로, 이러한 임상 경과를 잘 이해함으로써 불필요한 검사나 치료약제 변경 등을 하지 않고 적절히 치료할 수 있게 될 것이다. 저자들은 적절한 항결핵제 치료 중에도 역설적 반응으로 결핵성 뇌농양, 결핵성 복막염이 연속적으로 병발한 환자 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. While receiving appropriate treatment, patients with tuberculosis occasionally have unusual, paradoxical reactions, with transient worsening of lesions or the development of new lesions. This report is a case of tuberculosis brain abscess and tuberculosis peritonitis with intra-abdominal abscess that developed during appropriate anti-tuberculosis chemotherapy. A 45-year-old male patient had been diagnosed as with all-drug susceptible pulmonary tuberculosis with pleurisy. Subsequently, the patient underwent standard treatment with anti-tuberculosis therapy; the pulmonary lesions improved. Three months after initial treatment, the patient developed brain abscesses and peritonitis. With the addition of corticosteroid treatment, the patient`s neurologic symptoms were relieved. Exploratory laparotomy with surgical drainage was performed and a diagnosis of tuberculosis peritonitis was confirmed on biopsy. Anti-tuberculosis therapy was continued for 19 months, the patient improved eventually without further complications, although the therapeutic regimen had not been altered. In this case, the paradoxical response to treatment may have been involved in the pathogenesis of disease.
게임을 활용한 기능성 콘텐츠가 노인 당뇨환자의 자기효능감, 인지기능에 미치는 영향

본 연구팀은 2016년에 제 2형 당뇨환자 식이교육용 기능성 콘텐츠(롤리폴리 160)를 개발하였고, 이후 당뇨환자와 보건교육자의 요구를 반영하여 당뇨병 전반에 걸친 자가관리용 기능성 게임 콘텐츠로 수정·보완하였다. 이에 임상효과성을 실험하였다. 본 연구는 단일집단 전후비교 실험설계이며, G지역소재 C대학병원에 입원한 당뇨환자 49명을 대상으로 2017년 2월 27일부터 4월 21일까지 하루에 1회 이상, 총 5일 동안 "롤리폴리 160"을 통한 자가관리 교육을 실시하였다. 연구결과 교육 후 대상자의 공복혈당과 식후 2시간 혈당 모두 통계적으로 유의하게 감소하였으며(p<.000), 자기효능감, 인지기능은 통계적으로 유의하게 상승하였다(p<.000). In 2016, we developed serious content for diabetes (Roly Poly 160) for diabetes, and then revised and supplemented them to serious game contents for self-care throughout diabets, reflecting the needs of diabets and health educators. We investigated the clinical efficacy. This study was one-group pretest-posttest design, 49 people with diabets who were admitted to a hospital in the G region were given self-management training through "Roly Poly 160" for 5 days, from February 27 to April 21, 2017, once a day. As a result of the study, the fasting blood glucose level and the postprandial blood glucose level of the subjects were significantly decreased (p<.000), and self-efficacy and cognitive function were significantly increased (p<.000).
제2형 당뇨 식이 기능성 게임 ‘롤리폴리 160’의 효용성 및 만족도 조사 연구

당뇨병 자가관리 기능성 게임 ‘롤리폴리 160’은 제 2형 당뇨 환자에게 자가 식이 요법을 통하여 혈당치를 낮추게 하기 위하여 개발되었다. 만족도 조사를 위하여 2018년 11월 27일부터 12월 4일까지 ‘롤리폴리 160’을 사용 중인 광주광역시 소재 D구 보건소와 S구 보건소에서 관리중인 당뇨환자 및 일반인을 대상으로 만족도 설문조사를 실시하였다. ‘롤리폴리 160’을 사용하는 전체인원 525명 중에서 설문에 응대를 한 사람들은 67명 이었다. App에 대하여 전체 만족도 조사결과 5점 만점에 평균 3.7점이 나왔으며 연령대별로 차이가 있는 분석 결과가 도출되었다. A diabetes self-management serious game, "Roly Poly 160" is developed for Type Ⅱ diabetes patients to decrease their blood Sugar level. For verifying the effectiveness of "Roly Poly 160", we examined the satisfaction survey. Data were collected from the diabetes patients and public those who have used "Roly Poly 160" through D Community health center and S Community health center in Gwangju, from November, 27, 2018 to December, 4, 2018. 67 people returned the survey through the "Roly Poly 160" APP. The overall average satisfaction level was 3.7 out of 5.
지각된 서비스가치, 서비스전환비용과 고객충성도의 관계분석 -씨푸드 레스토랑을 중심으로-

