A case of giant keratoacanthoma on unusual site

Giant keratoacanthoma is a rare variant of keratoacanthoma (KA) with a maximum size exceeding 2-3cm. In addition to its size, it often gives rise to the severe anatomic damage of the surrounding tissue, leaving a deforming scar even after the spontaneous involution. Giant KA occured predominately in elderly males. 98.3% of gaint KAs reported in English literatures to date were located on head and limbs. A 69-year-old male patient had 3x5cm sized pink colored irregular shaped protruding bridge-like mass with yellow crusted bumpy surface on sacral area, which had enlarged progressively over the previous 2 months. For diagnosis, incisional biopsy has done. Histopathologically, H&E stain showed marked hyperplastic epithelium with eosinophilic glassy kertinocytes, several horn-filled craters, intraepithelial microabscesses and mixed infltraton of inflammatory cells of the dermis. For non surgical approach, oral acitretin was administered. Mass showed partial regression and some pigmented scars left on the margin of the lesion after 2 months from first visit. We report giant KA on unusual site, also suggest that oral acitretin is a effective non-operative modality to minimize anatomic damage for the treatment of giant KA.

Cutaneous leiomyosarcoma is a rare soft tissue sarcoma with predilection for the lower extremities. Leiomyosarcoma of the face is exceedingly rare(1-5%). The supericial dermal type of cutaneous leiomyosarcoma usually arise from the arrector pili muscle and is associated with epidermal change such as ulceration and color changes. A 86-year-old woman had a relatively well defined 1.5 X 1.5 cm sized dark red colored nodule with ulceration and crust above the right eyebrow, which had enlarged progressively over the previous 5 years. The tumor was excised with margin of 0.5cm, and the defect was reconstructed using H-flap. Histopathologically, H&E stain showed a densely cellular dermal tumor with intersecting fascicles of spindled cells and many mitotic figures were found. Immunohistochemical study of the tumor cells showed positive reactivity with smooth muscleactin and desmin, diffusely, and negative for S-100, CD34, CD68. The scar completely healed up 8 months later without recurrence.

Lack of efficacy with adjuvant carbon dioxide laser abrasion for treatment of Tinea unguium

Background: The carbon dioxide laser system has good accessibility to treat tinea unguium in clinic. However, there is limited evidence of their efficacy. Objectives: To evaluate the clinical efficacy of adjuvant carbon dioxide laser abrasion with systemic antifungal therapy in the treatment of tinea unguium. Methods: 14 patients who had bilateral dystrophic great toenails that were clinically consistent with tinea unguium and confirmed by direct microscopy using 20% potassiumhydroxide of subungual debris were included. All patients were administered oral antifungals. Only one of bilaterally involved great toenails was treated with adjuvant carbon dioxide laser abrasion therapy consisted of 3 to 13 sessions at 2-week intervals. Involved great toenail of the other side treated with no adjuvant therapy. Results: Minimal to moderate response was seen in 5 patients(35.7%) and no response in 9 patients on physician``s assessment. Thickness of nail plate treated with adjuvant carbon dioxide laser abrasion therapy declined(2.79【1.97 to 2.20【1.25mm), but, is statistically not significant, compared with control side of involved toenail(p=.979). Conclusion: There was no significant clinical response with adjuvant carbon dioxide laser abrasion therapy compared with control. But, Our study followed up only 14 patients and satisfactions of the patients were variable. Larger scale and optimal follow-up period to assess the efficacy of the carbon dioxide laser therapy will be required for further study.

Primary cutaneous nocardiosis caused by Nocardia takedensis

Nocardia is an aerobic, Gram-positive, filamentous, actinomycete strain which is found worldwide in the soil and decaying organic plant matter. Most of the infections of Nocardia species are through the respiratory tract and disseminate systemically, but rarely, resulting from direct inoculation of the skin, primarily. Early notice of nocardiosis is particularly important, because of the high risk of dissemination. Hence, prompt identification of this organism is indispensable. We describe a case of 87-year-old women diagnosed with primary cutaneous nocardiosis on her forearm caused by Nocardia takedensis, which was identified by bacterial culture and 16S ribosomal RNA sequencing. Histopathologically, H&E stain showed neutrophil dominant mixed infliltration and abscess formation in the dermis. Grocott-Gomori methenamine silver stain showed several filamentous bacilli in the tissue specimen. Gram stain and Periodic acid-Schiff were also positive. The patient were improved with trimethoprim-sulfamethoxazole and ceftriaxone antibiotics therapy for 2 months. Herein, we report the 1st case of primary cutaneous nocardiosis caused by Nocardia takedensis in Korea.

