        Immunohistochemistry of Janus Kinase 1 (JAK1) Expression in Vitiligo

        Background: Vitiligo is a chronic autoimmune disease in which the destruction of melanocytes causes white spots on the affected skin. Janus kinase (JAK) is a family of intracellular, non-receptor tyrosine kinases that transduce cytokine-mediated signals via the JAK–signal transducer and activator of transcription pathway. The aim of the present study is to explore the possible role of JAK1 in the pathogenesis of vitiligo using immunohistochemical methods. Methods: The current study was conducted in a sample of 39 patients who presented with vitiligo and 22 healthy individuals who were age and sex matched as a control group. We used immunohistochemistry to evaluate JAK1 status (intensity and distribution) and assess the percentage of residual melanocytes using human melanoma black 45 (HMB45). Results: Intense and diffuse JAK1 expression was significantly more likely to indicate vitiliginous skin compared to normal skin (p < .001). Strong and diffuse JAK1 expression was associated with short disease duration, female sex, and lower percentage of melanocytes (detected by HMB45) (p < .05). Conclusions: JAK1 may be involved in the pathogenesis of vitiligo, as indicated by intense and diffuse expression compared to control and association with lower percentage of melanocytes detected by HMB45 immunostaining.

        A Hardware Implementation of Flexible Attitude Determination and Control System for Two‑Axis‑Stabilized CubeSat

        This paper presents a hardware implementation of fexible and low-cost attitude determination and control system (ADCS) for two-axis-stabilized CubeSat. As small satellite missions are increasing, the CubeSat requires precise ADCS with attitude drift adjustment. This attitude drift if not properly compensated, will cause a slow attitude information loss as the error in attitude rises between the actual and estimated signals. The proposed ADCS comprises two steps; the attitude determination which estimates the current CubeSat’s attitude and a novel simplifed intelligent proportional-integral control algorithm that accurately adjusts the attitude. The control algorithm is based on the multi degree-of-freedom controller concept and has no controller gains parameters. The proposed ADCS employs sun sensor, magnetometer, and a micro-electro-mechanical gyroscope sensor to correct the attitude drift by ofering a comparative attitude that is utilized for updating the estimated attitude delivered to the Kalman flter for determining the CubeSat’s attitude and angular velocity. The ADCS model verifcation and validation are accomplished via Matlab/Simulink and hardware implementation. A comparison with other ADCS techniques is presented. The ADCS simulated model demonstrates precision results with error of less than 0.1°.

        Diagnostic accuracy and temporal impact of ultrasound in patients with dyspnea admitted to the emergency department

        Objective Few studies have prospectively evaluated the diagnostic accuracy and temporal impact of ultrasound in the emergency department (ED) in a randomized manner. In this study, we aimed to perform a randomized, standard therapy controlled evaluation of the diagnostic accuracy and temporal impact of a standardized ultrasound strategy, versus standard care, in patients presenting to the ED with acute dyspnea. Methods The patients underwent a standardized ultrasound examination that was blinded to the team caring for the patient. Ultrasound results remained blinded in patients randomized to the treating team but were unblinded in the interventional cohort. Scans were performed by trained emergency physicians. The gold standard diagnosis (GSDx) was determined by two physicians blinded to the ultrasound results. The same two physicians reviewed all data >30 days after the index visit. Results Fifty-nine randomized patients were enrolled. The mean±standard deviation age was 54.4±11 years, and 37 (62%) were male. The most common GSDx was acute heart failure with reduced ejection fraction in 13 (28.3%) patients and airway diseases such as acute exacerbation of asthma or chronic obstructive pulmonary disease in 10 (21.7%). ED diagnostic accuracy, as compared to the GSDx, was 76% in the ultrasound cohort and 79% in the standard care cohort (P=0.796). Compared with the standard care cohort, the final diagnosis was obtained much faster in the ultrasound cohort (mean±standard deviation: 12±3.2 minutes vs. 270 minutes, P<0.001). Conclusion A standardized ultrasound approach is equally accurate, but enables faster ED diagnosis of acute dyspnea than standard care.

