        HER2 양성 간세포양 위선암 1예

        Gastric hepatoid adenocarcinoma is a rare adenocarcinoma that develops in the stomach. The prognosis of gastric hepatoid adenocarcinoma is poorer than that of ordinary gastric adenocarcinoma. Here, we report the first case of human epidermal growth factor receptor 2 (HER2)-positive gastric hepatoid adenocarcinoma in Korea. A 57-year-old male presented with abdominal distension and underwent endoscopic gastric biopsy and percutaneous core needle liver biopsy. The pathological findings were consistent with HER2-positive gastric hepatoid adenocarcinoma. He received six cycles of chemotherapy with cisplatin-capecitabine plus trastuzumab, which is a HER2 targeted agent. After chemotherapy, a follow-up abdominal computed tomography scan showed a partial tumor response. This case emphasizes the importance of using trastuzumab in a patient with HER2-positive gastric hepatoid adenocarcinoma. (Korean J Med 2016;91:211-215)

        발파로 인한 노천광산 사면안정성 평가 및 인자분석

        노천채광에서 경제적인 채광을 위한 채광장 형성은 매우 중요하며, 대규모 채광을 위한 발파작업은 사면의 안정성에 영향을 미칠 수 있으므로 이에 대한 고려가 필요하다. 본 연구에서는 사면설계와 발파 조건을 달리하면서 사면 안정성에 미치는 영향 인자의 기여율을 검토하였다. 설계인자는 암반의 물성, 사면각도, 벤치높이이며, 발파 조건은 장약량과 거리를 달리하여 반영하였다. 사면의 안전율은 3차원 모델링을 통해 전단강도감소기법으로 산출하였으며, 기여율은 암반사면의 물성이 94.8%로 다른 설계인자들에 비해 상대적으로 높았으며, 사면각도 0.89%, 벤치높이 0.58%, 발파는 3.73% 정도의 영향을 미치는 것으로 나타나, 사면과 근접거리에서의 발파는 사면의 안정성에 영향을 줄 수 있음을 알 수 있다. In surface mining, it is very important to create a mining area for economical mining. This study examined the contribution of design factors on slope stability with different slope design and blasting conditions. The design factors were the properties of the rock, the slope angle and the bench height, and the blasting conditions were reflected at different explosive weight and distances. The safety factor of slope was calculated by shear strength reduction method through 3D modeling, and the contribution rate of rock slope was 94.8%, which is relatively higher than other design factors, slope angle 0.89%, bench height 0.58%, and blasting It is shown that it affects about 3.73%, and it can be seen that blasting at a close distance can affect the stability of the slope.

        파괴인성의 축소모형실험 적용 연구

        암석의 파괴인성은 발파, 굴착 등으로 인한 균열의 개시와 전파를 나타낼 수 있는 상수이다. 터널의 거동, 석회석 광산의 안정성 평가 등에 축소모형실험이 다양하게 적용되고 있다. 축소모형을 통해 발파로 인한 손상영역평가도 이뤄지고 있는데, 파괴 관련 인자에 대한 축소율 적용은 이뤄지지 않고 있다. 본 연구에서는 DCT(diametral compression test) 값과 유한요소법인 ATENA2D 수치해석 결과를 비교하여 암석의 파괴인성에 축소율을 적용할 수 있는지 확인하였다. 암석의 파괴인성에 이론적으로 계산된 축소율을 적용한 값과 DCT 시험결과 및 수치해석 결과가 각각 0.21~ 0.46, 0.40, 0.99MPa√m로 편차가 있으므로 암석의 파괴인성에 축소율을 적용 시에는 이 세 가지 값을 고려하여 적합한 축소율을 도출해야하고, 축소모형 제작 시 축소율 적용 대상이 되는 길이, 시간, 질량과 함께 이로부터 산출되는 일축압축강도, 밀도 등의 주요 설계인자들의 축소율이 적용된 값을 함께 검토해야 할 것이다. Fracture toughness of rock is a constant that can indicate the initiation and propagation of cracks due to blasting, excavation, etc. Scaled model tests have been applied to the behavior of tunnels and the stability of limestone mines. Through the scaled model, damaged zone evaluation due to blasting is also carried out, and the scale factor is not applied to the failure-related factors. In this study, DCT (diametral compression test) and finite element method ATENA2D numerical analysis results were compared to determine whether the scale factor could be applied to the fracture toughness of rock. The theoretical values of the scale factor applied to the fracture toughness of the rock and the DCT test results and the numerical results are 0.21~0.46, 0.40, and 0.99MPa√m respectively, so these three values should be considered when determining scale factor. It is necessary to derive a suitable scale factor in consideration of the length, time, and mass to which the scale factor is applied, as well as the values of the scale factor of major design factors such as uniaxial compressive strength and density.

