The malfunction of an expiratory unidirectional valve (EV) can cause rebreath of expired gas, barotraumas, or ventilatory failure during the general anesthesia. The following is a report on a case of ventilatoryfailure caused by an EV that failed to open during the induction of anesthesia. A 57-year-old man was scheduled for the biopsy of a vocal cord polyp. After intubation, we could not detect the evidence of ventilation through the endotracheal tube. Suspecting the esophageal intubation, we administered extubation. The patient was still having difficulty in ventilating even after a retrial of intubation. Then we discovered the EV was failing to open properly with ventilation and thus not able to function properly. The common cause of ventilatory failure immediately after intubation is malposition of an endotracheal tube, like esophageal intubation, and equipment failure also has reported. Accordingly, we should remember possible causes of ventilatory failure after intubations and routine conscientious inspection of the ventilator. (Korean J Anesthesiol 2007; 53: 774∼7)

A Bezold-Jarisch reflex (BJR) mediated vasovagal syncope is the most common neurally mediated reflex characterized by the sudden failure of the autonomic nervous system and have been reported in patients undergoing a peripheral nerve block. The concomitant administration of a β-adrenoreceptor antagonist and calcium channel blocker have a synergistic suppressant effect on the autonomic nervous system and interact with the anesthetics significantly. We report a case of a 57-year-old female patient with essential hypertension controlled with lercanidipine and carvedilol. Cardiac arrest developed with spontaneous respiration 50 min after the brachial plexus block to remove a plate in the humerus. Epinephrine was administered and the pulse immediately returned to a normal sinus rhythm. This event might be vasovagal syncope mediated by BJR, and realted to the antihypertensive medication used. 정신질환교육이 고등학생들의 정신질환에 대한 태도에 미치는 영향

The objectives of this study are to find out the positive effect of mental illness education on the attitudes toward mental illness of high school students. 198 students, both male and femane, at S High School located in K City were sampled for this study from 2nd February 1998 until 20th February 1998. Documents were compiled through questionaires to the experimental group and comparative group of 4 classes after visiting to the school to explain the purpose of the study to the students and teachers. The instrument used for this study was a questionaire developed by S. Martim Taylor and Michael J. Dear in 1981 and translated by Lee, Joo-Hoon, et al. which was verified its reliance and validity. CAMI is consists of 40 question with 4 subordinate factors i.e. authoritarianism, benevolence, social restrictiveness and concept of community mental health. The mental health education was provided with the subject of the concept of mental illness, causes and types of mental illness, characteristics of behaviour, treatment, prognosis, prevention etc. for 50 minutes for two times each class. Findings of the study demonstrate as follows : A significant difference between two groups shown in 3 factors among 4 in a test carried out after an education on mental illness. That is, the experimental group who was educated on mental illness shows lover points in authoritarianism (t=4.81, p=.01) and social restrictiveness(t=5.25, p=.010 and higher points in concept of community mental health(t=7.50, p=.01) than that of comparative group. There is no significant difference in the points of benevolence, by the way. At it is shown above, the mental illness education for high school students can change the attitudes towards mental illness. Their attitudes on mental illness was quite positive and favorable after having an education. This shows that, in order to change the attitudes toward mental illness, a systematic and specified on-going preventive education on mental illness reponding to the needs of adolescent is necessary.
