        Characteristics and Risk Factors for Delirium in The Surgical Step-Down Unit

        Purpose: This study aimed to investigate the incidence rate, types and risk factors for delirium in patients admitted to the surgical Step-Down Unit (SDU). Methods: This study was a retrospective study of the electronic medical records of patients admitted to a surgical SDU from February 2020 to July 2020. The delirium assessment was conducted using the short Confusion Assessment Method (a tool that allows quick and easy screening of delirium) and the incidence and risk factors for delirium were determined. Results: Among a total of 227 patients in the study, the incidence rate of delirium was 35 cases (16.7%). It predominantly occurred on the first day (n = 16, 45.7%) and the second day of admission (n = 15, 42.8%). The peak occurrence of delirium was between 14:00 and 22:00 (n = 30, 85.7%). The most common type of delirium was hypoactive (n = 19, 54.3%). The results of multiple logistic regression analysis indicated that the factors influencing the occurrence of delirium in the surgical step down unit (SDU) were age, hypertension, stroke, white blood cell count, and the use of restraints. Conclusion: Considering the characteristics of high-risk groups for delirium in the surgical SDU, it is necessary to establish nursing practice guidelines to minimize delirium.

        The Effect of Systematic Approach to Tracheostomy Care in Patients Transferred from the Surgical Intensive Care Unit to General Ward

        Background: The aim of this study was to investigate the effects of using a systematic approachto tracheostomy care by a clinical nurse specialist and surgical intensivists for patientswith a tracheostomy who were transferred from the surgical intensive care unit (SICU) to thegeneral ward. Methods: In this retrospective study, subjects were limited to SICU patients with a tracheostomywho were transferred to the general ward. The study period was divided into a preinterventionperiod (January 1, 2007 to December 31, 2010) and a postintervention period (January1, 2011 to December 31, 2014), and electronic medical records were used to analyze andcompare patient characteristics, clinical outcomes, and readmission to the SICU. Results: The analysis included 44 patients in the preintervention group and 96 patients in thepostintervention group. Decannulation time (26.7±25.1 vs. 12.1±16.0 days, P=0.003),length of stay in the general ward (70.6±89.1 vs. 40.5±42.2 days, P=0.008), length of totalhospital stay (107.5±95.6 vs. 74.7±51.2 days, P=0.009), and readmission rate of SICU decreaseddue to T-cannula occlusion (58.8% vs. 5.9%, P=0.010). Conclusions: Using a systematic approach to tracheostomy care in the general ward led toreduction in decannulation time through professional management, which resulted in ashorter hospital stay. It also lowered SICU readmission by solving problems related to direct Tcannula.

        Status and Needs of Continuing Education for Trauma Nursing

        Purpose: This study was conducted to status and needs for continuing education for trauma hospital nurses in Korea. Methods: Thirty nurses from the seven level I trauma center hospitals or trauma treatment systems were randomly selected and surveyed. The survey was conducted from March 1 to May 31, 2017. Categorical data were analyzed with Pearson chi-square tests and Continuous variables were analyzed with ANOVA. Results: Only 86 out of 204 nurses had received continuing education (42.1%). The current status of continuing education programs, delivering institution (p<0.001), education method (p<0.001), education period (p=0.003), number of participants (p=0.007), and instructors (p=0.014) were also significantly different from trauma center to trauma center. There were 108 (52.9%) nurses who responded that continuing education programs were “needed” 92 (45.1%) and “very much needed 16 (7.8%). According to each trauma center’s characteristics were significantly differences in the need for continuing education (p=0.089), subject selection method (p<0.001) and the number of continuing education sessions (p=0.043) depending on the hospital. Conclusions: It is necessary to consider differences between the hospitals to develop continuing education programs that reflect the needs of nurses, in order to improve the efficiency of and satisfaction with the educational programs.

      • Impact of Isovalent and Aliovalent Doping on Mechanical Properties of Mixed Phase BiFeO<sub>3</sub>

        <P>In this study, we report the effect of doping in morphotropic BiFeO3 (BFO) thin films on mechanical properties, revealing variations in the elasticity across the competing phases and their boundaries. Spectroscopic force-distance (F-D) curves and force mapping images by AFM are used to characterize the structure and elastic properties of three BFO thin-film candidates (pure-BFO, Ca-doped BFO, La-doped BFO). We show that softening behavior is observed in isovalent La-doped BFO, whereas hardening is seen in aliovalent Ca-doped BFO. Furthermore, quantitative F-D measurements are extended to show threshold strengths for phase transitions, revealing their dependence on doping in the system. First-principles simulation methods are also employed to understand the observed mechanical properties in pure and doped BFO thin films and to provide microscopic insight on them. These results provide key insight into doping as an effective control parameter to tune nanomechanical properties and suggest an alternative framework to control coupled ferroic functionalities at the nanoscale.</P>

      • Enhanced conductivity at orthorhombic–rhombohedral phase boundaries in BiFeO3 thin films

