        사회적 자본 이론으로 살펴본 폐자원 수거 노인의 삶에 관한 질적 연구

        The purpose of this study is to explore the life of waste picking older adults by utilizing the social capital theory developed by Bourdieu, Coleman, and Putnam. Using qualitative research methods, we studied the background of older adults who have made money by picking up waste resources, their experiences in picking up waste resources, and their continued doing it despite of various difficulties. Interviews with 16 elderly and nine people around them, including junk shop officials and social workers, and data from participatory observation were analyzed using the thematic analysis method. The results are presented as follows. First, the life of older adults who pick up waste resources can be examined in six types, depending on when to enter the world of waste resources-picking up and the purpose of the activity, and the way the elderly interact with the environments in the process of collecting, managing, and selling waste resources. Second, although the background of waste resource collection and the appearance of social capital working are slightly different, making money by picking up waste resources is reasonable choices according to their circumstances. Third, older adults interact with the world with long-lived neighbors of similar circumstances who know the situation intimately, and the world that threatens and ignores the ‘old man who is picking up’,giving meaning to activities of picking up waste resources, and further giving meaning to their own lives. Fourth, despite various difficulties such as low income compared to time and labor, negative societal attitudes toward them, and poor health, the activities of picking up waste resources continues because they have become an expression of their familiar lifestyle, a way to participate in society, and a way to communicate with the world. Based on the results of this study and related discussions, we suggested a few ways of improving the quality of life and expanding social capital for older adult street waste pickers. 본 연구의 목적은 부르디외, 콜맨, 퍼트넘 등에 의해 발전된 사회적 자본 이론을 활용하여 폐자원 수거 노인의 삶을 탐색하는것이다. 질적 연구방법을 통해 노인들이 폐자원 수거의 길로 들어선 배경, 이들의 폐자원 수거 활동의 경험, 그리고 여러 가지 어려움에도불구하고 이들이 폐자원 수거를 계속하는 현상을 사회적 자본이라는 ‘일깨우는 개념’을 활용하여 살펴보고자 하였다. 16명 폐자원수거 노인과의 면담, 고물상 관계자, 사회복지사 등 주변 인물 9명과의 면담, 그리고 참여관찰의 자료를 중심주제 분석 방법으로 분석하였다. 분석 결과는 다음과 같다. 첫째, 폐자원 수거 활동 진입 시기 및 활동 목적, 그리고 폐자원 수거, 관리, 판매하는 과정에서 노인이관계 맺고 교류하는 방식에 따라 폐자원 수거 노인의 삶의 모습을 여섯 가지 유형으로 살펴볼 수 있음. 둘째, 폐자원 수거의 길로들어선 배경과 사회적 자본이 작동하는 모습은 조금씩 달라도 폐자원 수거는 이들의 형편과 상황에 맞춘 합리적 선택임. 셋째, 노인들은사정을 속속들이 아는 비슷한 형편의 오랜 이웃과의 세계, 그리고 ‘폐지나 줍는 노인’을 무시하고 위협하는 세계와 상호작용하며 폐자원수거 활동에 대해 의미 부여, 나아가 자신의 삶에 대해 의미 부여함. 넷째, 시간과 수고 대비 낮은 소득, 부정적인 사회 인식, 건강악화 등 여러 가지 어려움에도 불구하고 폐자원 수거를 계속하는 것은 폐자원 수거 활동이 이들에게 익숙해진 삶의 방식의 표현이자사회 참여 방식, 그리고 세상과 소통하는 방식이기 때문임. 본 이론 주도 연구는 사회적 자본 개념이 저소득 노년의 삶을 이해하는하나의 이론적이고 발견적인(heuristic) 도구로서 충실함을 보여준다. 본 연구를 통해 불평등하게 축적된 사회적 자본의 결과뿐 아니라폐자원 수거 노인의 삶에 사회적 자본이 형성되는 과정, 그리고 그 과정에서 노인과 주변의 역할과 상호작용을 살펴볼 수 있다. 사회적자본의 틀에서 볼 때, 노인에게 폐자원 수거 활동은 자신의 상황에서 취할 수 있는 최선의 합리적인 선택일 뿐 아니라 구성원 간의기대와 의무를 기반으로 하는 상호호혜적인 사회활동이며, 나아가 사회 참여 방식이자 세상과의 소통방식일 수도 있다. 본 연구 결과와관련 논의를 바탕으로 폐자원 수거 노인의 삶의 질 향상과 사회적 자본 확장을 지원하는 방안을 제안하였다

