        Estimation of Optimum Flip Angle using 3D VANE XD Technique: Focused on Pre-Contrast and Hepatobiliary Phase

        To investigate the optimum flip angle that can enhance image quality, SNR (signal to noise ratio), and CNR (contrast to noise ratio), comparing the images obtained, applying flip angles, 11°, 14°, 17°, 20°, and 23° in getting Liver Hepatobiliary Phase image using 3D VANE XD(3D Multivane mDixon, Philips Healthcare) technique. Experiments were conducted on a total of 30 outpatients and inpatients to our hospital (HCC:10, Metastasis:10, Abscess:10). As for the equipment used in the experiments, Philips Ingenia 3.0T CX was used, and all parameters other than the flip angle were set the same to conduct the tests. As for the image analysis method, using the Image-J program (National Institutes of Health and LOCI), the SNR of the liver, kidney, and pancreas obtained from the images by flip angle before the contrast medium injection and the CNR between the lesion and the normal tissue after the contrast medium injection were measured to conduct comparative analysis. As a result of a comparison of images before and after the contrast medium injection by disease, when the flip angle of 17° was applied, SNR and CNR were measured higher than in the images of other flip angles (p<0.05). In the comparisons of the images taken before and after the injection of contrast medium by disease, when a flip angle of 17° was applied, the SNR before contrast medium injection was 28-29 % higher, and the SNR after the injection of contrast medium was 11 % up to 49 % higher than that at other flip angles. There was a difference in CNR before contrast medium injection of 30-43 % and CNR after contrast medium injection of 58-68 %. The measured value increased up to 17° and then decreased after that. Additionally, in the qualitative evaluation, Lesion Conspicuity (p=0.003), Image Artifact (p=0.0001), Lesion Delineation (p=0.0002), and Vascular Anatomy (p=0.0002) received the most excellent evaluations at 17°. In conclusion, in this study, the flip angle of 17° provided the highest SNR and CNR values when the tests were conducted using the free breath hold technique, 3D VANE XD Sequence. Thus, in liver MRI protocol tests, the overall diagnostic information was provided, including hypervascular tumor.

        A Study on Development of Phase Array Coil for MRI of Mouse in the 3.0 T High Magnetic Field

        <P>In this study, we manufactured an animal-dedicated radio frequency (RF) coil with a small diameter and multiple channels that could improve both image uniformity and signal-to-noise ratio (SNR). Quality of images from the manufactured coil was assessed compared to the loop coil used primarily for animal experiments and that had the same diameter. For our experiments, we used Balb/C nude mice that are most commonly used, and 3.0 T systems. The mouse body coil was a receive-only (Rx-only) coil with four channels. The coil had a diameter of 40 mm, length of 100 mm, and an overlapped distance between channels of 4 mm. A dual echo steady state (DESS) 2D pulse sequence, the type most frequently used for animal experiments, was applied. To evaluate the SNR of the coil cylinder that had a different number of channels, we used the loop coil typically designated for human use and the manufactured coil specific for animal use to obtain and compare phantom and mouse images. According to the results the image analysis, image uniformity and SNR were high for the image obtained from the animal-specific coil that had many channels. In conclusion, our experiment demonstrated that a 3.0 T phase array coil with multiple channels designed for animal use only produced images with excellent uniformity and SNR.</P>

        Comparison of Image Quality in Magnetic Resonance Imaging of the Abdominal Organ at 1.5T and 3.0T before the Gadolinium Injection

