      • Report on oral health status of adults with mental health disorders in South Korea

        Objective: The aim of this review is to examine the relationship between oral health and mental health disorders lawfully regarded as disabilities in South Korea: schizophrenia, schizoaffective disorder, bipolar disorder, and depression. Background: Mental health and oral health are two of the most prevalent factors that affect health-related quality of life. People with mental health disorders are prone to poor oral health, but research on the relationship between mental health and oral health is lacking. Discussion: Individuals with mental health disorders tended to show exacerbated oral health conditions overall, and there seems to be a connection between the type of mental health disorder and the corresponding oral health outcomes. However, the cause and degree of the relationships could not be clearly established, due to the small quantity of studies conducted on the oral health status of individuals with specific mental health disorders, as well as the methodological inconsistencies among studies. Conclusion: Future research should establish a consistent standard of methodology for studying the oral health of individuals with mental health disorders.

      • 2016년 전북대학교 치과병원 소아치과를 방문한 장애인환자에 관한 조사연구

        According to the Ministry of Health and Welfare, the portion of physical disability was 51.4% which was the highest among the registered disabled in December 2015, followed by visual disability(10.1%), disability of the brain lesions(10.0%), hearing disability(10.0%). The aim of this study is to investigate the types of disability in patients with disabilities visiting the Pediatric Dentistry, Chonbuk National University Dental Hospital in 2016. Of the patients who visited the pediatric dentistry, 196 patients (7.5%) had disabilities, and 19 patients had two or more disabilities. Twelve patients (6.1%) had both disabilities of brain lesion and intellectual disorder. Of the 177 patients with only one disability, 54 patients (30.5%) had physical disorders and 123 patients (69.4%) had psychiatric disorders. In psychiatric disorders, intellectual disorder was the most with 98 patients (55.3%), followed by autistic disorder with 23 patients (12.9%). In physical disorders, disability of brain lesion was the most with 37 patients (20.9%), followed by speech disability with 8 patients (4.5%). In this study, as a result of analyzing the types of disability with the patients who visited the Pediatric Dentistry of Chonbuk National University Dental Hospital, most of them had either intellectual disorder, disability of brain lesion, or autistic disorder. Dentists need to understand their general and dental characteristics to treat children and adolescents with disabilities. Since the most chief complaint of the patients was dental caries, early interventions should be made by dentist to reduce the incidence of the disease.

      • 장애인의 구강건강권 확립을 위한 구강의료 서비스 전달 시스템 개발

        The need for oral health rights for people with disabilities is very high, and current oral health care system does not fully reflect these demands. Efforts to promote oral health of people with disabilities are urgently needed. In order for the disabled to have oral health rights, access to oral health services for people with disabilities should be improved and barriers to access should be resolved. In this study, we propose oral health service delivery system to guarantee oral health rights for the disabled. In addition, before applying the proposed oral medical delivery system, the external effects of the system application were predicted and the expert verification was conducted to find out the solution. There are some controversies about the development of the service delivery system proposed in this study. As a result of the expert verification, there were disagreements about the suitability of the service provider, the suitability of the service recipient, the appropriateness of the service content and scope, and the appropriateness of the cost and the revenue source. Subsequent Delphi surveys require the development of structured questionnaires for discussions that require consensus. It is expected that a reasonable consensus of expert opinions will be derived.

      • Orthodontic traction of impacted canines in a patient with Cornelia de Lange syndrome: A case report

