        High Preoperative Fibrinogen and Systemic Inflammation Response Index (F-SIRI) Predict Unfavorable Survival of Resectable Gastric Cancer Patients

        Purpose: This study was to investigate the prognostic significance of the preoperative fibrinogen and systemic inflammation response index (F-SIRI) in a Chinese cohort of resectable gastric cancer. Materials and Methods: Baseline characteristics, preoperative fibrinogen levels and peripheral neutrophil, monocyte, and lymphocyte counts were retrospectively reviewed in 240 patients who underwent radical gastrectomy. The optimal cut-off values for fibrinogen and SIRI were defined as 4.0 g/L and 1.2. Then patients with hyperfibrinogenemia (≥4.0 g/L) and high SIRI (≥1.2) were assigned with an F-SIRI of 2 (both of these hematological abnormalities), 1 (one of these abnormalities), and 0 (neither abnormality), respectively. The prognostic value was examined by univariate and multivariate survival analysis. Results: Preoperative F-SIRI was significantly correlated with tumor size, fibrinogen level, and adjuvant chemotherapy. Whereas there was no significant difference in age, gender, tumor location or other characteristics between groups. In addition, high preoperative F-SIRI was significantly associated with worse disease-free survival (DFS) (hazard ratio [HR], 2.299; 95% confidence interval [CI], 1.482–3.566; P<0.001) and overall survival (OS) (HR, 2.461; 95% CI, 1.584–3.824; P<0.001) by univariate survival analysis. Moreover, it remained an independent predictor for impaired DFS (HR, 2.023; 95% CI, 1.273–3.215; P=0.003) and OS (HR, 2.341; 95% CI, 1.480–3.705; P<0.001) in multivariate Cox regression analysis. Conclusions: Preoperative F-SIRI could serve as a significantly prognostic marker for long-term survival in Chinese patients who underwent radical gastrectomy.

        Semi-Rational Screening of Probiotics from the Fecal Flora of Healthy Adults against DSS-Induced Colitis Mice by Enhancing Anti-Inflammatory Activity and Modulating the Gut Microbiota

        Ulcerative colitis (UC), a chronic inflammatory bowel disease, substantially impacts patients’ health-related quality of life. In this study, an effective strategy for discovering high-efficiency probiotics has been developed. First, in order to survive in the conditions of the stomach and intestine, high bile salt-resistant and strong acid-resistant strains were screened out from the fecal flora of healthy adults. Next, the probiotic candidates were rescreened by examining the induction ability of IL-10 (anti-inflammatory factor) production in dextran sodium sulfate (DSS)-induced colitis mice, and Lactobacillus sakei 07 (L07) was identified and selected as probiotic P. In the end, fourteen bifidobacterium strains isolated from stools of healthy males were examined for their antimicrobial activity. Bifidobacterium bifidum B10 (73.75% inhibition rate) was selected as probiotic B. Moreover, the colonic IL-6 and TNF-α expression of the DSS-induced colitis mice treated with L. sakei 07 (L07) - B. bifidum B10 combination (PB) significantly decreased and the IL-10 expression was up-regulated by PB compared to the DSS group. Furthermore, Bacteroidetes and Actinobacteria decreased and Firmicutes increased in the DSS group mice, significantly. More interestingly, the intestinal flora biodiversity of DSS colitis mice was increased by PB. Of those, the level of B. bifidum increased significantly. The Bacteriodetes/Firmicutes (B/F) ratio increased, and the concentration of homocysteine and LPS in plasma was down-regulated by PB in the DSS-induced colitis mice. Upon administration of PB, the intestinal permeability of the the DSS-induced colitis mice was decreased by approximately 2.01-fold. This method is expected to be used in high-throughput screening of the probiotics against colitis. In addition, the L. sakei 07 - B. bifidum B10 combination holds potential in UC remission by immunomodulatory and gut microbiota modulation.

        Improved Convolutional Neural Network for Laser Welding Defect Prediction

        In order to predict the laser welding defects, a convolutional neural network prediction model is established. The keyhole image and plume image collected by a high-speed camera are processed to obtain visual information such as keyhole area and plume area. The rolling mean and standard deviation methods are used to calculate the fluctuation degree indicators of the visual information and the optical radiation information obtained by the photoelectric sensor. Finally, three improved one-dimensional convolutional neural network prediction models with a learning rate dynamic adjustment mechanism are established to predict welding defects. Experimental results indicate that the improved one-dimensional convolutional neural network prediction model can avoid premature convergence four times to achieve the best performance. The fluctuation degree indicators of sensor features can distinguish the welding state more easily than the sensor features. The reliability test of the new weld is carried out. The prediction accuracy of fusion detection model of sensor features and fluctuation degree indicators is 99.21%. The improved model can accurately predict laser welding defects.