        본 연구는 조영제 주입 전 복부 검사 시 필수적으로 적용되고 있는 HASTE, HASTE(f/s), FFE(in, out)를 중심으로 두 기기에 대한 복부장기의 신호 대 잡음비(SNR; Signal to Noise Ratio)와 대조도 대 잡음비(CNR;Contrast to Noise Ratio)를 정량적으로 평가함으로서 최적의 장비선택을 알아보고자 하였다. 데이터분석은 1.5T 와 3.0T 자기공명영상기기(Philips medical system, Netherland)를 이용하여 검사한 복부영상을 무작위로 선정하여 분석을 하였다. 정량적 분석결과 간(Liver), 신장(Kidney), 비장(Spleen)에서는 1.5와 비교했을 때 3.0T가 SNR, CNR 값이 높게 나타났고(p<0.05), 위(Stomach), 복부지방(Abdominal Fat), 췌장(Pancreas)에서는 1.5T 가 높은 결과를 얻었다(p<0.05). 결론적으로 두기기별 장기에 대한 정량적 평가를 했을 때 인체의 바같 부분 조직은 전반적으로 3.0T 가 높게 나타났고 가스를 포함하여 자화율의 차이를 많이 발생시키는 안쪽부 분의 장기는 1.5T 가 높은 결과를 얻었다. 이러한 결과는 환자상태에 따라 조영제를 사용하지 못하고 MRI 검사를 하는 경우 정확한 진단학적 정보를 제공하는데 가이드라인이 될 것이다. The sudy was intended to evaluate the optimal equipment selection by quantitatively assessing the SNR(signal to noise ratio) and CNR(contrast to noise ratio) on the abdominal organ. This study performed on 1.5 T and 3.0 T MRI units focusing on HASTE, HASTE(f/s) and FFE(in of phase), FFE(out of phase) without using the contrast medium(Gadolinium). The data analysis was performed by randomly selecting on 1.5 T and 3.0 T abdominal MRI images. As a results, SNR and CNR values of 3.0 T is higher than 1.5 T at liver, kidney and spleen(p<0.05). Stomach, abdominal fat and pancreas was obtained a higher value at 1.5 T(p<0.05). On conclusion, the organs of outer part in the body showed generally a high value at 3.0 T, and the organs of inner part in the body including the gas showed a high value at 3.0 T because of a large difference on magnetic susceptibility.

        Assessment of Magnetic Resonance Image Quality For Ferromagnetic Artifact Generation: Comparison with 1.5T and 3.0T.

        In this research, 15 patients were diagnosed with 1.5T and 3.0T MRI instruments (Philips, Medical System, Achieva) to minize Ferromagnetic artifact and find the optimized Tesla. Based on the theory that the 3.0T, when compared to 1.5T, show relatively high signal-to-ratio(SNR), Scan time can be shortened or adjust the image resolution. However, when using the 3.0T MRI instruments, various artifact due to the magnetic field difference can degrade the diagnostic information. For the analysis condition, area of interest is set at the background of the T1, T2 sagittal image followed by evaluation of L3, L4, L5 SNR, length of 3 parts with Ferromagnetic artifact, and Histogram. The validity evaluation was performed by using the independent t test. As a result, for the SNR evaluation, mere difference in value was observed for L3 between 1.5T and 3.0T, while big differences were observed for both L4, and L5(p<0.05). Shorter length was observed for the 1.5T when observing 3 parts with Ferromagnetic artifact, thus we can conclude that 3.0T can provide more information on about peripheral tissue diagnostic information(p<0.05). Finally, 1.5T showed higher counts values for the Histogram evaluation(p<0.05). As a result, when we have compared the 1.5T and 3.0T with SNR, length of Ferromagnetic artifact, Histogram, we believe that using a Low Tesla for Spine MRI test can achieve the optimal image information for patients with disk operation like PLIF, etc. in the past. 본 연구는 총 15명의 정상인 지원자를 대상으로 1.5T와 3.0T의 자기공명영상기기(Philips, Medical System, Achieva)를 이용하여 강자성 인공물을 최소화하기 위한 최적의 Tesla를 알아보고자 하였다. 평가는 신호대 잡음비 평가, 강자성 인공물이 형성된 부위의 길이와 Histogram을 평가하였다. 분석방법으로는 T1, T2 sagitt al 영상의 Background의 4 부위에 관심영역을 설정하고 L3, L4, L5의 각 추체부에 관심영역을 설정한 후 신호대잡음비 값을 측정하였고, 인공물이 형성된 3 부위에 관심영역을 설정하고 길이 값을 측정하였고, Im age J 프로그램을 이용해 인공물 영역의 히스토그램 분포 값을 측정하였다. 본 실험에 대한 결과로 신호대 잡음비 평가에서 L3에서는 1.5T와 3.0T 사이에 별 차이가 없는 것으로 나타났고, L4, L5에서는 큰 차이가 있는 것으로 나타났다(p<0.05). 강자성 인공물이 형성된 3 부위의 길이는 3.0T에 비해 1.5T가 더 짧아 주변 조직에 대한 진단적 정보를 더 얻을 수 있는 것으로 나타났다(p<0.05). Histogram평가에서는 3.0T보다 1.5T 가 Count값이 높게 나타났다(p<0.05). 결론적으로 1.5T와 3.0T의 신호대 잡음비, 강자성 인공물의 길이, Hist ogram을 비교 평가해 봤을 때, Spine MRI 검사 시에 PLIF 등 디스크 수술을 받은 환자에게는 Low Tesla로 검사가 최적의 영상정보를 얻을 수 있었다.

