      • Nursing Staffing and Quality of Life in Western New York Nursing Homes

        <P>This study investigated the relationship between nurse staffing and quality of life (QOL) in Western New York State nursing homes. This was a cross-sectional, correlational study. The independent variables were hours per resident day (HPRD), skill mix, and turnover rate of nursing staff. The outcomes were measured using the self-reported QOL instrument. No coefficients were statistically significant with registered nurses’ (RNs) HPRD. Certified nursing assistant (CNA) HPRD had a statistically significant positive impact on the spiritual well-being domain. There was a statistically negative relationship between the amount of licensed practical nurse (LPN) HPRD and food enjoyment; and the ratio of more RNs to fewer LPNs and CNAs had a statistically significant negative influence on the meaningful-activity, food-enjoyment, and security domains. The turnover of RNs had a statistically negative relationship with the sum of each domain. None of the coefficients was statistically significant with LPN turnover.</P>

      • Trace Metals Optimization in Ceria Abrasive for Material Removal Rate Enhancement during ILD CMP

        <P>Ceria abrasive has been widely used in chemical mechanical planarization (CMP) to achieve a high material removal rate. To achieve optimum CMP performance, Ce<SUP>3+</SUP> ions need to be generated on the ceria particle surface so they can react with SiO<SUB>2</SUB>. Based on the redox reaction, trace metals can be good candidates for forming the required ions. In this study, FeCl<SUB>2</SUB> and CrCl<SUB>2</SUB> trace metals with ceria were evaluated in terms of their CMP performance and effectiveness. The role of oxygen vacancies formed on the ceria surface using trace metals was experimentally investigated. Compared to FeCl<SUB>2</SUB>, CrCl<SUB>2</SUB> exhibits better CMP performance. We observed that with the addition of CrCl<SUB>2</SUB> trace metals in the ceria abrasive, the removal rate of SiO<SUB>2</SUB> was enhanced by 1.3 times that of only ceria abrasive. Further, in the presence of CrCl<SUB>2</SUB>, the SiO<SUB>2</SUB> wafer surface roughness was reduced from 0.473 nm to 0.390 nm.</P>

      • Monitoring the Physicochemical Degradation of Polishing Pad Soaked in Hydrogen Peroxide during Chemical Mechanical Polishing

        <P>The porous polyurethane polishing pads are extensively used in the chemical mechanical polishing. The pad property is very important and influences the polishing effectiveness and the wafer surface quality. Hence, in the present work, the physicochemical degradation of unused polyurethane polishing pads during metal chemical mechanical polishing (CMP) in the presence of hydrogen peroxide (H2O2) was studied. It was found that the polishing pads were reacted chemically with H2O2 resulting in degradation of the polymer chain network of polishing pad and reduction in the pad elastic modulus. Pad polymer network chain scission was observed during FT-IR evaluation confirming the degradation of the pad. CMP experiments with the H2O2 soaked pads resulted in change in material removal rate (MRR) and the within-wafer non-uniformity (WIWNU).         Evaluation of Size Distribution Measurement Methods for Sub-100 nm Colloidal Silica Nanoparticles and Its Application to CMP Slurry

        <P>Various methods such as transmission electron microscopy, scanning electron microscopy, dynamic light scattering (DLS), and scanning mobility particle sizer were utilized to measure size distributions for sub-100 nm colloidal silica. Compared to electron micrographs, hydrodynamic diameter of sample is dependent on its concentration from ∼60 nm to ∼90 nm and it is difficult to measure the poly dispersed nanoparticles and analyze its own properties due to limitation of DLS method, whereas, nano nebulizer (NN)-SMPS method distinguished two different size nanoparticles (51 nm and 82 nm) perfectly and is considerably stable to alternations of concentration and dispersity.</P>

        Clinical Characteristics and Outcomes of COVID-19 Cohort Patients in Daegu Metropolitan City Outbreak in 2020

