병원 아웃소싱 근로자의 QWL과 이직의도에 관한 분석 - 이중몰입의 조절효과를 중심으로 -

[purpose] This study was conducted to identify how a hospital's outsourcing workers' QWL influences Turnover Intention and to determine Dual Commitment cause any discrepancies in regards to the link between QWL and Turnover Intention. [Methodology/Approach] The study is based on 254 survey sheets answered by outsourcing employees working at general hospitals with more than 300 beds in Seoul and the methods of analysis used are as follows: reliability analysis, correlation analysis, multiple regression analysis, and hierarchical regressions analysis. [Findings] The study results First, analyzing how different types of QWL (individual, organizational, managemental) influence Turnover Intention, R2 turned out to be 30.7% and organizational factors and managemental factors show a statistically significant difference. In addition, organizational and managemental factors have negative effects on Turnover Intention while that of organization factors has a bigger influence. Second, verifying regulation effects of Dual Commitment in regards to the link between QWL and Turnover Intention, it is learned that Dual Commitment (hiring firm) has no regulation effects on the link. However, it does have a regulation effect on the link between QWL (managemental) and Turnover Intention. Moreover, after verifying regulation effects of Dual Commitment (client's firm), it is learned that it has regulation effect on the link between QWL (organizational and managemental) and Trunover Intention. [Practical Implications] In conclusion, lower Turnover Intention, it is necessary to impose tasks that suit the employees' competence and to encourage efficiency throughout clear communications and accurate directions.
관골골절에 있어 관상피판 접근법시 혈액학적 변화에 대한 연구

Background : The coronal incision is versatile surgical approach to upper and middle region of the facial skeletal including the zygomatic arch. The advantages of coronal approach are minimal injury of facial tissue including facial nerve and satisfactory cosmetic result by hidden scar at hair. But wide exposure of scalp, its disadvantages are operation time and massive blood loss. Methods : Thirty patients undergoing elective surgery were divided 3 groups. Group I used only coronal approach, group II used coronal with subciliary approach and group III used coronal with subciliary and intraoral approach. And then retrospected of the preoperative, postoperative red blood cell count, hemoglobin(Hb), hematocrit, transfused red blood cell units and platelet cell units, and the amount of infused crystalloids and colloids, and postoperative hemovac count was estimated. Results: 1. Red blood cell count were decreased in all groups at immediated postoperation and decreased in all group of postoperative first day and decreased in group I, II but increased group III of postoperative third day. 2. Hemoglobin and hematocrit were decreased in all group at immediated postoperation and decreased in all group of postoperative first day and decreased in group I, II., but increased group III postoperative third day. 3. Platelet was decreased in all group at immediated postoperation, and decreased in group II, III but increased in group I of postoperative first day and decreased in group I but increased group II, III of postoperative third day. 4. Mean postoperative hemovac mean drainage group I of first day is $48.63{\pm}21.12ml$ and second day is $23.92{\pm}19.53ml$ and third day is $7.82{\pm}5.32ml$ and group II of first day $60.45{\pm}22.65ml$ and second day is $22.14{\pm}13.21ml$ and third day is $7.32{\pm}6.25ml$. III group of first day $58.16{\pm}10.13ml$ and second day is $21.27{\pm}11.72ml$ and third day is $7.13{\pm}4.90ml$. 5. Infusion of group I is mean PRC $1.08{\pm}0.91$ pint, FFP $1.03{\pm}0.75$ pint, crystalloid $2562.23{\pm}1345.53ml$ and group II is mean PRC $1.05{\pm}0.89$ pint, FFP $1.71{\pm}0.78$, crystalloid $2650.47{\pm}1096.36ml$ and group III is mean PRC $1.79{\pm}1.45$ pint, crystalloid $3295.43{\pm}1472.432ml$.

