        Depression masquerading as chest pain in a patient with Wolff Parkinson White syndrome

        Wolff Parkinson White (WPW) syndrome is a condition in which there is an aberrant conduction pathway between the atria and ventricles, resulting in tachycardia. A 42-year-old patient, who was treated for WPW syndrome previously, presented with chronic somatic pain. With her cardiac condition in mind, she was thoroughly worked up for a recurrence of disease. As part of routine screening of all patients at our pain clinic, she was found to have severe depression as per the Patient Health Questionnaire-9 (PHQ-9) criteria. After ruling out sinister causes, she was treated for depression using oral Duloxetine and counselling. This led to resolution of symptoms, and improved her mood and functional capability. This case highlights the use of psychological screening tools and diligent examination in scenarios as confusing as the one presented here. Addressing the psychological aspects of pain and adopting a holistic approach are as important as treatment of the primary pathology. (Korean J Pain 2016; 29: 262-5)

      • 스마트 폰 사용에 따른 사용자의 태도 예측

        Recently, predicting personality with the help of smartphone usage is become very interesting and attention grabbing topic in the field of research. At present there are some approaches towards detecting a user’s personality which uses the smartphones usage data, such as call detail records (CDRs), the usage of short message services (SMSs) and the usage of social networking services application. In this paper, we focus on the predicting user attitude based on MBTI theory by using their smartphone usage data. We used Naive Bayes and SVM classifier for classifying user personalities by extracting some features from smartphone usage data. From analysis it is observed that, SVM classifier works well as compared to Naive Bayes.

        In Vitro Antibacterial Activity, Gas Chromatography-Mass Spectrometry Analysis of Woodfordia fruticosa Kurz. Leaf Extract and Host Toxicity Testing With In Vitro Cultured Lymphocytes From Human Umbilical Cord Blood

        Objectives: To locate a plant with suitable phytochemicals for use as antimicrobial agents to control multidrug-resistant (MDR) bacteria as a complementary medicine, without host toxicity as monitored through cultured lymphocytes from human umbilical cord blood. Methods: The methanol crude leaf extract of the plant Woodfordia fruticosa was subjected to antimicrobial assay in vitro with nine pathogenic MDR bacteria from clinical samples. This was followed by bioassay-guided fractionation with seven non-polar to polar solvents, gas chromatographyemass spectrometry analysis of the n-butanol fraction, and monitoring of the host toxicity of the leaf extract with in vitro grown lymphocytes from human umbilical cord blood. Results: The leaf extract of W. fruticosa had a controlling capacity for MDR bacteria. The minimum inhibitory concentration and minimum bactericidal concentration of the n-butanol fraction were < 1.89 mg/mL extract and 9.63 mg/ mL extract, respectively. The gas chromatographyemass spectrometry spectrum of the n-butanol fraction confirmed the presence of 13 peaks of different compounds with retention times of 9.11 minutes, 9.72 minutes, 10.13 minutes, 10.78 minutes, 12.37 minutes, 12.93 minutes, 18.16 minutes, 21.74 minutes, 21.84 minutes, 5.96 minutes, 12.93 minutes, 24.70 minutes, and 25.76 minutes. The six leading compounds were: diethyl phthalate: IUPAC name: diethyl benzene-1,2- dicarboxylate; 5-methyl-2-(1-methylethyl) phenol: IUPAC name: 5-methyl-2-propan-2-ylphenol; (E )-3,7-dimethylocta-2,6-diene-1-thiol: IUPAC name: (2Z)- 3,7-dimethylocta-2,6-diene-1-thiol; 2,6,10-dodecatrien-1-ol, 3,7,11-trimethyl-, (E,E ): IUPAC name: 2,6,10-dodecatrien-1-ol; 3,7,11-trimethyl-, (E,E); 2- methoxy-4-(2-propenyl) phenol: IUPAC name: 2-methoxy-4-[(1E)-prop-1-en-1-yl] phenol; hexadecanoic acid: IUPAC name: hexadecanoic acid. Conclusion: The presence of antimicrobial compounds that are therapeutically potent against MDR bacteria was confirmed in W. fruticosa. The crude leaf extract showed no host toxicity with human lymphocytes; the n-butanol fraction of the extract was the most suitable bioactive fraction. The terpenes isolated were: 5-methyl-2-(1-methylethyl) phenol, 2-methoxy-4-(2-propenyl) phenol, 2,6-octadien-1-ol, 3,7-dimethyl-(E )-2,6-octadienal, 3,7-dimethylcyclohexanol, and cyclohexanol, 2-methylene-5-(1-methylethenyl) which were reported to have specifically antimicrobial activity.