        Background: A coronavirus disease 2019 (COVID-19) outbreak started in February 2020 and was controlled at the end of March 2020 in Daegu, the epicenter of the coronavirus outbreak in Korea. The aim of this study was to describe the clinical course and outcomes of patients with COVID-19 in Daegu. Methods: In collaboration with Daegu Metropolitan City and Korean Center for Diseases Control, we conducted a retrospective, multicenter cohort study. Demographic, clinical, treatment, and laboratory data, including viral RNA detection, were obtained from the electronic medical records and cohort database and compared between survivors and non-survivors. We used univariate and multi-variable logistic regression methods and Cox regression model and performed Kaplan–Meier analysis to determine the risk factors associated with the 28-day mortality and release from isolation among the patients. Results: In this study, 7,057 laboratory-confirmed patients with COVID-19 (total cohort) who had been diagnosed from February 18 to July 10, 2020 were included. Of the total cohort, 5,467 were asymptomatic to mild patients (77.4%) (asymptomatic 30.6% and mild 46.8%), 985 moderate (14.0%), 380 severe (5.4%), and 225 critical (3.2%). The mortality of the patients was 2.5% (179/7,057). The Cox regression hazard model for the patients with available clinical information (core cohort) (n = 2,254) showed the risk factors for 28-day mortality: age > 70 (hazard ratio [HR], 4.219, P = 0.002), need for O2 supply at admission (HR, 2.995; P = 0.001), fever (> 37.5°C) (HR, 2.808; P = 0.001), diabetes (HR, 2.119; P = 0.008), cancer (HR, 3.043; P = 0.011), dementia (HR, 5.252; P = 0.008), neurological disease (HR, 2.084; P = 0.039), heart failure (HR, 3.234; P = 0.012), and hypertension (HR, 2.160; P = 0.017). The median duration for release from isolation was 33 days (interquartile range, 24.0–46.0) in survivors. The Cox proportional hazard model for the long duration of isolation included severity, age > 70, and dementia. Conclusion: Overall, asymptomatic to mild patients were approximately 77% of the total cohort (asymptomatic, 30.6%). The case fatality rate was 2.5%. Risk factors, including older age, need for O2 supply, dementia, and neurological disorder at admission, could help clinicians to identify COVID-19 patients with poor prognosis at an early stage.

        스마트폰 카메라를 이용한 눈꺼풀처짐 환자의 눈꺼풀 깜박임 특성 분석

        목적: 눈꺼풀처짐 환자군과 정상 대조군 사이의 눈꺼풀 깜박임 특성을 스마트폰을 통해 비교하고자 하였다. 대상과 방법: 눈꺼풀각막반사간거리가 2.5 mm 이하인 노년 널힘줄눈꺼풀처짐 환자군 20명과 정상 대조군 10명을 대상으로 눈꺼풀처짐 유무로 2개 군으로, 환자군을 70세 기준으로 2개 군으로 나누어 대조군 포함 3개 군으로 나누었다. 스마트폰으로 촬영한 사진과 깜박임 영상을 통해 각 군에서 눈꺼풀틈새, 눈꺼풀올림근 기능, 눈꺼풀각막반사간거리, 눈깜박임 사이 간격, 눈깜박임 시간, 분당 눈깜박임 횟수, 눈깜박임 속도를 비교하였다. 결과: 정상 대조군과 비교해 눈꺼풀처짐 환자군에서 눈꺼풀틈새, 눈꺼풀올림근 기능, 눈꺼풀각막반사간거리, 눈깜박임 속도는 작았다. 70세 기준으로 나눈 2개의 환자군과 대조군을 포함한 총 3개 군을 비교했을 때 환자군에서 상기 값들은 나이에 의한 차이는 없었고 대조군에 비해 작았다. 눈꺼풀틈새, 눈꺼풀올림근 기능, 눈꺼풀각막반사간거리가 눈깜박임 속도와 상관관계를 보였으며 눈깜박임 속도의 receiver operating characteristic (ROC) curve에서 area under the receiver operating characteristic (AUROC) curve값이 0.969로 높았고 판별기준점은 32.36 mm/s이었다. 결론: 스마트폰을 통해서도 눈꺼풀 깜박임 운동에 대해 분석할 수 있었으며, 노년 널힘줄눈꺼풀처짐 환자군에서 눈꺼풀틈새, 눈꺼풀올림근 기능, 눈꺼풀각막반사간거리, 눈깜박임 속도가 정상 대조군에 비해 작으며 나이의 영향을 받지 않는 점을 확인할 수 있었다. 또한 눈꺼풀처짐 정도와 눈깜박임 속도 사이의 상관관계와 눈깜박임 속도의 ROC curve를 통해 눈깜박임 속도가 눈꺼풀처짐 진단에 가치가 있을 것이라고 본다. Purpose: To compare eyelid blink characteristics between patients with ptosis and healthy controls using a smartphone camera. Methods: The ptosis group consisted of 20 senile aponeurotic ptosis patients with margin reflex distance1 ≤2.5 mm and the control group consisted of 10 healthy subjects without ptosis. The ptosis group was further divided into two groups based on an age cutoff of 70 years. Palpebral fissure height, levator function, margin reflex distance1, inter-blink interval, blink duration, blink rate, and blink velocity were measured and compared between the three groups based on photographs of the eyelids and videos of blinking taken with a smartphone camera. Results: The palpebral fissure height, levator function, margin reflex distance1, and blink velocity were lower in the ptosis groups than in the control group but these values did not differ between the two ptosis groups. The palpebral fissure height, levator function, and margin reflex distance1 were correlated with blink velocity. In the receiver operating characteristic (ROC) curve of blink velocity, the area under the receiver operating characteristic (AUROC) curve value was as high as 0.969 and the cut-off value was 32.36 mm/s. Conclusions: It is possible to analyze eyelid blink characteristics using a smartphone camera and the results confirmed that palpebral fissure height, levator function, margin reflex distance1, and blink velocity were lower in the senile aponeurotic ptosis group than in the healthy control group and were unaffected by age. Additionally, blink velocity is valuable for diagnosis of ptosis due to the correlation between the degree of ptosis, blink velocity, and the ROC curve of blink velocity.