        The Evaluation of Image Quality in Gradient Echo MRI of the Pancreas

        본 데이터 분석은 췌장의 묘출을 위한 2D T1 FFE 과 3D T1 THRIVE 기법을 비교하고자 하였다. 1.5T에서 검사한 총 85명(남자 45명, 여자 40 명, 연령범위 38-80세, 평균연령 58세)의 데이터를 PACS 네트워크(network)로 분석을 하였다. 평균 2D T1 FFE(SNR: 46.42 ± 0.67, CNR: 28.16 ± 0.50,)보다 3DT1 THRIVE(SNR: 53.84 ± 1.20, CNR: 35.48 ± 0.70)값이 의미 있게 높은 값을 보여주었고(p<0.05), 영상의 질적인면(2D T1 FFE: 2.2 ± 0.05, 3D T1 THRIVE: 2.63 ± 0.14)에서도 2D T1 FFE 기법에 비해 3D T1 THRIVE 기법이 모두 우위의 값을 얻게 되었다(p<0.05). 그러나 3D T1 THRIVE 기법이 높은 값을 얻었지만 영상의 질을 감소시키는 몇 가지 인공물이 발생하였다. 결론적으로 1.5 T MRI 기기를 이용한 3D T1 THRIVE 기법이 SNR, CNR, 영상의 질적 측면에서 증가된 결과를 얻었다. The purpose of this analysis is to compare 2D T1 FEE and 3D T1 THRIVE for demonstration of the pancreas. A total of 85(45 men, 40 women; 58 years) PACS network datum were analysis clinically indicated pancreas MRI at 1.5 T. The SNRs and CNRs of 3D T1 THRIVE(SNR: 46.42 ± 0.67, CNR: 28.16 ± 0.50) showed significantly higher values than those from 2D T1 FEE(SNR: 53.84 ± 1.20, CNR: 35.48 ± 0.70), p<0.05, The image quality of the 3D T1 THRIVE(2.63 ± 0.14) was significantly superior to that with the 2D T1 FEE(2.2 ± 0.05), but 3D T1 THRIVE revealed several artifacts resulting in poor quality. In conclusion, The 3D T1 THRIVE technique with a 1.5 T resulting in improved SNRs, CNRs and image quality was demonstrated.

        A Study of Whiter Matter Fiber Tractography in Young Internet Addiction Disorder using a Brain Diffusion Tensor Magnetic Resonance Imaging

        3.0 T 자기공명영상기기에서 확산텐서영상을 이용하여 청소년을 대상으로 정상인과 인터넷 중독자의 뇌량과 내섬유막 구조의 변화에 대하여 정량적으로 알아보고자 하였다. 10명의 인터넷 중독자와 정상인 12명을 대상으로 검사를 하였고, 정량적 분석을 위하여 뇌량의 5부위와 내섬유막의 2부위를 View Forum 프로그램을 이용하여 두 그룹에 대한 신경섬유의 손상 정도를 FA, ADC(10 -3 mm 2 /s), Length(mm)값으로 측정하였다. 그룹 간 통계적 유의성은 Independent t-test와 pearson 상관관계로 하였다(p<0.05). 정상인과 인터넷 중독자에 관하여 신경섬유 손상정도는 7부위에 대하여 FA, ADC(10 -3 mm 2 /s), Length(mm)값에 대하여 의미 있는 차이를 보였다(p<0.05). 또한 두 그룹에서 IAD scale 과 뇌량의 FA값은 음의 적으로 강하게 상관관계가 있었다(p<0.000). 결론적으로, 확산텐서영상은 FA, ADC(10 -3 mm 2 /s), Length(mm)값에 대하여 유의성 있는 차이를 보여고, 이 결과는 뇌 백질에 대한 손상정도에 관하여 정량적으로 임상적 정보를 제공하였다. The goal of this study is to investigate corpus callosum and both internal capsule changes with the internet addiction disorder compared to control group using MR diffusion tensor imaging. A total of 22 teenager volunteers who had 10 high-risk group with internet addiction and 12 normal control group were conducted for this study. Imaging was conducted on a 3 T using a EPI sequence. Image evaluation was analysed of the FA, ADC(10 -3 mm 2 /s), length(mm). We did select ROI for image tracking on corpus callosum of 5 and including 2(internal capsule). The data from these ROIs were compared statistically among the groups using independent t-test, correlation coefficient. There were significant inter-group differences(p<0.05) among FA, ADC(10 -3 mm 2 /s) and length(mm). And also significantly negative correlations were fond between FA values of corpus callosum and IAD scale(p=0.000). DTI was shown significant changes of FA and ADC, LNF values in IAD compared to control group. Therefore, our results may provided clinical information for brain wite matter functions.