      • The Critical Factors of Project Management in Sudanese Construction Projects

        The Sudanese construction sector is characterized by many small and large projects and high labor intensity, and accounted for 3.2% of the Sudanese country's GDP. The basic problems facing the Sudanese construction projects are the factors that affect on construction project performance. The objectives include identifying the factors affecting the performance of Sudanese construction projects, and to determine the critical factors. The literature review has been done to gather the information about the causes and their factors that affect on the performance of construction projects from the previous researches. The research methodology was conducted to gather the data by questionnaire which was examined to be reliable and valid according to statistical tests. The (34) factors were identified as factors affect on construction projects and the (10) factors were the critical factors which may lead to poor performance of Sudanese construction projects. This study has some conclusions such as the instrument for measuring the critical factors on the performance is reliable and valid, so the project management stage performance is 64.2%.

        Reliability and Safety of Cross-Leg Free Latissmus Dorsi Muscle Flap in Reconstruction of Mutilating Leg Injuries Using End-to-Side Anastomosis

        Background Free tissue transfer is considered the gold standard option for the reconstruction of distal leg defects. Free tissue transfer using recipient vessels in the contralateral leg (cross-leg bridge) is a potential option to supply the flap if there are no suitable recipient vessels in the injured leg. Most studies have described this technique using end-to-end anastomosis which sacrifices the main vessel in the uninjured leg. This study evaluated the use of a cross-leg free latissimus dorsi muscle flap for the reconstruction of defects in single-vessel legs, using end-to-side anastomosis to recipient vessels in the contralateral leg without sacrificing any vessel in the uninjured leg.Methods This is a retrospective study that included 22 consecutive patients with soft tissue defects over the lower leg. All the reconstructed legs had a single artery as documented by CT angiography. All patients underwent cross-leg free latissimus dorsi muscle flap using end-to-side anastomosis to the posterior tibial vessels of the contralateral leg.Results The age at surgery ranged from 12 to 31 years and the mean defect size was 86 cm2. Complete flap survival occurred in 20 cases (91%). One patient had total flap ischemia. Another patient had distal flap ischemia.Conclusion Cross-leg free latissimus dorsi muscle flap is a reliable and safe technique for the reconstruction and salvage of mutilating leg injuries, especially in cases of leg injuries with a single artery. As far as preservation of the donor limb circulation is concerned, end-to-side anastomosis is a reasonable option as it maintains the continuity of the donor leg vessels.

        Why do consumers use Facebook brand pages? A case study of a leading fast-food brand fan page in Egypt

        This paper identifies the reasons that make consumers actively engage on Facebook brand pages. By adopting the Uses and Gratification Theory, this article presents a number of uses for these online brand communities. A qualitative study was conducted to collect data from the Facebook page of a leading fast-food brand in Egypt. A content analysis for the comments that were posted by consumers on the brand page over the period of three months was conducted. The findings showed that consumers actively contribute to the content of the brand page for eight reasons which are: search for information, complaining, socializing, obtaining incentives, entertainment seeking, expression of emotions, brand advocacy and enhancing social image. Prior research has focused on consumer participation in offline brand communities. Given the rapid adoption of social media based brand communities in marketing, this article expands the research focus to the social media domains, which have largely replaced traditional marketing methods. The article provides some useful guidelines for companies to follow when adopting marketing on Facebook brand pages. By understanding why consumers use these pages, companies can execute more effective social media marketing strategies that allow them to fully grasp the potentials of these new media.

