        Idiopathic Atrial Standstill in an Old English Sheepdog Cross Dog

        An 1 year old intact male mixed dog (weighing 20 kg) was presented with primary complaints of abdominal distension and severe exercise intolerance. Electrocardiogram found profound bradycardia (46-79 beats/min) with atrial standstill. Laboratory studies found no particular abnormalities. Serum cortisol and T4 concentration were within normal range. Diagnostic imaging studies revealed generalized cardiomegaly, ascites, dilation in all cardiac chambers, absence of atrial contraction, absence of transmitral A-peak, mitral and tricuspid valve insufficiency and normal left ventricular systolic dimension. Based on diagnostic findings, the case was diagnosed as idiopathic atrial standstill. The dog was treated with conventional therapy for heart failure.

        Effect of Medetomidine and Combination of Medetomidine/tiletamine/ zolazepam and Medetomidine/tiletamine/zolazepam/tramadol on Echocardiographic Cardiac Contractility in Dogs

        This study evaluated the myocardial performance on echocardiography after the sedation/anesthesia of medetomidine (D), the combination of medetomidine and tiletamine/zolazepam (DZ), and the combination of medetomidine, tiletamine/zolazepam and tramadol (DZT) in Beagle dogs. Ten healthy adult Beagle dogs (weighing 8.6 ± 1.0 kg) were enrolled in this study. Heart rate (HR), fractional shortening (%FS), left ventricular ejection fraction (%LVEF), stroke volume (SV), cardiac output (CO), left ventricular internal diameter in systole (LVIDs) and left ventricular internal diameter in diastole (LVIDd) using M-mode echocardiography were measured prior to anesthesia, then every 10 min for 60 min. The HR, %FS, %LVEF, SV and CO were significantly decreased during sedation/ anesthesia with D, DZ and DZT combination of anesthesia. Although those anesthetic protocols provided acceptable quality of sedation/anesthesia, levels of cardiovascular suppression were substantial and persistent and thus the continuous monitoring on vital signs should be accompanied in any situation. Close attention is required for dogs with pre-existing heart diseases, when those anesthetic protocols were applied.

        전산화단층촬영을 이용한 폐엽구분

        Computed Tomograms of 45 patients with or without lung mass were reviewed to determine the characteristics of the major and minor fissures. Also pulmonary vascular distributions were analyzed to localize each pulmonary lobe. The results were as follows: 1. Each major fissure area was imaged frequently as lucent band, and less frequently dense band. 2. The minor fissure area was imaged frequently as lucent band, and less frequently dense band. 2. The minor fissure area was imaged frequently as lucent zones were as same frequency. 3. Avascular planes without above mentioned lucent zones were also correspond to minor fissure areas. 4. Slices of abascular zones were observated between pulmonary arteries to posterior segment or apico-posterior segment of upper lobe and to superior segment of lower lobe on both lungs.

        근위축성 측삭경화증의 MR 소견 : 1예 보고

        중추신경 운동계의 퇴행성 질환으로 국내에서는 보고가 드물었던 근위축성 측삭 경 화증(amyotropic lateral sclerosis)의 뇌자기공명영상(MR) 소견 1예를 보고한다. 운동신경계 질환의 가장 흔한 형태인 근위축성 측삭경화증은 상부 및 하부 운동신경원 모두을 침범하여 점진적인 운동악화와 구음장애, 심부건반사의 항진 등의 임상소견과 경과, 침범된 근육의 탈 신경화와 경축등의 근전도 소견을 근거로 임상적으로 진단해 혼 질환이다. 저자들은 상지 에서 하지로 진행되는 사지 근육악화와 구음장애, 침범된 근육의 탈신경화와 경축을 보인 근전도 소견, 지연된 운동신경 전도속도를 보이고, 감각신경계와 동안운동 신경계는 보존되 어 있는 59세 여자환자에서 시행한 뇌자기 공명영상에서 뇌피질척수경로에 양자밀도강조영 상과 T2강조영상에서 양측에 대칭적인 경계가 분명한 둥근 고신호강도가 관찰되고, 중심전 회운동피질에 T2강조영상에서 양측성의 대칭적인 약한 저신호강도가 보여 여타 질환과 혼 동 없이 근위축성 측삭 경화증을 진단하였다. Amyotropic lateral sclerosis(ALS) is a progressive degenerative illness of unknown cause; we present its characteristic brain magnetic resonance(MR) findings in one patient. A 58-year-old woman who for two years had been suffering from progressive motor weakness and dysarthria was admitted to our hospital. Physical examination and laboratory findings showed a pattern of both upper and lower motor neuron disease such as decreased motor power(Grade III), tongue atropy, increased deep tendon reflex, a pattern of lower motor neuron disease, as seen on electromyogram, and a pattern of sparing sensory nervous system, extraocular muscle movement, bladder, and bowel function. On axial brain MR proton-density and T2-weighted images, small round areas of high signal intensity were seen bilaterally in the posterior limb of the internal capsule; these corresponded to the corticospinal tract. Additionally, bilateral, subtle linear low signal intensity in the precentral gyrus was noted on T2-weighted imaging. On the basis of the findings of clinical and laboratory examination, and of typical MR imaging findings, ALS was diagnosed.

        이번 연구는 비글 개에서 medetomidine(D), medetomidine, tiletamine/zolazepam(DZ) 합제, medetomidine, tiletamine/zolazepam, tramadol(DZT) 합제를 이용한 진정/마취 후 심근 기능을 평가하였다. 10 마리의 건강한 성견 비글(체중 $8.6{\pm}1.0$ kg)를 이번 연구에 사용하였다. M-mode 심장초음파를 사용하여 심박수(HR), 구획단축률(%FS), 좌심실 박출기계수(%LVEF), 박출량(SV), 심박출량(CO), 수축기 좌심실 내강 직경 (LVIDs), 이완기 좌심실 내강 직경(LVIDd)을 마취 전, 그리고 마취 후 10분 간격으로 60분 동안 측정하였다. HR, %FS, %LVEF, SV, CO 는 D, DZ 합제, DZT 합제 로 진정된 동안 심각하게 감소하였다. 이번 실험에 사용된 마취 프로토콜은 사용 가능할 정도의 진정/마취가 이뤄졌지만, 심혈관계 압박의 정도가 심했고 지속적이었고, 따라서, 어떠한 상황에서도 생체지수에 대한 모니터링이 동반되어야 한다. 본 마취 프로토콜은 심장질환을 가지고 있어서 심맥관계 억압의 가능성이 높은 환자에서는 주의해서 사용해야 한다.