        영국 커뮤니티라이브러리의 프로그램 운영 및 공간 구성에 관한 연구 - 런던 루이샴자치구 4개관을 중심으로 -

        This study aims to analyze the guidelines and cases of Community Library in UK from which to derive insights of program operation and space organization of Small Libraries in South Korea. Community Libraries and Small Libraries serve as a community center of local community with the smallest scales of library spaces. Over the past decade, Small Libraries in Korea have failed to enhance the quality of library service due to the lack of guidelines and exemplary cases; on the contrary, the UK government has offered more specific and various guidelines and exemplary cases for Community Libraries in terms of its program and spaces. In this background, this study reviews the library guidelines of UK government, and analyzes the four cases of Community Libraries of Lewisham in London, based on the field research and interviews of the key library staffs. The findings point out that the programs should be shaped by local needs, in particular the needs of disadvantaged groups; and the spaces are needed to be flexible and integrated to deal with the limited areas, using movable furniture. This study is expected to be a useful database to propose the guidelines for Small Libraries in Korea in follow research.

        International Classification of Diseases 10th edition-based disability adjusted life years for measuring of burden of specific injury

        Objective We aimed to develop an International Classification of Diseases (ICD) 10th edition injury code-based disability-adjusted life year (DALY) to measure the burden of specific injuries. Methods Three independent panels used novel methods to score disability weights (DWs) of 130 indicator codes sampled from 1,284 ICD injury codes. The DWs were interpolated into the remaining injury codes (n=1,154) to estimate DWs for all ICD injury codes. The reliability of the estimated DWs was evaluated using the test-retest method. We calculated ICD-DALYs for individual injury episodes using the DWs from the Korean National Hospital Discharge Injury Survey (HDIS, n=23,160 of 2004) database and compared them with DALY based on a global burden of disease study (GBD-DALY) regarding validation, correlation, and agreement for 32 injury categories. Results Using 130 ICD 10th edition injury indicator codes, three panels determined the DWs using the highest reliability (person trade-off 1, Spearman r=0.724, 0.788, and 0.875 for the three panel groups). The test-retest results for the reliability were excellent (Spearman r=0.932) (P<0.001). The HDIS database revealed injury burden (years) as follows: GBD-DALY (138,548), GBD-years of life disabled (130,481), and GBD-years of life lost (8,117) versus ICD-DALY (262,246), ICD-years of life disabled (255,710), and ICD-years of life lost (6,537), respectively. Spearman’s correlation coefficient of the DALYs between the two methods was 0.759 (P<0.001), and the Bland-Altman test displayed an acceptable agreement, with exception of two categories among 32 injury groups. Conclusion The ICD-DALY was developed to calculate the burden of injury for all injury codes and was validated with the GBD-DALY. The ICD-DALY was higher than the GBD-DALY but showed acceptable agreement.

        바이오의약품 접근성 확대 방안: 유럽과 미국의 사례를 중심으로

        Backgrounds: Biopharmaceutical market is the fastest growing market in the pharmaceutical industry. However, since better drugs are sold at higher price, these drugs are less accessible by the patient population. Methods: For this study, literatures from 2018 to 2023 were searched. Pubmed (MEDLINE), Web of Science Core Collection, and Google scholar were selected for electronic databases. As a keyword ('biosimilar' OR 'biosimilars') AND ('regulatory' OR 'guideline'', 'ICH' OR 'FDA' OR 'EMA') were used, and the search was conducted by specifying that the key words should be included in the title and abstract. Result: Efforts of regulators such as EMA(European Medicines Agency) and FDA (U.S. Food and Drug Administration) - reducing approval requirements by omitting animal testing and clinical trials and promoting market penetration of biosimilars - are needed to expand market entry of biopharmaceuticals. Furthermore, expanding the indication of biosimilars through extrapolation and approving biosimilars' substitution and switching can also improve accessibility of biopharmaceuticals. Lastly, the expansion of biosimilar awareness and the prevention of illegal transactions will also contribute to the improvement of accessibility of biopharmaceuticals. Conclusion: The significance of this study is that it examines how the use of biosimilars can be promoted and ultimately the accessibility of biopharmaceuticals can be expanded, focusing on specific cases in Europe and the United States.
