        Ulnar artery access for intracranial mechanical thrombectomy procedure: A salvage option after failed trans-femoral and trans-radial access

        84 years old gentle man with past medical history of hypertension and diabetes presented with sudden onset right sided weakness and aphasia for two hours. Initial neurological assessment revealed National Institute of Health Stroke Scale (NIHSS) 17. Computed tomography (CT) scan demonstrated minimal early ischemic changes along left insular cortex with occlusion of left middle cerebral artery (MCA). Based on clinical and imaging findings, decision was made to perform mechanical thrombectomy procedure. Initially, right common femoral artery approach was utilized. However, due to unfavorable type-III bovine arch, left internal carotid artery could not be engaged via this approach. Subsequently, access was switched to right radial artery. Angiogram revealed small caliber radial artery, with larger caliber ulnar artery. Attempt was made to advance the guide catheter through the radial artery, however significant vasospasm was encountered. Subsequently, ulnar artery was accessed and successful thrombolysis in cerebral infarction (TICI) III left MCA reperfusion was achieved with a single pass of mechanical thrombectomy via this approach. Post procedure neurological examination demonstrated significant clinical improvement. Doppler ultrasound 48 hours after the procedure demonstrated patent flow in radial and ulnar arteries with no evidence of dissection.

        Salvage flow diverter stent across the posterior communicating artery for persistent retrograde filling of a giant internal carotid artery aneurysm after parent vessel occlusion

        Giant internal carotid artery (ICA) aneurysms are complex vascular lesions which are difficult to treat with open as well as endovascular surgery. Parent vessel occlusion is a well-established treatment option for such aneurysms. However, there have been a few reported cases of ruptured aneurysms related to the persistent retrograde filling after parent vessel occlusion. We report a case which highlights the usage of the flow diverter stent as a potential treatment strategy for the management of retrograde filling of aneurysms. A 54-year-old female was found to have a giant left ICA aneurysm on a brain magnetic resonance imaging during workup for headaches. She underwent occlusion of the left ICA proximal to the aneurysm using multiple coils. However, follow up angiograms after 6 months and 2 years demonstrated persistent retrograde filling of the left ICA aneurysm through the posterior communicating (PCOM) artery. Eventually, she was successfully treated with a flow diverter stent across the PCOM artery into the distal ICA. Follow up angiogram after 6 months showed patent flow in the PCOM artery and the distal ICA. with complete occlusion of the aneurysm. Using a flow diverter stent after insufficient parent vessel occlusion for giant intracranial aneurysms may be a feasible treatment option and an addition to the neurovascular treatment armamentarium.

        The First 100 Cases of Endoscopic Combined Intrarenal Surgery in Korea: Matched Cohort Analyses versus Shock-Wave Lithotripsy

        Purpose: This study presents our initial experience with endoscopic combined intrarenal surgery (ECIRS) for large renal stonesand compares the results of a propensity score-matched cohort of patients undergoing shock-wave lithotripsy (SWL). Materials and Methods: A total of 100 adults underwent ECIRS for renal stones between August 2017 and January 2019. For comparison,2172 patients who underwent a first session of SWL between January 2005 and May 2018 were included in the SWL cohort. Propensity score matching was performed using maximal stone length (MSL), mean stone density (MSD), and stone heterogeneityindex (SHI) scores. Stone-free rate (SFR) and success rate were compared between ECIRS and SWL. Results: In the ECIRS group, the mean MSL, mean MSD, and mean SHI were 28.7±15.2 mm, 1013.9±360.0 Hounsfield units (HU),209.4±104.0 HU, respectively. The SFR was 70%, and the success rate was 82.0% in this group. Although the ECIRS group had larger,harder, and more homogeneous stones than the SWL group, ECIRS showed a higher SFR and success rate than SWL. After propensity-score matching, SFR and success rate remained higher with ECIRS than with SWL (both, p<0.001). In multivariate logistic regression,smaller stone size [odds ratio (OR): 0.947, 95% confidence interval (CI): 0.913–0.979, p=0.002] and lower Seoul NationalUniversity Renal Stone Complexity score (OR: 0.759, 95% CI: 0.610–0.935, p=0.011) were independent predictors of successful ECIRS. Conclusion: ECIRS showed a higher SFR and success rate than SWL for large renal stones. Smaller stone size and lower complexityof stones were associated with a higher likelihood of successful ECIRS.