        A Study on the Image Quality According to the Change of Flip Angle in Flow-Related Enhancement Magnetic Resonance Angiography

        본 연구는 Image J 프로그램을 사용하여 유속증가 자기공명영상기법의 숙임각 변화에 따른 SNR와 CNR 을 측정하여 최적의 숙임각을 알아보기 위해 연구하였다. 총 30명의 정상인 지원자를 대상으로 1.5T 자기공명영상기기(Philips, Medical System, Achieva)를 이용하여 뇌동맥검사 후 평가를 실시하였다. 분석 방법으 로 유속증가 자기공명 혈관 조영술에 대하여 4 부위에 관심영역을 설정하고 SNR와 CNR을 평가하였다. 5가지 숙임각에서의 정량적 분석으로 SNR과 CNR에 대한 통계적 유의성은 일원분산분석으로 계산되었으며, 사후 분석으로는 Bonferroni 법을 적용하였고, 통계에 사용된 프로그램은 SPSS 14.0을 이용하여 p 값을 0.05 이하일 때 유의성을 두었다. 본 실험에 대한 결과로서 전교통동맥(SNR:876.59 ± 14.22, CNR:1999.7 ± 12.5), 후교통동맥 (SNR: 863.48 ± 13.29, CNR:1870.18 ± 12.56), ICA(SNR: 1116.87 ± 08.34, CNR:2979.37 ± 14.69), 중대뇌동맥(SNR:848.66 ± 15.25, CNR:2199.25 ± 13.48)의 값으로 25°에서 가장 높은 신호강도를 보였다(p<0.05). 작은 혈관 묘출로서 a1, a2, a3, p1, p2, p3, m1, m2, m3 값 또한 동일한 결과 이었다(p<0.05). 사후분석결과로, 숙임각에서 25° 기준으로 10°, 15°, 20°에서 유의성 있는 결과를 얻었지만(p=0.000) 30° 에서는 유의한 차이가 없었다(p>0.05). 결론적으로, 본 연구에서 30° 에서는 신호강도가 떨어지기 때문에 뇌혈관 자기공명조영술에서 최적의 숙임각은 25°로 나타냈다. The purpose of this study was to investigate the optimal flip angle by measuring the SNR and CNR according to the angle of changes of the MRI technique using the Image J program. A total of 30 normal volunteers were assessed by using a 1.5T magnetic resonance imaging system (Philips, Medical System, Achieva). For the MRI angiography, we set the region of interest in four regions and evaluated the SNR and CNR. The statistical significance of SNR and CNR was calculated by one-way ANOVA using quantitative analysis at five different positions. The Bonferroni method was used for post-hoc analyzes. Statistical significance was determined by using ANOVA analysis at p<0.05 and Bonferroni method was used as a post-hoc analysis. The results of this study, the measurement values o f ACA(SNR:876.59 ± 14.22, CNR:1999.7 ± 12.5), PCA(SNR:863.48 ± 13.29, CNR: 1870.18 ± 12.56), ICA(SNR:1116.87 ± 08.34, CNR:2979.37 ± 14.69) and MCA(SNR:848.66 ± 15.25, CNR: 2199.25 ± 13.48) were obtained with the high signal intensity at 25°(p<0.05). The values of a1, a2, a3, p1, p2, p3, m1, m2 and m3 were also the same (p<0.05). Post-hoc analysis results, There was a statistically significant difference (p=0.000) between 10°, 15°, 20° on the 25° reference for the flip angle, but no significant results were obtained with 30°(p<0.05). In concision, because the signal intensity decreased at 30°, this study revealed that the optimal flip angles were 25° in cerebrovascular MR angiography.