Nephrogenic fibrosing dermopathy (NFD) is a rare cutaneous fibrosing disorder that primarily affects patients with a history of renal disease. NFD manifests with induration, thickening and hardening of the skin with brawny hyperpigmentation. Lesions are typically symmetrical and usually develop on the limbs and trunk. Flexion contractures of the joints may be a feature of the disease. Histopathological features of NFD include proliferation of dermal fibroblasts and dendritic cells, thickened collagen bundles, increased elastic fibers and focal mucin deposition. Although the pathogenesis remains largely unknown, some of the factors implicated in the pathogenesis include renal dysfunction, circulating fibrocytes, vascular injury, and gadolinium which is a contrast material used in magnetic resonance imaging. Currently, no definitive or uniformly effective therapies are available for the treatment of NFD. We herein describe the case of a 44-year-old female NFD patient who undergoes significant improvement of skin lesions and associated joint contracture after renal transplantation. The steam generator tubes of power plant damaged by sliding wear due to flow-induced motion of foreign object. Amount of wear have been predicted by Achard's wear equation until now. However, there are large error and low reliability, because this equation regards wear coefficient(k) as constant. The sliding wears tests have been performed at room temperature to examine parameters of wear (wear distance, contact stress). The steam generator tube material for wear test is used Inconel 600 and foreign object material is used 304 austenite stainless steel. The sliding wear tests show that the amount of wear is not linearly proportional to the wear distance(for 374 austenite stainless steel). According to experimental result, wear coefficient is not constant k but function k(s) of wear distance. The newly modified wear predictive equation V=k(s)F have small error and high reliability.
병원 2세경영의 리더십 변화가 내부환경 및 조직유효성에 미치는 영향

This study aims to examine the effect of a change of leadership on internal environment and organization effectiveness and suggest measures on effective organizational management by analyzing a change of leadership before and after 2nd generation management. There was analysis of a difference of leadership type, organizational structure, organizational culture, management resources, job satisfaction, and organizational commitment between the period of the founders and the period of 2nd generation successors by collecting data from employees of 6 hospitals in Seoul. Also there was analysis of the effect of a change of leadership on internal environment and organizational effectiveness. According to the analysis, it was demonstrated that the factor affecting job satisfaction during the period of the founder was complexity of organizational structure, and there were no factors affecting organizational commitment. In contrast, during the period of the 2nd generation successors, it was revealed that transformational leadership, centralism of organizational structure, and human resources of management resources affect job satisfaction, and transformational leadership, complexity of organizational structure, hierarchical culture of organizational culture, and human resources of management resources affect organizational commitment. Further, after the succession from the founders to 2nd generation successors, as transformational leadership increased, job satisfaction rose, and as developmental culture was reinforced and human resources increased, organizational commitment was heightened. Furthermore, as transformational leadership increased and developmental culture was reinforced, organizational commitment was heightened. Namely, as transformational leadership was reinforced resulting from the succession to 2nd generation, employees`` job satisfaction and organizational commitment increased, with circumstances aiming for the actual change.

Purpose: We are to describe the clinical characteristics and treatment outcomes in patients aged between 80 and 89 years who underwent inguinal hernia repair under local anesthesia. Methods: We retrospectively reviewed the medical records of patients aged between 20 and 89 years who underwent tension free hernia repair under local anesthesia by a single surgeon between June 2001 and January 2009 and compared clinical characteristics and outcomes between octogenarians who were the eldest and sexagenarians whose incidence was the highest. Results: Of the 514 patients, the number of octogenarians was 52 (8.6%) and sexagenarians were 225 (35.0%). Body mass index (BMI) was 22.5 ㎏/㎡ in octogenarians and 23.7 ㎏/㎡ in sexagenarians (P=0.003). Underlying diseases were present in 67.3% of octogenarians and 73.5% of sexagenarians (P=0.238). The incidence of liver disease was significantly lower in the octogenarians (1.9% vs 15.7%, P=0.004). The proportion of patients who underwent local anesthesia was significantly higher among the octogenarians (94.5% vs 82.2%, P=0.014). Operative time and postoperative hospital stay had no significant difference between both groups. Postoperative complications developed in 4 (7.7%) of the octogenarians and in 18 (9.7%) of the sexagenarians. Scrotal swelling was developed most frequently and surgery-related mortality did not developed in both groups. Conclusion: If local anesthesia is used appropriately, inguinal hernia repair in octogenarians is a safe procedure as like in sexagenarians and may reduce the psychiatric burden related to complications for both surgeons and elderly patients.
