Background: Denosumab, a humanised monoclonal antibody against receptor activator of nuclear factor kappa beta ligand (RANKL), reduces bone turnover via inhibition of osteoclasts and osteoclastogenesis and is used in the management of osteoporosis and metastatic bone disease. While hypocalcaemia complicating denosumab treatment has been reported, the real world incidence, clinical and biochemical risk factors are not fully elucidated. This study aims to investigate the incidence of denosumab-associated hypocalcaemia (DAH) and identify relevant clinical and investigation features. Methods: We performed a retrospective observational audit of patients administered denosumab (60mg/120mg) over a 12 month period at a tertiary hospital in Australia. Data collected: denosumab dosage and indication, 25-hydroxyvitamin D concentration, parathyroid hormone, estimated glomerular filtration rate (eGFR), nadir and duration of hypocalcaemia (albumin adjusted serum calcium concentration <2.15 mmol/L or ionised calcium <1.13 mmol/L upto 6 months post denosumab administration), calcium and colecalciferol pre- and post-administration of denosumab. The primary outcome was the incidence proportion of DAH. Results: Of 161 patients administered denosumab (106 osteoporosis, 55 bone metastases), 20 patients (12.4%, mean age 78.5 years, 11 male, 11 osteoporosis, 9 bone metastases) developed hypocalcaemia. Median calcium nadir was 2.06 mmol/L (interquartile range (IQR) 1.81-2.11), with the median time to diagnosis 24.50 days (IQR 9.25-41.25). One patient required intravenous calcium gluconate treatment. 75% of affected patients had a 25-hydroxyvitamin D concentration >50nmol/L and 90% of affected patients were on calcium or colecalciferol supplementation. DAH was associated with sex (27% males versus 8% females, p=0.004), but not age (8% under 60 years versus 13% aged 60 or more, p=0.74) or indication (16% with bone metastases versus 10% with osteoporosis, p=0.32). Conclusions: Denosumab-associated hypocalcaemia occurred in over 12% of patients in this population, despite wide use of appropriate calcium and colecalciferol supplementation.

Synthesis of Host Polymers and Guests for Electrophosphorescence

Significant progress has been realized in the design and synthesis of light emitting polymers that emit over the entire visible spectrum. However, up to seventy-five percent of charge recombination events can lead to triplet states that decay non-radiatively. Following the pioneering work in the field of small molecule organic light emitting devices, it has been found that solution processible iridium polymer complexes can be used to harness the wasted triplet energy. In this paper, new results with respect to the electrophosphorescence of solution processible tethered iridium polymer derivatives are presented. Furthermore, our approaches to the design of new high triplet energy conjugated polymer hosts are also reported.
HOUSEHOLD USE OF SYNTHETIC CHEMICALS AND LABOR FOR DO-IT-YOURSELF YARD CARE

Use of synthetic fertilizers and pesticides on yards is increasingly scrutinized by government officials for health and environmental risks but has not been extensively analyzed by economists. In our model, estimated with data from a national survey, households choose initially how much, if any, money to spend on synthetic chemicals and subsequently how much time to spend on other yard tasks. Expenditures on synthetic chemicals are income inelastic. Time spent on subsequent yard care decreases as income increases but increases as prior chemical expenditures increase. These effects on time allocation and our theoretical model help us to characterize and qualitatively analyze the private benefits of synthetic chemical use and costs of a ban on that use. Data on prices and quantities are necessary to estimate these costs and determine the extent to which politically feasible financial incentives would encourage use of 'environmentally friendlier' alternatives. The non-effect of the number of children on expenditures underscores the importance of future research about expenditures on specific risk-classes of pesticides and household perceptions of risks.
Effect of Dupilumab on Sleep Apnea Severity in Patients With Chronic Rhinosinusitis

Patients with chronic rhinosinusitis (CRS) report improved sleep quality after dupilumab, an anti IL4/13 therapy. Concurrent CRS and obstructive sleep apnea (OSA) cases are not rare, and CRS seemingly raises nasal resistance. Thus, we hypothesized that improved sleep quality by dupilumab therapy in CRS patients might be due to lowered nasal resistance and subsequent improvement of unrecognized comorbid OSA. Patients with concurrent CRS and OSA were recruited. Nasal resistance was measured invasively with transnasal pressure and flow data collected during normal respiration in the supine position. Results from the first five participants did not support our hypothesis. Subjective and objective measures for CRS and nasal resistance values were improved with dupilumab therapy in CRS patients with nasal polyps. However, apnea severity and sleep-related subjective parameters did not change. In the patients with CRS without nasal polyps, no significant changes in either CRS or OSA-related measures were observed.
161Tb‑PSMA Unleashed: a Promising New Player in the Theranostics of Prostate Cancer

Radiotheranostics with 177Lu-PSMA have changed the treatment paradigm in patients with prostate cancer, becoming thenew standard in certain settings. Terbium-161 (161Tb) has been recently investigated as a potential radionuclide for radiotheranosticsin various types of cancer, including metastatic castration-resistant prostate cancer (mCRPC). The nuclearmedicine team at King Hussein Cancer Center (KHCC) in Amman, Jordan, recently published the first-in-human SPECT/CT imaging results following a well-tolerated dose of 161Tb-PSMA radioligand therapy with no treatment-related adverseevents, adding to the potential of radiotheranostics in prostate cancer. Two clinical trials for 161Tb-PSMA radioligand therapyin prostate cancer are currently underway and will provide valuable insights. This review will shed light on the expandingfield of radiotheranostics in prostate cancer, which is not without challenges, and will discuss how the introduction of a newtherapeutic option like 161Tb-PSMA may help to combat these challenges and build on the proven success of 177Lu-PSMAbasedradiotheranostics for the benefit of prostate cancer patients worldwide.
