We irradiated the ears of C3H mice with UVB 200mJ/㎤ and 400mJ/㎤ in a single dose at one time and 5 fractionated doses for 5 days and 10 fractionated doses for 10 days and performed biopsies on the ears of the control and experimental groups after, 2, 7, 14, 21 days of irradiation and stained them by the immunoperoxidase method. The results are summarized as follows. 1. A more significant decrease in the number of Ia-positive epidermal Langerhans cells was observed in single-dose-exposed group than fractionated-dose-exposed groups on the second day of irradiation. 2. On the seventh day of exposure, the number of the Ia-positive epidermal cells in single-dose-exposed group returned to normal, but in fractionated-dose-exposed groups the number of Langerhans cells decreased most remarkably. 3. In fractionated-dose-exposed groups, the number of Langerhans cells returned to normal on the 14th or 21st day of expposure. (Kor J Dermatol 29(4) : 459-465, 1991)

The antigenic potential of CFA-001, cefazolin, a cephalosporin derivative produced by an enzymatic semisynthesis, was determined in Hartley guinea pigs. A battery of tests employed consisted of active systemic anaphylaxis (ASA), passive cutaneous anaphylaxis (PCA), and indirect hemagglutination test (IHA). The results were as follows: 1) In ASA, no signs attributable to anaphylaxis was observed in guinea pigs sensitized with CFA-001, whereas OVA-sensitized animals induced severe anaphylactic symptoms; 2) guinea pigs did not produce antibodies against CFA-001 when sensitized with or without Freund's complete adjuvant (FCA) in homologous PCA tests. Meanwhile, antibodies against ovalbumin (OVA) were clearly detected; 3) No CFA-001-specific hemagglutination was observed in the IHA using sera obtained from CFA-001-sensitized guinea pigs. These results suggest that CFA-001 has no antigenicity potential in guinea pigs.

Background: There are several active tuberculosis (TB) cases in Korean high schools each school year. The risk of transmission in schools is extremely high due to the considerable time spent in closed classrooms. We evaluated the control of latent tuberculosis infection in Korean high schools. Methods: When a student was identified with active TB, tuberculin skin testing was performed on their classmates and on students in their same school grade. When a student had a positive tuberculin skin tests (TST), they underwent follow-up testing with QuantiFERON-TB Gold In-Tube (QFT). The manufacturer recommended a cut-off of 0.35 IU/mL to determine QFT positivity was applied. Results: A total of 131 pulmonary tuberculosis (TB) patients were included based on the criteria for screening TB contacts in the National Tuberculosis Control Program. Seventy-five (57.2%) students tested smear positive. TST were performed on 7,109 students who were classmates of, or in the same grade as, a TB patient. Of the contacts, 1,231 students (17.3%) were TST positive and they were screened with QFT. Six hundred-sixty-six (55.0%) of the tested students returned a positive QFT result and the rate of positivity was significantly associated with the increasing size of TST indurations (p<0.0001). Conclusion: The use of QFT resulted in approximately 45% of TST positive students not being given chemoprophylaxis.

Background: Adipose tissue is recently recognized as not only energy reservoir but also endocrine organ producing proin ammatory cytokines. Especially the impact o visceral adipose tissue(VAT) in critical illnesses has been proposed, but researches on the association between VAT and sepsis were scarce and quantitative measurement of VAT had not been used. Methods: In this retrospective cohort study, we enrolled patients with sepsis who examined abdominal computed tomography(A-CT) within 1 month of occurrence of sepsis, among the patients admitted to intensive care unit(ICU). Age, sex, anthropometric values, comorbidities and APACHE II score were reviewed. The areas of VAT and total adipose tissue(TAT) on the section of A-CT image of the umbilicus level were measured by calculating pixels presenting fat density. Results: Among 287 patients admitted to ICU due to sepsis, 178 patients were included for this study. Median age was 65 and 59.0% were men. In-hospital mortality rate was 57.9%. Women had higher TAT and subcutaneous adipose tissue, lower VAT/TAT ratio compared to men. The amount of VAT and VAT/TAT ratio were higher in in-hospital mortality group than in survivor group(90.41cm2 vs. 63.83cm2 and 45.88% vs. 32.79%, p=0.001 and <0.001, respectively). After adjusting age, sex, APACHE II score and comorbidities, multivariate logistic regression analysis revealed that the amount of VAT and VAT/TAT ratio were independent prognostic factors of sepsis with obvious dose-dependent relationship(VAT/TAT ratio quartile 3: OR 8.832, p<0.001 and quartile 4: OR 29.477, p<0.001, compared to quartile 1 respectively). Conclusion: The amount of VAT and VAT/TAT ratio quantitatively measured by A-CT were positively correlated with mortality in sepsis, and this association was dose-dependent. Visceral obesity should be considered as the poor prognostic factor of sepsis.