Circumscribed palmar hypokeratosis(CPH) is a rare condition characterized by a sharply circumscribed area of erythematous depressed macule or patch with scaly borders on palms. It usually reveals solitary lesion on thenar area in elderly women. The pathogenesis of this entity still remains unclear. Some authors suggested that this entity presents a rare morphological manifestation of verruca vulgaris. Recently, presence of HPV type 4, 6, 16, 18 specific DNA were demonstrated. A 51-year-old woman presented with CPH concurrent with multiple verrucae on thenar area of palm. For diagnosis, punch biopsy at the edge of the lesion was done. Histopathologically, H&E stain showed a step-like thinning of the stratum corneum with underlying epidermal changes. HPV real-time PCR of tissue specimen of the center of the lesion showed expression of HPV type 6, 16. Topical ingenol mebutate was applied for treatment. After 5 weeks, the flesh depressed lesion was faded and partially flattened. We suggests reconsideration about HPV for etiology of CPH, and ingenol mebutate is an effective treatment modality for this entity.

The psychological well-being(PWB: Ryff, 1989) is important theory because of contribute to an individual's enhancing the positive affect, contentment, and happiness. The study of psychological well-being has been guided by two primary conceptions of positive functioning. First, Bradburn's (1969) seminal work, distinguished between positive and negative affect and defined happiness as the balance between the two. Second, primary conception, which has gained prominence among sociologists, emphasizes life satisfaction as the key indicator of well-being. The convergence of these multiple frameworks of positive functioning served as the theoretical foundation to generate a multidimensional model of well-being(Ryff, 1989, 1995) so that PWB consist of 6 dimensions are self-acceptance, positive relationships with other people, autonomy, environmental mastery, purpose in life, and personal growth which is attained by achieving a state of balance affected by both challenging and rewarding life events. The aim of this research was to investigate the effects of individualistic value orientation (IVO), collectivistic value orientation(CVO) on the employees’ PWB. Also we test the moderating effects of person-job fit(PJF), person-organization fit(POF) between those of relationship. IVO-CVO are one of the cultural value orientations should be conceptualized as residing within a culture whether the focus is exclusively on representations of the self more specially broader values and belief systems. We draw the these IVO-CVO are critical determinants of PWB which dispositional approach such as trait activation theory and reflected context of person-environment fit as interaction between IVO-CVO and environment(organization, job). PJF defined the relationship between a person’s characteristics and those of the job or tasks that are performed at work and POF, which addresses the compatibility between people and entire organizations. Based on literatures, we proposed direct, and moderating effect hypothesis. To test hypothesis, total of 312 questionnaires, multi souce which self-report and supervision report, were used for analysis. The results are as follows. First, CVO have significantly positive impact on PWB, but IVO does not. Second, PJF have significantly positive impact on PWB, but POF does not. Third POF have moderate relationship between IVO, CVO and PWB. Base on the results, this study CVO and PJF is critical determinant of PWB that Individuals with strong CVO and match the requirement of job are likely to engage in psychological states that are beneficial to the their happy working life. Both of strong IVO, CVO employee amplify PWB that organization should more pay attents to their goal, value to matching the employees.

본 논문에서는 최근 금융, 보험 등에서 빈번하게 발생하는 불법/이상 행위를 탐지하기 위해 데이터 그래프에서 사용자가 찾고자 하는 이상 패턴을 찾아 결과를 보여주는 그래프 가시화 툴을 제안한다. 개발한 툴은 정점과 간선 추가 및 삭제 등의 유용한 기능을 제공하기 때문에, 동적 그래프에 대한 불법/이상 행위 탐지를 위한 응용 프로그램에서도 널리 사용될 수 있을 것이다.

Automatic Reproduction of a Genius Algorithm: Strassen's Algorithm Revisited by Genetic Search

<P>In 1968, Volker Strassen, a young German mathematician, announced a clever algorithm to reduce the asymptotic complexity of <I>n×n</I> matrix multiplication from the order of <I>n</I> <SUP>3</SUP> to <I>n</I> <SUP>2.81</SUP>. It soon became one of the most famous scientific discoveries in the 20th century and provoked numerous studies by other mathematicians to improve upon it. Although a number of improvements have been made, Strassen's algorithm is still optimal in his original framework, the bilinear systems of 2 × 2 matrix multiplication, and people are still curious how Strassen developed his algorithm. We examined it to see if we could automatically reproduce Strassen's discovery using a search algorithm and find other algorithms of the same quality. In total, we found 608 algorithms that have the same quality as Strassen's, including Strassen's original algorithm. We partitioned the algorithms into nine different groups based on the way they are constructed. This paper was made possible by the combination of genetic search and linear-algebraic techniques. To the best of our knowledge, this is the first work that automatically reproduced Strassen's algorithm, and furthermore, discovered new algorithms with equivalent asymptotic complexity using a search algorithm.</P>
