Backgroud: Herpes zoster is caused by the reactivation of varicella zoster virus. Many factors are involved in the reactivation of viruses, but most of them are unknown, except for immunosuppression and old age. A relationship between an increased level of total serum IgE and viral infection has been observed in a few previous studies. Objective: The purpose of this study was to evaluate the quantitative changes, as well as the role of the total serum IgE, in patients suffering with herpes zoster. Methods: We retrospectively reviewed the medical records and laboratory data of 205 patients with herpes zoster. We evaluated the clinical findings such as the affected dermatomes, the duration of illness and the persistence of herpes zoster-associated pain. The relationships of the clinical findings and the level of total serum IgE, which was measured by competitive chemiluminescence immunoassay, were also evaluated. Results: The level of total serum IgE was elevated in 95 (46.3%) of 205 patients with herpes zoster and who did not have any medical history that would influence the level of total serum IgE. The amount of elevation of the total serum IgE was higher in the younger patient group than that in the older patient group. However, there were no statistically significant relationships between the level of total serum IgE and the clinical features. Conclusion: The level of total serum IgE in patients with herpes zoster was increased to more than the normal level. This may contribute to an overt defect of the cellular immune system in patients with herpes zoster. (Korean J Dermatol 2009;47(1):50~54)

Tungiasis is an ectoparasitic disease that is caused by the penetration of Tunga penetrans into the epidermis of the host. Tungiasis is indigenous to South Americaand the Caribbean regions, but it has spread to Latin America, India, and sub-Saharan Africa. The area of soft skin such as the soles, toes, webs and subungual regions are the favored sites for sand flea embedding. As the flea enlarges within epidermis a pruritic, painful white or erythematous papulonodule develops. A black central punctum is characteristic. A 32-year-old woman presented with a small black crust on the right sole after traveling to an endemic area(Congo). Histopathologic examination revealed the internal organs of the arthropod embedded intothe epidermis, which allowed a definitive diagnosis of tungiasis. Although tungiasis is common in endemic areas, there has been no report of tungiasis in the Korean literature. We report herein an interesting case of tungiasis in a patient after traveling to Congo. Apert`s syndrome is a rare autosomal dominant disease that`s characterized by craniosynostosis and severe syndactyly of both the hands and feet. It is caused by a mutation of the fibroblast growth factor receptor (FGFR) type-2 gene. The development of widespread acne, involving the face, chest, back and forearms, is a characteristic finding at puberty in Apert`s syndrome patients. The acne of Apert`s syndrome patients is generally resistant to conventional therapy, including topical agents and/or oral antibiotics, but it shows a good response to isotretinoin. We report here on a case of acne that was successfully treated with isotretinoin in a teenager with Apert`s syndrome. (Korean J Dermatol 2009;47(5):576~579)

몬테소리 유아상상력 교육의 의미

본 연구는 몬테소리 교육사상에서 다소 제한적으로 연구되어온 상상력의 의미를 분석하여 몬테소리 상상력 개념과 교육적 의의를 제시하고자 하였다. 이를 위해 몬테소리 저서와 강연집 뿐만 아니라 상상력과 관련되는 국·내외 자료를 탐색하여 몬테소리가 강조하고 있는 상상력의 의미를 분석하였다. 그 결과 몬테소리의 상상력의 의미는 첫째, 정신적 능력의 확장으로서의 상상력 둘째, 내적 규율의 추상화로서 상상력 셋째, 자연적 실재의 표현을 강조하는 상상력 넷째, 삶의 궁극 목표인 사랑의 실현으로서 상상력을 나타낸다. 몬테소리는 상상력 발달을 위해 유아기 상상력 교육의 중요성과 유아상상력 교육은 자발적 활동을 통하여 이루어져야 함을 강조하였으며, 상상력은 이성과 정서의 종합적 특성으로 총체적 의미의 상상력 발달이 중요함을 제시한다. 또한 몬테소리 상상력은 자연적 실재의 표현을 강조하여 인위적이고 상업화된 형식적인 유아교육의 잘못된 방법들에 시사점을 준다. 몬테소리의 상상력은 오늘날 유아기에 적합하지 않은 과도한 조기교육, 폭력성이 난무하는 미디어 등에 대한 올바른 방향을 제시한다. 상상력은 교육의 궁극목적으로서 사랑의 차원으로까지 확대된다. 몬테소리는 교육은 자발적인 상상의 과정이며 이러한 상상은 선을 지향해야한다고 하였다. 몬테소리 교육에서 상상력은 ‘사랑’, ‘우주적 통일체’, ‘정상화’ 를 실현할 수 있도록 돕는 유아중심교육에 의의가 있다. 뿐만 아니라 몬테소리 유아상상력 교육은 유아교사의 올바른 역할과 자연교육에 많은 영향을 준다. This study examines the nature of imagination in Montessori early childhood education. Imagination is a characteristically human capacity, an innate power of children to develop as their mental capacities are established through interaction with the environment. The imaginative ability to grasp the whole generates a sense of wonder and gratitude for the world``s large-scale diversity. Wonder is a magnet; it impels from within. Gratitude fosters well being and harmony; it bonds with the universe. First, Montessori``s Imagination is an extension of humans`` mental capacity, an extension further supported by scientific classification, as Montessori suggested when she said, “the world always repeats more or less the same elements.” Thus, it is with the aid of both imagination and abstraction that the elementary child can expand the idea. Second, it is Imagination as Love of Interdisciplinary Abstraction. The nature if the imaginative investigations is a passion demonstrated within and among all subjects, a way of experiencing the disciplines in a common world, a seamless web of interconnected knowledge to be explored uniquely by each teacher and child. Third, Imagination emphatically expresses nature``s reality. Montessori emphasized that the imagination of a six-year-old cannot take flight without prior grounding in sensory experience, without a well- articulated reality achieved in the first plane. Fourth, Imagination is the basis for philosophy of life and Service. Imagination is an engine that drives the elementary years; it powers the quest for knowledge across disciplines. The imagination, now a conscious resource in early childhood, forms a child``s life vision while integrating the child``s personality.

