청소년 내담자의 반복적 자해행동의 의미탐색: ‘고통으로 고통을 견디기’ = Exploring the Meaning of Repeated Self-injurious Behavior in Youth Client: ‘to endure the pain with suffering’

The purpose of this study was understanding experience and meaning of the youth client committing’s repeatedly self-injure. We interviewed four students aged from 15 to 18 having those experiences. The interview data were analyzed using phenomenological method in order to uncover their self-injurious behavior experiences and those meanings. Results showed that repetitive self-injurious behavior experiences and those meanings could be classified into 28 concepts and 8 categories. Eight categories were ‘wanting to hide’, ‘just impulsively’, ‘strong addiction’, ‘self-treatment of pain’, ‘soothe angry-car airbag’, ‘self comforting through self-injury’, ‘clearly express feelings’ and ‘vicarious suffering-to endure the pain with suffering’. Self-injurious youths were comforted their own painful mind through self-injurious behavior as self treatment of psychological distress and were maintained addictive properties of repeated self-injurious behavior because of absence of an alternative way. Finally implications for counseling youth committing self-injurious behavior repeatedly and limitations of the current study are discussed.

국문 초록 (Abstract)

본 연구는 반복적으로 자해를 하는 청소년 내담자를 대상으로 이들의 자해행동의 경험과 의미를 이해하는데 목적이 있다. 이러한 연구목적을 위해 반복적 자해행동의 어려움을 지닌 4명의 청소년들에게 자해 경험을 묻는 반구조적인 질문형식으로 심층면담하고 현상학적 방법으로 분석절차를 거쳐 자해행동의 경험과 의미는 무엇인지 알아보았다. 연구 결과 자해행동의 경험과 의미에 대하여 28개의 주제와 8개의 범주가 도출되었다. 8개의 범주는 ‘감추고 싶은 마음’, ‘충동적으로 그냥’, ‘강한 중독성’, ‘스스로 고통을 처리’, ‘화를 진정시킴-자동차의 에어백’, ‘상처를 통한 자기 위로’, ‘확실한 감정전달’, ‘대리적 고통-고통으로 고통을 견디기’로 나타났다. 반복적으로 자해를 하는 청소년들은 자해행동을 통해 심리적 고통을 스스로 처리하여 힘든 마음을 위로하고 있었으며, 주변에 자신의 감정을 전달하고자 하는 마음과 동시에 이를 감추고 싶어 하는 상반된 태도를 가지고 있었으나, 다른 대안적 방법이 부재한 상황에서 자해가 반복되는 중독적 특성을 지니고 있었다. 이러한 연구의 결과를 바탕으로 반복적인 자해행동을 하는 청소년 내담자에 대한 상담적 의의와 제한점을 제시하였다.
