A laparoscopic cholecystectomy is the gold standard for symptomatic gallbladder stones. Reports of a pseudoaneurysm following a cholecystectomy have recently increased. Herein, the case of a 36-year-old man, who had an unusual complication following a routine laparoscopic cholecystectomy for gallbladder stones, is reported. On postoperative day 6, the patient was readmitted via the emergency room with severe abdominal pain and right shoulder pain. A selective hepatic angiography revealed a pseudoaneurysm of the branch of the right hepatic artery, which was treated with transarterial embolization. A pseudoaneurysm is an uncommon complication of a laparoscopic cholecystectomy. If any suspicion of a pseudoaneurysm is presented, prompt attention and angiography are necessary, as the condition can cause life-threatening bleeding.
제7차 교육과정 시행에 따른 고등학교 도덕과 수업효과 분석

The purpose of this thesis is to analyze on effect of ethics education on the 7th curriculum of highschool. With the purpose on the ground, we discussed the general aim of ethics education in the 7th curriculum. In this study, in order to achieve the aim we make up a questionnaire which is the subject of highschool teacher. And we elucidate the characteristics of the four textbook in the ethics education, which are ``Moral`` ``Ethics and thought`` ``Traditional ethics`` ``Civil ethics``. The correlations between educational aim and context in the ethics textbook are not appropriateness, besides the applied methodology of ethics education is insufficient. In this study, we elucidate that the ethics education in the 7th curriculum contain many problems. We conclude that the keys of overcoming the problem are meaningful description of the ethics textbook and building the effective educational system including social support. Temporal bone CT 검사에서 Ultra algorithm 적용시 algorithm의 특성과 영상의 화질평가

목적 : 높은 공간분해능을 요구하는 Temporal bone CT 검사에서 새로운 Ultra algorithm을 적용하여 기존의 bone plus, edge algorithm과 비교평가해 algorithm의 특성을 파악하고 Temporal bone CT 검사시 Ultra algorithm의 유용성을 평가해보고자 한다. 대상 및 방법 : 본원에 내원하여 Temporal bone CT를 시행한 환자 30명에 동일한 temporal bone CT porotocol에 bone plus, edge, ultra algorithm을 적용하고 FBP와 ASIR30%의 재구성방식을 통해 6종류의 데이터를 획득하였다. ACR phantom 실험에서도 위와 동일한 방법으로 데이터를 획득하였다. 획득한 환자 데이터를 이용해 soft tissue와 bone 2부분에 ROI를 설정하여 영상을 분석하였다. 해상력 측정을 위해 ACR phantom 데이터를 이용해 LSF를 이용한 MTF50%, MTF10%를 각각 측정하여 비교평가 하였다. 결과 : bone plus algorithm을 대조군으로 환자데이터를 분석하였다. soft tissue에서 SD값이 ultra algorithm은 FBP와 ASIR30% 모두 bone plus algorithm과 차이가 없었다. edge algorithm 은 SD값이 FBP와 ASIR30% 각각 28.78%, 34.63% 증가했다. bone에서 SD값이 ultra algorithm은 FBP와 ASIR30% 가각 18.88%, 19.78% 증가했고, edge algorithm은 FBP와 ASIR30% 각각 10%, 9.1% 감소했다. 공간분해능과 선예도 측정결과 bone plus, edge, ultra algorithm의 MTF50% 값은 FBP에서 각각 4.28, 0.62, 5.64였고, ASIR30%는 4.39, -3.77, 6.17이었다. MTF10%값은 FBP에서 6.50, 5.85, 10.01이었고, ASIR30%는 6.91, 5.74, 21.98이었다. 측정결과 ultra algorithm의 공간분해능과 선예도가 가장 좋았다. 결론 : Ultra algorithm은 기존의 bone plus, edge algorithm과 비교했을 때 저주파영역의 노이즈 증가없이 고주파영역만 강조하면서 영상의 화질 또한 우수하다는 것을 알 수 있었다. 임상 Temporal bone CT검사에서 ultra algorithm은 bone plus algorithm과 비교하여 적절히 사용한다면 매우 유용할 것이라 사료된다. Purpose : To find out the usefulness ultra algorithm in temporal bone CT which requires high spatial resoultion by comparing ultra with conventional bone plus and edge algorithm. Materials and Methods : Temporal bone CT imgaes of 30 patients were reconstructed by 6 type 3 different algorithms(bone plus, edge, and ultra algorithm) and 2 different reconstruction techniques (FBP and ASIR30%). ACR pahntom was scanned and acquired images were reconstructed by same way. We set the ROIS on soft tissue and bone of paitent’s CT images and analyzed the images. We also evaluated spatial resolution using LSF by measuring MTF 50% and MTF 10% of ACR phantom images. Result : bone plus algorithm was in the conrtol group for analysis of patient data. In SD of soft tissue, there was no difference between ultra and bone plus algorithms. SD increased by 28.78% and34.63% in FBP and ASIR 30% images of edge algorithm, respectively. in case of bone, images of ultra algorithm showed increased SD by 18.88% and 19.78% in FBP and ASIR 30%, repectively, whilethose of edge algorithm showed decreased SD by 10% and 9.1% in FBP and ASIR 30%. As a result of spatial resolution and sharpness, the value of MTF 50% was 4.28, 0.62 and 5.64 in FBP images and 4.39, -3.77 and 6.17 in ASIR 30% images for bone plus, edge and ultra algorithm, respectively. The value of MTF 10% was 6.50. 5.85 and 10.01 in FBP images and 6.91, 5.74 and 21.98 in ASIR 30% images for bone plus, edge and ultra algorithm, respectively. Spatial resolution and sharpness of ultra algorithm showed thr best. conclusion : Ultra algorithm emphasizes high-frequency without increase of low-frequency noise and shows considered to be usefulexcellent image quality compared to conventional bone plus or edge algorithm. Therefore, ultra algorithm is considered useful in clinical temporal bone CT.