        Cornelia de Lange syndrome (CdLS) is a rare genetic disorder that approximately 1 in every 10,000 to 30,000 newborns with no racial predilection, but its precise genetic mechanisms are still unclear. The primary clinical features of this condition include a delay in growth and development, excessive hair growth, abnormalities in limb structure, and unique facial characteristics. The oral manifestations of this condition are micrognathia, dental malocclusion, delayed eruption of teeth, microdontia, periodontal disease, and dental erosion caused by gastric reflux. This report presents an overview of the orthodontic traction of an impacted canine in a 17-year-old female patient who exhibited the primary clinical manifestations associated with CdLS. The patient’s overall health condition was acceptable without any cardiac or respiratory alterations. The clinical and radiological examination revealed the presence of skeletal class III malocclusion, a large cystic lesion located on the right maxilla, the prolonged retention of multiple primary teeth, the impaction of several permanent teeth, and the presence of periodontal disease. Surgical intervention including cyst enucleation and button bonding on the impacted maxillary canines was carried out under general anesthesia due to a lack of patient cooperation. Following a period of approximately two years, orthodontic traction of impacted teeth and arch alignment with fixed orthodontic appliances was completed. Despite mild intellectual disability, she had been cooperative during orthodontic procedures. It will be imperative to provide ongoing care, including routine check-ups and scaling for the maintenance of the patients oral health.

      • 뇌병변 장애 환자와 자폐성 장애 환자의 교정치료: 증례 보고

        장애인의 교정 치료는 환자에 따라 치료 목표와 방법이 비장애인의 교정 치료와 달라질 수 있다. 교정 치료를 시작하기 전에 환자의 교정 치료의 한계에 대한 보호자와의 충분한 의사소통이 먼저 이루어져야 하고, 교정 치료는 환자가 견딜 수 있으면서 도움이 되는 방향으로 시행되어야 한다. 또한 교정치료 시 보호자들에게 구강위생관리의 중요성을 항상 강조하고 집에서의 구강 위생 관리를 위한 교육을 반드시 시행해야 한다. Malocclusion occurs more frequently in Special Health Care Needed (SHCN) patients than those in general. As caregiver’s needs for orthodontic treatment tend to increase, the dentist should know how to decide the extent of treatment. This case report is about orthodontic treatment for two SHCN patients; one patient with cerebral palsy, and another patient with autism. A 10-year-old patient with cerebral palsy showed protrusion and rotation of maxillary anterior teeth. To resolve his chief complaints and make better oral hygiene, he underwent orthodontic treatment using micro tube appliances for 6 months. Another 11-year-old patient with autism had anterior crossbite and showed space deficiency of #13 and chronic gingivitis because of poor oral hygiene. She underwent orthodontic treatment with maxillary skeletal expander, facemask and AP expansion appliance. After 18 months we found positive overjet and ended the treatment. When giving SHCNs orthodontic treatment, the extent of treatment can be chosen according to the patient’s cooperative ability and the traits of disabilities. Before initiating orthodontic treatment, the caregivers should be aware of their limitations of the treatment. Since oral hygiene is crucial factor in every dental treatment, education of oral hygiene process for the caregivers and SHCN patient must be done before the orthodontic treatment.

      • 대한장애인치과학회지에 게재된 논문의 연구경향 분석

        The purpose of this study was to examine the state of the articles in the Journal of Korean Association for Disability and Oral Health. All the papers in the Journal of Korean Association for Disability and Oral Health of 2005 to 2018 were analyzed. A total of 181 papers were classified according to its type, field of the study, school, the number of authors, references, and written language. According to the type of the paper, 38 (21.0%) were original articles, 12 (6.6%) were review articles, 124 (68.5%) were case reports and 7 (3.9%) were others. In relation to the field, the most were pediatric dentistry. According to author’s school, some schools showed preponderances. As to the number of researchers, the most common number of researchers was four (39 articles, 21.5%), and the number of the articles written by six (37, 20.4%) was similar to that of the articles written by three and five (28, 15.5%). As for the number of references, this number ranged from minimum 0 to maximum 40, and the average was 14.8. Regarding the language of the articles, English was used in 5 articles (2.8%), and Korean was used in 176 articles (97.2%). Thirty-nine (21.5%) papers were done in collaboration with workers of the other field. The efforts of this study was to provide useful information of the journal and the development of research in Special care dentistry.

