In the injection-molded light guide plate the replication quality, i.e. the reproduction accuracy, ofmicro-patterns should be high and uniform over the entire surface area. However technical difficulty inmeeting the necessary replication quality arises as the plate size becomes large for TV applications. Wepropose a simulation technique to optimize the distribution of micro-patterns on a 55-inch injection moldedlight guide plate considering non-ideal replication quality of micro-patterns. The luminance uniformity couldbe improved by more than 16% by optimizing the pattern distribution in spite of the same replicationquality

A Polymorphous Low-Grade Adenocarcinoma (PLGA) of the major salivary gland is an extremely rare case over the world. The PLGA is usually a well-recognized tumor with a predilection for minor salivary glands. Furthermore, A PLGA arising in the parotid gland is difficult to diagnose clinically and cytohistologically. Because the tumor present an indolent clinical course and its histologic pattern is similar to adenocystic carcinoma and pleomorphic adenoma. We present a rare case of PLGA arising within the left parotid gland in a 23-year-old man with a review of the literature.

Optical structures of a tapered ultra-thin light guide film (LGF) were optimized by optical simulationfor increasing coupling efficiency between light sources and the LGF. A serration pattern on the entranceside surface could provide a comparable coupling efficiency to that of the conventional LGF where a linear,asymmetric prism array was formed on the taper surface. Several micro-patterns were applied to the topand/or bottom surface of the LGF for achieving better luminance property, and it was found that an optimizedmicro-pyramid pattern exhibited the highest average luminance together with satisfactory luminance uniformity.

Microlens array (MLA) films have been used as brightness-enhancement components in many illumination systems, such as a backlight, which should meet the industrial needs of low cost and high brightness performance. A hexagonally-arranged MLA (HA-MLA) was chosen due to its high filling factor, which was a prerequisite for achieving high brightness-enhancement capability. The moiré fringes produced in a film stack composed of two HA-MLA were investigated, under four different lens configurations, by optical simulation. It was found that randomizing the lens configuration, while keeping the filling factor high, was effective way to develop high-gain MLA films for backlight applications.

The classic surgical technique for cochlear implantation was first introduced by House in 1961. This technique includes a mastoidectomy and a posterior tympanotomy, which has been most widely used for cochlear implantation. However, this approach may result in the injury of the chorda tympani nerve or the facial nerve. To avoid the risk of the neural injury, several alternative surgical methods have been developed. The endomeatal approach is one of the several alternative techniques. We present, along with a review of literatures, a case of cochlear implantation using the endomeatal approach performed in a child with an extremely hypoplastic mastoid. The objective of this study is to develop a highly accurate micro tool alignment system applicable to the micro machining technology. In a specific application such as micro hole punching, radial clearance between micro tools is order of a few micron. Under this micron scale tool clearance, accuracy of tool alignment is very important for ensuring hole quality. In the present study, a two-way image acquisition system was developed, which can produce overlapped image of both micro tools that face each other, and applied to the tool alignment in the micro punching. Also, to meet alignment accuracy of tools within $1\mu\textrm{m}$, the cross correlation image processing algorithm was employed. With this system, $25\mu\textrm{m}$ punching tools with $1\mu\textrm{m}$ radial clearance could be accurately aligned.

A stapedial dislocation is a rare complication of ossicular trauma that is most commonly caused by direct, penetrating injury to the external ear canal. In this type of ossicular dislocation, patients usually suffer from cochleovestibular symptoms including progressive sensorineural hearing loss, conductive hearing loss, tinnitus, and vertigo. We present rare cases of internal stapedial dislocation after penetrating trauma, which resulted in the preservation of bone conduction hearing thresholds and complete resolution of the vestibular symptoms after an urgent repair. A skull base defect mainly occurs as a result of congenital defect, trauma, iatrogenic injury, inflammatory erosion, increased intracranial pressure or neoplasm. Meningoencephalocele is one of them resulting in skull base defects. Among the several types of meningoencephalocele, protrusion to lateral wall of sphenoid sinus in the basal type is less frequently encountered than other types and has not been reported in Korea. We report a patient with meningoencephalocele protruded to sphenoid sinus wall. It was successfully repaired with nasoseptal flap through the intranasal endoscopic approach.

Milk-alkali syndrome (MAS) is a disorder featuring hypercalcemia, renal failure and metabolic alkalosis due to ingestion of large amounts of calcium and absorbable alkali. MAS was first reported after Milk-alkali treatment of peptic ulcer in early 1910. But since the late 20th century, the wide availability and increasing consumption of calcium carbonate and vitamin D supplementation mostly for osteoporosis prevention has been noted as reemerging clinical backgrounds of MAS. We experienced a case of a woman who had taken calcium and vitamin D for a year due to hypoparathyroidism after total thyroidectomy. She was admitted suffering from nausea, vomiting, anorexia and irritability with the triad of hypercalcemia, metabolic alkalosis and acute renal insufficiency. Every abnormal symptom, calcium level and renal function were normalized with fluid and diuretic therapies.