The objective of this study was to identify relationships among customers` perceived value of service, services switching costs, and customer loyalty in seafood restaurants. In particular, this study focused on showing the factors of customers` perceived value including economic value, psychological value, and social value, which have a significant effect on services switching costs, which have an effect on customer loyalty. In this survey, 285 questionnaires were used for the statistical analysis, and the study examined reliability and validity of the data by using PASW Statistics 18.0. The results showed that perceived values, such as economic value and social value had a significant influence on switching costs, and switching costs also had an significant effect on customer loyalty.

본 연구는 제2형 당뇨 환자 식이교육 기능성 게임(Roly-Poly 160)을 개발하고 그 효과를 검증하고자 수행하였다. 당뇨환자의 식이교육 기능성 게임은 식사자가관리, 카드게임, 퀴즈게임으로 총 3가지 종류의 모듈로 이루어져 있다. 식사자가관리는 매일의 식사정보를 관리하여 12개월 동안의 변화추이를 관찰할 수 있도록 개발되었다. 카드게임은 권장식사메뉴를 바탕으로 변화된 카드를 제한시간동안 찾아내는 게임이며 퀴즈게임은 식이요법에 관한 퀴즈를 풀면서 지식을 습득하는 게임이다. 이 게임은 2016년 9월 28일부터 10월 28일까지 G지역소재 C병원과 G병원에 내원하여 당뇨식이교육을 신청한 제2형 당뇨병환자 30명에게 1인당 1시간씩 총 5회 실시하여 당뇨식이지식과 식후 2시간 혈당을 실험전과 후에 반복 측정하였다. 실험 후 제2형 당뇨환자의 당뇨지식은 실험전보다 통계적으로 유의하게 상승하였고(p=0.01), 공복시 혈당과 식후 2시간 혈당은 각각 통계적으로 유의하게 감소하여(p<.05) Roly-Poly 160의 임상효과를 검증하였다. The purpose of this study was to develop the customized diet education game (Roly-Poly 160) for type II diabetes and to test its effectiveness. The diet education game for type II diabetic is composed of three kinds of modules as Meal self-management, Card Game, and Quiz Game. Meal self-management was developed to manage the dietary information of each day and to observe changes in the 12-month period. Card game is to find a changed card based on the recommended meal menu during a limited time and Quiz game is to learn knowledge while solving the quizzes about diet. Data was collected from September 28th to October 28th, 2016 at C hospital and G hospital in G district, and 5 times for 1 hour for every 30 people with type II diabetes who applied for diabetic diet education. Knowledge of diabetes and 2 hour postprandial blood glucose were measured repeatedly before and after the experiment. After the Roly-Poly 160 experiment, the knowledge of type II diabetes was statistically significantly increased (p = 0.04), and the fasting blood glucose and the 2 hour postprandial blood glucose decreased statistically decreased (p <.05) and Roly-Poly 160 game clinical efficacy was verified.

A field study was performed to investigate the effect of water saving irrigation method on water use efficiency and rice yield. The field plot was 40a (40 × 100m) in size and located at Buryangmyun, Kimjae city, Chonbuk province. Field measurements were made during the growing seasons, May to September of the year 1998 and 1990. Irrigation water volume, drainage water volume, rainfall and ponding depth were measured. Irrigation water management practice employed was such that to keep the ponding depth about 3 to 4cm by intermittent irrigation with drying the soil surface until hair cracks emerge before the next irrigation. The amounts of water volume irrigated and drained were measured by pipe flow meter and ponding depth was observed by using a partly buried 120mm diameter PVC pipe. The results showed that the irrigation water depths, the rainfalls, and the drainage depths were 379mm, 458mm, and 448mm in 1988, and 274mm, 819mm, and 736mm in 1990, respectively. The average yield was 590kg per 10a. The water saving irrigation method saved irrigation water by about 20% with higher yield compared with the traditional method.