      • Orthodontic treatment of anterior crossbite using Frankel appliance for a patient with intellectual disability

        In children with disabilities, malocclulsion prevalence is relatively high. Malocclusion negatively affects these patients and reduces quality of life. However, it is often difficult to perform ideal orthodontic treatment due to lack of cooperation. Therefore, simple and feasible plans should be established for patients with disabilities. This case report is on treatment of anterior crossbite using Frankel appliance in a disabled patient. A 6-year-old patient with intellectual disability visited the Department of Pdeiatric Dentistry, Yonsei University Dental Hospitalwith the chief complaint of anterior crossbite. Upon oral examination, The patient was diagnosed with pseudo class III malocclusion. Considering the patient’s medical history and degree of cooperation, it was decided to use the Frankel appliance III, with a focus on resolving the anterior crossbite. The patient was well adapted to the appliance, and the anterior crossbite resolved two months after the start of treatment. Since then, it has been maintained without relapse. The patient and their caregivers were very satisfied with the treatment results.

      • 장애인 무료 치과 진료소에 내원한 장애인의 구강진료 현황 분석

        The purpose of study was to analyze the types of dental treatment and their frequencies in a charity dental clinic for the disabled people, and to figure out baseline data for developing oral health policies. This study was based on the 141 patient records who visited the charity free dental clinic between 2011 and 2016. Data were evaluated according to gender, types of disability, types of treatment, and the average number of caries. The number of dental treatment according to sex was 70% for men and 30% for women. Patients were classified according to types of disability. The most common type of disability is intellectual disabilities (53.9%). After that, developmental disabilities were 37.5%, and brain lesions were 8.51%. The highest frequency of dental treatment is preventive treatment (45.1%). The mean number of dental caries was higher than the number of initial dental caries in all types of disability. Preventive oral care education and regular oral screening are necessary for the oral health of patients with disabilities.

      • 장애인의 치과의료서비스 이용 시 만족 요인에 관한 연구

        This study aims to help utilize the dental treatment institutions for the disabled and provide high quality of medical services in the future by identifying the satisfaction factors for the patients of the dental clinics for the disabled. As a study method, surveys were completed 200 patients who visited the dental hospital for the disabled. Among these, the surveys from 187 patients were statistically analyzed. The main findings of this study are as follow. Since the study showed that there were differences in the satisfaction factors between the disabled and non-disabled people, subsequent studies needs to be done to develop measuring tools to accurately accommodate the satisfaction factors of the disabled. In addition, as the low spatial accessibility in terms of distance and location was found in this study, more dental treatment institutions for the disabled are required. In the meantime, the survey results found that the most satisfactory aspect was the lower treatment cost compared to other clinics, and that the most important factor when choosing a clinic was also treatment. In addition, given the fact that the group that took a prosthetic treatment, which was non-insured treatment, showed higher satisfaction compared to the group that did not, it suggests that the economic burden plays an important role for the dental treatment of the disabled.

      • A study for oral health status of patients with disabilities in Jeonbuk National University Dental Hospital between 2015-2018

        Aim: Patients with inherited or acquired disabilities have severe oral health problems. The aim of this study is to analyze chief complaints of the patients with disabilities visiting Jeonbuk National University Dental Hospital and to demonstrate recent data on their oral health status based on the analysis. Study Design: The retrospective study was performed on 1,773 cases with dental treatment taken at Jeonbuk National University Dental Hospital from 2015 to 2018. The patients’ information including sex, age, types of disability, residence, general anesthesia experience, visiting route, and chief complaints was investigated with their hospital records. Results: The number of males was 1.4 times bigger than that of females and the group of patients aged between 50 and 59 was the largest. The number of the patients with physical disability was 1.8 times bigger than that of the patients with mental disability. The most common chief complaints were dental caries in the 0-39 years group, periodontal disease in the 40-79 years group, and pathology in the over 80 years group. Conclusion: As the dental treatment for the disabled has expanded, expectations for the treatment have also increased. Therefore, the training for the skilled and professional dental personnel to treat the disabled is needed according to the characteristics of the patients by their age and disease.