        Light and scanning electron microscopic studies of Lepeophtheirus salmonis (Siphonostomatoida: Caligidae) infecting the rosy goatfish Parupeneus rubescens

        Parasites, particularly crustaceans, are considered as an important limiting factor in the development ofintensified fish culture. The parasitic copepod of the Arabian Gulf fish is poorly known. So, the currentinvestigation aimed to record the caligid species infecting the rosy goatfish Parupeneus rubescens in theArabian Gulf. One copepod species was identified belonged to family Caligidae and labeled as Lepeophtheirussalmonis Krøyer 1837 depends mainly on its morphological, morphometric, and ultrastructuralcharacteristics. It is characterized by lacking lunules, the anterior margin of the cephalothorax is notfolded, three-segmented exopod of the fourth leg and longer than the protopod, leg 4 with the middleterminal claw as long as the inner one, accessory processes on terminal elements II and III of the exopodof the first leg, a genital complex distinctly wider than being long, and a relatively short abdomen. Therefore, the present study demonstrated that the rosy goatfish represents a new host for Lepeophtheirusspecies and added locality records for this infection.

        Synthesis, Structural Characterization, and Antimicrobial Activities of Mn(II), Co(II), Ni(II), Cu(II) and Zn(II) Complexes of Triazole‐based Azodyes

        <P><B>Abstract</B></P><P>The synthesis and characterization of new transition metal complexes of Mn(II), Co(II), Ni(II), Cu(II) and Zn(II) with 3‐(2‐hydroxynaph‐1‐ylazo)‐1,2,4‐triazole (<B>HL<SUP>1</SUP></B>) and 3‐(2‐hydroxy‐3‐carboxynaph‐1‐ylazo)‐1,2,4‐triazole (<B>HL<SUP>2</SUP></B>) have been carried out. Their structures were confirmed by elemental analyses, thermal analyses, spectral and magnetic data. The IR and <SUP>1</SUP>H NMR spectra indicated that <B>HL<SUP>1</SUP></B> and <B>HL<SUP>2</SUP></B> coordinated to the metal ions as bidentate monobasic ligands via the hydroxyl O and azo N atoms. The UV‐Vis, ESR spectra and magnetic moment data revealed the formation of octahedral complexes [Mn<B>L<SUP>1</SUP></B>(AcO)(H<SUB>2</SUB>O)<SUB>3</SUB>] (<B>1</B>), [Co<B>L<SUP>1</SUP></B>(AcO)(H<SUB>2</SUB>O)<SUB>3</SUB>]·H<SUB>2</SUB>O (<B>2</B>), [Mn<B>L<SUP>2</SUP></B>(AcO)(H<SUB>2</SUB>O)<SUB>3</SUB>] (<B>6</B>) and [Co<B>L<SUP>2</SUP></B>(AcO)(H<SUB>2</SUB>O)<SUB>3</SUB>] (<B>7</B>), [Ni<B>L<SUP>1</SUP></B>(AcO)(H<SUB>2</SUB>O)] (<B>3</B>), [Zn<B>L<SUP>1</SUP></B>(AcO)(H<SUB>2</SUB>O)]·H<SUB>2</SUB>O (<B>5</B>), [Ni<B>L<SUP>2</SUP></B>(AcO)(H<SUB>2</SUB>O)] (<B>8</B>), [Zn<B>L<SUP>2</SUP></B>(AcO)(H<SUB>2</SUB>O)]·10H<SUB>2</SUB>O (<B>10</B>) have tetrahedral geometry, whereas [Cu<B>L<SUP>1</SUP></B>(AcO)(H<SUB>2</SUB>O)<SUB>2</SUB>] (<B>4</B>) and [Cu<B>L<SUP>2</SUP></B>(AcO)(H<SUB>2</SUB>O)<SUB>2</SUB>]·5H<SUB>2</SUB>O (<B>9</B>) have square pyramidal geometry.. The mass spectra of the complexes under EI‐con‐ ditions showed the highest peaks corresponding to their molecular weights, based on the atomic weights of <SUP>55</SUP>Mn, <SUP>59</SUP>Co, <SUP>58</SUP>Ni, <SUP>63</SUP>Cu and <SUP>64</SUP>Zn isotopes; besides, other peaks containing other isotopes distribution of the metal. Kinetic and thermodynamic parameters of the thermal decomposition stages were computed from the thermal data using Coats‐Redfern method. <B>HL<SUP>2</SUP></B> and complexes <B>6</B>–<B>10</B> were found to have moderate antimicrobial activities against <I>Staphylococcus aureus </I>(gram positive), <I>Escherichia coli </I>(gram negative) and <I>Salmonella </I>sp bacteria, and antifungal activity against <I>Fusarium oxysporum</I>, <I>Aspergillus niger</I> and <I>Candida albicans</I>. Also, in most cases, metallation increased the activity compared with the free ligand.</P>