Clear cell renal cell carcinoma (RCC) is the most common histological subtype of malignant renal tumors. Although RCC frequently metastasizes to many organs, it rarely metastasizes to the thyroid gland without evidence of other organ involvement. We report a case of a metastatic renal cell carcinoma to the thyroid gland, which was misdiagnosed preoperatively as a follicular neoplasm from the fine needle aspiration cytology.
갑상선암 세포주에서 Na-4-Phenylbutyrate의 세포 증식 억제 및 분화 유도 효과

Purpose: Sodium-4-phenylbutyrate (Na-4-PB) is an analogue of phenylacetate, which is a well-known redifferentiating agent. In vitro and in vivo studies on this agent have been done and the clinical relevance of Na-4-PB has been studied in other malignancies, but not in thyroid cancer. We investigated the effect of Na-4-PB on cell proliferation and differentiation in thyroid cancer cell lines. Methods: We used 5 thyroid cancer cell lines: TPC-1, FTC-133, FTC-236, FTC-238 and XTC-1. MTT assay and flowcytometry were used to measure the agent’s antiproliferative effects and the cell cycle change. We evaluated the PPARγ expression via western blotting and the mRNA expressions of NIS, Tg and CD 97 were determined by performing RT-PCR. Troglitazone, a potent PPARγ agonist, was used in combined treatment with Na-4-PB. Results: Na-4-PB inhibited cell proliferation in a dose and time dependent manner in all 5 thyroid cancer cell lines. By performing flowcytometry in the FTC-133 and TPC-1 cell lines, we identified that the antiproliferative effect of Na-4-PB was associated with an increased apoptotic cell population. Treatment with Na-4-PB upregulated the PPARγ expression, but the combined treatment of Na-4-PB with troglitazone did not seem to be synergistic for the antiproliferative effect. Treatment with Na-4-PB downregulated the CD97 mRNA expression and it upregulated the NIS and Tg mRNA expressions in both the FTC-133 and TPC-1 cell lines. Conclusion: Na-4-PB inhibited thyroid cancer cell proliferation by inducing apoptosis in a dose dependent manner. Treatment with Na-4-PB increased the expression of PPARγ and it upregulated such differentiation markers as NIS and Tg, and it downregulated CD97, a dedifferentiation marker. Na-4-PB should be further evaluated as a new potential therapeutic agent for patients with thyroid cancer.