        Carotid Artery Angioplasty and Stenting for Carotid Stenosis: A Single-Center Experience from Saudi Arabia

        Purpose: Atherosclerotic stenosis of the extracranial carotid artery accounts for approximately 20% of all strokes. Both carotid artery endarterectomy and carotid artery angioplasty with stenting (CAAS) are recommended for symptomatic patients with 50% or more stenosis or asymptomatic patients with 70% or more stenosis. CAAS is under-reported in Saudi Arabia, as evidenced by a thorough literature search. In this article, we aim to share our experience of CAAS to call for the necessity of conducting more research on stroke and emphasize the local need of utilizing more endovascular treatments like CAAS.Materials and Methods: A retrospective single-center observational study was conducted at King Abdulaziz Medical City in Jeddah, Saudi Arabia. The inclusion criteria consisted of all adult patients (18 years and above) with carotid stenosis who were treated with CAAS.Results: A total of 16 patients were included in the study. The mean age of the participants was 66.9±13.5 years (range 30–87 years). All patients were symptomatic (had a previous stroke or transient ischemic attack). The procedure was successful in 14 patients (87.5%), while it failed in 2 patients (12.5%) due to technical reasons. All patients had no stroke or myocardial infarction within 30 days of the procedure.Conclusion: Despite the advancement in medicine with free healthcare services in Saudi Arabia, the interventional procedures for secondary prevention of strokes are underutilized. Collaboration between different hospitals will be extremely helpful since few centers in each city are providing such treatments by an expert neurointerventionist and/or strokologist. The good selection of candidates, optimal management of comorbid conditions, and multidisciplinary care may improve outcomes and reduce mortality.

        Efficient electrochemical detection of L-lactic acid using platinum nanoparticle decorated Chitosan/ZnTiO3 nanocomposites

        Herein, a facile L-lactic acid electrochemical sensor based on Pt-nanoparticles (NPs)@Chitosan/ZnTiO3nanocomposites (NCs) coated on a glassy carbon electrode (GCE) is demonstrated. Ultra-sonicationmethod followed by irradiation using an Osram Hg-lamp was applied to prepare Pt-NPs@Chitosan/ZnTiO3 NCs, and the characterization of prepared NCs was executed by Field Emission ScanningElectron Microscopy FESEM, Energy Dispersive Spectroscopy EDS, Transmission Electron MicroscopyTEM, High Resolution Transmission Electron Microscopy HRTEM, Fourier Transform InfraredSpectroscopy FTIR, Ultraviolet–visible spectroscopy UV–vis., and X ray Diffraction XRD analysis. A linearcurrent versus potential responses relation was obtained in a concentration range of L-lactic acid of 0. 30 2.40 mM at differential pulse voltammetric (DPV) analysis in a pH 7.0 buffer medium and resultedconcentration range was defined as the dynamic detection range (LDR) for L-lactic acid analysis. The Llacticacid sensor sensitivity (0.4529 lAlM-1cm2), limit of detection (LOD; 22.36 ± 1.12 lM), and limitof quantification (LOQ; 79.88 lM) were obtained. Besides this, the sensor reproducibility and responsetime were found to be reliable. Finally, the assembled sensor probe was validated by the testing of realsamples, which exhibited acceptable and satisfied results. It is introduced a new route for the detection ofchemicals using novel nanocomposite materials by electrochemical approach for the safety of healthcarefields in a broad scales.

        Spectroscopic and cloud point studies of the interaction and thermodynamics of ciprofloxacin hydrochloride+surfactants mixture in different solvents: Effect of temperature and composition

        Surfactant is one of the most important chemical entities in drug formulation which can bind with drug molecules. Herein, the binding interaction of ciprofloxacin hydrochloride (CFH) drug with two different surfactants (sodium dodecyl sulfate (SDS) and Triton X-100 (TX-100)) has been investigated through UV-Visible spectroscopic and cloud point measurement techniques at different conditions. The absorption spectrum of CFH was found to be dependent on presence of additives/temperature change. The binding constant (Kb) of CFH+SDS/CFH+TX-100 was found to be increased primarily, reached a maximum value and then decreased with the increase of temperature, except in water medium (pH=2.0) and 30% (v/v) methanol. The Kb values for CFH+SDS were found to be higher in the aqueous medium than almost all medium studied herein, while better binding was observed in the alcoholic medium in the case of the CFH+TX-100 system. The Gibbs free energy of binding (Gb o) for both CFH+SDS and CFH+TX- 100 systems were attained negative in each case studied, inferring the spontaneous binding phenomenon. The cloud point (CP) value of CFH+TX-100 mixture was lessened in ZnSO4·7H2O solution and the CP values exhibited a gradual reduction through the upsurge of electrolyte concentration. The positive values of the Gibbs free energy of clouding indicated the nonspontaneous clouding phenomena. To disclose the interaction between drug and surfactant, other thermodynamic parameters, e.g., enthalpy (Hb o) and entropy (Sb o), different transfer energies as well as entropyenthalpy compensation parameters of binding/clouding were evaluated and clarified with proper explanation.

      • Machine learning-based techniques to facilitate the production of stone nano powder-reinforced manufactured-sand concrete

        This study aims to examine four machine learning (ML)-based models for their potential to estimate the splitting tensile strength (STS) of manufactured sand concrete (MSC). The ML models were trained and tested based on 310 experimental data points. Stone nanopowder content (SNPC), curing age (CA), and water-to-cement (W/C) ratio were also studied for their impacts on the STS of MSC. According to the results, the support vector regression (SVR) model had the highest correlation with experimental data. Still, all of the optimized ML models showed promise in estimating the STS of MSC. Both ML and laboratory results showed that MSC with 10% SNPC improved the STS of MSC.
