        Giant Ovarian Tumor Presenting as an Incarcerated Umbilical Hernia: A Case Report

        We report a rare case of a giant ovarian tumor presenting as an incarcerated umbilical hernia. A 61-yr-old woman was admitted to the hospital with severe abdominal pain, an umbilical mass, nausea and vomiting. On examination, a large, irreducible umbilical hernia was found. The woman underwent an urgent operation for a possible strangulated hernia. A large, multilocular tumor was found. The tumor was excised, and a total abdominal hysterectomy and bilateral salphingo-oophorectomy were performed. The woman was discharged 6 days after her admission. This is the first report of incarcerated umbilical hernia containing a giant ovarian tumor within the sac.

        Spinal Hydatid Cyst Disease : Challenging Surgery - an Institutional Experience

        Objective : Hydatid cyst disease is caused by the parasite Echinococcus granulosus. It is rarely seen in the vertebral system, occurring at a rate of 0.2-1%. The aim of this study is to present 12 spinal hydatid cyst cases, and propose a new type of drainage of the cyst. Methods : Twelve cases of spinal hydatid cysts, surgical operations, multiple operations, chronic recurrences, and spinal hydatic cyst excision methods are discussed in the context of the literature. Patients are operated between 2005 and 2016. All the patients are kept under routine follow up. Patient demographic data and clinicopathologic characteristics are examined. Results : Six male and six female patients with a median age of 38.6 at the time of surgery were included in the study. Spinal cyst hydatid infection sites were one odontoid, one cervical, five thoracic, two lumbar, and three sacral. In all cases, surgery was performed, with the aim of total excision of the cyst, decompression of the spinal cord, and if necessary, stabilization of the spinal column. Mean follow up was 61.3 months (10-156). All the patients were prescribed Albendazole. Three patients had secondary hydatid cyst infection (one lung and two hepatic). Conclusion : The two-way drainage catheter placed inside a cyst provides post-operative chlorhexidine washing inside the cavity. Although a spinal hydatid cyst is a benign pathology and seen rarely, it is extremely difficult to achieve a real cure for patients with this disease. Treatment modalities should be aggressive and include total excision of cyst without rupture, decompression of spinal cord, flushing of the area with scolicidal drugs, and ensuring spinal stabilization. After the operation the patients should be kept under routine follow up. Radiological and clinical examinations are useful in spotting a recurrence.

        Effectiveness of seismic repairing stages with CFRPs on the seismic performance of damaged RC frames

        This study aims at evaluating the performance of repairing technique with CFRPs in recovering cyclic performance of damaged columns in flexure in terms of structural response parameters such as strength, dissipated energy, stiffness degradation. A 2/3 scaled substandard reinforced concrete frame was constructed to represent the substandard RC buildings especially in developing countries. These substandard buildings have several structural deficiencies such as strong beam-weak column phenomenon, improper reinforcement detailing and poor material properties. Flexural plastic hinges occurred at the columns ends after testing the substandard specimen under both constant axial load and reversed cyclic lateral loading. Afterwards, the damaged columns were externally wrapped with CFRP sheets both in transverse and longitudinal directions and then retested under the same loading protocol. In addition, ambient vibration measurements were taken from the undamaged, damaged and the repaired specimens at each structural repair steps to identify the effectiveness of each repairing step by monitoring the change in the natural frequencies of the tested specimen. The ambient vibration test results showed that the applied repairing technique with external CFRP wrapping was proved to recover stiffness of the pre-damaged specimen. Moreover, the lateral load capacity of the pre-damaged substandard RC frame was restored with externally bonded CFRP sheets.