      • 반복측정된 이항자료의 GEE 분석에서 가상관행렬에 관한 연구

        반복측정된 자료의 분석을 위해 제안된 Liang and Zeger(1986)의 회귀모형은 반복측정된 이항자료의 분석에도 사용될 수 있으며 이 모형은 일반화추정식(generalized estimating equations, GEE)을 이용하여 모형의 모수를 추정한다. Liang and Zeger 방법의 장점으로 지적된 사실은 잘못된 형태의 가상관행렬(working correlation matrix)을 사용하더라도 모형의 모수에 대한 추정량과 이 추정량의 분산에 대한 추정량이 일치적으로(consistently) 얻어질수 있다는 것이다. 그러나 최근에 Crowder(1995)는 잘못된 형태의 가상관행렬을 사용하게되면 추정의 효율이 떨어지는 것 외에도 일치추정량이 존재하지 않을 수가 있음을 지적하였다. 그러나 Crowder의 결과는 대표본이론에 근거한 것이기 때문에 본 논문에서는 반복측정된 이항 자료에 대하여 표본의 크기를 실제 상황과 비슷하게 설정한 후에 모의실험을 통해서 가상관행렬의 형대가 회귀모형의 모수의 추정에 미치는 영향을 구체적으로 고찰해 보았다. 결론적으로 Crowder의 지적과는 반대로 반복측정된 이항 자료의 분석에 있어서 올바른 형태의 가상관행렬을 사용한 경우와 잘못된 형태의 가상관행렬을 사용한 경우에 대하여 큰 차이를 발견할 수 없었다. 따라서 실제 상황에서의 GEE방법은 가상관의 형태에 robust함을 알 수 있다. The model proposed by Liang and Zeger can be used for analyzing repeated binary data. The estimation is based on the generalized estimating equations (GEE). One of the advantages of the GEE modelling approach is its robustness on the structure of the working correlation matrix. The parameter estimator and its covariance matrix can be consistently estimated even if an incorrect working correlation matrix is used. Recently, however, Corwder (1995) pointed out that a serious estimating problem occurs if an incorrect correlation matrix is used, based on the asymptotic theory. For moderate sample sizes, we investigate the effect the correlation structures on estimating the regression parameters through a Monte-Carlo simulations. On the contrary to Crowder's findings, the result shows that there are no differences between the cases when the correct correlation matrix is used and when the incorrect one is used. Thus, for moderate sample sizes the GEE approach is robust to the choice of the working correlation structure.