        <P>Enhanced properties in modern functional materials can often be found at structural transition regions, such as morphotropic phase boundaries (MPB), owing to the coexistence of multiple phases with nearly equivalent energies. Strain-engineered MPBs have emerged in epitaxially grown BiFeO3 (BFO) thin films by precisely tailoring a compressive misfit strain, leading to numerous intriguing phenomena, such as a massive piezoelectric response, magnetoelectric coupling, interfacial magnetism and electronic conduction. Recently, an orthorhombic-rhombohedral (O-R) phase boundary has also been found in tensile-strained BFO. In this study, we characterise the crystal structure and electronic properties of the two competing O and R phases using X-ray diffraction, scanning probe microscope and scanning transmission electron microscopy (STEM). We observe the temperature evolution of R and O domains and find that the domain boundaries are highly conductive. Temperature-dependent measurements reveal that the conductivity is thermally activated for R-O boundaries. STEM observations point to structurally wide boundaries, significantly wider than in other systems. Therefore, we reveal a strong correlation between the highly conductive domain boundaries and structural material properties. These findings provide a pathway to use phase boundaries in this system for novel nanoelectronic applications.</P>

        가시박 추출물의 항산화 및 항염증 효과

        생태계 교란 식물로 알려진 가시박 추출물을 활용하여 생리 활성 효과를 알아보고, 기능성 화장품 소재로써의 활용 가능성을 확인하고자 하였다. 본 실험 방법으로는, 총 폴리페놀 함량, 총 플라보노이드 함량을 측정하고, DPPH radical 소거능, 세포 내 ROS 생성 억제, NO 및 COX-2 발현 억제효과를 평가하고자 하였다. 본 실험 결과, 가시박 추출물의 21 mg/g, 0.72 mg/g의 총 폴리페놀과 총플라보노이드 함량을 확인하였으며, 높은 라디칼 소거 활성을 통한 항산화 활성을 확인 하였다. RAW 264.7세포와 HDF세포에서 100 μg/mL 농도까지 유의한 세포 독성은 확인되지 않았으며, HDF세포에서의 농도 의존적인 ROS 생성 억제와 RAW 264.7 세포에서의 높은 NO 생성 억제와 COX-2 단백질 발현 억제 효과를 확인 하였다. 이와 같은 결과를 통하여 항산화 효과가 우수하고, 세포 내 ROS 생성억제와 NO 생성 억제, COX-2 단백질 발현 억제 활성을 통하여 항산화 및 항염증의 효과를 가진 기능성 화장품 소재로써의 활용 가능성을 확인 하였다. This study was carried out to find out the physiological activity effect of Sicyos angulatus L. extract which is known as an ecosystem disturbance plant and confirm the availability as a functional cosmetic material. Total polyphenol and flavonoid contents were measured, and DPPH radical scavenging, intracellular ROS, and its inhibitory effect on the expression Nitric oxide of and COX-2 were evaluated. The content of polyphenol and flavonoid was found to be 3.079 mg(CA)/100g and 72 mg(Q)/100g of Sicyos angulatus L. extract and antioxidant activity through high radical scavenging activity was confirmed. ∼Significant cytotoxicity was not observed up to a concentration of 100 μg / mL in RAW 264.7 cells and HDF cells and concentration-dependent inhibition of ROS production in HDF cells, inhibition of high NO production and inhibition of COX-2 protein expression in RAW 264.7 cells were confirmed. Through these results, we found the possibility of use as a functional cosmetic material with excellent antioxidant effect and antioxidant and anti-inflammatory effects through inhibition of intracellular ROS generation, inhibition of NO generation, and inhibition of COX-2 protein expression.

        외상환자의 치료에서 외상 전문간호사 도입 결과

        Purpose: Ongoing treatment and care, as well as initial stabilization, are required for trauma patients. With increasing number of sickest trauma patients and shortage of surgeons, the need for advanced practice nurse to provide and coordinate trauma care has been greater. The purpose of this study is to analyze the effect of hiring a trauma clinical nurse specialist and its influence on the treatment of trauma patients. Methods: Based on the employment of the clinical nurse specialist in December 2010, the patients were divided into two groups: patients admitted from January 1, 2010 to November 30, 2010 and patients admitted from December 1, 2010 to December 31, 2011. Retrospectively, data were collected using electronic medical records. The general characteristics, clinical courses, and ICU re-admission rates, collaboration (transfers to other departments and collaborative surgery) were compared. Results: To have a clinical nurse specialist on the trauma team resulted in a statistically significant reductions in the length of general ward hospital stay (p<0.05), the ICU re-admission rate, (p<0.03), the lead-time before transfer to other departments (p<0.05). Conclusion: The clinical nurse specialist, as a professional practitioner, improved the quality of treatment through early detection and management of problems. In addition, as a coordinator, the clinical nurse specialist maintained a cooperative relationship with multi-disciplinary medical personnel. The trauma clinical nurse spe-cialist contributed to the treatment of trauma patients positively through a decrease in ICU re-admission rate and length of hospital stay.