      • Multicenter Evaluation of Patients with Cutaneous Malignant Melanoma in Turkey: MELAS Study

        Background: Malignant melanoma is a cancer that demonstrates rapid progression and atypical clinically features with a poor prognosis. Aim: This study was performed to determine the clinical characteristics and treatment outcomes of patients with malignant melanoma in Turkey. Methods: The medical records of 98 patients between 2007-2012 at our centers were retrieved from the patient registry. Overall survival (OS) was calculated using the Kaplan-Meier method. Results: In our study, with the median follow-up of all patients with cutaneous MM of 46.3 months, the median OS rate of all cases was 43.6 months and 5-year OS was 48.6%. However, five-year OS rates of patients with localized disease (stage I-II) and node involvement (stage III) were 60.3% and 39.6%, respectively. The median OS of stage IV patients was 8.7 months and 1-year OS rate was 26.2%. We showed that advanced stage, male gender, and advanced age in all patients with MM were significant prognostic factors of OS. Conclusions: Compared with the results of current studies from Western countries, we found similar findings concerning demographical features, histological variables and survival analyses for our patients with cutaneous MM in Turkey.

        Does the Time From Biopsy to Radical Prostatectomy Affect Gleason Score Upgrading in Patients With Clinical T1c Prostate Cancer?

        Purpose: It is debated whether treatment delay worsens oncologic results in localizedprostate cancer (PCa). Few studies have focused on the role of a delay between the timeof biopsy and the time of surgery. Thus, we aimed to investigate the effect of the timeperiod between biopsy and surgery on Gleason score upgrading (GSU). Materials and Methods: A total of 290 patients who underwent radical retropubic prostatectomyin Ankara Training and Research Hospital were included in the study. Thebiopsy Gleason score, age, total prostate-specific antigen (PSA) value, prostate volumes,and PSA density (PSAD) were analyzed in all patients. The patients were dividedinto two groups: patients with GSU (group 1) and patients without GSU (group 2). Variables having a p-value of ≤0.05 in the univariate analysis were selected and thenevaluated by use of multivariate logistic regression models. Results were consideredsignificant at p<0.05. Results: GSU occurred in 121 of 290 patients (41.7%). The mean age of the patientswas 66.0±7.2 years in group 1 and 65.05±5.60 years in group 2 (p=0.18). The mean PSAvalues of groups 1 and 2 were 8.6±4.1 and 8.8±4.3 ng/dL, respectively. The mean prostatevolumes of groups 1 and 2 were 43.8±14.1 and 59.5±29.8 mL, respectively. ThePSAD of group 1 was significantly higher than that of group 2 (0.20 vs. 0.17, p=0.003). The mean time to surgery was shorter in group 2 (group 1, 52.2±22.6 days; group 2,45.3±15.5 days; p=0.004). According to the logistic regression, time from biopsy to surgeryis important in the prediction of GSU. Conclusions: We suggest that the time period between biopsy and surgery is a significantfactor that affects GSU in patients with clinically localized PCa.

        Comparative Analysis of Vertebroplasty and Kyphoplasty for Osteoporotic Vertebral Compression Fractures

        Study Design: A retrospective study. Purpose: The aim of this study is to compare the efficacy and outcome of vertebroplasty compared with unipedicular and bipedicular kyphoplasty for the treatment of osteoporotic vertebral compression fractures in terms of pain, functional capacity and height restoration rates. Overview of Literature: The vertebroplasty procedure was first performed in 1984 for the treatment of a hemangioma at the C2 vertebra. Kyphoplasty was first performed in 1998 and includes vertebral height restoration in addition to using inflation balloons and high-viscosity cement. Both are efficacious, safe and long-lasting procedures. However, controversy still exists about pain relief, improvement in functional capacity, quality of life and height restoration the superiority of these procedures and assessment of appropriate and specific indications of one over the other remains undefined. Methods: Between 2004 and 2011, 296 patients suffering from osteoporotic vertebral compression fracture underwent 433 vertebroplasty and kyphoplasty procedures. Visual analogue scale (VAS), the Oswestry Disability Index (ODI) and height restoration rates were used to evaluate the results. Results: Mean height restoration rate was 24.16%±1.27% in the vertebroplasty group, 24.25%±1.28% in the unipedicular kyphoplasty group and 37.05%±1.21% in the bipedicular kyphoplasty group. VAS and ODI scores improved all of the groups. Conclusions: Vertebroplasty and kyphoplasty are both effective in providing pain relief and improvement in functional capacity and quality of life after the procedure, but the bipedicular kyphoplasty procedure has a further advantage in terms of height restoration when compared to unipedicular kyphoplasty and vertebroplasty procedures.
