앵두 Ethanol 추출물의 항균력과 Flavonoid

앵두의 기능성 물질을 검색하기 위하여 50%, 70% 및 95% (v/v)의 ethanol 추출물에 대한 flavonoid류를 정량하고 MIC와 disk test를 이용한 항균력 및 자외선 차단효과를 조사하였다. 앵두의 70%(v/v) ethanol 추출액에서 가장 높은 flavonoid함량을 나타냈고 quercitrin의 함량은 ethyl acetate 분획에서 12.70 ㎎%, buthanol분획에서 2.11 ㎎%로 대부분 ethyl acetate 분획에서 분리되었다. 또한 앵두 70%(v/v) ethanol추출물의 flavonoid류는 quercitrin 14.9 ㎎%, tannin 6.0 ㎎% 및 catechin 5.2 ㎎%로 정량되어 quercitrin이 앵두의 주요 flavonoid임을 확인하였다. 앵두의 ethanol 추출물에 대한 Staphylococcus aureus(ATCC 6538)와 Staphylococcus epidermidis(ATCC 12228)의 항균력은 flavonoid함량이 높은 ethyl acetate 분획에서 가장 높은 항균력을 나타내어 앵두의 flavonoid에 의한 결과로 확인되었다. 앵두의 ethanol 추출물의 자외선 차단효과는 인체의 피부에 손상을 주는 UVB 파장인 290~320 ㎚를 포함하는 210~380 ㎚로 넓게 자외선 흡수대를 형성하고 catechin과 quercitrin의 흡수대와 동일한 패턴을 나타내어 그의 기능성이 예상되었다. 따라서 앵두의 기능성을 나타내는 주요 성분은 quercitrin으로 피부에 관련된 항균효과와 자외선 차단효과를 예상할 수 있었다. Quantitaties of flavonoids were determined by HPLC in three different ethanol extract (50, 70 and 95%, v/v) from Korean cherry. Antimicrobial activity of the ethanol extract and its solvent fraction (ethyl acetate and n-butanol fractions) against Staphylococcus aureus (ATCC 6538) and Staphylococcus epidermidis (ATCC 12228) was evaluated using MIC and disk test. Additionally, UV protective effect was also determined. The highest flavonoid quantity was found in 70% ethanol extract of Korean cherry. The quantities of quercitrin were 12.7 ㎎% and 2.1 ㎎% in ethyl acetate fraction and in buthanol fraction, respectively. In addition, the flavonoid in 70% (v/v) ethanol extract of Korean cherry included 14.9 ㎎% of quercitrin, 6.0 ㎎% of tannic acid and 5.2 ㎎% of catechin. Therefore, it was confirmed that main flavonoid of Korean cherry was quercitrin. For antimicrobial activity of ethanol extract of Korean cherry against Staphylococcus aureus (ATCC 6538) and Staphylococcus epidermidis (ATCC 12228), MIC and disk test using ethyl acetate fraction and butanol fraction were carried out. As a result, ethyl acetate fraction with higher quantity of flavonoid showed higher antimicrobial activity. Therefore, it was confirmed that the higher antimicrobial activity was brought about by flavonoid in Korean cherry. For UV protective effect of Korean cherry, ethanol extract showed the absorption zone of ultraviolet rays in 210~380 ㎚ including 290~320 ㎚ that is the wave length of UV-B harmful to the skin of human body. The pattern is the same as the absorption zone of 220~300 ㎚ in catechin and 210~400 ㎚ in quercitrin. Then, the functionalities of catechin and quercitrin were anticipated. It is expected that quercitrin in Korean cherry has the antimicrobial effect and UV protective effect related to the skin.