미만형 신경섬유종

Diffuse neurofibroma is an uncommon variant of neurofibroma that usually presents on the head and neck. It is common in children and young adults. Clinically, the lesion presents as large, poorly circumscribed swelling. A 25 year-old female presented with diffuse swelling on the occipital region of the scalp for several years. Magnetic resonance imaging showed 8x9 cm sized, ill defined infiltration on the occipital scalp. The histologic examination revealed unencapsulated loosely arranged spindle cells within the dermis and subcutaneous tissue. Meissnerian differentiation was observed within the tumor cells. The tumor cells were positive for S-100 protein. We report a case of diffuse neurofibroma on the occipital region of the scalp. (Korean J Dermatol 2008;46(3):355∼358)

다원주의 사회에서 몬테소리 유아인성교육의 의미

본 연구는 다양한 가치가 혼재하는 다원주의 사회에서 유아인성교육의 의미를 몬테소리가 제시하는 유아인성개념과 인성교육에서 탐색하여 오늘날 다원주의 사회에서 요구되는 바람직한 유아인성교육의 의미를 제시하고자 하였다. 이를 위해 몬테소리 저서와 강연집 뿐만 아니라 인성과 관련되는 국․내외 자료를 탐색하여 몬테소리가 강조하고 있는 인성과 인성교육의 의미를 분석하였다. 그 결과 몬테소리의 인성의 의미는 첫째, 자연과학적 인간 둘째, 자유로운 인간 셋째, 종교적 인간 넷째, 총체적 인간으로서 인성개념을 나타낸다. 다원주의 사회에서 몬테소리 유아인성교육은 이러한 인성개념을 바탕으로 첫째, 경험중심, 활동중심 유아인성교육을 강조한다. 둘째, 유아 인성교육은 자유로운 유아의 정신을 확장하는 교육되어야 한다. 셋째, 종교적 인간본성을 기초로 자연의 실재뿐 아니라 영성, 경외심, 자연과 초자연에 대한 상상력을 함양하는 교육을 강조한다. 넷째, 총체적 인성개념으로서 몬테소리 유아인성교육은 통합성, 다원성, 배려, 연대성, 창의성 등을 강조하는 전인교육을 의미한다. The study aims to propose the proper meaning of early childhood humanity education demanded in today"s pluralistic society by examining the meaning of early childhood humanity education in pluralistic society composed of various values within early childhood humanity concept and education suggested by Montessori. For this end, this study analyzed the meaning of humanity and humanity education emphasized by Montessori and I explored not only the books and lectures of Montessori, but also the national and international materials related to humanity. As results, the meaning of humanity by Montessori has first, natural scientific human, Second, a free human being, Third, a religious human being, and Forth, an overall human being. Within a pluralistic society, Montessori humanity education for early childhood takes its ground on such concept of humanity and emphasizes following: First, humanity education for early childhood should be experiential and include activities. Second, humanity education for early childhood should be aiming for extending free spirit of children. Third, humanity education for early childhood should be based on the religious human nature and cultivate not only the reality of nature, but also the imagination of spirituality, admiration, as well as nature and supernatural. Fourth, Montessori humanity education for early childhood, as a concept of overall humanity, stands for holistic education emphasizing integration, pluralism, consideration, solidarity, and creativity.