        성견치주질환 이환 발치와내 이식 치근과 발치와 치조골 재생에 대한 연구;I. 치근활택술의 영향

        The authors have transplanted periodontally involved roots which had been root planed into healthy and periodontally involved extraction sockets, and studied the root resorption patterns as well as its effect on new bone formation and wound healing. Alveolar bone around mandibular premolars of 6 adult dogs has been surgically removed, followed by ligation of orthodontic elastic wires for 8 weeks inducing chronic periodontal disease. After removing the crown portions, roots were extracted, and notches were made on the root surfaces discriminating healthy and periodontally involved areas using burs. Controls and experimental groups were divided as follows. Control I : Transplantation of periodontally involved root into healthy extraction sockets. Control II : Transplantation of periodontally involved root into diseased extraction sockets. Experimental group I : Transplantation of root planed roots into healthy extraction sockets. Experimental group II : Transplantation of root planed roots into diseased extraction sockets. Extraction sockets were sutured after transplantations, completely submerging the roots. Healing progress was histologically observed at 2nd, 8th, 12th, and 20th weeks, and the results were as follows ; 1. No inflammation or infection within the extraction sockets had been observed in all groups throughout the experimental period. 2. Reversal lines were observed at week 2 in all groups, clearly discriminating socket walls and new bone, and numerous blood vessels were observed in the new bone trabeculae. 3. Experimental groups showed markedly less root resorption compared to the controls at week 2, but as time progressed, severe resorptions were present in all groups. 4. Localized areas of new bone ankylosis were observed, and the rest of the areas showed collagen fiber insertion with new bone formation at its periphery. 5. No clear differences were found in healing and alveolar bone regeneration between healthy and diseased extraction sockets. 6. The amount of root resorption and ankylosis had increased up to week 8 and 12, showing ankylosis of new bone and the roots. However, no further increase in ankylosis was observed at week 20. 7. Most of the cementum on healthy roots was directly ankylosed to new bone at week, 2, and were gradually resorbed and replaced by new bone thereafter. These results appear to indicate that root planing may inhibit early root resorption of transplanted roots, but gradual replacement by alveolar bone and collagen fibers eventually occur. Condition of the roots or presence of disease in extraction sockets do not appear to make marked differences in alveolar bone regeneration process.

        다층 주방식 채광 광산에서 낙반이 채광장 천반과 광주에 미치는 영향

        다층 주방식 광체의 개발에서 상부 채광장 천반의 붕락이 하부 편 광주에 미치는 영향을 보기 위하여 AUTODYN으로 수치해석 하였다. 그 결과 하부 채광장 천반에서의 최대 변위와 응력은 각각 0.001mm LC 36MPa 정도였고, 광주에서의 최대 변위와 응력은 각각 0.0003 mm 및 3 MPa 정도였다. 상부 채광장의 천반암석의 절반이 붕락하는 상황을 가정했을 때 하부 채광장 천반과 광주의 손상도는 0.03 정도로 나타났다. Influence of rock fall from upper-level roofs to lower-level roofs and pillars at a multi layered room and pillar mine was numerically simulated by using AUTODYN. The analysis results showed that the maximum displacement and stress in the roof of the lower-level stope are respectively 0.001 mm and 36 MPa, and those in the pillars of the lower-level stope are 0.0003 mm and 3 MPa. The maximum damage levels in the roof and pillar of the lower-level stope were evaluated to be about 0.03 when a half of the roof rock of the upper-level stope was assumed to be fallen to the floor.