Two polysaccharides, ACP-UMF and ACP-ULF, were purified from the aerial part of Artemisia capillaris by anion-exchange chromatography, ultrafiltration, and gel filtration chromatorgraphy. The polysaccharides appeared to be homogenious from the results of HPLC. The molecular weights of ACP-UMF and ACP-ULF were estimated to be 16305.92 D and 3292.23 D, respectively, by MALDI-TOF MS. The sugar compositions were determined by GC to be arabinose 10.05%, xylose 1.67%, mannose 5.45%, galactose 39.06%, glucose 15.43% for ACP-UMF and arabinose 11.60%, xylose 11.15%, mannose 6.37%, galactose 32.47%, glucose 18.35% for ACP-ULF. A polysaccharide, SP-M, was also purified from Artemisia sylvatica in the same manner as in A. capillaris. The molecular weight of SP-M was determined to be 2462.52 D by MALDI-TOF MS. SP-M consisted mainly of rhamnose 36.49%, arabinose 29.00%, and glucose 19.38%. Incubation of CCl4-intoxicated hepatocytes with ACP-UMF reduced the levels of glutamic pyruvic transaminase(GPT) and cellular malondialdehyde(MDA) to 62.8% and 23.8%. ACP-ULF also reduced the levels of GPT and MDA to 46.1% and 38.1% and SP-M to 38.8% and 26.3%, respectively.
교통사고상해증후군 한의표준임상진료지침 개발을 위한 환자의 의료이용 경험과 의료체계 인식도 설문조사 연구

Objectives To explore the traffic injury patients’ experience on health care utilization of Korean Medicine (KM) practice and perception of health care system for developing a Korean medicine clinical practice guideline (CPG) of traffic injuries by survey method. Methods Two hundred ten patients in suffering from traffic injuries were surveyed from September 1st, 2019 to January 31th, 2020 at 2 University Hospitals (Pusan National Korean Medicine Hospital and Kyung Hee Korean Medicine Hospital at Gangdong) and 1 Spine Specialty Hospital (Jaseng Hospital of Korean Medicine). A structured questionnaire of experience on health care utilization of KM practice and perception of health care system of was distributed to responders by visits and all data were statistically analysed. Results Survey results showed high satisfaction of patients with the experience of KM treatments in order of daoyin exercise (7.8±2.3), chuna manual therapy (7.7±4.0), pharmacoacupuncture (7.4±3.0) etc. Safety concerns were reported in 9.1% subjects and 205 (97.6%) patients answered that collaboration with KM and western medicine is necessary for patients with traffic injuries. For the patients’ requirement for extending insurance coverage, the most required therapy was chuna manual therapy (57.5%) and pharmacoacupuncture (42.0%). Conclusions This study presented the realistic patient-centered perception of KM practice and health care system in Korea. These results will provide basic data to be reflected in the process of adaptation for the revision of Korean Medicine CPG for traffic injuries. (J Korean Med Rehabil 2020;30(2):139-152)
소아 알레르기 비염에 대한 추나요법의 효과: 체계적 문헌 고찰과 메타분석

Objectives : This review determines the evidence of effectiveness and safety of Chuna manual therapy (CMT) for pediatric allergic rhinitis. Methods : We searched 12 electronic databases (Pubmed, EMBASE, Cochrane Library, Web of Science, CAJ, J-STAGE, RISS, DBpia, NDSL, KISTI, KISS, and KMBASE) and two related journals up to the end of April, 2019. We only included randomized controlled trials (RCTs) investigating CMT for the treatment of pediatric allergic rhinitis. The methodological quality of the included RCTs was evaluated using the Cochrane risk of bias tool 1.0. Results : Twelve RCTs were eligible in our inclusion criteria. A meta-analysis of five studies demonstrated positive results for the using CMT for the treatment of pediatric allergic rhinitis. When used in conjunction with traditional Chinese medicine, CMT treatment significantly improved total efficacy rate compared with traditional Chinese medicine alone (P<0.02, n=5). Conclusions : Based on a published meta-analysis, there is reliable evidence for the use of CMT in treating pediatric allergic rhinitis. However, it should be noted that the studies included in this systematic review were heterogeneous and were of low quality, warranting further investigation using well-designed RCTs.
