The blepharoptosis is the disease which is caused by the congenital or acquired weakness of the levator palpebrae superioris. From September 1984 to December 1992, authors had treated 19 blepharoptosis patients(22 cases of operation), and the operative procedures were chosen according to the patient's levator function : levator resection in cases with levator function above 4mm(6 cases), frontalis transfer in cases with levator function below 2mm(5cases), levator resection(7cases) or frontalis transfer(4 cases) in cases with levator function between 2-4mm. And the operative results were evaluated with the average follow up period of 34.5 months. The results were summarized as follows. 1. 15patients were congenital and 4 patients were acquired type of blepharoptosis and sex ratio was 11:4(male:female) in 15 congenital blepharoptosis patients. 2. We performed 13 cases of levator resection and 9 cases of frontalis transfer. 3. In cases of moderate blepharoptosis, whose levator function is 4-8mm or 2-4mm, results of levator resection and the frontalis transfer are both good. 4. In cases of severe blepharoptosis, whose levator function is 2-4mm, result of levator resection is relatively fair, the frontalis transfer is good. 5. Results of frontalis transfer in acquired type showed excellent regardless of levator function. 6. The incidence of undercorrection after levator resection in congenital blepharoptosis was 72.7%, and the frontalis transfer showed frequent severe complication such as persistent lagophthalmos, exposure keratitis, hollowness of temporal area, headache, overcorrection & undercorrection.

The auricle is frequently exposed to trauma due to its protruding anatomical position and it is sometimes severed completely. Many kinds of techniques such as composite graft or cartilage preservation methods for the reattachment or reconstruction of and amputated ear have been tried. But their indications are limited and results are not satisfactory. For the complete survival of ear cartilage and skin of the amputated part, microvascular replantation is known as the best procedure. But cases of successful replantations have been rare until now, because of technical difficulties due to the small size of auricular vessels and uncertain vascular anatomy. In 1980, Pennington et al. reported the first successful microvasclar replantation of a totally avulsed ear using vein graft. After that about 10 cases of world wide successful micro replantation of servered ears were reported. Here, we report a case of a successful replantation of a completely amputated lower half of auricle including the ear lobule due to a human bite. Replantation was done by direct anastomosis of transected peripheral branches of the posterior auricular artery and vein without vein graft. We again emphasize that, in spite of the technical difficulties and long operative time, successful replantation of an amputated ear with microvascular surgery is superior to any other methods of reattachment or reconstruction.
Apoptotic cell death of human leukaemia U937 cells by ubiquinone-9 purified from Pleurotus eryngii

<P> A chloroform extract of the edible mushroom Pleurotus eryngii showed an inhibitory effect on mammalian DNA topoisomerase I. The topoisomerase I inhibitory compound was purified and identified as ubiquinone-9. Ubiquinone-9 was shown to inhibit the activity of topoisomerase I with IC50 of about 50 µM. Concentration of 110 µM ubiquinone-9 caused 50% growth inhibition of human leukaemia U937 cells, but not that of normal fibroblast NIH3T3 and 3Y1 cells. Ubiquinone-9-induced cell death was characterised with the cleavage of poly (ADP-ribose) polymerase and pro-caspase 3. Furthermore, ubiquinone-9 induced the fragmentation of DNA into an apoptotic DNA ladder, indicating that the inhibitor triggered apoptosis. The induction of apoptosis by ubiquinone-9 was also confirmed using flow cytometry analysis. Taken together, these results suggest that ubiquinone-9 may function by inhibiting oncogenic disease, at least in part, through the inhibition of topoisomerase I activity.</P>

<P>The papain-like protease (PL(pro)), which controls replication of the severe acute respiratory syndrome coronavirus (SARS-CoV), has been identified as a potential drug target for the treatment of SARS. An intensive hunt for effective anti-SARS drugs has been undertaken by screening for natural product inhibitors that target SARS-CoV PL(pro). In this study, diarylheptanoids 1-9 were isolated from Alnus japonica, and the inhibitory activities of these compounds against PL(pro) were determined. Of the isolated diarylheptanoids, hirsutenone (2) showed the most potent PL(pro) inhibitory activity, with an inhibitory concentration (IC(50)) value of 4.1 ?M. Structure-activity analysis showed that catechol and α,β-unsaturated carbonyl moiety in the molecule were the key requirement for SARS-CoV cysteine protease inhibition.</P>