This comprehensive literature review on ethical issues of health communication was performed in databases (PubMed and Google Scholar). All English-language relevant articles with keywords “medical confidentiality” and “confidentiality breach” for “confidentiality breach” search and “miscommunication in doctor patient relationship” in “miscommunication” search were analyzed. Their reference lists were reviewed and the relevant ones were obtained and analyzed. Confidentiality breaches happen commonly and most of them are accidental. It is mostly committed by doctors and severe confidentiality breaches occur more often with the most common disclosed information is patients’ clinical and/or personal data. Breaching confidentiality can happen during conversation and data entry. It results mainly in professional consequences and sanctions for the doctors, as well as poorer health outcomes of the patients. Poor health outcomes can also be resulted from miscommunication. There are numerous factors grounding the occurrences of miscommunication. It can happen between medical professionals, medical professionals and patients, and medical professionals and patients’ relatives. Confidentiality breach and miscommunication are two major issues in most countries which result in poor doctor-patient relationship that cause numerous disadvantages, particularly poor patients’ health outcome. Although there are various reasons, they usually happen due to awareness lack of the doctors. Awareness of confidentiality and communication importance should be improved.

        Influence of temperature on the carbonization context, oxidation properties and mechanism of a porous biomorphic carbon template

        A porous biomorphic carbon template (BCT) was prepared using a carbonized native aspen under an Ar atmosphere. Microstructural properties of the BCT were characterized by scanning electron microscopy (SEM), energy dispersive X-ray spectroscopy (EDS) and X-ray diffraction (XRD). Non-isothermal oxidation properties and the mechanism of the BCT formation were studied by thermogravimetric analysis (TGA). Experimental results show that microstructure of the BCT exhibits a honeycomb interconnected porous network and double-peaked distribution of pore diameters. The BCT phase belongs to amorphous carbon, with an increase of the carbonization temperature, the (002) peak of the XRD spectrum becomes stronger, the interplanar spacing decrease, and the structure of BCT slowly evolved towards that of ideal graphite. The non-isothermal oxidation process of BCT is controlled by a chemical reaction and gas diffusion together, and the two stages have different influences on the whole reaction rate with an increase of the conversion (a). The corresponding activation energy (E a) is also calculated.

        Influence of calcined temperatures on the microstructure and electrochemical properties of LiFePO4/C nano-particles with a core-shell structure and It’s thermal stability study

        Influence of calcined temperatures on the microstructure and electrochemical properties of LiFePO4/C nano-particles as well as it’s thermal stability were studied using HRTEM, XRD, electrochemical workstation and TGA. The results indicated that when calcined at 973 K, the LiFePO4/C nano-particles consisted of a well-crystalline LiFePO4 core with size of 58.6-80.1 nm and an amorphous carbon shell with thickness of 2 nm. With the increase of calcined temperature, the electrochemical properties of LiFePO4/C materials increased first and then decreased, it reached maximum when temperature equaled to 973 K. The initial discharge capacity of the sample was 142 mAh/g, the discharge capacity of it maintained 132 mAh/g with capacity retention of 93.0% after 40 cycles. The decomposition reaction of LiFePO4/C material calcined at 973 K occurred at 938.38- 1194.52 K under 10 K • min−1 in N2 atmosphere and corresponded to approximately 5.8% of the total weight. The decomposition mechanism of it consisted of three stages: the first stage was controlled by gas diffusion in carbon shell; the second stage was controlled by chemical reaction and gas diffusion; the third stage was controlled by chemical reaction

        It is an important to quickly and accurately forecasting road network traffic flow in intelligent transportation systems, Aiming at the forecasting problem of short-term traffic flow, this paper proposed a traffic flow prediction algorithm, which based on traffic flow sequence partition and neural network model. Firstly, the algorithm divided the traffic flow into different patterns and time sequence by clustering, secondly, described and predicted traffic flow model according to BP neural network. Finally, the experiment shows that based on combined model is much accurate.

        Failure Mechanisms and Modes of Tunnels in Monoclinic and Soft-Hard Interbedded Rocks: A Case Study

        The issue of large deformation mechanism in soft rock tunnels has puzzled tunnel scholars for decades. Previous studies have not evolved a clear and common understanding. Therefore, detailed on-site measurement, full investigation and statistical analysis have been conducted on the instability and failure of Muzhailing Tunnel since its construction, whose length is beyond 15 km. The study aims at systematically analyzing the failure mechanisms and modes of Muzhailing Tunnel in monoclinic and soft-hard interbedded rock strata. Study results show that the angle between strata strike and tunnel axis greatly determines the magnitude of deformation, the dip direction significantly controls the bias direction and maximum deformation direction, and the dip angle deeply affects the deformation form. The failure modes of surrounding rock mainly include four types: spalling and overturning failure, bending failure, shear slip failure and buckling failure. Large deformation characteristics are summarized from six aspects: failure form, groundwater, sensitivity to influencing factors, deformation degree, deformation speed and deformation duration. The instability modes of primary lining include in-plane (transverse) instability and out-plane (longitudinal) instability. Finally, the causes of large deformation are analyzed from geological, structural, engineering and human factors.
