Codex Alimentarius Commission (Codex) has adopted the maximum permissible concentration of cadmium (Cd) in polished rice and other relevant crops, which requires an alleviation of the Cd contamination in rice grain. Various chemicals were tested for their Cd extraction efficiency by using three paddy soils, selecting ferric chloride (FeCl3) as a promising chemical for on-site soil washing. The comparison of FeCl3 extraction ability to that of various iron, manganese, and zinc salts revealed the primary extraction mechanism of FeCl3 to be proton release coupled with hydroxide generation (hydrolysis). This indicates that proton release from FeCl3 is controlled by the chemical equilibrium of hydroxide formation, and minimizes the negative effect on soil properties and environment, which are different from hydrochloric acid (HCl). Washing with FeCl3 led to the formation of Cd?chloride complexes, enhancing Cd extraction from the soils. We also developed in situ three-step washing method for Cd-contaminated paddy fields with FeCl3. The method was comprised of 1) chemically washing the field soil with a FeCl3 solution; 2) washing the treated soil with water to eliminate residual Cd and FeCl3; and 3) on-site treatment of wastewater using a portable wastewater treatment system. Concentrations of Cd in the treated water were below Japan`s environmental quality standard (0.01 mg L-1). The on-site soil washing confirmed the effectiveness of FeCl3 for decreasing Cd in soil and rice grains without negative effect on rice yield.

A surge of needs for the low carbon society promotes a spread of electric vehicle (EV). EVs could be charged at night simultaneously. That could cause severe voltage drop. Typical charger for EV employs the self-commutated inverter, which can control both active and reactive powers in principle. The authors have investigated potential availability of EVs for the distribution system operation qualitatively, and have concluded that the utilization of reactive power from EV charger could provide great potential for voltage regulation in the distribution system without loss of EV owner's convenience. The authors have proposed some voltage regulation algorithms using EVs. This paper proposes a new algorithm - a centralized control method in which the distribution system operator collects information of each EV in the distribution system and controls them.

Acute heart failure is associated with high mortality and frequent rehospitalization, resulting in enormous healthcare costs and declining physical function, activities of daily living, and quality of life. Cardiac rehabilitation has been recommended as one of the non-pharmacologic treatments for patients with heart failure. However, much of the evidence for cardiac rehabilitation interventions reported to date has been limited to chronic heart failure. In recent years, the effectiveness of rehabilitation intervention in patients with acute heart failure has been reported, led by the Rehabilitation Therapy in Older Acute Heart Failure Patients (REHAB-HF) trial. This review overviews the recent evidence of rehabilitation in patients with acute heart failure.
Giant Superior Mesenteric Artery Aneurysm Treated by Endovascular Treatment in a Very Elderly Female
Ryo Okubo,Shinsuke Kikuchi,Norifumi Otani,Masahiro Tsutsui,Hiroyuki Kamiya 대한혈관외과학회 2023 Vascular Specialist International Vol.39 No.2
Superior mesenteric artery (SMA) aneurysms (SMAAs) are rare and account for approximately 7% of all visceral artery aneurysms. If the anatomical complexity permits and the patency of organ perfusion is allowed, then an endovascular approach is the first choice for minimally invasive procedures. We report the case of a 92-year-old female with a giant SMAA and challenging anatomy, including a short proximal sealing zone from the origin of the SMA and a short distal sealing zone from the hepatic artery bifurcation. In view of her advanced age, she was treated endovascularly with covered stents. Reintervention was required to correct a postoperative endoleak; however, a favorable outcome was achieved with endovascular therapy.

Background: Achieving external access to and manual occlusion of the left atrial ap- pendage (LAA) during minimally invasive mitral valve surgery (MIMVS) through a small right thoracotomy is difficult. Occlusion of the LAA using an epicardial closure device seems quite useful compared to other surgical techniques. Methods: Fourteen patients with atrial fibrillation underwent MIMVS with concomitant surgical occlusion of the LAA using double-layered endocardial closure stitches (n=6, endocardial suture group) or the AtriClip Pro closure device (n=8, AtriClip group) at our institution. The primary safety endpoint was any device-related adverse event, and the primary efficacy endpoint was successful complete occlusion of blood flow into the LAA as assessed by transthoracic echocardiography at hospital discharge. The primary efficacy endpoint for stroke reduction was the occurrence of ischemic or hemorrhagic neurologic events. Results: All patients underwent LAA occlusion as scheduled. The cardiopulmonary by- pass and aortic cross-clamp times in the endocardial suture group and the AtriClip group were 202±39 and 128±41 minutes, and 213±53 and 136±44 minutes, respectively (p=0.68, p=0.73). No patients in either group experienced any device-related serious adverse events, incomplete LAA occlusion, early postoperative stroke, or neurologic complication. Conclusion: Epicardial LAA occlusion using the AtriClip Pro during MIMVS in patients with mitral valve disease and atrial fibrillation is a simple, safe, and effective adjunctive procedure.