This study was conducted to investigate the effects of heating, fiber type used in solid-phase microextraction (SPME, two phase vs three phase) and storage time on the volatile compounds of porcine M. longissimus dorsi (LD). Volatile compounds were measured using a gas chromatography and mass spectrometry (GC/MS) with a quadrupole mass analyzer. Among the volatile compounds identified, aldehydes (49.33%), alcohols (24.63%) and ketones (9.85%) were higher in pre-heated loins (100oC/30 min), whereas, alcohols (34.33%), hydrocarbons (22.84%) and ketones (16.88%) were higher in non-heated loins. Heating of loins induced the formation of various volatile compounds such as aldehydes (hexanal) and alcohols. The total contents of hydrocarbons, alcohols, and carboxylic acids were higher in two phase fibers, whereas those of esters tended to be higher in three-phase fibers (p<0.05). Most volatile compounds increased (p<0.05) with increased storage time. Thus, the analysis of volatile compounds were affected by the fiber type, while heating and refrigerated storage of pork M. longissimus dorsi increased the volatile compounds derived from lipid oxidation and amino acid catabolism, respectively. Antinociception Effect and Mechanism of Ruta graveolens L. in Mice

Antinociceptive profiles of Ruta graveolens L extract were examined in ICR mice. R. graveolens L extract administered orally (200 mg/kg) showed an antinociceptive effect as measured by the tailflick and hot-plate tests and attenuated the writhing numbers in the acetic acid-induced writhing test. Cumulative nociceptive response time for intrathecal (i.t.) injection of substance P (0.7 ${\mu}g$) was decreased by the extract. Intraperitoneal (i.p.) pretreatment with naloxone (opioid receptor antagonist) or yohimbine (${\alpha}_2$-adrenergic receptor antagonist) attenuated antinociceptive effect induced by the extract in the writhing test. However, methysergide (5-HT serotonergic receptor antagonist) did not affect antinociception induced by the extract in the writhing test. These results suggest that R. graveolens L extract exerts antinociceptive property in various pain models. Furthermore, this antinociceptive effect of R. graveolens L extract may be mediated by opioidergic and ${\alpha}_2$-adrenergic receptors, but not by serotonergic receptors.
Factors Determining the Timing of Tracheostomy in Medical ICU of a Tertiary Referral Hospital

Background: Tracheostomy is a common procedure for patients requiring prolonged mechanical ventilation. However, the timing of tracheostomy is quite variable. This study was performed to find out the factors determining the timing of tracheostomy in medical intensive care unit (ICU). Methods: Patients who were underwent tracheostomy between January 2008 and December 2009 in the medical ICU of Seoul National University Hospital were included in this retrospective study. Results: Among the 59 patients, 36 (61.0%) were male. Median Acute Physiology And Chronic Health Evaluation (APACHE) II scores and Sequential Organ Failure Assessment scores on the admission day were 28 and 7, respectively. The decision of tracheostomy was made on 13 days, and tracheostomy was performed on 15 days after endotracheal intubation. Of the 59 patients, 21 patients received tracheostomy before 2 weeks (group I) and 38 were underwent after 2 weeks (group II). In univariate analysis, days until the decision to perform tracheostomy (8 vs. 14.5, p<0.001), days before tracheostomy (10 vs. 18, p<0.001), time delay for tracheostomy (2.1 vs. 3.0, p<0.001), cardiopulmonary resuscitation (19.0% vs. 2.6%, p=0.049), existence of neurologic problem (38.1% vs. 7.9%, p=0.042), APACHE II scores (24 vs. 30, p=0.002), and $PaO_2/FiO_2$ <300 mm Hg (61.9% vs. 91.1%, p=0.011) were different between the two groups. In multivariate analysis, APACHE II scores${\geq}20$ (odds ratio [OR], 12.44; 95% confidence interval [CI], 1.14~136.19; p=0.039) and time delay for tracheostomy (OR, 1.97; 95% CI, 1.11~3.55; p=0.020) were significantly associated with tracheostomy after 2 weeks. Conclusion: APACHE II scores${\geq}20$ and time delay for tracheostomy were associated with tracheostomy after 2 weeks.

Invasive pulmonary aspergillosis (IPA) is rarely reported in patients who have normal immune function. Recently, IPA risk was reported in nonimmunocompromised hosts, such as patients with chronic obstructive pulmonary disease and critically ill patients in intensive care units. Moreover, influenza infection is also believed to be associated with IPA among immunocompetent patients. However, most reports on IPA with influenza A infection, including pandemic influenza H1N1, and IPA associated with influenza B infection were scarcely reported. Here, we report probable IPA with a fatal clinical course in an immunocompetent patient with influenza B infection. We demonstrate IPA as a possible complication in immunocompetent patients with influenza B infection. Early clinical suspicion of IPA and timely antifungal therapy are required for better outcomes in such cases.
