S-388 Two cases Pill induced bronchitis : Showing importance of timely removal by bronchoscopy

Introduction:?An aspirated pill can dissolve in the tracheobronchial tree causing inflammation and stenosis. Prompt recognition and timely removal of aspirated pill is essential to avoid serious sequelae in the airways. Here we report two cases of pill induced bronchitis resulted in airway inflammation and narrowing.?Case1:?A 83-year-old man presented with a 1-week history of blood tinged sputum, chest discomfort. A chest computed tomography (CT) scan showed 1.1cm sized ovoid material in right lower lobar (RLL) bronchus. We found out a partially dissolved pill impacted in RLL bronchus by bronchoscope. Bronchial mucosa were swollen and covered with inflammatory exudates. Four days after, inflammation of bronchus was still ongoing. Growth of granulation tissue, swelling and necrotic debris led to narrowing of bronchus.?Case2:?A 87-year-old man was visited our emergency room for chest discomfort and cough. A chest CT scan revealed a foreign body in RLL bronchus with peribronchial consolidation. We could perform the bronchoscopy. A pill was not present but we presumed that the pill had dissolved into the bronchial mucosa. After 1 week, necrotic change of bronchus intermedius and narrowing of RLL bronchus were progressing. After one month, narrowing of RLL bronchus was aggravated with granulation tissue, necrotic debris and fibrotic changes.?Conclusions:?Our cases suggest that if pill aspiration is suspected, early diagnosis and urgent management by bronchoscope is necessary in order to reduce the serious sequelae in the airway.
수확 후 에틸렌 및 1-MCP 처리가 사과 ‘홍로’ 과실의 품질에 미치는 영향

시장 출하 성숙기에 수확한 사과 ‘홍로’ 과실에 에틸렌 (100μℓㆍℓ?¹ for 24 hours at 10℃) 및 1-Methylcyclopropene (1-MCP)(1μℓㆍℓ?¹ for 24 hours at 10℃)를 수확 후 즉시 처리하고 0℃ 및 20℃ 저장 과정에서 생리대사의 변화와 50일 저장 후 품질을 조사하였다. 수확 후 1-MCP 처리는 0℃와 20℃ 저장 과일의 에틸렌 발생을 효과적으로 억제한 반면 호흡속도 감소에 미치는 효과는 20℃ 저장 과일에서만 관찰되었다. 수확 후 에틸렌 처리는 20℃ 저장 사과의 에틸렌 발생과 호흡을 증가시켰다. 저장기간 중 에틸렌 발생과 호흡의 증가현상은 무처리와 에틸렌 처리 과일에서만 나타났으며 1-MCP 처리 사과에서는 보이지 않았다. 한편, 0℃ 저온저장은 에틸렌 발생과 호흡을 효과적으로 억제하면서 증가현상이 나타나는 시점을 지연시켰다. 과실 품질에 미치는 1-MCP 처리 효과는 20℃ 상온저장 후 과육경도와 종합식미 유지 및 표피 미끌거림 방지에 유의성을 보였다. 실험결과를 종합해 볼 때, 1-MCP 처리는 상온저장 ‘홍로’ 사과의 품질유지에는 효과가 뚜렷한 반면, 단기간 0℃ 저온저장 시에는 그 효과가 제한적인 것으로 판단되었다. Effects of postharvest ethylene and 1-methylcyclopropene (1-MCP) treatments on the physiological metabolism and quality of ‘Hongro’ apples (Malus domestica) were investigated. Fruit were harvested at commercial maturity, treated with 100 μℓㆍℓ?¹ ethylene for 24 hours at 10℃ or 1 μℓㆍℓ?¹ 1-MCP for 24 hours at 10℃, and then stored at 0℃ and 20℃ for 50 days, respectively. Ethylene evolution was completely inhibited by 1-MCP treatment at both storage temperatures. In contrast, 1-MCP treatment effects on respiration were distinct only in apples stored at 20℃. Ethylene treatment tended to increase ethylene evolution and respiration, especially during 20℃ storage. Rises in the ethylene evolution and respiration rates appeared clearly in control and ethylene-treated apples, while not detectable in 1-MCP treated apples during 50-day storage. Storage at 0℃ also effectively reduced ethylene evolution and respiration while delaying the ethylene and respiration peak times. Effects of 1-MCP on fruit quality were significant for maintaining flesh firmness and overall taste while reducing surface greasiness during storage. Overall data indicated that postharvest 1-MCP application could maintain fruit quality during storage at ambient temperature. However, potential of 1-MCP treatment may be limited to ‘Hongro’ apples programmed to be stored at 0℃ for short period.
