        All that glitters is not <i>Ramularia</i>

        <P><I>Ramularia</I> is a species-rich genus that harbours plant pathogens responsible for yield losses to many important crops, including barley, sugar beet and strawberry. Species of <I>Ramularia</I> are hyphomycetes with hyaline conidiophores and conidia with distinct, thickened, darkened, refractive conidiogenous loci and conidial hila, and <I>Mycosphaerella</I> sexual morphs. Because of its simple morphology and general lack of DNA data in public databases, several allied genera are frequently confused with <I>Ramularia</I>. In order to improve the delimitation of <I>Ramularia</I> from allied genera and the circumscription of species within the genus <I>Ramularia</I>, a polyphasic approach based on multilocus DNA sequences, morphological and cultural data were used in this study. A total of 420 isolates belonging to <I>Ramularia</I> and allied genera were targeted for the amplification and sequencing of six partial genes. Although <I>Ramularia</I> and <I>Ramulariopsis</I> proved to be monophyletic, <I>Cercosporella</I> and <I>Pseudocercosporella</I> were polyphyletic. <I>Phacellium</I> isolates clustered within the <I>Ramularia</I> clade and the genus is thus tentatively reduced to synonymy under <I>Ramularia</I>. <I>Cercosporella</I> and <I>Pseudocercosporella</I> isolates that were not congeneric with the ex-type strains of the type species of those genera were assigned to existing genera or to the newly introduced genera <I>Teratoramularia</I> and <I>Xenoramularia</I>, respectively. <I>Teratoramularia</I> is a genus with ramularia-like morphology belonging to the <I>Teratosphaeriaceae</I>, and <I>Xenoramularia</I> was introduced to accommodate hyphomycetous species closely related to <I>Zymoseptoria</I>. The genera <I>Apseudocercosporella</I>, <I>Epicoleosporium</I>, <I>Filiella</I>, <I>Fusidiella</I>, <I>Neopseudocercosporella</I>, and <I>Mycosphaerelloides</I> were also newly introduced to accommodate species non-congeneric with their purported types. A total of nine new combinations and 24 new species were introduced in this study.</P>

        Acanthosis nigricans as a clinical marker of insulin resistance among overweight adolescents

        Purpose: Acanthosis nigricans (AN) is a hyperpigmented dermatosis associated with obesity and insulin resistance (IR). There is no consensus whether AN extension scoring offers added value to the clinical estimation of IR. In this study we aimed to assess and score AN using both a short and an extended version of the scale proposed by Burke et al. and analyze the relationships of both versions with hyperinsulinemia and IR. Methods: We analyzed data from 139 overweight adolescents (body mass index ≥85th percentile) aged 12–18 with (n=67) or without (n=72) AN who were followed at a pediatric obesity clinic. Results: Adolescents with AN had higher levels of insulin (d=0.56, P=0.003) and HOMA­IR (d=0.55, P=0.003) compared to those without. Neither the short nor the extended versions of AN scores explained either hyperinsulinemia (β=1.10, P=0.316; β=1.15, P=0.251) or IR (β=1.07, P=0.422; β=1.10, P=0.374). The presence of AN alone predicted hyperinsulinemia and the presence of IR in 7.3% (β=2.68, P=0.008) and 7.1% (β=2.59, P=0.009) of adolescents, respectively. Conclusion: Screening for AN at the neck and axilla is a noninvasive and costeffective way to identify asymptomatic overweight adolescents with or at risk of developing IR.

        Metabolically healthy overweight adolescents: definition and components

        Purpose: In adolescents, the definition and clinical implications of metabolically healthy overweight (MHO) status have not been established. This study aimed to investigate the prevalence of MHO according to its most widespread definition, which is based on metabolic syndrome (MS), and to explore further metabolic indicators such as Homeostatic Model Assessment of Insulin Resistance, total cholesterol, low-density lipoprotein cholesterol, alanine aminotransferase, and C-reactive protein levels, together with metabolic health predictors in a sample of adolescents attending a pediatric obesity clinic. Methods: Data from 487 adolescents categorized as overweight (52.6% females, 88.1% white), with a mean body mass index (BMI) z-score of 2.74 (±1.07 standard deviation [SD]), and a mean age of 14.4 years (±2.2 SD) were cross-sectionally analyzed. From this original sample, a subsample of 176 adolescents underwent a second assessment at 12 (±6 SD) months for longitudinal analysis. Results: From the 487 adolescents originally analyzed, 200 (41.1%) were categorized as MHO, but only 93 (19.1%) had none of the metabolic indicators considered in this study. According to longitudinal analysis, 30 of the 68 adolescents (44%) categorized as MHO at baseline became non-MHO over time. BMI z-score was the best predictor of metabolic health both in cross-sectional and longitudinal analyses. Increased BMI z-score reduced the odds of being categorized as MHO (odds ratio [OR], 0.6; 95% confidence interval [CI], 0.4–0.9; P=.008) and increased the odds of having hypertension (OR 2.1, 95% CI: 1.4–3.3, P=0.001), insulin resistance (OR, 2.4; 95% CI, 1.4–4.1, P=0.001), or a proinflammatory state (OR, 1.2; 95% CI, 1.1–1.3, P=0.002). Conclusion: Diagnosis of MHO should not be exclusively based on MS parameters, and other metabolic indicators should be considered. Adolescents categorized as overweight should participate in weight-management lifestyle interventions regardless of their metabolic health phenotype.

        Intraductal Migration of a Breast Tissue Marker Placed Under Ultrasound Guidance during COVID-Induced Delay of Surgery

        Breast tissue markers are common in current clinical practice and are susceptible to migration. Herein, we present the case of a 47-year-old woman with invasive breast carcinoma diagnosed through ultrasound-guided core biopsy, who underwent placement of a breast marker (HydroMARK®) under ultrasound guidance 30 days after core biopsy and with subsequent marker migration to the nipple. The correct position of the marker was documented by mammography after its placement and by magnetic resonance imaging (MRI) after neoadjuvant chemotherapy. Migration of the marker to the nipple was evident only by mammography on the day of surgery. We hypothesized that an intraductal path was the route of marker migration in this patient. Marked ductal ectasia evident on MRI and histopathologic examination supported this hypothesis. To the best of our knowledge, this is the first published case of intraductal migration of a breast tissue marker.

        Not a neuroendocrine tumor: A case of hepatocellular carcinoma in ectopic liver tissue in the pancreas

        Hepatocellular carcinoma (HCC) accounts for most of the hepatic neoplasms and can also occur in ectopic liver tissue. We present a case of a 55-year-old male complaining of weight loss. The imaging studies reported a 2.9 cm nodule in the pancreatic body, with a neuroendocrine tumor diagnosis by cytology. A corpo-caudal pancreatectomy was performed. Pathology showed a well-differentiated HCC developed in ectopic liver tissue with free margins and no lymph node metastases. HCC presenting in ectopic liver tissue is rare. In this case, the preoperative study did not establish the diagnosis, warranting the need for suspicion of this neoplasm.