        데이터 공학 : 가중 윈도우를 통한 사용자 이력 기반 추천 시스템

        온라인에서 물품을 구매하고자 할 때, 추천 시스템은 사용자에 맞춘 추천을 하게 되며, 사용자가 관심을 가질만한 새로운 물품까지 추천해준다. Collaborative filtering 등, 여러 모델들이 보다 정확한 추천을 위해 제안되었으며, 활발히 연구되고 있다. 그중 Collaborative filtering은 사용자 선호도를 예측하는 데 좋은 결과를 보여주지만 사용자 개체 및 데이터가 부족한 환경에서는 사용자들끼리의 비교를 힘들게하여 collaborative filtering이 적용되기 힘들게 한다. 새로 시작하는 시스템이거나 사용자 개체 수가 적은 경우, 문제가 발생하며, 이와 같은 상황에서는 content-based filtering이 사용된다. 하지만 content-based filtering은 비슷한 물건만 추천해주거나, 사용자 성향 변화를 제대로 반영하지 못하는 등의 여러 단점을 가지고 있다. 이러한 한계점들을 극복하기 위해서 사용자 구매 기록에 가중 윈도우를 적용하고, 사용자 구매 기록 분석을 통한 윈도우 가중치 조정을 수행하는 시스템을 제안한다. 사용자 성향의 변화에 민감하게 반응할 수 있고, 이를 기반으로 무의미한 추천을 제거하며, 사용자가 찾기 어려운 관련 물품 추천이 가능한 새로운 상품도 추천하는 시스템을 제시하며, 언급된 사용자 개체 및 데이터가 부족한 환경에서의 동작을 검증하기 위해, 스타트업 무역업체에서 제공된 상품정보 기반 실험을 통해 제안된 시스템의 동작을 검증하였다. When we buy items in online stores, it is common to face recommended items that meet our interest. These recommendation system help users not only to find out related items, but also find new things that may interest users. Recommendation system has been widely studied and various models has been suggested such as, collaborative filtering and content-based filtering. Though collaborative filtering shows good performance for predicting users preference, there are some conditions where collaborative filtering cannot be applied. Sparsity in user data causes problems in comparing users. Systems which are newly starting or companies having small number of users are also hard to apply collaborative filtering. Content-based filtering should be used to support this conditions, but content-based filtering has some drawbacks and weakness which are tendency of recommending similar items, and keeping history of a user makes recommendation simple and not able to follow up users preference changes. To overcome this drawbacks and limitations, we suggest weighted window assisted user history based recommendation system, which captures user’s purchase patterns and applies them to window weight adjustment. The system is capable of following current preference of a user, removing useless recommendation and suggesting items which cannot be simply found by users. To examine the performance under user and data sparsity environment, we applied data from start-up trading company. Through the experiments, we evaluate the operation of the proposed recommendation system.

        Assessing the Relationship between MBTI User Personality and Smartphone Usage

        최근 스마트폰 사용 형태의 도움을 받아 사용자 특성을 예측하는 것은 매우 흥미롭고 주의를 사로잡는 연주 주제이다. 현재 몇몇 연구들은 사용자의 특성을 예측하기 위해 전화 사용 기록, 문자 메시지 사용 기록, 소셜 네트워크 서비스 사용 기록 등을 이용하고 있다. 이 논문에서, 우리는 MBTI 사용자 특성과 스마트폰 사용로그 간의 관계를 평가한다. 이를 위해, 스마트폰 사용 기록에서 부터 몇몇 특징들을 추출하고 이를 Naive Bayes와 SVM등의 분류기에 적용하여 사용자의 특성을 구분하였다. 사용자 특성 분석 결과의 분석을 통해 facebook사용 기록이 외향적인 사람과 내향적인 사람을 가장 잘 구분하는 것을 알 수 있었고, SVM 분류기가 Naive Bayes보다 사용자의 특성을 잘 예측하는 것을 확인하였다. We used $Na{\ddot{i}}ve$ Bayes and SVM classifier for classifying user personalities by extracting some features from smartphone usage data. From analysis it is observed that, among all extracted features facebook usage log working as the best feature for classification of introverts and extraverts; and SVM classifier works well as compared to $Na{\ddot{i}}ve$ Bayes.

        Risk based approach for design and optimization of site specific delivery of isoniazid

        The research envisaged focuses on risk managementapproach for development and optimization ofenteric coated tablet of isoniazid giving extended release inpH 6.8 phosphate buffer. Risk assessment using failure modeand effects analysis was done to depict the effects ofunambiguous failure modes related to particular formulation/process variable. A 2³ full factorial design wasemployed for optimization of core tablet to investigate effectof amount of Polyox WSR303 (A), hardness (B) and amountof ethyl cellulose (C) on percent drug release in pH 6.8phosphate buffer. Main effects and interaction plots weregenerated to study effects of variables. The selection ofoptimized formulation was done on overlay contour plotsand desirability function. The optimized formulationexhibited percent drug release at first hour of 26.97 %, secondhour of 44.20 %, fourth hour of 66.15 %and eighth hourof 97.9 % in phosphate buffer pH 6.8. Akaike informationcriteria and Model selection criteria revealed that the modelwas best described by Korsmeyer–Peppas power law. TheKopcha and Peppas–Sahlin model revealed diffusion aspredominant mechanism of release which may be due to highsolubility of drug and drug loading. Enteric coating optimizationrevealed weight gain of 10 %w/w as optimum; givingnil release of isoniazid in 0.1 N hydrochloric acid. Thecomposite desirability for optimized formulation computedusing equations and software were 0.91 and 0.90 respectively. Capability analysis on reproducibility batchesrevealed all indices above 1.33 signifying process was withincontrol of producing batches as per desired specifications.