        Selective Harmonic Elimination in Multi-level Inverters with Series-Connected Transformers with Equal Power Ratings

        This study applies the selective harmonic elimination (SHE) technique to design and operate a regulated AC/DC/AC power supply suitable for maritime military applications and underground trains. The input is a single 50/60 Hz AC voltage, and the output is a 400 Hz regulated voltage. The switching angles for a multi-level inverter and transformer turns ratio are determined to operate with special connected transformers with equal power ratings and produce an almost sinusoidal current. As a result of its capability of directly controlling harmonics, the SHE technique is applicable to apparatus with congenital immunity to specific harmonics, such as series-connected transformers, which are specially designed to equally share the total load power. In the present work, a singlephase 50/60 Hz input source is rectified via a semi-controlled bridge rectifier to control DC voltage levels and thereby regulate the output load voltage at a constant level. The DC-rectified voltage then supplies six single-phase quazi-square H-bridge inverters, each of which supplies the primary of a single-phase transformer. The secondaries of the six transformers are connected in series. Through off-line calculation, the switching angles of the six inverters and the turns ratios of the six transformers are designed to ensure equal power distribution for the transformers. The SHE technique is also employed to eliminate the higher-order harmonics of the output voltage. A digital implementation is carried out to determine the switching angles. Theoretical results are demonstrated, and a scaled-down experimental 600 VA prototype is built to verify the validity of the proposed system.

        Extent of Hyperostotic Bone Resection in Convexity Meningioma to Achieve Pathologically Free Margins

        Objective : Hyperostosis in meningiomas can be present in 4.5% to 44% of cases. Radical resection should include aggressive removal of invaded bone. It is not clear however to what extent bone removal should be carried to achieve pathologically free margins, especially that in many cases, there is a T2 hyperintense signal that extends beyond the hyperostotic bone. In this study we try to investigate the perimeter of tumour cells outside the visible nidus of hyperostotic bone and to what extent they are present outside this nidus. This would serve as an initial step for setting guidelines on dealing with hyperostosis in meningioma surgery. Methods : This is a prospective case series that included 14 patients with convexity meningiomas and hyperostosis during the period from March 2017 to August 2018 in two university hospitals. Patients demographics, clinical, imaging characteristics, intraoperative and postoperative data were collected and analysed. In all cases, all visible abnormal bone was excised bearing in mind to also include the hyperintense diploe in magnetic resonance imaging (MRI) T2 weighted images after careful preoperative assessment. To examine bony tumour invasion, five marked bone biopsies were taken from the craniotomy flap for histopathological examinations. These include one from the centre of hyperostotic nidus and the other four from the corners at a 2-cm distance from the margin of the nidus. Results : Our study included five males (35.7%) and nine females (64.3%) with a mean age of 43.75 years (33-55). Tumor site was parietal in seven cases (50%), fronto-parietal in three cases (21.4%), parieto-occipital in two cases (14.2%), frontal region in one case and bicoronal (midline) in one case. Tumour pathology revealed a World Health Organization (WHO) grade I in seven cases (50%), atypical meningioma (WHO II) in five cases (35.7%) and anaplastic meningioma (WHO III) in two cases (14.2%). In all grade I and II meningiomas, bone biopsies harvested from the nidus revealed infiltration with tumour cells while all other bone biopsies from the four corners (2 cm from nidus) were free. In cases of anaplastic meningiomas, all five biopsies were positive for tumour cells. Conclusion : Removal of the gross epicentre of hyperostotic bone with the surrounding 2 cm is adequate to ensure radical excision and free bone margins in grade I and II meningiomas. Hyperintense signal change in MRI T2 weighted images, even beyond visible hypersototic areas, doesn't necessarily represent tumour invasion.