      • 초음속 공기 흡입구 개념설계 및 성능 예측 프로그램

        압축성 유체역학 이론에 기초한 초음속 공기 흡입구 개념설계 및 탈설계점 성능 예측 프로그램을 개발하였으며, batch 프로세스를 이용하여 별도의 상용 프로그램을 이용한 후처리까지 일괄 처리하도록 패키지를 구성하였다. 본 논문에서는 마하수 3.0 조건에서 출입구 면적비에 따른 축대칭 및 사각형 흡입구 설계 결과를 제시하였으며, 이 결과를 바탕으로 한 탈설계점 성능 예측 결과를 제시하였다. A design tool is developed for the conceptual design of the supersonic air intake based on the compressible fluid dynamics theories. A performance prediction tool is also incorporated into to the code to evaluate the performance at off-design conditions. The package includes the post-processing by a separate commercial program by using a batch process. Axi-symmetric and rectangular intake designs are presented at Mach number 3.0 flight condition using the ratio of exit area /capture area as design variable. The off-design performance of the designs are also presented.

        Benign cystic lesions, have to be differentiated, are common in the calcaneus, Among the lesions the intraosseous lipoma is extremely rare tumor in bone. In 1996, Unni reported 7 in 11,087 cases of bone tumors at Mayo Clinic. In Korea, Ahn reported the first intraosseous lipoma in the distal fibula. We experienced two cases of intraosseous lipoma of calcaneus, and report the clinical, radiological and pathological findings with a review of the literatures.

        GTR치료시 e-PTFE 차단막의 노출에 따른 하악이개부의 치유효과

        In order to evaluate the effects of the early exposure of e-PTFE membrane on the periodontal regeneration, 21 cases of 21 patients diagnosed as the chronic adult periodontitis were evaluated. All were class II furcation involvement cases. The control group was composed of 7 cases treated only by the flap operation. 14 cases were treated by the e-PTFE membrane as the experimental group, the membranes of 7 cases were exposed more than 1mm during healing period, which were named as the experimental group I, and the others, experimental group II. Clinical parameters such as probing pocket depth, clinical attachment level, bone probing depth, and gingival recession were recorded before the treatment and 6 months after the treatment. The results were as follows. 1. Significant probing depth reductions were observed for all groups(p<0.05), but no group shows significantly greater reductions than another. 2. Significant clinical attachment gains were observed for the experimental group II(p<0.05), no significant gains were observed in the other groups. 3. Significant bone probing depth reductions were observed for the experimental group II(p<0.05), no significant reductions were observed in the other groups. 4. All but the experimental group II exhibited a significant increase in gingival recession(p<0.05). The result suggested that is case of the e-PTFE membrane is exposed, the result is similar to that of flap operation without membrane. Therefore selecting the proper treatment case, intricate surgical procedure and infection control are essential for minimizing the chance of membrane exposure and finally for the good treatment results.

      • 건물통합도면과 연계한 지진 시 지반 액상화 위험지도 작성 방안 연구

        본 연구에서는 재난 예방 및 완화, 준비와 계획, 대응, 복구 전 단계에 걸친 효과적인 재난관리대책 마련을 위해 건물통합도면과 연계한 액상화 위험지도 작성 방안을 제안하였다. 각 지점별로 도출된 지반 액상화 가능성지수(LPI) 및 액상화 심각성지수(LSN)를 기반으로 Kriging기법을 통해 격자형태로 보간하여 포항시를 대상으로 액상화 위험지도를 작성하였다. 이를 국토교통부 건물통합도면 데이터와 연계하여 3차원 공간정보로 표현함으로써, 지진 발생 시 공공시설물 및 다중밀집시설 등 재난피해가 높을 것으로 예상되는 건물에 대한 위험성을 평가하고자 하였다.