        Qualitative Assessment of Contrast-Enhanced Magnetic Resonance Angiography using Breath-Hold and Non-Breath-Hold Techniques in the Portal Venous System

        The purpose of this study is to evaluate the image quality in delineation of the portal venous systems with two different methods, breath-hold and non-breath-hold by using the 3D FLASH sequence. We used a 1.5 T system to obtain magnetic resonance(MR)images. Arterial and portal phase 3D FLASH images were obtained with breath-hold after a bolus injection of GD-DOTA. The detection of PVS on the MR angiograms was classified into three grades. First, the angiograms of the breath-hold method showed well the portal vein, the splenic vein and the superior mesenteric vein systems in 13 of 15 patients (86%) and the inferior mesenteric vein system in 6 of 15 patients (40%), Second, MR angiograms of the non-breath-hold method demonstrated the PVS and the SMV in 12 of 15 patients (80%) and the IMV in 5 of 15 patients (33%). Our study showed contrast-enhanced 3D FLASH MR angiography, together with the breath-hold technique, may provide reliable and accurate information on the portal venous system.

        Development of Automatic Voice System for Foreign Patients in Testing of Magnetic Resonance Imaging

        본 연구의 목적은 자기공명영상을 이용하여 외국인 환자를 검사할 때 언어의 소통을 위하여 자동음성시스템을 개발하고자 하였다. 자동음성을 위해 사용된 문장은 병원에서 자주 사용하고 있는 12개의 문장과 5개의 외국어를 선택하였다. 이러한 번역된 문장은 Oddcast’s website로 부터 무료로 재공 되며 Wav 파일로서 각 언어 특징에 소리로 기록하게 된다. 기록된 오디오 파일은 Mobile phone 환경에서 적용되는 PHP 프로그램을 사용하여 MS-파워포인트(extension; ppsx) 와 Web-enabled system 형태로 재생하게 된다. 음성 시스템 개발은 방사선사에게 언어 소통의 어려움에 관하여 스트레스를 해소될 것이며, 다양한 언어로 설계된 자동음성시스템은 외국인 환자를 검사 할 때 환자에게 진단적 정보를 증가시킬 것이다. 본 연구를 위해 개발된 음성 시스템은 한국의 방사선사와 외국인 환자 사이에서 검사의 이해를 증가시킬 수 있기 때문에 임상실무에서 더욱더 유용하게 이용될 것이라고 기대된다 The purpose of this study intend to develop the automatic voice system for language communicating over foreign patients in testing of magnetic resonance imaging. Used sentences for the automatic voice system was selected with twelve sentences and five foreign languages in hospital in use. These translated texts were provided free of charge from Oddcast’s website, which is recorded each language-specific voice as Wav files, The recorded audio file were produced by the two types of a power-point show of MS-office 2013(extension; ppsx) form and Web-enabled system using the PHP program to be applied in the mobile phone environment. It is considered to relieve the stress about an language barrier to medical technician and the voice system designed with the variable language will be enhanced the diagnostic information on foreign patients. Because automatic voice system designed for this study can enhance the understanding of test in between korean medical technician and foreign patients, It will be expected to utilize more usefully in clinical practice.

        The Study on Correlation between Carotid IMT and Colon Polyps

        본 연구에 목적은 연령별 대장용종의 발생과 경동맥 내중막 두께와의 상관관계를 알아보고자 하였다. 대장내시경과 경동맥 초음파를 동시에 시행한 환자를 대상으로 연령별로 나누어 경동맥 내중막 두께에 따라 대장용종 발생의 상관관계를 교차분석을 토대로 확인하였다. 분석결과 대사증후군 환자 중 연령별로 대장용종과 경동맥 내중막 두께와의 연관성은 고령일수록 높은 일치결과를 나타냈고, 경동맥 내중막 두께가 1.1mm이상일 때 용종발생비율이 높게 나타났다. 결론적으로 대사증후군 환자의 용종발생비율은 경동맥 초음파 검사와 대장내시경과 높은 상관관계가 있었고, 고령일수록 경동맥 내중막 두께가 두꺼울수록 높았다. This study aims to investigate the correlation between the occurrence of colon polyps and carotid IMT by age. This study checked the correlation between the occurrence of colon polyps and carotid IMT, grouping patients who had a colonoscopy and carotid ultrasonography simultaneously by age based on cross-tabulation. As a result of the analysis, by age, the older the patients with metabolic syndrome, the higher the correlation between the occurrence of colon polyps and carotid IMT became. Also, when carotid IMT was more than 1.1mm, the incidence of polyps was high. In conclusion, there was a high correlation of the occurrence rate of colon polyps with carotid ultrasonography and colonoscopy, and the older the patient and the thicker the carotid IMT, the higher the correlation became.