<P>Pseudohypoparathyroidism (PHP) comprises a heterogeneous group of endocrine disorders with the common feature of resistance to parathormone (PTH), manifested by hypocalcemia, hyperphosphatemia, and elevation of serum PTH despite normal renal function. Herein, the first Korean cases of PHP type Ia are reported. The two patients (6-yr-old female, 7-yr-old male) had typical signs of Albright hereditary osteodystrophy. Genomic DNA was isolated from their peripheral blood leukocytes and the GNAS gene was amplified by PCR and analysed by bidirectional sequencing including all coding exons. Two GNAS mutations were found: c.94A>T and c.344_345insT. Patient 1 had a nonsense mutation of c.94A>T (p. K32X), which has not been previously described; the mother also had c.94A>T, and it therefore was a familial mutation. Patient 2 had a known frame shift mutation for c.344_345insT (p.V117RfsX23). The family members of patient 2 had wild-type sequences. In summary, two Korean patients with PHP-Ia were confirmed by genetic analysis and a novel p.K32X product of the GNAS mutation was identified.</P>

<P><B>Purpose</B></P><P>This study sought to determine the 1-year clinical effectiveness and safety of a biodegradable, polymer-containing Biolimus A9™-eluting stent (BES) in Korean patients with acute coronary syndrome (ACS).</P><P><B>Materials and Methods</B></P><P>A total of 1000 ACS patients with 1251 lesions who underwent implantation of BESs at 22 centers in Korea were enrolled between May 2011 and July 2013. We assessed major adverse cardiac events (MACE) defined as the composite of cardiac death, non-fatal myocardial infarction (MI), and clinical-driven target vessel revascularization at 12 months.</P><P><B>Results</B></P><P>Patient mean age was 62.6±11.4 years. 72.8% of the patients were male, 28.5% had diabetes, 32.8% had multi-vessel disease (MVD), and 47.9% presented with acute MI (AMI). The mean global registry of acute coronary events risk score of all patients was 103.0±27.6. The number of stents per patient was 1.3±0.6. The incidences of MACE and definite stent thrombosis at 12 months were 3.9% and 0.2%, respectively. On multivariate Cox-regression analysis, age ≥65 years was identified as an independent predictors of 1-year MACE (hazard ratio=2.474; 95% confidence interval=1.202−5.091). Subgroup analyses revealed no significant differences in the incidence of MACE between patients with and without diabetes (4.3% vs. 3.7%, <I>p</I>=0.667), between those who presented with and without AMI (4.4% vs. 3.4%, <I>p</I>=0.403), and between those with and without MVD (4.6% vs. 3.5%, <I>p</I>=0.387).</P><P><B>Conclusion</B></P><P>Our study demonstrated excellent 1-year clinical outcomes of BES implantation in patients at low-risk for ACS.</P>
Compliance-Based Robotic Peg-in-Hole Assembly Strategy Without Force Feedback

<P>The peg-in-hole assembly is a representative robotic task that involves physical contact with the external environment. In this paper, in contrast to past research in the area, which has involved the utilization of such expensive devices as force/torque sensors or remote compliance mechanisms, an inexpensive method is proposed for peg-in-hole assembly without force feedback or passive compliance mechanisms. The method consists of an analysis of the state of contact between the peg and the hole as well as a strategy to overcome the inevitable positional uncertainty of the hole incurred in the recognition process. A control scheme was developed to yield compliant behavior from the robot with physical contact under the condition of hybrid position/force control. The effectiveness of the proposed method was experimentally verified using a robot manipulator with 8-degrees of freedom and a peg-in-hole apparatus with a small clearance (0.1 mm).</P>
