The diagnosis and management of gastro-esophageal reflux (GER) and GER disease (GERD) in infants and children remains a challenge. Published guidelines and position papers, along with Embase, MEDLINE, and the Cochrane Database were reviewed and summarized with the intent to propose a practical approach and management of GER and GERD for healthcare providers and to standardize and improve the quality of care for infants and children. For this purpose, 2 algorithms were developed, 1 for infants <12 months of age and the other for older children. None of the signs and symptoms of GER and GERD are specific and there is no gold standard diagnostic test or tool. Nutritional management is recommended as a first-line approach in infants, while in children, a therapeutic trial with antacid medication is advised for early management. The practical recommendations from this review are intended to optimize the management of GER in infants and older children and reduce the number of investigations and inappropriate use of medication.

Gastrointestinal infections kill over two million people each year, and pathogen contamination of livestock causes many cases of food poisoning. Two candidate intestinal probiotic strains, L. rhamnosus GR - 1 and L. fermentum RC - 14 were found to inhibit the growth of Salmonella typhi, Shigella dysenteriae, E. coli O157:H7, Listeria monocytogenes, L. innocua, Enterococcus faecalis, and Bacteroides fragilis. In a series of mouse experiments, L. rhamnosus GR - 1 and L. rhamnosus GG protected against S. typhimurium infection and translocation to the liver and spleen, reduced mortality and induced intestinal phagocytic and immunoglobulin responses. In a second series of experiments, the combination of L. rhamnosus GR - 1 and L. fermentum RC - 14 was superior to L. rhamnosus GG and placebo in protecting the mice from the lethal effect of salmonella. In summary, the use of combinations of probiotic lactobacilli as dietary supplements or foods could be considered for people at high risk of salmonella intestinal infection. Given the post - infection complications that can arise, such natural methods warrant further exploration especially given the increasing problem of antibiotic resistance and the lack of alternative measures available to many developing countries.

Soursop (Annona muricata) is a tropical fruit that can be infected by Colletotrichum gloeosporioides and Rhizopus stolonifer. Traditional methods used for postharvest disease control include the application of fungicides, however due to their excessive use, as well as their persistence in the environment, the development of new strategies that control pathogens are required. The application of chitosan (Chi), salicylic acid (SA) and methyl jasmonate (MJ) is an environmentally-friendly alternative with antimicrobial properties and also induces defense mechanisms in plant tissues. In this study, Colletotrichum was reactivated and Rhizopus was identified using morphological features and molecular tools. In vitro, the application of 0.5 and 1.0% of Chi alone or in combination with SA and MJ decreased mycelial growth and sporulation, a complete inhibition of spore germination was obtained. Thus, the application of Chi in combination with SA and MJ could be a smart strategy to inhibit the development of pathogens that attack soursop fruit.

Central venous disease (CVD) is a serious complication in hemodialysis patients. Neurological manifestations are rare. We describe a female with end-stage renal disease with throbbing headache accompanied by paresthesia, weakness, and abnormal posture of her right hand during dialysis sessions. Motor symptoms completely resolved after each dialysis session, although the headaches persisted for several hours. No neurological deficit was evidenced on physical examination. Digital subtraction angiography identified an incomplete thrombosis of the left brachiocephalic vein with retrograde flow in the internal jugular vein, sigmoid sinus, and transverse sinus on the left side. This case illustrates that cerebral venous congestion due to CVD can produce neurological symptoms. Furthermore, we systematically review the literature to identify the characteristics of the cases described so far. This allows clinicians to know the entity and have a high index of suspicion in a hemodialysis patient who develops neurological symptoms.

Characterization of Cellulases of Fungal Endophytes Isolated from Espeletia spp.

Endophytes are microorganisms that asymptomatically invade plant tissues. They can stimulate plant growth and/or provide defense against pathogen attacks through the production of secondary metabolites. Most endophyte species are still unknown, and because they may have several applications, the study of their metabolic capabilities is essential. We characterized 100 endophytes isolated from Espeletia spp., a genus unique to the paramo ecosystem, an extreme environment in the Andean mountain range. We evaluated the cellulolytic potential of these endophytes on the saccharification of the oil palm empty fruit bunch (OPEFB). The total cellulolytic activity was measured for each endophyte on filter paper (FPA). In addition, the specific carboxymethyl cellulase (CMCase), exoglucanase, and β-glucosidase activities were determined. We found four fungi positive for cellulases. Of these fungi, Penicillium glabrum had the highest cellulolytic activity after partial purification, with maximal CMCase, exoglucanase and β-glucosidase enzyme activities of 44.5, 48.3, and 0.45 U/ml, respectively. Our data showed that the bioprospection of fungi and the characterization of their enzymes may facilitate the process of biofuel production.
