유구치에서 electrosurgery와 MTA를 이용한 치수절단술

This study investigates the success rate of pulpotomy using electrosurgery operated on a primary molar in caries. The pulpotomy using electrosurgery was done on 253 primary molars of 111 young patients from 2 to 9 years old during the period of the first day of January 2011 to the last day of December 2015. After the amputation of pulp and hemostasis of primary molar were done using electrosurgery, MTA as pulp capping material was applied to the primary molar and the tooth was restored with the stainless steel crown. The follow up period after the treatment ranged from 4-46 months. The clinical and radiographic success rate ranged from 92.1 - 94.3%. Which is comparable to formocresol and ferric sulfate pulpotomy. Due to its non-pharmacological characteristic, electrosurgery can minimize harmful effect on the pulp tissue. Its fast bleeding control makes it easy and safe to use in pediatric and disabled patients in comparatively simple manner. Electrosurgery can be an alternative for pulp therapy considering the side effects of pharmacological ways.

본 연구는 만 11세에서 18세 사이에 해당하는 소년원 여학생과 대학병원 소아치과에 내원한 동일한 연령대 환자들의 구강 건강상태를 조사하여 차이점을 비교하고자 시행되었다. 최근 5년간 누적된 소년원 여학생과 아주대학교병원 소아치과에 내원한 환자의 치과기록을 수집하여 치과치료에 분석하였다. 소년원 여학생과 대학병원 환자 모두 수복치료가 가장 높은 비중을 차지하였다. 소년원 여학생에서는 치아우식에 의한 영구치의 발거가 중,고등학교 시기에 점차 증가한 반면에 대학병원 환자에서는 영구치의 발거 비율이 적었다. 현재 수용되어있는 만 11부터 18세까지의 소년원 여학생 130명과 최근 1년간 대학 병원 소아치과에 내원한 환자에 대해 구강검진을 시행하여 우식, 상실, 충전 치아 수를 검사하여 구강건강상태를 조사하였다. 우식경험영구치지수가 대학병원에서 4.15인 반면에 소년원 여학생에서는 7.53로 약 2배 높았다. This research investigates into overall oral health status of adolescent girl inmates in a juvenile detention facility and ordinary school students in the same age range of 11 to 18 years old and compares the results focusing on the difference between the two groups. The last 5 years dental records for girl inmates in Juvenile detention facility and for students who have visited the Department of Pediatric Dentistry at Ajou University Hospital was colleted and analyzed. The restorative treatments took up the greatest part in their history of treatments. While the percentage of extractions of permanent teeth due to dental caries had been gradually increased, in Juvenile detention facility students, throughout the transition period from middle school to high school, in dental hospital patients, the phenomenon of the same kind has hardly been detected. After that record analysis, oral examination for Juvenile detention facility students and dental hospital patients was conducted. DMFT index for dental hospital patients are 4.15, whereas, the value for Juvenile detention facility students are as twice as high to be 7.53.
